Description
- Hintergrund der Erfindung
- Die vorliegende Erfindung betrifft im Allgemeinen das Gebiet der Herzklappenreparatur. Insbesondere betrifft die vorliegende Erfindung eine Vorrichtung für die Reduktion der myokardialen Wandspannung und der Reparatur einer Mitralklappeninsuffizienz.
- Die dilatierte Kardiomyopathie wird oftmals von einer Mitralklappeninsuffizienz begleitet. Es gibt mehrere Gründe für das Vorliegen einer Mitralklappeninsuffizienz, die im Zusammenhang mit einem dilatierten Herz steht. Zunächst erhöhen die Kammerdilatation und die damit im Zusammenhang stehenden hohen Wandspannungen den Durchmesser des Mitralklappenringes. Zusätzlich, wenn das Herz dilatiert wird, wird die Position der Papillarmuskeln verändert. Die Papillarmuskeln und die Chorda in einem dilatierten Herz haben sich in radialer Richtung von der Mitralklappe weg und nach unten bewegt. Diese erneute Anordnung des vaskulären Systems und die Vergrößerung des Ringes verhindern ein ordnungsgemäßes Schließen der Klappe.
- Gegenwärtig wird die Mitralklappeninsuffizienz durch Reparatur oder Austausch der Klappe behandelt. Chirurgische Verfahren, die zur Reparatur der Klappe verwendet oder eingesetzt werden, umfassen die posteriore Ring-Annuloplastik, bei dem ein C- oder D-förmiger Ring um das posteriore Blatt der Mitralklappe genäht und in den Ring gezogen wird, wodurch sein zuvor vergrößerter Durchmesser verringert wird. Ein weiteres Verfahren besteht darin, die anterioren und posterioren Mitralblättchen (Alfieri-Reparatur) anzunähern, indem ein Nähfaden durch die Mitte beider Blättchen gebracht wird. Dies verleiht der Klappe eine Achterform, wenn die Klappe geöffnet wird. Wird die Mitralklappe ausgetauscht, werden die ursprünglichen Blättchen entfernt und die Chorda getrennt. Eine künstliche Klappe besteht aus mechanischen oder Gewebeblättchen, die an Streben, welche an einem Metallstent angebracht sind, aufgehängt sind, und die an Ort und Stelle an dem Mitralring angenäht werden.
- Es ist behauptet worden, dass die Reparatur der Klappe gegenüber einem Austausch der Klappe bevorzugt wird, falls die Blättchen-Chorda-Papillarverbindungen aufrechterhalten werden können. Die Spannungen der Herzwand nehmen zu, falls das Chorda beim Austausch der Klappe getrennt wird. Es ist gezeigt worden, dass durch Trennen des Chorda eine 30%-ige Verringerung der Kammerfunktion erzielt werden kann. Der Mitralklappenaustausch zieht eine hohe Sterblichkeit bei sehr kranken Patienten nach sich, die an einer chronischen Herzinsuffizienz leiden.
- Die WO-A-95/06447 offenbart eine Vorrichtung zum Reparieren einer Herzklappe.
- Zusammenfassung der Erfindung
- Die vorliegende Erfindung betrifft eine Vorrichtung für die Mitralklappenreparatur. Die Mitralklappe ist im Allgemeinen bestimmt als ihre Blättchen oder Cuspis, aber in Wirklichkeit besteht sie tatsächlich aus der gesamten linken Ventrikelkammer. Durch Erzeugen einer verbesserten Kammergeometrie werden sowohl die Kammer als auch die Klappenfunktion verbessert. Die Vorrichtung der vorliegenden Erfindung und das Verfahren zur Klappenreparatur/Austausch kann die Behandlung von chronischer Herzinsuffizienz umfassen, indem die linke ventrikuläre Wandspannung reduziert wird.
- Die Klappenreparaturvorrichtung umfasst ein längliches Spannungselement mit einem ersten Ende und einem zweiten Ende. Der basale Anker ist an dem ersten Ende, und der sekundäre Anker ist an dem zweiten Ende angeordnet.
- Der basale Anker kann ein Polster und einen annuloplastischen Ring oder dergleichen umfassen. Alternativ kann eine künstliche Herzklappe als basaler Anker dienen.
