Un sistema (10) para estimular la cicatrización de tejido, que comprende: una almohadilla porosa (11); un apósito estanco (13); un medio de conexión del extremo distal (16b) de un conducto (16) a través del apósito (13); una cubeta (18) conectada de forma desmontable a un extremo proximal (16a) del conducto (16); un medio (14) para la aplicación de presión negativa en la zona de una herida; un filtro hidrófobo (20) colocado entre dicha cubeta (18) y dicho medio (14) para la aplicación de presión negativa; un medio para modificar dicha presión negativa durante un intervalo de tiempo, caracterizado porque el sistema incluye un filtro de vapor para el olor (23) colocado entre dicho filtro hidrófobo (20) y dicho medio (14) para la aplicación de presión negativa y porque el sistema incluye otro filtro hidrófobo (22) colocado entre el filtro hidrófobo (20) y dicho medio (14) para la aplicación de presión negativa.
Description
Esta invención trata en general sobre los sistemas de tratamiento de tejidos. Concretamente, esta invención hace referencia a los sistemas de tratamiento asistido por vacío empleados en la cicatrización de heridas abiertas.
El proceso de cicatrización de heridas abiertas por inducción de vacío ha ido ganando adeptos en los últimos años gracias a la empresa Kinetic Concepts, Inc. de San Antonio, Texas, con su línea de productos V.A.C.® disponible en el mercado. El proceso de cicatrización por inducción de vacío se ha descrito en la patente cedida comúnmente de los EE.UU nº 4.969.880 emitida el 13 de noviembre de 1990 a Zamierowski, así como sus adiciones y adiciones parciales, la patente de los EE.UU. nº 5.100.396, emitida el 31 de marzo de 1992, la patente de los EE.UU. nº 5.261.893, emitida el 16 de noviembre de 1993, y la patente de los EE.UU. nº 5.527.293, emitida el 18 de junio de 1996. Las modificaciones y mejoras posteriores del proceso de cicatrización por inducción de vacío también se recogen en la patente de los EE.UU. nº 6.071.267, emitida el 6 de junio de 2000 a Zamierowski y las patentes de los EE.UU. nº 5.636.643 y 5.645.081 emitidas a Argenta et al, el 10 de junio de 1997 y 8 de julio de 1997, respectivamente. Las mejoras adicionales también se describen en la patente de los EE.UU. nº 6.142.982, emitida el 13 de mayo de 1998 a Hunt, et al.
En la práctica, la aplicación de presión manométrica negativa en una herida, comercializada por el Cesionario o su matriz bajo la denominación de terapia de “cierre asistido por vacío" (conocida por sus siglas "V.A.C.®") normalmente implica la contracción de tipo mecánica de la herida, acompañada de la eliminación del líquido sobrante. De esta forma, la terapia V.A.C.® incrementa el proceso inflamatorio natural del cuerpo, a la vez que alivia muchos de los conocidos efectos secundarios intrínsecos, como la producción de edema debido a un mayor flujo sanguíneo por la falta de la estructura vascular necesaria para un retorno venoso adecuado. En consecuencia, la terapia V.A.C. ® ha resultado altamente exitosa en el cierre de heridas abiertas, cicatrizando muchas heridas para las que antes no existía tratamiento.
La frecuencia con la que se aplica la presión negativa en la herida, así como la frecuencia en el cambio de presión, influye directamente en el grado de cicatrización de la herida. Se cree que una variación según el momento en el cambio de presión, que hasta ahora los dispositivos para la terapia de cierre inducido por vacío no eran capaces de proporcionar, puede incrementar de forma significativa el grado de cicatrización de las heridas. Asimismo, una incorporación inmediata a su actividad habitual por parte del paciente sometido a este tipo de terapia también puede mejorar el grado de curación de las heridas, ya que el incremento de la actividad física a menudo va acompañado de una mayor circulación vascular, lo que implica un mayor flujo sanguíneo en la zona de la herida. Uno de los inconvenientes de la incorporación a la actividad habitual es la limitada duración de la batería, como consecuencia de la energía que necesitan los actuales sistemas de tratamiento asistido por vacío para la cicatrización de heridas. Además, la zona de la herida ha de examinarse con regularidad para cerciorarnos de que no se infecta. No obstante, una incorporación inmediata a la actividad habitual no debe impedirnos tomar las precauciones necesarias durante la terapia de tratamiento asistido por vacío con el fin de evitar que se derrame involuntariamente el líquido supurado por la herida contenido en la cubeta, o evitar la entrada en el mecanismo de bombeo del líquido supurado por la herida.
