Description
- Die vorliegende Erfindung betrifft allgemein eine Einrichtung für die Behandlung und Entfernung bzw. Ablation von Körpermassen, wie z. B. Tumoren, und genauer gesagt eine zurückziehbare Nadelelektrodenvorrichtung mit mehreren Elektroden, die das Äußere eines Tumors mit einer Mehrzahl von Nadelelektroden umgibt und so ein Ablationsvolumen definiert.
- Derzeit angewandte offene Verfahren zur Behandlung von Tumoren wirken extrem zerstörerisch und verursachen großen Schaden an gesundem Gewebe. Während des chirurgischen Eingriffs muß der Arzt sorgfältig vorgehen, um den Tumor nicht in einer Weise anzuschneiden, die zu einem Streuen des Tumors und damit zu Metastasenbildung führt. In den letzten Jahren war die Entwicklung von Produkten verstärkt auf eine Minimierung des traumatischen Charakters herkömmlicher chirurgischer Eingriffe gerichtet.
- Besonders aktive Forschung wurde auf dem Gebiet der Hyperthermie als Hilfsmittel bei der Behandlung von Tumoren betrieben. Das Erhöhen der Temperatur von Tumoren ist bei der Behandlung und Handhabung von karzinomatösem Gewebe als hilfreich bekannt. Die Mechanismen der selektiven Vernichtung von Krebszellen durch Hyperthermie sind nicht vollumfänglich verstanden. Es wurden jedoch vier zelluläre Auswirkungen der Hyperthermie auf karzinomatöses Gewebe beobachtet: (i) Veränderungen in der Permeabilität oder der Fluidität der Zell- oder Zellkernmembran, (ii) lysomale cytoplasmatische Zersetzung, die zur Freisetzung von Verdauungsenzymen führt, (iii) thermische Schädigung von Proteinen, die sich auf die Zellatmung und die Synthese von DNA oder RNA auswirkt, und (iv) mögliche Anregung der Immunsysteme. Behandlungsverfahren mit Aufbringen von Wärme auf Tumore beinhalten die Verwendung von Direktkontakt-Hochfrequenz- (RF-) Applikatoren, Mikrowellenstrahlung, induktiv gekoppelten RF-Feldern, Ultraschall und einer Reihe einfacher Techniken der thermischen Leitung.
- Eines der Probleme bei der Anwendung all dieser Verfahren besteht in dem Erfordernis, daß stark lokalisierte Wärme in Tiefen von mehreren Zentimetern unterhalb der Körperoberfläche erzeugt werden muß. Bestimmte Techniken mit Mikrowellenstrahlung und Ultraschall sind entwickelt worden, um die Energie in verschiedenen gewünschten Tiefen zu fokussieren. RF-Anwendungen können während einer Operation in der Tiefe eingesetzt werden. Die örtliche Begrenzung ist jedoch im allgemeinen schlecht, was zur Schädigung von gesundem Gewebe führen kann.
- Auch die Induktionsheizung führt zu einer schlechten örtlichen Begrenzung der zugeführten Energie. Obwohl Induktionsheizung dadurch bereitgestellt werden kann, daß eine Antenne auf der Körperoberfläche plaziert wird, werden in unmittelbarer Nähe der Antenne oberflächliche Wirbelströme erzeugt. Wenn sie unter Verwendung von RF-Strom betrieben wird, kommt es zu einer unerwünschten Erwärmung der Oberfläche und damit zu einer verminderten Erwärmung des darunterliegenden Gewebes.
- Daher hatten nicht-invasive Verfahren zum Bereitstellen von Wärme an im Körper liegenden Tumoren Schwierigkeiten, eine bedeutende gezielte und selektive Behandlung unter Verwendung zu erreichen.
- Durch die Hyperthermie, welche von einer RF-Mikrowellenquelle erzeugt werden kann, wird Wärme auf Gewebe aufgebracht, wobei allerdings 45°C nicht überschritten werden, so daß normale Zellen überleben. Bei der Wärmebehandlung wird Wärmeenergie von mehr als 45°C eingesetzt, was zu histologischen Schäden, zu Austrocknung und der Denaturierung von Proteinen führt. In jüngster Zeit wurde die Hyperthermie zur Behandlung bösartiger Tumore eingesetzt. Bei Verwendung der Hyperthermie ist es wünschenswert, in einem bestimmten Gebiet einen durch interstitielle Stromheizung ortsbegrenzten Zustand der Überwärmung zu erzeugen und gleichzeitig eine nur minimale thermische Schädigung umliegenden gesunden Gewebes sicherzustellen. Oft befindet sich der Tumor unterhalb der Haut, und seine Behandlung erfordert entweder eine Operation, endoskopische Verfahren oder äußerliche Bestrahlung. Es ist schwierig, in tiefliegendem Körpergewebe eine Überwärmung von außen zu induzieren, weil die Stromdichte aufgrund der Absorption des Stroms durch gesundes Gewebe vermindert wird. Des weiteren wird ein Teil der RF-Energie an den Grenzflächen von Muskeln/Fett und Knochen reflektiert, was zu dem Problem des direkten Aufbringens einer bekannten Energiemenge an einem kleinen Tumor beiträgt.
- Versuche der Anwendung interstitieller örtlicher Überwärmung haben sich als nicht sehr erfolgreich erwiesen. Im Ergebnis wurden oft uneinheitliche Temperaturen in dem Tumor erzeugt. Es wird angenommen, daß die Reduzierung der Tumormasse durch Hyperthermie mit der Wärmedosis in Beziehung steht. Unter Wärmedosis versteht man die minimale wirksame Temperatur, die während einer festgelegten Zeitdauer auf die gesamte Tumormasse aufgebracht wird. Da der Blutfluß hauptverantwortlich für den Wärmeverlust von Tumoren ist, die erhitzt werden, und der Blutfluß innerhalb des Tumors variiert, wird eine gleichmäßigere Erhitzung von Tumorgewebe benötigt, um eine wirksamere Behandlung zu gewährleisten.