- Die Spannungselemente können im Wesentlichen steif oder im Wesentlichen elastisch sein. Der sekundäre Anker kann eine hakenförmige Papillarmuskelgewebeschlaufe, einen schraubenförmigen Gewebeanker oder ein transmurales Ankerpolster umfassen.
- Ein Verfahren, bei dem die vorliegende Erfindung eingesetzt wird, sieht ein Spannungselement mit einem ersten Ende und einem zweiten Ende vor. Das Spannungselement besitzt einen basalen Anker an dem ersten Ende und einen sekundären Anker an dem zweiten Ende. Der basale Anker ist in unmittelbarer Nähe der Klappe derart verankert, dass das Spannungselement in der Kammer angeordnet ist. Der sekundäre Anker ist an einem Abschnitt des Herzens, der von dem basalen Anker beabstandet ist, verankert, so dass das Spannungselement unter Spannung gesetzt und die Geometrie der Kammer durch Einsetzen des Spannungselements verändert wird.
- Der basale Anker kann eine künstliche Herzklappe, einen annuloplastischen Ring oder dergleichen umfassen. Der sekundäre Anker kann an einem Papillarmuskel oder transmural verankert sein.
- Mehr als nur ein Spannungselement kann verwendet werden. Zusätzlich kann ein seitliches Spannungselement quer in der Kammer im Allgemeinen senkrecht zu den anderen Spannungselementen eingesetzt werden, um ferner die Geometrie des Herzens zu verändern, die Wandspannung zu verringern und die Leistungsfähigkeit der Kammer zu verbessern. Die Erfindung ist im Anspruch 1 bestimmt. Eine Ausführungsform, die sich im Widerspruch zu dem Gegenstand des Anspruchs 1 befindet, ist nicht Teil der Erfindung.

- Bezugnehmend nun auf die Zeichnungen, in denen gleiche Bezugszeichen auf ähnliche Elemente in sämtlichen Ansichten hinweisen, zeigt

Die Papillarmuskeln12 sind im Querschnitt gezeigt.2 ist ein vertikaler Querschnitt eines menschlichen Herzens10 . Eine Mitralklappe ist in der Nähe der Oberseite des linken Ventrikels10 angeordnet. Die Mitralklappe14 umfasst zwei Blättchen oder Cuspen16 . Die Chorda18 verläuft zwischen den Blättchen16 und den Papillarmuskeln12 . -
3 ist ein Querschnitt des Herzens10 , das gegenüber dem in1 gezeigten Herzen modifiziert ist, indem eine Klappenreparaturvorrichtung20 entsprechend der vorliegenden Erfindung, wie in4 gezeigt ist, eingesetzt ist.4 ist ein vertikaler Querschnitt des linken Ventrikels10 , dessen Geometrie durch die Vorrichtung20 modifiziert ist. Bei dieser Ausführungsform der Erfindung umfasst die Vorrichtung20 einen basalen Anker22 , beispielsweise einen anuloplastischen oder Nahtring, der in unmittelbarer Nähe des Ringes der Klappe14 angenäht ist. Längliche Spannungselemente24 erstrecken sich von dem basalen Anker22 . Jedes besitzt ein erstes Ende, das mit dem basalen Anker22 verbunden ist, und ein zweites Ende, das mit den Papillarmuskeln12 oder der Herzwand verankert ist. - Wie in den
3 und4 zu sehen ist, ist sowohl der seitliche Radius und die vertikale Abmessung des linken Ventrikels10 im Vergleich zu dem linken Ventrikel der1 und2 verringert worden, indem die Papillarmuskeln12 zur Klappe14 hin mit Hilfe der Spannungselemente24 gezogen werden. Diese Änderung der Geometrie verringert die Spannungen in der Herzwand und erhöht folglich die Kammerfunktion. Die Klappenfunktion ist ebenso verbessert, wie dies im Detail unter Bezugnahme auf die5 und6 erläutert wird. -
5 ist ein im Allgemeinen vertikaler Querschnitt einer insuffizienten Mitralklappe eines Herzens, das an einer chronischen Herzinsuffizienz leidet. In diesem Fall, da das insuffiziente Herz dilatiert worden ist, sind die Papillarmuskeln12 von der Mitralklappe14 weggezogen worden. Die Chordaverbindungen zwischen den Papillarmuskeln12 und der Klappe14 wiederum ziehen die Blättchen16 auseinander, so dass während des normalen Herzzyklus die Blättchen16 sich nicht vollständig schließen können. Auf diese Weise verbleibt eine Öffnung26 zwischen den Blättchen16 während des gesamten Herzzyklus. Die Öffnung26 bewirkt, dass Blut durchtritt, was die Kammereffizienz verringert. -