En la técnica anterior se plantean otras limitaciones relacionadas con el uso de bombas oscilantes de frecuencia constante. Dichas limitaciones vienen impuestas ante la necesidad de un tamaño de bomba determinado para poder mantener en la zona de la herida la presión negativa deseada y / o ante una menor duración de la batería ya que las bombas oscilantes requieren de esta energía para su funcionamiento. Las bombas oscilantes, tal y como sabemos por la técnica, están pensadas normalmente para operar bajo unas condiciones de funcionamiento limitadas. Por ejemplo, para aprovechar al máximo el caudal de baja presión a una frecuencia constante. Por lo general, la masa y/o dureza de diversos componentes se ve modificada para cambiar la frecuencia resonante de la bomba según las condiciones de funcionamiento del diseño. Si aumenta la presión a través de la bomba, la dureza del sistema se ve incrementada por la presión de retroceso que se produce a través del diafragma de la bomba oscilante. La frecuencia resonante de la bomba cambia y el accionamiento de frecuencia constante no hace funcionar la bomba a la frecuencia óptima. En consecuencia, el caudal baja drásticamente, mermándose la capacidad de la bomba para impulsar el aire a una presión elevada. Por ello, para poder proporcionar un mayor caudal a altas presiones es necesario sacrificar el caudal a bajas presiones, o disponer de una bomba que sea suficientemente grande cuando se utilice una bomba oscilante de frecuencia constante.
En el documento WO 01/37922 se divulga un sistema de acuerdo con el preámbulo de la reivindicación 1.
Según las razones descritas anteriormente, se necesita un sistema de tratamiento de heridas asistido por vacío que sea capaz de cambiar automáticamente la presión. Además, existe la necesidad de un sistema de tratamiento de heridas asistido por vacío más eficaz, capaz de ofrecer mayor movilidad al paciente, y de minimizar los riesgos de derramar el líquido supurado o contaminar la bomba.
De acuerdo con la presente invención se proporciona un sistema para estimular la
cicatrización de tejido, que comprende:
una almohadilla porosa;
un apósito estanco;
un medio para conectar un extremo distal de un conducto a través del apósito
una cubeta conectada de forma desmontable a un extremo proximal del conducto;
una bomba eléctrica para aplicar presión negativa a una zona de herida;
Un filtro hidrófobo clocado entre dicha cubeta y dicho medio para aplicar presión negativa; un medio para variar dicha presión negativa durante un intervalo de tiempo,
que se caracteriza porque el sistema incluye un filtro de vapor para el olor colocado entre dicho filtro hidrófobo y dicho medio para aplicar presión negativa, y
otro filtro hidrófobo hidrófobo colocado entre dicho filtro hidrófobo y dicho medio para la aplicación de presión negativa.
Preferentemente dicho medio para variar dicha presión negativa comprende medios para ajustar la presión real que cumpla una presión objetivo variable.
Convenientemente, dicha presión objetivo variable oscila entre una presión objetivo máxima y una presión objetivo mínima.
De forma ventajosa, la bomba eléctrica tiene una fuente de suministro eléctrico para alimentar la bomba eléctrica y el sistema incorpora un medico para manipular la fuente de suministro eléctrico.
Preferentemente, dicho medio para manipular la fuente de suministro eléctrico está compuesto por medios para evitar que la potencia eléctrica alcance un motor eléctrico de la bomba hasta que se haya generado suficiente potencia para activar dicho motor.
De forma conveniente, dicho suministro eléctrico para dicha bomba eléctrica comprende una unidad de potencia portátil.
De forma ventajosa, dicha bomba eléctrica es una bomba oscilante tiene un medio para maximizar el caudal de la bomba en un intervalo de presión previamente determinado.
Preferentemente, dicho medio para maximizar el caudal de la bomba comprende un medio para variar una frecuencia de accionamiento de un circuito accionador de la bomba oscilante.
Convenientemente, dicho medio para variar dicha frecuencia de accionamiento comprende:
un sensor de presión para medir la presión a través de dicha bomba;
un sistema de control para determinar la frecuencia de accionamiento óptima para dicha bomba con relación a la presión detectada por dicho sensor de presión; y
dicho circuito de accionamiento de frecuencia variable para accionar dicha bomba.