- Gleiches gilt für die Entfernung des Tumors selbst durch Verwendung von RF-Energie. Verschiedene Verfahren wurden für die RF-Ablation von Massen, wie z. B. Tumoren, eingesetzt. Statt den Tumor zu erwärmen, wird er durch das Anlegen von Energie entfernt. Dieses Verfahren gestaltet sich jedoch aufgrund einer Reihe von Faktoren als schwierig, einschließlich (i) der Positionierung von RF-Ablationselektroden zur wirksamen Entfernung der gesamten Masse, (ii) der Einführung der RF-Ablationselektroden an den Tumorort und (iii) des geregelten und überwachten Anlegens von RF-Energie zur Entfernung des Tumors, ohne das Gewebe außerhalb des Tumors zu schädigen.
- Es gibt eine Reihe verschiedener Behandlungsverfahren und -vorrichtungen zur minimal invasiven Behandlung von Tumoren. Ein Beispiel hierfür ist ein Endoskop, das RF-Hyperthermie in Tumoren erzeugt, wie in dem US-Patent Nr. 4,920,978 beschrieben wird. Eine Mikrowellenendoskopvorrichtung ist in dem US-Patent Nr. 4,409,993 beschrieben. In dem US-Patent Nr. 4,920,978 wird ein Endoskop für RF-Hyperthermie beschrieben.
- Wie in dem US-Patent Nr. 4,763,671 beschrieben, verwendet ein minimal invasives Verfahren zwei Katheter, die interstitiell in den Tumor eingeführt werden. Die Katheter werden innerhalb der Tumormasse plaziert, wobei jeder einzelne mit einer Hochfrequenzstromquelle verbunden ist.
- In dem US-Patent Nr. 4,565,200 wird ein Elektrodensystem beschrieben, bei dem eine einzelne Eingangsbereichskanüle verwendet wird, um eine Elektrode an einer ausgewählten Stelle im Körper einzuführen.
- Die in der WO-A-94-04220 beschriebene medizinische Sondenvorrichtung beinhaltet einen Katheter zur Einführung in eine Körperöffnung und hat ein Stilett-Führungsgehäuse mit einem oder mehreren Stilettanschlüssen in einer seiner Seitenwände und eine Stilettführung, um ein flexibles Stilett nach außen durch einen Stilettanschluß und durch dazwischenliegendes Gewebe mit einem vorbestimmten einstellbaren Winkel zu einem Zielgewebe zu führen. Der Katheteraufbau beinhaltet ein Stilett-Führungslumen, das mit dem Stilettanschluß in Verbindung steht, und ein in dem Stilett-Führungslumen angeordnetes Stilett für die Bewegung in Längsrichtung von dem Anschluß durch dazwischenliegendes Gewebe zu einem Zielgewebe. Das Stilett kann ein in einer nicht-leitfähigen Schicht eingeschlossener elektrischer Leiter sein, wobei der elektrische Leiter eine Hochfrequenzelektrode ist.
- Das deutsche Gebrauchsmuster DE-U-8909492 beschreibt eine interokulare Phacofragmentationsvorrichtung mit einem Paar von Greifarmen, die durch eine Hornhaut eingeführt werden können. Durch die Einwirkung von Hochfrequenzstrom über die Greifarme wird eine Denaturierung der Proteine im Linsenkern erzielt. Die Greifarme ermöglichen auch ein Zerquetschen des für eine Ultraschallfragmentierung zu harten Linsenkerns bei der Rückwärtsbewegung dieser Arme. Die Greifarme erstrecken sich von einer Führungshülse, die benachbart zur äußeren Oberfläche der Hornhaut gehalten wird.
- Gemäß Seite 1 der veröffentlichten deutschen Patentanmeldung DE-A-21 24 684 beschreibt dieses Dokument eine Einsetzelektrode zur biomedizinischen Verwendung, die eine Einführung größerer Gegenstände wie Elektroden oder Massageköpfe in Körpergewebe oder Körperöffnungen ermöglicht. Gemäß Seite 3 können die Elektroden beispielsweise aus Drähten, leitfähigen Gasen oder leitfähigen Flüssigkeiten gebildet sein. Gemäß Seite 4 können die Drähte als Röhren ausgebildet sein, so daß die Flüssigkeiten und Gase hier hindurchgeleitet werden können. Die einzigen Verwendungen der Elektroden, die auf Seite 3 erwähnt sind, sind die Verwendung als Antennen, als Heizung für die Kurzwellentherapie sowie die Kauterisierung, des weiteren als Operationsinstrumente, für die lonophorese oder Kaskadenionophorese, für die Zerstörung von Tumoren und schließlich für mechanische Vibratoren zum Lösen von Gewebe und der Zerstörung von Tumoren.
- Um einen Tumor wirksam behandeln zu können, müssen die Elektroden in Bezug auf den Tumor geeignet positioniert werden. Sobald die Elektroden positioniert wurden, ist eine kontrollierte Anwendung und Anlegung von RF-Energie zur Entfernung des Tumors wünschenswert. Dadurch wird die Zerstörung gesunden Gewebes reduziert.
- Dementsprechend besteht ein Ziel der vorliegenden Erfindung darin, eine RF-Gewebeablationsvorrichtung bereitzustellen, die eine gewünschte Stelle im Gewebe, z. B. einen Tumor, auf minimal invasive Weise entfernt.