6 ist eine Ansicht der Mitralklappe14 der5 , die durch das Einsetzen einer Klappenreparaturvorrichtung20 , wie gezeigt, modifiziert worden ist. Der Nahtring22 wird in unmittelbarer Nähe des Ringes der Klappe14 angenäht, wie es denjenigen bekannt ist, die Nahtringe verwenden. Der Ring der Klappe14 kann hinsichtlich seiner Größe verringert werden, indem der Ring zu dem Nahtring hin durch Nähfäden, die zum Verbinden des Ringes22 mit der Klappe verwendet werden, gezogen werden. Das Ziehen des Ringes der Klappe14 zu dem Nahtring22 hin ist dabei behilflich, die Öffnung26 zu beseitigen. Das Spannungselement24 wird anschließend mit dem Papillarmuskel12 verankert, so dass der Papillarmuskel12 zur Klappe14 hin gezogen wird. Unabhängig davon, ob der Nahtring alleine ausreicht, die Öffnung26 zu beseitigen, sieht das Ziehen des Papillarmuskels12 zur Klappe14 hin einen zusätzlichen Spannungsabbau auf dem Blättchen16 vor, was wiederum das vollständige Schließen der Klappe14 fördert. Das Ziehen des Papillarmuskels12 zu14 hin verringert ebenso die Herzwandspannung und erhöht die Kammereffizienz, wie voranstehend beschrieben wurde. -
7 ist eine höchst vereinfachte Ansicht des linken Ventrikels10 und der in4 gezeigten Klappenreparaturvorrichtung20 . Es kann darauf hingewiesen werden, dass die Spannelemente24 von dem basalen Anker22 zu einem benachbarten Papillarmuskel12 verlaufen. Im Gegensatz dazu ist8 eine ähnliche Querschnittsansicht des linken Ventrikels10 , bei der allerdings eine Klappenreparaturvorrichtung120 derart eingesetzt ist, dass ihre Spannelemente124 zwischen einem basalen Anker122 und einem Papillarmuskel12 seitlich entgegengesetzt zu dem Punkt verlaufen, an dem das Spannelement124 mit dem basalen Anker122 verbunden ist. Diese Anordnung, im Gegensatz zu der in10 gezeigten, kann die seitliche Komponente der Spannkraft in den Spannelementen124 relativ zu der vertikalen Komponente zu dieser Zugkraft erhöhen. -
9 zeigt eine noch weitere Ausführungsform der Klappenreparaturvorrichtung entsprechend der vorliegenden Erfindung, die mit dem Bezugzeichen220 bezeichnet ist. Bei dieser Ausführungsform ist die Vorrichtung220 in dem linken Ventrikel10 auf eine Weise angeordnet, die ähnlich der in7 gezeigten Vorrichtung20 ist, indem nämlich die Spannelemente224 der Vorrichtung220 von einem basalen Anker222 zu einem benachbarten sekundären Ankerpunkt verlaufen. Der sekundäre Ankerpunkt wird durch die seitliche Verlängerung eines Spannelements225 quer über den linken Ventrikel10 gebildet. Das Spannelement225 wird transmural an der Herzwand an ihren gegenüberliegenden Enden durch Polster227 verankert. Die Spannelemente224 werden wiederum mit dem Spannelement225 verankert oder verbunden. - Das Spannelement
225 kann dazu verwendet werden, die Geometrie des linken Ventrikels10 weiter auf eine Weise zu ändern, wie es in der US-Patentanmeldung mit der Nummer 08/933456 offenbart ist, die den Titel trägt "HEART WALL TENSION REDUCTION APPARATUS AND METHOD", die am 18. September 1997 angemeldet wurde. -
10 zeigt eine noch weitere Ausführungsform einer Klappenreparaturvorrichtung entsprechend der vorliegenden Erfindung, die mit dem Bezugszeichen320 bezeichnet ist. Diese Ausführungsform umfasst einen basalen Anker322 und Spannelemente324 und ein seitliches Spannelement325 mit Ankerpolster327 , die ähnlich denen der Vorrichtung220 sind. In Bezug auf die Vorrichtung320 verlaufen die Spannelemente324 jedoch über Kreuz, und zwar ähnlich zu denjenigen der Vorrichtung120 der8 , um die horizontale Komponente relativ zu der vertikalen Komponente der Zugkraft in dem Spannelement324 zu erhöhen. -