Por tanto, el objetivo de la presente invención es ofrecer un sistema de tratamiento de heridas asistido por vacío capaz de favorecer la estimulación del crecimiento celular a través de variaciones en el cambio de presión.
Uno de los objetivos preferentes es poder ofrecer un sistema que no interrumpa su funcionamiento en caso de que se produzca un corte en el suministro eléctrico de corriente alterna.
Por último, otro de los objetivos preferentes que se desea conseguir con la presente invención es ofrecer un dispositivo de terapia para el tratamiento de heridas asistido por vacío que pueda fijarse a un objeto con el fin de reducir las posibilidades de alterar el dispositivo, y que a la vez pueda colocarse en un lugar práctico donde funcione con normalidad.
De acuerdo con los anteriores objetivos, la forma de realización preferida de la presente invención comprende, en general, una almohadilla porosa para la inserción principalmente en la zona de la herida y un apósito que proporcione un cierre estanco de la almohadilla en la zona de la herida. Se conecta un extremo distal de un tubo al apósito para proporcionar presión negativa en la zona de la herida. Se prepara un orificio de muestreo del líquido en el tubo que permita extraer muestras del líquido supurado por la herida a través del tubo de la zona de la herida. Una fuente de presión negativa está en comunicación con un extremo proximal del tubo. Se conecta una cubeta desmontable al tubo dirigida a recoger los líquidos que supure la herida durante la aplicación de presión negativa. Un primer filtro se acopla en una apertura de la cubeta, mientras que un segundo filtro se coloca entre la cubeta y la fuente de presión negativa. Como la fuente de presión negativa puede ser una bomba eléctrica alimentada mediante corriente alterna o corriente continua, se incorpora un dispositivo de gestión de energía, con su correspondiente protocolo de gestión de energía, con el fin de maximizar la duración de la batería cuando la unidad recibe la alimentación de la corriente continua. Se utiliza un mecanismo de fijación para asegurar el sistema a un objeto estacionario, como por ejemplo una barandilla de la cama o un mástil como los utilizados para colgar las bolsas de líquidos intravenosos.
La almohadilla, consistente en una espuma con relativamente pocas celdillas abiertas en contacto con la zona en la que se necesita estimular el crecimiento celular, con el fin de evitar con ello adhesiones no deseadas, pero a la vez con las suficientes celdillas abiertas como para que la terapia de presión negativa y drenaje puedan seguir su curso sin verse afectados, se coloca en comunicación fluida con una fuente de vacío para estimular un drenaje fluido, tal y como sabemos por la técnica. La almohadilla porosa de la presente invención puede consistir en una espuma de alcohol polivinílico. La comunicación fluida puede establecerse al conectar un tubo a un apósito, como el que se describe en la Aplicación Internacional WO 99/13793, titulada "Surgical Drape and Suction Heads for Wound Treatment" [Apósitos Esterilizados y Cabezales de Succión para el Tratamiento de Heridas].
Una vez colocada la almohadilla se forma un cierre estanco en la zona de la herida que evita pérdidas del vacío. Dicho cierre estanco se consigue al colocar un apósito en la herida de tal forma que se adhiera a la piel sana alrededor de la zona de la herida, mientras al mismo tiempo se mantiene un cierre estanco en la propia herida.
Se coloca un conducto o tubo en comunicación fluida con la almohadilla de espuma, su extremo distal conectado a una cubeta para el drenaje de líquidos que esté en comunicación fluida con una fuente de vacío. La presión negativa varía según el momento, para una mayor estimulación del crecimiento celular, el cual a su vez puede reducir el proceso de cicatrización. La presión negativa inducida en la herida se ajusta para admitir una presión objetivo variable, la cual oscila entre una presión objetivo mínima y máxima.
El caudal de una bomba de desplazamiento variable se maximiza por encima de un intervalo de presiones al variar la frecuencia de accionamiento de la bomba. Un sistema se encarga de ajustar continuamente la frecuencia de accionamiento óptima, así como de monitorizar periódica y constantemente la presión de la bomba con el fin de determinar la frecuencia de accionamiento óptima para dicha presión. Por tanto, el rendimiento de la bomba mejora en comparación con las bombas de desplazamiento variable empleadas en la técnica anterior, sin tener que aumentar las dimensiones o peso de la bomba. Asimismo, el rendimiento de una bomba convencional de desplazamiento variable se puede conseguir con una bomba más pequeña, lo que a su vez reduce las dimensiones y el peso de todo el sistema con el fin de facilitar la utilización y portabilidad para el paciente. También puede utilizarse otra fuente de presión negativa alternativa, como una bomba de desplazamiento fijo, también conocida como bomba de desplazamiento positivo.