- Gemäß der vorliegenden Erfindung wird eine Ablationsvorrichtung bereitgestellt, die eine längliche Zuführungseinrichtung einschließlich eines Lumens und einer Energiezuführungseinrichtung aufweist, wobei die Energiezuführungseinrichtung folgendes beinhaltet: eine Mehrzahl von RF-Elektroden, die jeweils einen gewebedurchdringenden distalen Abschnitt haben, und ein Glied für das Vorrücken der RF-Elektroden, das innerhalb eines Lumens positionierbar ist und mit den RF-Elektroden verbunden ist, wobei das Glied zum Vorrücken der RF-Elektroden derart konfiguriert ist, daß es die RF-Elektroden durch das Gewebe vorrückt, wobei die RF-Elektroden in einem zurückgezogenen Zustand in der länglichen Zuführungseinrichtung positioniert und derart betreibbar sind, daß sie sich von dem distalsten Ende der länglichen Zuführungsvorrichtung mit einer Krümmung entfalten, wenn sie von dem distalsten Ende der länglichen Zuführungseinrichtung zu einem entfalteten Zustand vorgerückt werden, und wobei die entfalteten Elektroden derart betreibbar sind, daß sie ein definiertes Ablationsvolumen erzeugen, das eine Gewebemasse umgibt, die fern von dem distalsten Ende der Zuführungseinrichtung lokalisiert ist. Bevorzugte Ausführungsformen werden in den abhängigen Ansprüchen beansprucht.
- Eine Ausführungsform der Erfindung kann eine RF-Gewebeablationsvorrichtung zur Verfügung stellen, die eine Mehrzahl von Elektroden aufweist, die sich aus einem Zuführungskatheter heraus- und wieder in ihn hineinschieben lassen. Die Elektroden sind in dem Zuführungskatheter zumindest teilweise in einem nicht-entfalteten Zustand angeordnet und dehnen sich in einen entfalteten Zustand aus, wenn sie aus einem distalen Ende des Zuführungskatheters herausbewegt werden und das Ablationsvolumen definieren.
- Eine Ausführungsform der Erfindung kann eine RF-Gewebeablationsvorrichtung mit entfalteten Elektroden bereitstellen, die einen ersten Abschnitt mit einem ersten Krümmungsradius und einen sich über den ersten Abschnitt hinaus erstreckenden zweiten Abschnitt mit einem zweiten Krümmungsradius oder einer im wesentlichen linearen Geometrie aufweisen.
- Eine weitere Ausführungsform der Erfindung kann eine RF-Gewebeablationsvorrichtung mit entfalteten Elektroden mit zwei oder mehreren Krümmungsradien bereitstellen, z. B. mit zumindest einem Krümmungsradius in zwei oder mehreren Ebenen.
- In einer Ausführungsform der Erfindung kann eine RF-Gewebeablationsvorrichtung mit zumindest einer entfalteten Elektrode einen in der Nähe eines distalen Endes des Zuführungskatheters angeordneten gekrümmten Abschnitt und einen sich über den gekrümmten Abschnitt der entfalteten Elektrode hinaus erstreckenden nicht-gekrümmten Abschnitt aufweisen.
- Alternativ oder zusätzlich dazu kann die Gewebeablationsvorrichtung eine entfaltete Elektrode mit zumindest einem gekrümmten Abschnitt in der Nähe eines distalen Endes des Zuführungskatheters und einem sich über den gekrümmten Abschnitt der entfalteten Elektrode hinaus erstreckenden nicht-gekrümmten Abschnitt aufweisen.
- In einer speziellen Ausführungsform der Erfindung beinhaltet die Gewebeablationsvorrichtung einen Zuführungskatheter mit einem distalen und einem proximalen Ende. Am proximalen Ende des Zuführungskatheters ist ein Griff angebracht. Dieser beinhaltet eine Mehrzahl von Elektroden, die sich aus dem distalen Ende des Katheters hinaus- und wieder hineinbewegen lassen. Die Elektroden befinden sich in einem nicht-entfalteten Zustand, wenn sie in den Zuführungskatheter eingesetzt werden. Sobald sie aus dem distalen Ende des Katheters herausbewegt werden, entfalten sie sich und definieren so ein Ablationsvolumen. Jede Elektrode beinhaltet einen ersten Abschnitt mit einem ersten Krümmungsradius und einen sich über den ersten Abschnitt hinaus erstreckenden zweiten Abschnitt mit einem zweiten Krümmungsradius oder einer im wesentlichen linearen Geometrie.
- Alternativ dazu kann jede entfaltete Elektrode mindestens zwei Krümmungsradien aufweisen, die gebildet werden, wenn die Nadel durch das distale Ende des Katheters vorgerückt und an einer ausgewählten Stelle im Gewebe positioniert wird.
- In einer weiteren Ausführungsform hat jede entfaltete Elektrode mindestens einen Krümmungsradius in zwei oder mehreren Ebenen. Des weiteren kann die Elektrodenenttaltungseinrichtung zumindest eine entfaltete Elektrode mit zumindest zwei Krümmungsradien und zumindest eine entfaltete Elektrode mit zumindest einem Krümmungsradius in zwei oder mehreren Ebenen beinhalten.
- In einer weiteren Ausführungsform beinhaltet die Elektrodenentfaltungseinrichtung zumindest eine entfaltete Elektrode mit zumindest einem gekrümmten Abschnitt in der Nähe des distalen Endes des Zuführungskatheters und einem sich über den gekrümmten Abschnitt der entfalteten Elektrode hinaus erstreckenden nicht-gekrümmten Abschnitt. Darüber hinaus weist die Elektrodenentfaltungseinrichtung zumindest eine entfaltete Elektrode mit zumindest zwei Krümmungsradien auf.
- In einer anderen Ausführungsform der Erfindung hat jede entfaltete Elektrode zumindest einen gekrümmten Abschnitt in der Nähe des distalen Endes des Zuführungskatheters und einen sich über den gekrümmten Abschnitt der entfalteten Elektrode hinaus erstreckenden nicht-gekrümmten Abschnitt.
- Am distalen Ende des Zuführungskatheters kann eine Elektrodenschablone positioniert sein. Sie unterstützt die Führung der Entfaltung der Elektroden in einer Umgebungsbeziehung zu der Außenseite einer ausgewählten Masse in einem Gewebe. Die Elektroden können hohl sein. Ein justierbarer Elektrodenisolator kann in einer benachbarten Umgebungsbeziehung zu allen oder einigen der Elektroden angeordnet sein. Der Elektrodenisolator ist einstellbar und kann entlang der Elektroden vorgerückt oder zurückgezogen werden, um eine leitfähige Elektrodenoberfläche zu definieren.