11 ist eine noch weitere Ausführungsform420 der Klappenreparaturvorrichtung des vorliegenden Verfahrens. Die Klappenreparaturvorrichtung420 umfasst einen basalen Anker422 und Spannelemente424 . Die Spannelemente424 sind in einer Anordnung ähnlich der Spannelemente24 der Vorrichtung20 , die in7 gezeigt ist, angeordnet, außer dass die Spannelemente424 transmural mit Hilfe der Polster427 und nicht in die Papillarmuskeln12 verankert sind. Die relativ größere Dicke der Spannelemente424 , wie in11 gezeigt ist, und verglichen mit den Spannelementen24 , die in7 gezeigt sind, stellt lediglich dar, dass die Spannelemente im Wesentlichen steif oder im Falle der Spannelemente24 im Wesentlichen elastisch sein können. Es sollte klar sein, dass bei sämtlichen hier gezeigten Ausführungsformen die Spannelemente jedoch vorteilhafterweise derart gebildet sein können, dass sie im Wesentlichen elastisch oder im Wesentlichen steif sind. -
12 ist eine Draufsicht oder eine posteriore Ansicht der Klappe14 . Bei dieser Ausführungsform ist der basale Anker für die Klappenreparaturvorrichtung als einzelne Polster28 gezeigt, die an der posterioren Seite der Klappe14 angenäht sein können. Die Spannelemente24 sind so gezeigt, dass sie von den jeweiligen Polstern28 in den linken Ventrikel verlaufen. -
13 ist die gleiche Ansicht der Klappe14 wie die12 . In13 ist der basale Anker22 jedoch als ein mondförmiger Nahtring gezeigt. Die Spannelemente24 verlaufen von dem basalen Anker22 durch die Klappe14 in den linken Ventrikel. -
14 ist eine Seitenansicht einer künstlichen Herzklappe30 . Falls es notwendig ist, die Klappe auszutauschen und nicht zu reparieren, kann eine künstliche Klappe30 als ein basaler Anker für die Spannelemente24 verwendet werden. -
15 ist eine Draufsicht einer alternativen Ausführungsform eines Nahtringes als basaler Anker32 . Der Ring32 besitzt eine Sichelform und einen Pylon34 , der durch die Mitralklappe verläuft.16 ist eine Seitenansicht des Nahtringes32 , die die an dem Pylon34 angebrachten Spannelemente24 zeigt. - Die Spannelemente
24 verlaufen vorzugsweise durch das Gewebe der Klappe14 , und nicht durch die Klappenöffnung. Es ist zu erkennen, dass die Spannelemente24 jedoch durch die Klappenöffnung angeordnet sein können. Im Falle der Ausführungsform der15 und16 würde der Pylon34 jedoch durch die Klappenöffnung angeordnet sein. Die Spannelemente24 , die dem Pylon34 zugeordnet sind, würden an der gegenüberliegenden Seite der Klappe14 von dem Nahtring32 angeordnet sein. Der Pylon34 würde vorzugsweise durch die Klappenöffnung und nicht an der gewebebildenden Klappe14 angeordnet sein. - Die
17 und18 sind weitere, zusätzliche alternative Ausführungsformen von Nahtringen, die als basale Anker entsprechend der vorliegenden Erfindung verwendet werden können. Die Form der Ringe ist derart ausgewählt, dass die Nähfäden dazu verwendet werden können, um das Gewebe zur Innenseite des Ringes hin zu ziehen, da sie an die Klappe14 angenäht werden, und auf diese Weise wird die seitliche und/oder vertikale Querschnittsfläche der damit im Zusammenhang stehenden Herzkammer verringert. Dies verringert vorteilhafterweise die Herzwandspannung, was wiederum besonders nützlich ist, falls der Patient an einer Herzinsuffizienz leidet. - Es ist zu erkennen, dass die Spannelemente