El sistema de gestión de energía se utiliza para maximizar la duración de la batería cuando el sistema recibe la alimentación de la corriente continua. El sistema de gestión de energía incluye la desactivación de la retroiluminación de un terminal de representación visual, o panel de control de una pantalla táctil de cristal líquido (LCD), una vez transcurrido un intervalo predeterminado. La duración de la batería se prolonga cuando el sistema de gestión de energía impide la entrada de energía eléctrica a un motor eléctrico hasta que el ajuste de la potencia fijado es lo suficientemente grande como para activar el motor. En tal caso, el motor se utiliza para proporcionar presión negativa mediante el accionamiento de una bomba eléctrica tal y como sabemos por la técnica.
Cuanto antecede da una idea general de algunos de los objetivos más importantes de la presente invención. Estos objetivos deben interpretarse como mera ilustración de algunas de las características y aplicaciones más destacadas de la invención. Es posible conseguir muchos otros resultados beneficiosos aplicando la invención divulgada de manera diferente o bien modificando la invención tal y como se describe más adelante. En consecuencia, pueden plantearse otros objetivos y entender mejor la invención al consultar la siguiente Descripción Detallada de la Invención, que incluye la realización preferente.

A pesar de que aquellas personas con conocimientos normales de la técnica reconocerán de inmediato muchas de las realizaciones alternativas, sobre todo a la luz de las ilustraciones incluidas en este documento, esta descripción detallada es un ejemplo de la realización preferente de la presente invención, cuyo alcance está limitado sólo por las reivindicaciones que se extraen.
La presente invención es un sistema asistido por vacío diseñado para estimular la cicatrización de los tejidos.
Si observamos en particular la Figura 1, veremos que en ella aparecen ilustrados los principales componentes de un sistema que funciona de acuerdo con la presente invención. La presente invención 10 incluye una almohadilla de espuma 11 diseñada para insertarse principalmente en la zona de la herida 12 y un apósito 13 para sellar herméticamente la almohadilla de espuma 11 en la zona de la herida 12. La almohadilla de espuma 11 puede consistir en un material polímero de celdillas abiertas hecho de alcohol polivinílico (PVA), u otro material similar con un tamaño de celdilla lo suficientemente grande como para facilitar la cicatrización de la herida. Lo recomendable es que la densidad sea superior a 38 poros por pulgada lineal. Una densidad de poro entre 40 y 50 poros por pulgada lineal sería la situación más recomendable. Aunque lo ideal es que la densidad sea superior a 45 poros por pulgada lineal. Dicha densidad de poro se traduce a un tamaño de poro de aproximadamente 400 micrones.
La incorporación de un agente indicador como violeta cristal, azul de metileno u otros agentes parecidos y ya conocidos en la técnica provoca un cambio de color en la espuma 11 ante la presencia de un agente bacteriano. Esto permite a un usuario o profesional sanitario determinar fácilmente y de inmediato si la zona de la herida 12 presenta alguna infección. Está previsto que el agente indicador también puede colocarse en el conducto 16 entre la zona de la herida 12 y la cubeta 18. Este tipo de configuración (no se muestra) permitiría comprobar fácilmente y de inmediato la presencia de bacterias contaminantes en la zona de la herida 12, sin alterar la herida, ya que se produciría un cambio de color casi de inmediato y la herida infectada con bacterias supuraría líquido el cual se extraería de la zona de la herida 12 y se transportaría por el conducto 16 durante la aplicación de presión negativa.
También se contempla que la almohadilla de espuma 11 pueda estar recubierta de un agente bacteriostático. La incorporación de ese agente serviría para limitar o reducir la densidad bacteriana en la zona de la herida 12. La almohadilla de espuma 11 puede impregnarse o empaparse con el agente antes de insertarse en la zona de la herida, al igual que durante un proceso de envasado con esterilización. Alternativamente, el agente puede inyectarse en la almohadilla de espuma 11 después de insertarse en la zona de la herida 12.