- Die Elektrodenentfaltungseinrichtung kann einen Nocken aufweisen, der die Elektroden aus dem distalen Ende des Zuführungskatheters vorrückt und sie wieder dorthin zurückzieht. Optional können in dem Zuführungskatheter eines oder mehrere Führungsrohre in Verbindung mit einer oder mehreren Elektroden vorgesehen sein. Die Führungsrohre sind am distalen Ende des Zuführungskatheters positioniert.
- Quellen von Infusionsmedien, die elektrolytische und chemotherapeutische Lösungen beinhalten, aber nicht auf diese beschränkt sind, können mit den hohlen Elektroden verbunden sein.
- Die Elektroden können angespitzte, spitz zulaufende Endabschnitte haben, die die Einführung der Elektroden in das Gewebe und das Vorrücken zu der ausgewählten Stelle in dem Gewebe erleichtern.
- Die Elektrodenenttaltungseinrichtung ist von dem Zuführungskatheter abnehmbar. Anfänglich ist ein Absperrorgan in dem Zuführungskatheter positioniert. Er kann ein angespitztes distales Ende aufweisen. Der Zuführungskatheter kann mit dem darin angeordneten Absperrorgan perkutan zu einem inneren Organ oder einer Stelle im Körper bewegt werden. Sobald der Katheter positioniert wurde, wird das Absperrorgan entfernt, und die Elektrodenentfaltungseinrichtung wird in den Zuführungskatheter eingesetzt. Die Elektroden befinden sich beim Einsetzen in den Zuführungskatheter in einem nicht-entfalteten Zustand und sind vorzugsweise gedrängt oder gefaltet. Die Elektroden sind aus einem Material ausreichender Stärke gefertigt, so daß sie sich bei ihrem Austritt aus dem distalen Ende des Zuführungskatheters in einer seitlichen Richtung weg von diesem dreidimensional entfalten. Die Elektroden setzen ihre seitliche Bewegung fort, bis der Widerstand des Gewebes die Nadeln zu einer Richtungsänderung zwingt.
- Jede Elektrode kann nun entweder beinhalten: (i) einen ersten Abschnitt mit einem ersten Krümmungsradius und einen sich über den ersten Abschnitt hinaus erstreckenden zweiten Abschnitt mit einem zweiten Krümmungsradius oder einer im wesentlichen linearen Geometrie, (ii) zwei Krümmungsradien, (iii) einen Krümmungsradius in zwei oder mehreren Ebenen oder (iv) eine Kombination aus zwei Krümmungsradien, von denen sich einer in zwei oder mehreren Ebenen erstreckt. Zusätzlich kann die Elektrodenenttaltungseinrichtung eine oder mehrere solcher entfalteten Geometrien/Strukturen für die verschiedenen Elektroden aus der Mehrzahl von Elektroden umfassen. Die Elektroden müssen dabei jeweils nicht notwendigerweise die gleiche entfaltete Geometrie aufweisen.
- Nachdem die Elektroden um eine Masse, wie z. B. einen Tumor, herum positioniert wurden, kann eine Reihe von Lösungen, einschließlich, aber nicht beschränkt auf elektrolytische Lösungen, in einem vor der Ablation durchgeführten Schritt durch die Elektroden zu der Masse geleitet werden. RF-Energie wird angelegt, und die Masse wird ausgetrocknet bzw. verbrannt. In einem nach der Ablation stattfindenden Schritt kann ein chemotherapeutischer Wirkstoff zu der betreffenden Stelle geleitet werden, und die Elektroden werden anschließend in den Zuführungskatheter zurückgezogen. Die gesamte Ablationsvorrichtung kann nun entfernt werden, oder es lassen sich weitere Ablationsbehandlungen durchführen.

10 gemäß der vorliegenden Erfindung ist in1 gezeigt. Die Ablationsvorrichtung10 beinhaltet einen Zuführungskatheter12 mit einem proximalen Ende14 und einem distalen Ende16 , wie er Fachleuten auf dem Gebiet gut bekannt ist. Der Zuführungskatheter kann eine Größe von etwa 5 bis 16 F haben. Ein Griff18 ist lösbar an dem proximalen Ende14 angebracht. Eine Elektrodenenttaltungseinrichtung ist zumindest teilweise in dem Zuführungskatheter12 positioniert und beinhaltet eine Mehrzahl von Elektroden20 , die sich aus dem distalen Ende16 heraus- und wieder in dieses hineinbewegen lassen. Die Elektroden20 können verschiedene Größen, Umrisse und Ausgestaltungen aufweisen. - In einer Ausführungsform sind die Elektroden Nadelelektroden mit einer Größe im Bereich von 27 bis 14 Gauge. Die Elektroden
20 befinden sich in einem nicht-entfalteten Zustand, solange sie im Zuführungskatheter zurückgehalten werden. In dem nicht-entfalteten Zustand können die Elektroden20 in einem gedrängten Zustand, federbelastet, allgemein begrenzt oder im wesentlichen gerade sein, wenn sie aus einem geeigneten erinnerungsfähigen Metall wie Nitinol gefertigt sind. Wenn die Elektroden20 aus dem distalen Ende16 herausbewegt werden, dehnen sie sich in einen entfalteten Zustand aus, der ein Ablationsvolumen definiert, aus welchem Gewebe entfernt wird, wie mehr im Detail in2 gezeigt ist. Die Elektroden20 arbeiten entweder im bipolaren oder im monopolaren Modus. Werden die Elektroden im bipolaren Modus verwendet, so wird das Ablationsvolumen im wesentlichen durch die Außenflächen der Mehrzahl von Elektroden20 definiert. In einer Ausführungsform beträgt der Querschnitt des Ablationsvolumens etwa 4 cm. Es versteht sich jedoch, daß mit der Gewebeablationsvorrichtung10 unterschiedliche Ablationsvolumina bestimmt werden können. - Das Ablationsvolumen wird zunächst derart bestimmt, daß es eine zu entfernende Masse, wie z. B. einen Tumor, definiert. Die Elektroden