24 an dem basalen Anker fest oder lösbar angebracht sein können. Vorzugsweise werden die Spannelemente fest an dem basalen Anker bei der Klappenreparatur angebracht. - Die
19 bis21 zeigen verschiedene Konfigurationen der Ankervorrichtungen, die an dem zweiten Ende des Spannelements24 gezeigt sind. Es ist zu erkennen, dass diese Ankervorrichtungen mit jedem der zuvor beschriebenen Spannelemente verwendet werden können. In19 umfasst das zweite Ende des Spannelements24 einen sekundären Anker40 , der als Schraube gebildet ist, und die in den Papillarmuskel12 eingeschraubt gezeigt ist.20 zeigt einen sekundären Anker42 einschließlich einer Schlaufe, die durch den Papillarmuskel12 angenäht ist.21 zeigt ein Spannelement24 , das in transmuraler Richtung zu einem äußeren Polster44 verläuft, mit dem es verbunden ist. Das Spannelement24 könnte an dem Polster44 angenäht oder ansonsten mechanisch mit diesem verbunden sein. - Es ist zu erkennen, dass verschiedene biokompatible Materialien vorteilhafterweise verwendet werden können, um die verschiedenen Komponenten der Vorrichtung der vorliegenden Erfindung zu bilden. Es ist abzusehen, dass die vorliegende Vorrichtung gewöhnlich dauerhaft implantiert wird. Beim Auswählen der Materialien für jede der Komponenten sollte auf. diese Weise auf die Folgen des lang anhaltenden Kontakts der Vorrichtung mit Gewebe und des Gewebes mit der Vorrichtung geachtet werden.
- Zahlreiche Eigenschaften und Vorteile der Erfindung, die in diesem Dokument enthalten sind, sind in der voranstehenden Beschreibung erörtert worden. Es ist verständlich, dass diese Offenbarung jedoch in vielerlei Hinsicht lediglich illustrativ ist. Veränderungen können in Bezug auf Details vorgenommen werden, besonders in Bezug auf Form, Größe und Anordnung der Teile, ohne dass der Bereich der Erfindung überschritten wird. Der Bereich der Erfindung ist natürlich in der Sprache, in der die beigefügten Ansprüche formuliert sind, bestimmt.
Claims (12)
- Vorrichtung zum Reparieren einer in situ Mitralklappe mit: einem länglichen Spannelement mit einem ersten Ende und einem zweiten Ende; einem ersten Anker an dem ersten Ende des länglichen Spannelements, der zur Verankerung benachbart der Klappe ausgebildet ist, so dass das längliche Spannelement in der Kammer angeordnet werden kann, und einem zweiten Anker an dem zweiten Ende des länglichen Spannelements, der zur Verankerung an einem Abschnitt des Herzens beabstandet von dem ersten Anker ausgebildet ist, so dass das längliche Spannelement unter Spannung steht und die Geometrie des Herzens durch Einsetzen des länglichen Spannelements verändert ist.
- Vorrichtung nach Anspruch 1, bei der der erste Anker eine Unterlage umfasst.
- Vorrichtung nach Anspruch 1, bei der der erste Anker einen sichelförmigen Ring umfasst.
- Vorrichtung nach Anspruch 1, bei der der erste Anker einen Ring umfasst, der zum Annähen an eine Klappe ausgebildet ist.
- Vorrichtung nach Anspruch 1, bei der der erste Anker einen Nähring mit veränderlichem Krümmungsradius umfasst.
- Vorrichtung nach Anspruch 1, bei der der erste Anker einen annuloplastischen Ring umfasst.
- Vorrichtung nach Anspruch 1, bei der das mindestens eine Spannelement ein Paar Spannelemente umfasst.
- Vorrichtung nach Anspruch 1, bei der das mindestens eine Spannelement im Wesentlichen starr ist.
- Vorrichtung nach Anspruch 1, bei der das mindestens eine Spannelement im Wesentlichen elastisch ist.
- Vorrichtung nach Anspruch 1, bei der der zweite Anker schraubenförmig ist.
- Vorrichtung nach Anspruch 1, bei der der zweite Anker einen schraubenförmigen Gewebeanker umfasst.
- Vorrichtung nach Anspruch 1, bei der der zweite Anker eine hakenförmige Papillarmuskel-Gewebeschlaufe umfasst.