Una vez insertada en la zona de la herida 12 y sellada mediante el apósito para herida 13, la almohadilla de espuma 11 se coloca en comunicación fluida con una fuente de vacío 14 para estimular el drenaje de líquido y la cicatrización de la herida, tal y como saben aquellos con conocimientos normales de la técnica. La fuente de vacío 14 puede ser una bomba portátil que funcione con electricidad o una unidad de succión de pared, que es el método más frecuente en los centros de atención médica.
De acuerdo con la realización preferente de la presente invención, la almohadilla de espuma 11, el apósito para herida 13, y la fuente de vacío 14 se aplican según la técnica anterior, con excepción de las modificaciones que se describen en detalle aquí.
La almohadilla de espuma 11 consiste preferiblemente en unas espumas altamente reticuladas de poliuretano o poliéster con celdilla abiertas para que se puedan impregnar eficazmente los líquidos de la herida durante las tareas de succión. La almohadilla 11 se coloca preferiblemente en comunicación fluida, a través de un conducto 16 de plástico o material similar, con una cubeta 18 y una fuente de vacío 14. Un primer filtro de membrana hidrofóbico 20 se coloca entre la cubeta 18 y la fuente de vacío 14, para evitar que los líquidos supurados por la herida contaminen la fuente de vacío 14. El primer filtro de 20 también puede utilizarse como un sensor de llenado de la cubeta 18. En cuanto el líquido entra en contacto con el primer filtro 20, se envía una señal a la fuente de vacío 14, y ésta se cierra. Preferiblemente, el apósito para herida 13 consiste en un material elastomérico cubierto al menos periféricamente con un adhesivo sensible a la presión para la aplicación del sellado en la zona de la herida 12, de tal forma que se pueda mantener un cierre estanco en la zona de la herida 12. El conducto 16 puede colocarse en comunicación fluida con la espuma 11 a través de un apéndice 17, el cual puede adherirse al apósito 13.
De acuerdo con la presente invención, un segundo filtro hidrofóbico 22 se coloca entre el primer filtro 20 y la fuente de vacío 14. El segundo filtro 22 puede ser de gran ayuda cuando el primer filtro 20 se utiliza también como un sensor de llenado de la cubeta 18. En ese caso, el primer filtro 20 puede actuar como un sensor de llenado, mientras que el segundo filtro 22 impide que la fuente de vacío 14 se contamine más con los líquidos supurados. Esta separación de funciones en un dispositivo de seguridad y de control (o limitación) permite que cada dispositivo sea diseñado de forma independiente. Puede colocarse un filtro de emanaciones olorosas 23, como un filtro de carbón, entre el primer filtro de 20 y el segundo filtro 22, para contrarrestar los vapores malolientes emanados por los líquidos supurados. En una representación alternativa (no se muestra), el filtro de emanaciones olorosas 23 puede colocarse entre el segundo filtro hidrofóbico 23 y la fuente de vacío 14. Un segundo filtro de emanaciones olorosas 15 puede colocarse entre la fuente de vacío 14 y el orificio de escape exterior 25, para reducir aún más la salida de vapores malolientes del sistema actual. Otra representación permite la incorporación del primer filtro 20 y el segundo filtro 22 como partes integrantes de la cubeta 18 para garantizar que los filtros 20, 22, al menos uno de los cuales probablemente se contaminará durante el proceso normal, sean eliminados automáticamente con el fin de limitar la exposición del sistema a contaminantes que puedan quedar atrapados en los filtros 20 y 22.
También podrá utilizarse un medio para tomar muestras mediante un orificio de acceso resellable 24 desde el conducto 16. El orificio 24 se encuentra entre el extremo distal 16a del conducto 16 y el extremo distal 16b del conducto 16. El orificio 24, tal y como se describe más adelante en las Figuras 2a y 2b, se utiliza para poder extraer las muestras de líquidos de la zona de la herida 12. A pesar de que el orificio 24 aparece como un apéndice que sobresale del conducto 16, hay que entender que un orificio empotrado (no se muestra) puede hacer las mismas funciones. El orificio 24 incluye una membrana resellable 26 que aunque se perfore, por ejemplo con una aguja hipodérmica, la junta se mantiene estanca. Pueden utilizarse diversos materiales de tipo caucho, conocidos en la técnica anterior por mantener su estanqueidad si se perforaban.