20 werden für die volumetrische Ablation nach einem vorbestimmten Schema in Umgebungsbeziehung zu einer Masse oder einem Tumor plaziert. Ein Abbildungssystem wird verwendet, um zunächst das Volumen des Tumors oder der ausgewählten Masse zu bestimmen. Geeignete Abbildungssysteme beinhalten, sind aber nicht beschränkt auf Ultraschall, Computertomographie- (CT-) Abtastung, Röntgenfilm, Röntgendurchleuchtung, Kernspinresonanztomographie, elektromagnetische Abbildung usw. Die Verwendung solcher Einrichtungen zur Bestimmung des Volumens einer Gewebemasse oder eines Tumors ist Fachleuten auf dem Gebiet gut bekannt. - Bei der Verwendung von Ultraschall überträgt ein Ultraschalltransducer Ultraschallenergie in eine interessierende Körperregion eines Patienten. Die Ultraschallenergie wird von verschiedenen Organen und verschiedenen Gewebearten reflektiert. Die reflektierte Energie wird von dem Trans ducer erfaßt, und das resultierende elektrische Signal wird verarbeitet, um eine Abbildung der interessierenden Körperregion zu erzeugen. Auf diese Weise wird das Ablationsvolumen festgelegt, und das geeignete Elektrodenentfaltungsgerät wird in den Zuführungskatheter
12 eingesetzt. - Das Ablationsvolumen wird im wesentlichen bestimmt, bevor die Ablationsvorrichtung
10 an eine durch Ablation zu behandelnde Stelle bewegt wird. Dies unterstützt die geeignete Positionierung der Ablationsvorrichtung10 . Auf diese Weise wird die Menge des entfernten Gewebes reduziert und auf eine definierte Masse oder einen Tumor einschließlich des einen solchen Tumor umgebenden Gebiets begrenzt, das gut gesteuert und definiert wird. Ein kleines Gebiet um den Tumor herum wird entfernt, um eine vollständige Entfernung des Tumors sicherzustellen. - Wieder gemäß
2 befinden sich die Elektrodenabschnitte20(a) in einem entfalteten Zustand, wenn sie aus dem distalen Ende16 herausbewegt werden. Obwohl die Elektroden20 im allgemeinen in keiner bauchigen Konfiguration sind, während sie im nicht-entfalteten Zustand im Zuführungskatheter12 positioniert sind, können sie ebenso bauchig konfiguriert sein. Im allgemeinen befinden sich die Elektrodenabschnitte20(b) in einer zurückgezogenen Position, solange sie nicht entfaltet sind. Dies wird durch eine Vielzahl von Verfahren erreicht, die folgendes einschließen, aber nicht hierauf beschränkt sind: (i) Die Elektroden sind im Zuführungskatheter12 vorgefedert und eingeschränkt und federn erst (dehnen sich aus), wenn sie den Zuführungskatheter12 verlassen, (ii) die Elektroden sind aus einem erinnerungsfähigen Metall (Metall mit Memory-Effekt) hergestellt, wie unten mehr im Detail beschrieben wird, (iii) die Elektroden bestehen aus einem auswählbaren Elektrodenmaterial, das ihnen außerhalb des Zuführungskatheters12 eine erweiterte Form gibt, oder (iv) der Zuführungskatheter12 beinhaltet Führungsrohre, die der Begrenzung der Elektroden20 in dem Zuführungskatheter12 dienen und außerhalb des Katheters ihre Bewegungsrichtung steuern, um das gewünschte ausgedehnte Ablationsvolumen zu bilden. Wie in2 gezeigt, sind die Elektroden20 vorgefedert, solange sie im Zuführungskatheter12 zurückgehalten werden. Sobald sie aus dem Zuführungskatheter12 heraus und durch das Gewebe bewegt werden, federn die Elektroden20 und beginnen sich vom distalen Ende16 "aufzufächern", wobei sie sich in einer seitlichen Richtung relativ zu der Längsachse des Zuführungskatheters12 bewegen. Während die federnden Elektroden20 weiter vorrücken, vergrößert sich die Fläche des Fächers und erstreckt sich bis über den Durchmesser des distalen Endes16 hinaus. - Bezeichnenderweise ist jede Elektrode
20 in einem entfalteten Zustand ausgedehnt, und die entfalteten Elektroden20 definieren gemeinsam ein zu entfernendes Gewebevolumen. Soll ein entweder bösartiger oder gutartiger Tumor entfernt werden, so ist es, wie bereits erwähnt, bevorzugt, eine Menge an Gewebe zu entfernen, welche die durch die äußere Oberfläche des Tumors definierte Gewebemenge leicht überschreitet. Dies erhöht die Chancen auf vollständige Entfernung des Tumors. - Die entfalteten Elektroden