El proceso mediante el cual se toman muestras de los líquidos de la herida, según la presente invención, consiste en perforar la membrana 26 con un tomamuestras de líquidos 28, tales como una aguja hipodérmica o jeringuilla. El tomamuestras 28, se inserta a través de la membrana 26 y del orificio 24, hasta que entra en contacto con líquidos de la herida que fluyen a través de la cavidad interior 30 del conducto 16. Tal y como se ilustra en la Figura 2b y describe en más detalle en la patente de los EE.UU. 6.142.982, emitida a Hunt, et al. el 13 de mayo de 1998, la cavidad interior 30 puede estar rodeada por una o más cavidades exteriores 31. Las cavidades exteriores 31 pueden utilizarse como conductos de detección de presión capaces de detectar variaciones de presión en la zona de la herida 12. En otra representación (no se muestra), la cavidad o cavidades exteriores 31 pueden actuar como conducto de la presión negativa, mientras la cavidad interior 30 puede utilizarse como conducto de detección de la presión. En la presente invención, el orificio de muestreo de líquidos 24, se comunica únicamente con la cavidad interior 30, a fin de no interferir con la detección de presión que puedan estar llevando a cabo las cavidades exteriores 31. En otra representación (no se muestra), en la que la cavidad exterior 31 se utiliza como el conducto de presión negativa, el orificio de muestreo de líquidos 24 se comunica con la cavidad exterior 31.
La fuente de vacío 14 puede consistir en una bomba portátil dentro de un alojamiento 32, tal y como se ilustra en las Figuras 3a y 3b. Puede incluir o incorporarse un tirador 33 en el alojamiento 32 para que el usuario pueda agarrarse fácilmente o bien para mover el alojamiento
32.
De acuerdo con la realización preferente de la presente invención, se proporciona un medio en forma de abrazadera 34 para fijar el alojamiento 32 a un objeto estacionario, como por ejemplo un mástil para colgar bolsas de líquidos intravenosos. La abrazadera 34, que puede ser una prensa de tornillo, tal y como sabemos por la técnica, es retráctil, de manera que cuando no se utiliza está en posición de almacenamiento dentro de un hueco 36 del alojamiento 32. Se facilita un mecanismo articulable 38 para que la abrazadera 34 se pueda extender hacia fuera desde el alojamiento 32, hasta un ángulo de 90 grados desde su posición de almacenamiento. Otra representación (no se muestra) permite que la abrazadera 34 se coloque en un ángulo de hasta 180 grados desde su posición de almacenamiento. Cuando la abrazadera 34 está completamente extendida, el mecanismo articulable 38 se bloquea en su posición, de forma que el alojamiento 32 queda suspendido de la abrazadera 34. Un dispositivo de sujeción 40, como un perno roscado, se introduce a través de un orificio 42 de la abrazadera 34, para que la abrazadera 34 pueda ajustarse firmemente a diversos objetos estacionarios de diferentes grosores.
En algunos casos, el dispositivo de sujeción 40, puede consistir en un tornillo o pasador accionado por muelle, capaz de ajustarse automáticamente a varios objetos de diferentes grosores en sección, tales como mástiles para colgar bolsas de líquidos intravenosos.
Preferentemente, el sistema de la presente invención también permite la gestión del suministro de energía hasta la fuente de vacío 14, para maximizar la duración de la batería cuando la presente invención utilice corriente continua como fuente de alimentación. En la realización preferente, tal y como se ilustra en el diagrama de flujos de la Figura 4a, un control por motor 44 determina si la presión real es inferior o igual a una presión objetivo 46. Si la presión real es inferior a la presión objetivo, se calcula la potencia provisional de accionamiento del motor necesaria para alcanzar la presión objetivo 48. Si la potencia provisional de accionamiento del motor necesaria para alcanzar la presión objetivo es mayor o igual a la pérdida de potencia 49, la potencia provisional de accionamiento del motor se aplica al motor 50. Si la presión real es superior a la presión objetivo, la potencia provisional de accionamiento del motor se reduce y se decide si se necesita potencia adicional para compensar la pérdida de potencia 52. Si se decide que la potencia provisional no es suficiente para poder compensar la pérdida de energía, no se suministra potencia provisional al motor 54. Si la potencia provisional es suficiente para compensar la pérdida de energía, se aplica la potencia provisional al motor 50. El control por motor 44 funciona como un sistema de bucle cerrado, de manera que la presión real se mide constantemente comparándola con la presión objetivo predeterminada. La ventaja de este sistema es que impide que se suministre alimentación al motor cuando no es necesario para mantener la presión objetivo especificada por la terapia VAC. Por tanto, se amplía la duración de la batería ya que la energía no se malgasta innecesariamente alimentando al motor cuando no es necesario.