20 können viele verschiedene Entfaltungsgeometrien aufweisen, die einschließen, aber nicht beschränkt sind auf (i) einen ersten Abschnitt mit einem ersten Krümmungsradius und einen sich über den ersten Abschnitt hinaus erstreckenden zweiten Abschnitt mit einem zweiten Krümmungsabschnitt oder einer im wesentlichen linearen Geometrie, (ii) zumindest zwei Krümmungsradien, (iii) zumindest einen Krümmungsradius in zwei oder mehreren Ebenen, (iv) einen gekrümmten Abschnitt mit einer Biegung in der Nähe des distalen Endes16 des Zuführungskatheters und einen sich über den gekrümmten Abschnitt hinaus erstreckenden nicht-gekrümmten Abschnitt oder (v) einen gekrümmten Abschnitt in der Nähe des distalen Endes16 , einen ersten geraden Abschnitt und schließlich einen weiteren gekrümmten Abschnitt oder einen zweiten geraden Abschnitt, der gegenüber dem ersten geraden Abschnitt winklig ausgerichtet ist. Die entfalteten Elektroden20 müssen nicht parallel zueinander sein. Die Mehrzahl entfalteter Elektroden20 , die einen Teil der Nadelelektrodenentfaltungseinrichtung darstellen, können entweder alle dieselbe entfaltete Geometrie haben, d. h. Geometrien mit zumindest zwei Krümmungsradien, oder sie können eine Mehrzahl von Geometrien haben, d. h. eine Geometrie mit zwei Krümmungsradien, eine zweite Geometrie mit einem Krümmungsradius in zwei Ebenen, und die anderen können mit einem gekrümmten Abschnitt in der Nähe des distalen Endes16 des Zuführungskatheters12 und einem sich über den gekrümmten Abschnitt hinaus erstreckenden nicht-gekrümmten Abschnitt ausgestaltet sein. - Ein Nocken
22 oder eine andere Antriebsvorrichtung kann innerhalb des Zuführungskatheters12 positioniert sein und dazu verwendet werden, die Elektroden20 aus dem Zuführungskatheter heraus- und wieder in diesen hineinzuschieben. Die tatsächliche Bewegung des Nockens kann über den Griff18 gesteuert werden. Geeignete Nocken sind von konventioneller Ausgestaltung, wie sie Fachleuten auf dem Gebiet gut bekannt ist. - Die verschiedenen geometrischen Ausgestaltungen der Elektroden
20 sind in den3 bis6 dargestellt. In3 hat die Elektrode20 einen ersten Krümmungsradius20 und einen (c)zweiten Krümmungsradius20(d) . Sie kann mehr als zwei Krümmungsradien aufweisen. Wie in4 gezeigt, hat die Elektrode20 zumindest einen Krümmungsradius, der sich in drei Ebenen erstreckt. In5 weist jede Elektrode einen ersten gekrümmten Abschnitt20(e) in der Nähe des distalen Endes16 des Zuführungskatheters12 auf. Ein erster, im allgemeinen gerader Abschnitt20(f) erstreckt sich über den gekrümmten Abschnitt20(e) hinaus, und beide treffen sich an einer Krümmung20(g) . Die Elektroden20 können als Anoden und Kathoden dienen. Die Mehrzahl von Elektroden20 kann gerade Abschnitte20(f) aufweisen, die im allgemeinen parallel zueinander sind, aber auch nicht-parallel sein können.6 zeigt eine Elektrode20 mit einem in der Nähe des distalen Endes16 des Zuführungskatheters12 angeordneten ersten gekrümmten Abschnitt20(e) , einem ersten geraden Abschnitt20(f) und einem sich über den ersten geraden Abschnitt20(f) hinaus erstreckenden zweiten geraden Abschnitt20(h) . Der Abschnitt20(h) kann gerade, gekrümmt oder eine Kombination hieraus sein. Die in6 gezeigte Mehrzahl von Elektroden20 kann parallele oder nicht-parallele erste gerade Abschnitte20(f) aufweisen. - In einer Ausführungsform der Erfindung sind die Elektroden
20 in ihren nicht entfalteten Positionen federnd vorgespannt und gedrängt. Wenn die Elektroden20 aus dem distalen Ende16 des Zuführungskatheters12 herausbewegt werden, entfalten sie sich und fächern auf. Die Elektroden behalten diese Auffächerungsrichtung bei, bis der Widerstand des Gewebes größer ist als die Stärke des die Elektrode20 bildenden Materials. Dies bringt die Elektrode20 dazu, sich zu biegen und sich in eine relativ zu ihrer anfänglich nach außen gerichteten Auffächerungsrichtung nach innen zu bewegen. Die Biegung erzeugt die in3 gezeigten gekrümmten Abschnitte20(c) und20(d) und kann auch zur Bildung der anderen Geometrien der Elektrode20 führen, wie sie in den4 ,5 und6 dargestellt sind. Das Ausmaß der fächerartigen Ausdehnung der Elektrode20 ist abhängig von der Stärke des Materials, aus dem sie gebildet ist. Geeignete Elektrodenmaterialien beinhalten rostfreien Stahl, Platin, Gold, Silber, Kupfer und andere elektromagnetisch leitfähige Materialien einschließlich leitfähiger Polymere. Vorzugsweise besteht die Elektrode20 aus rostfreiem Stahl oder Nickeltitan und hat Abmessungen von etwa 27 bis 14 Gauge. - In einer Ausführungsform besteht die Elektrode
20 aus einem erinnerungsfähigen Metall, wie z. B. Nickeltitan, wie es von Raychem Corporation, Menlo Park, Kalifornien, kommerziell erhältlich ist. Zusätzlich kann ein Widerstandsheizelement in einem inneren Lumen der Elektrode20 positioniert sein. Das Widerstandsheizelement kann aus einem geeigneten Metall hergestellt sein, das Wärme an die Elektrode20 überträgt, was zu einer Ablenkung der entfalteten Elektrode20 führt, wenn die Temperatur der Elektrode20 ein Niveau erreicht, bei dem das Elektrodenmaterial, z. B. ein erinnerungsfähiges Metall, abgelenkt wird, wie aus dem Stand der Technik bekannt ist. Die Elektrode20 muß nicht komplett aus einem erinnerungsfähigen Metall bestehen. Es ist möglich, daß nur der in das Gewebe eingeführte distale Endabschnitt der Elektrode20 aus einem erinnerungsfähigen Metall gefertigt ist, um die gewünschte entfaltete geometrische Ausgestaltung zu bewirken. Weiterhin können mechanische Vorrichtungen, einschließlich, aber nicht beschränkt auf Führungsdrähte, an dem distalen Ende der Elektrode20 angebracht sein, um diese zu führen, zu biegen und sie in einer gewünschten Richtung durch das Gewebe zu bewegen, bis sie schließlich ihre Idealstellung zur Entfernung einer Gewebemasse erreicht hat. - Optional sind eines oder mehrere Führungsrohre
24 in dem Zuführungskatheter beinhaltet (7 ), die dazu dienen, die fächerartige Ausdehnung der Elektroden20 zu steuern, wenn sie aus dem distalen Ende16 des Zuführungskatheters12 herausbewegt werden. Die Führungsrohre24 können aus rostfreiem Stahl, Federstahl und Thermoplasten bestehen, die Nylon und Polyester einschließen, aber nicht hierauf beschränkt sind, und sind von ausreichender Größe und Länge, um die Elektroden zu einer bestimmten Stelle im Körper zu bringen. -
8 stellt eine Ausführungsform der Elektrode20 mit einem angespitzten distalen Ende24 dar. Das Vorsehen eines spitzen oder dornartigen Endes24 erleichtert das Vorrücken der Elektrode24 durch das Gewebe. Die Elektrode20 kann in Abschnitte unterteilt sein und eine Mehrzahl von gleichmäßig um die gesamte Elektrode20 oder nur um Teile herum angeordneten Fluidverteilungsanschlüssen26 aufweisen. Die Fluidverteilungsanschlüsse26 werden in der Elektrode20 gebildet, wenn diese hohl ist, und erlauben die Einführung und den Durchfluß flüssiger Medien durch die Elektrode20 bis zu einem gewünschten Gewebeort. Solche flüssigen Medien umfassen elektrolytische Lösungen, Pasten oder Gels sowie chemotherapeutische Wirkstoffe, sind aber nicht auf diese beschränkt. Beispiele geeigneter leitfähiger Gels sind aus wäßrigen Elektrolytlösungen, wie physiologischer Kochsalzlösung und dergleichen, hergestellte Carboxymethylzellulosegels. - Die Größe der Fluidverteilungsanschlüsse
26 kann abhängig von der Größe und der Form der Elektrode20 variieren. Mit der Elektrode20 ist weiterhin eine justierbare, an einer äußeren Oberfläche der Elektrode20 verschiebbar angeordnete Isolatorhülse28 verbunden. Die Isolatorhülse28 wird entlang der Elektrode20 vorgerückt und zurückgezogen, um die Größe einer leitfähigen Oberfläche der Elektrode20 zu definieren. Die Isolatorhülse wird durch den Arzt über den Griff18 betätigt und ist steuerbar an der Elektrode20 positioniert. Wenn die Elektrode20 aus dem Zuführungskatheter12 heraus- und in Gewebe hineinbewegt wird, kann die Isolatorhülse28 um die Elektrode20 herum angeordnet sein, während diese sich durch das Gewebe bewegt. Alternativ dazu kann die Isolatorhülse28 entlang eines gewünschten Abschnitts der Elektrode20 vorgeschoben werden, nachdem die Elektrode20 um eine zu entfernende Zielmasse herum angeordnet wurde. Die Isolatorhülse kann sich daher gemeinsam mit der Elektrode20 durch das Gewebe bewegen, oder sie kann sich ohne die Elektrode20 als Bewegungsquelle durch das Gewebe bewegen. Somit wird das gewünschte Ablationsvolumen sowohl durch die entfalteten Elektroden20 als auch durch die Anordnung einer Isolatorhülse28 an jeder Elektrode definiert. Auf diese Weise wird ein sehr präzises Ablationsvolumen festgelegt. Geeignete Materialien zur Bildung der Isolatorhülse umfassen Nylon, Polyimide, andere Thermoplaste und dergleichen, sind aber nicht hierauf beschränkt. -
9 veranschaulicht eine perkutane Anwendung der Gewebeablationsvorrichtung10 . Die Gewebeablationsvorrichtung10 kann verwendet werden, um die Elektroden20 perkutan zu der ausgewählten Masse oder dem Tumor zu führen. Die Elektroden20 können in ihrem nicht-entfalteten Zustand verbleiben, während sie perkutan in den Körper eingeführt und zu einem die ausgewählte, zu entfernende Masse beinhaltenden ausgewählten Organ bewegt werden. Der Zuführungskatheter12 ist von dem Griff18 lösbar. Wenn dieser entfernt wird, kann die Elektrodenentfaltungseinrichtung (die Mehrzahl von Elektroden20 ) in den Zuführungskatheter12 eingeführt und aus diesem wieder entfernt werden. Ein Absperrorgan30 wird anfänglich in den Zuführungskatheter12 eingesetzt, wenn das Verfahren perkutan durchgeführt werden soll. Wie in10 gezeigt, kann das Absperrorgan ein spitzes distales Ende32 haben, welches das Gewebe durchsticht und so die Einführung des Zuführungskatheters12 an einen ausgewählten Gewebeort unterstützt. Der ausgewählte Gewebeort kann ein Körperorgan mit einem Tumor oder einer anderen Masse oder auch der Tumor selbst sein. - Dann wird das Absperrorgan
30 aus dem Zuführungskatheter12 entfernt (11 ). Die Elektrodenentfaltungseinrichtung wird in den Zuführungskatheter12 eingesetzt, und der Katheter wird dann wieder am Griff18 befestigt (12 ). Wie in12 gezeigt, kann die Elektrodenentfaltungseinrichtung optional eine Elektrodenschablone34 beinhalten, um die Entfaltung der Elektroden20 in Umgebungsbeziehung zu der äußeren Oberfläche einer ausgewählten Gewebemasse zu bringen. - Die Elektroden