La duración de la batería se prolonga, tal y como se ilustra en el diagrama de flujos de la Figura 4b, al proporcionar un medio, como un software integrado en un procesador de ordenador, que desconecta de forma automática la retroiluminación de la pantalla de visualización 19 de la presente invención 10 (según se muestra en la Figura 3b). La entrada de información del usuario 55, como la presión objetivo deseada o la duración de la terapia, activa 57 una retroiluminación de la pantalla de visualización 19 tal y como se muestra en la Figura 3b. La entrada del usuario 55 también puede efectuarse simplemente tocando la pantalla de visualización 19, que podrá activarse mediante una pantalla sensible a un toque o táctil tal y como sabemos por la técnica. La activación de una alarma 55 también puede activar 57 la retroiluminación de la pantalla 19. Una alarma puede activarse automáticamente si se detecta una fuga de aire en la zona de la herida
12. Dicha fuga puede hacerse evidente al detectarse una disminución o reducción de la presión en la zona de la herida 12. La retroiluminación permanece activa hasta que se decida que el intervalo de tiempo predeterminado ha concluido 58. Si el intervalo de tiempo no ha transcurrido, la retroiluminación permanece activa 57. Si el intervalo de tiempo ha transcurrido, la retroiluminación se apaga automáticamente 59, hasta el momento en que el usuario introduzca información adicional, o suene una alarma 55.
Si observamos la Figura 1, la duración de la batería se prolonga gracias a un sistema de accionamiento por bomba de frecuencia variable 80, cuando la bomba 14 es una bomba oscilante. El sistema de accionamiento por bomba 80 consiste en un sensor de presión 82, un sistema de control 84 y un circuito de accionamiento de frecuencia variable 86. En la realización preferente el sensor de presión de 82 mide la presión a través de la bomba, la cual es transmitida al sistema de control 84. El sistema de control 84 determina la frecuencia de accionamiento óptima de la bomba 14, en función de la presión medida y transmitida por el sensor de presión 82. La frecuencia de accionamiento óptima de la bomba 14 viene determinada por el sistema de control 84 de forma repetida o continuada. El sistema de control 84 ajusta el circuito de accionamiento de frecuencia variable 86 para accionar la bomba a una frecuencia óptima determinada por el sistema de control 84.
El uso de un sistema de accionamiento por bomba de frecuencia variable 80 permite maximizar la presión de la bomba 14. En las pruebas llevadas a cabo en bombas oscilantes, la presión máxima conseguida se duplicó sólo modificando la frecuencia de accionamiento en un 30%. Además, el sistema 80 maximiza el caudal por encima del intervalo de frecuencia prolongado. Como resultado, el rendimiento de la bomba 14 mejora considerablemente en comparación con bombas de los sistemas existentes de accionamiento de frecuencia constante sin necesidad de aumentar el tamaño o el peso de la bomba. Por consiguiente, la duración de la batería se prolonga, lo que permite al usuario una mayor movilidad al no tener que estar atado a una fuente de alimentación estacionaria. Alternativamente, se puede conseguir con una bomba más pequeña, un rendimiento similar al conseguido por las bombas con sistema de accionamiento de frecuencia constante de la técnica anterior. Como resultado, se mejora la movilidad de los pacientes, al facilitarse la portabilidad de la unidad.
El sistema de acuerdo con la presente invención también incrementa la estimulación del crecimiento celular mediante la oscilación de presión, tal y como se muestra en la Figura 5. Dicha oscilación de la presión se consigue gracias a una serie de algoritmos de un programa de software, utilizado conjuntamente con una unidad de procesamiento computerizada para el control de la función de la fuente de vacío o bomba. El programa se inicializa cuando un usuario, como por ejemplo un profesional sanitario, activa el modo de impulsos de la bomba 60. El usuario fija un valor máximo para la presión objetivo y un valor mínimo para la presión objetivo 62. Entonces el software inicializa la dirección de la presión a "aumentar" 63. El software entonces entra en un bucle de control de software. En este bucle de control, el software determina primero si la presión está aumentando 64.
Si la presión real aumenta en la prueba 64, entonces se decide si una presión objetivo variable sigue estando por debajo de la presión objetivo máxima 70. Si la presión objetivo variable sigue estando por debajo de la presión objetivo máxima, a continuación el software decide si la presión real se ha equiparado a (aumentado hasta) la presión objetivo ascendente 66. Si la presión real ha alcanzado la presión objetivo ascendente, el software incrementa la presión objetivo variable en un intervalo 68. De lo contrario, se abstiene de hacerlo hasta que la presión real se haya equiparado a la presión objetivo ascendente. Si la presión objetivo variable ha alcanzado la presión objetivo máxima en la prueba del bloque 70, el software ajusta la dirección de la presión a "disminuir" 69 y la presión objetivo variable comienza a moverse en la parte descendente de su ciclo oscilatorio.
El intervalo se puede medir en mmHg o cualquier otra unidad habitual de medición de la presión. Preferiblemente, la magnitud del intervalo se encuentra dentro del intervalo 1 -10 mmHg, de acuerdo con las preferencias del usuario.
Si la presión real disminuye en la prueba 64, entonces se decide si la presión objetivo variable sigue siendo mayor que la presión objetivo mínima 74. Si la presión objetivo variable sigue siendo mayor que la presión objetivo mínima, a continuación el software decide si la presión real ha alcanzado (disminuido hasta) la presión objetivo descendente 76. Si la presión real se ha equiparado a la presión objetivo descendente, el software disminuye la presión objetivo variable en un intervalo 72. De lo contrario se abstiene de hacerlo hasta que la presión real se haya equiparado a la presión objetivo descendente. Si la presión objetivo variable ha alcanzado la presión objetivo mínima en la prueba del bloque 74, el software ajusta la dirección de la presión a "aumentar" 73 y la presión objetivo variable comienza a moverse en la parte ascendente de su ciclo oscilatorio. Este proceso oscilatorio continúa hasta que el usuario deshabilita el modo de impulsos.
Si bien la invención se ha descrito en el presente documento en relación a determinadas representaciones de preferencia, nótese que dichas representaciones se han presentado sólo a modo de ejemplo, sin limitar el alcance de la invención. En consecuencia, el alcance de la
invención únicamente se limitará con arreglo a las siguientes reivindicaciones.
Claims (8)
1. Un sistema (10) para estimular la cicatrización de tejido, que comprende: una almohadilla porosa (11); un apósito estanco (13); un medio de conexión del extremo distal (16b) de un conducto (16) a través del apósito
(13); una cubeta (18) conectada de forma desmontable a un extremo proximal (16a) del
conducto (16); un medio (14) para la aplicación de presión negativa en la zona de una herida; un filtro hidrófobo (20) colocado entre dicha cubeta (18) y dicho medio (14) para la
aplicación de presión negativa;
un medio para modificar dicha presión negativa durante un intervalo de tiempo, caracterizado porque el sistema incluye un filtro de vapor para el olor (23) colocado entre dicho filtro hidrófobo (20) y dicho medio (14) para la aplicación de presión negativa y porque el sistema incluye otro filtro hidrófobo (22) colocado entre el filtro hidrófobo (20) y dicho medio (14) para la aplicación de presión negativa.
- 2.
- Un sistema (10) de acuerdo con la reivindicación 1, en el cual dicho medio para variar dicha presión negativa comprende un medio para ajustar la presión en curso para que cumpla una presión objetivo variable.

5 7. Un sistema (10) de acuerdo con una cualquiera de las reivindicaciones precedentes, en el cual dicha bomba eléctrica (14) es una bomba oscilante y el sistema tiene un medio para maximizar el caudal de la bomba por encima de un intervalo de presión predeterminado.
8. Un sistema (10) de acuerdo con la reivindicación 7, en el cual dicho medio para
10 maximizar el caudal de la bomba comprende un medio (80) para variar una frecuencia de accionamiento de un circuito accionador de la bomba oscilante (14).

Un sistema (10) de acuerdo con la reivindicación 8, en el cual dicho medio (80) para variar dicha frecuencia de accionamiento comprende: 15 un sensor de presión (82) para medir la presión a través de dicha bomba (14); un sistema de control (84) para determinar la frecuencia de accionamiento óptima para
dicha bomba con relación a la presión detectada por dicho sensor de presión (82); y dicho circuito de accionamiento de frecuencia variable (86) para accionar dicha bomba.