20 werden dann aus dem distalen Ende16 des Zuführungskatheters12 herausbewegt und entfalten sich zu einem gewünschten, die Masse umgebenden Ablationsvolumen. In13 ist der Zuführungskatheter12 neben der Leber angeordnet. Die Elektrodenentfaltungseinrichtung wird mit der Elektrodenschablone34 in den Zuführungskatheter12 eingeführt. Die Elektro denentfaltungseinrichtung sticht nun in die Leber hinein, und der Nocken22 bewegt die Elektroden20 aus dem Zuführungskatheter12 heraus in ihren entfalteten Zustand. Jede einzelne Elektrode20 durchsticht die Leber und bewegt sich durch sie hindurch, bis die Elektronen sich in Umgebungsbeziehung mit dem Tumor befinden. Das Ablationsvolumen ist auswählbar und wird zunächst durch Abbilden des durch Ablation zu behandelnden Bereichs bestimmt. Das Ablationsvolumen wird durch die Außenflächen aller entfalteten Elektroden20 bestimmt, die das Äußere des Tumors umgeben. Sobald das Ablationsvolumen festgelegt wurde, wird ein Satz von Elektroden ausgewählt, die sich zur Festlegung des Ablationsvolumens entfalten. Eine Reihe verschiedener Faktoren ist bei der Erzeugung eines Ablationsvolumen wichtig. Im wesentlichen weisen verschiedene Elektroden20 basierend auf dem Elektrodenmaterial, dem Grad der Vorspannung der Elektroden und der geometrischen Ausgestaltung der Elektroden in ihrem entfalteten Zustand verschiedene Grade der Entfaltung auf. Damit eine Vielzahl von Ablationsvolumina bestimmt werden können, erlaubt die Gewebeablationsvorrichtung10 das Einsetzen verschiedener Sätze von Elektroden20 in den Zuführungskatheter12 . - Vor der Entfernung des Tumors kann ein vor der Ablation stattfindender Schritt durchgeführt werden. Eine Vielzahl verschiedener Lösungen, einschließlich elektrolytischer Lösungen wie Kochsalzlösung, können wie in
14 gezeigt an den Tumorort eingeführt werden.15 veranschaulicht das Anlegen von RF-Energie an den Tumor. Der Elektrodenisolator28 ist auf Abschnitten der Elektroden20 angeordnet, an denen keine Ablation stattfindet. Dies definiert das Ablationsvolumen noch genauer. Die eigentliche elektrische Austrocknung bzw. Verbrennung des Tumors oder anderer Zielmassen oder -gewebe ist in16 gezeigt. Wieder bestimmen die entfalteten Elektroden20 mit ihren entlang von Abschnitten der Elektroden positionierten Elektrodenisolatoren28 das Ablationsvolumen und schließlich die Menge der ausgetrockneten Masse. - Optional können die Elektroden
20 in einem nach der Ablation erfolgenden Schritt nach der Austrocknung eine Vielzahl von Lösungen einführen. Dieser Schritt ist in17 gezeigt. Geeignete Lösungen beinhalten chemotherapeutische Wirkstoffe, sind jedoch nicht auf diese beschränkt. -
18 veranschaulicht die in einem bipolaren Modus betriebene Gewebeablationsvorrichtung10 . Der monopolare Betrieb ist in19 gezeigt. Jede aus der Mehrzahl von Elektroden20 kann im Ablationsprozeß eine andere Rolle spielen. Zwischen den verschiedenen Elektroden kann es zu einer Polaritätsverschiebung kommen. - Ein Gewebeablationssystem
36 , welches modular sein kann, ist in20 gezeigt und beinhaltet ein Anzeigegerät38 . Das Gewebeablationssystem36 beinhaltet die RF-Energiequelle und kann weiterhin eine Mikrowellenquelle, eine Ultraschallquelle, Darstellungsgeräte wie Kameras und Videorecorder, Quellen elektrolytischer und chemotherapeutischer Lösungen sowie einen Controller umfassen, der zur Überwachung der Temperatur oder der Impedanz eingesetzt werden kann. Eine der entfalteten Elektroden20 kann eine an eine Mikrowellenquelle gekoppelte Mikrowellenantenne sein. Diese Elektrode kann anfänglich an die RF-Spannungsquelle42 gekoppelt sein und dann auf die Mikrowellenquelle umgeschaltet werden. - Gemäß
21 liefert eine Stromquelle40 Energie an den RF-Stromgenerator (Quelle)42 und dann an die Elektroden20 der Gewebeablationsvorrichtung10 . Ein Multiplexer46 mißt Strom, Spannung und Temperatur (über zahlreiche Temperatursensoren, die auf den Elektroden20 angeordnet sein können). Der Multiplexer46 wird von einem digitalen oder analogen Controller48 oder von einem Computer mit Software gesteuert. Wenn der Controller48 ein Computer ist, kann dieser eine durch einen Systembus gekoppelte CPU beinhalten. Dieses System kann eine Tastatur, ein Diskettenlaufwerk oder andere nicht-flüchtige Speichersysteme, einen Bildschirm und andere aus dem Stand der Technik bekannte Peripheriegeräte umfassen. Mit dem Bus sind weiterhin ein Programmspeicher und ein Datenspeicher gekoppelt. - Eine Benutzerschnittstelle
50 beinhaltet Benutzersteuerungen52 und einen Bildschirm38 . Der Controller48 ist mit Darstellungssystemen einschließlich Ultraschalltransducern, Temperatursensoren sowie Bildoptiken und optischen Fasern gekoppelt, sofern vorhanden. - Strom und Spannung werden zur Berechnung der Impedanz verwendet. Diagnosen werden mittels Ultraschall, CT-Abtastung oder anderer aus dem Stand der Technik bekannter Verfahren erstellt. Die Abbildung kann vor, während und nach der Behandlung erfolgen.
- Temperatursensoren messen die Spannung und den Strom, die geliefert werden. Der Ausgang dieser Sensoren wird von dem Controller
48 zur Steuerung der Zuführung von RF-Spannung verwendet. Die Menge der zugeführten RF-Energie bestimmt die Stärke des Stroms. Ein Profil des zugeführten Stroms kann in den Controller38 integriert sein. Eine voreingestellte, zuzuführende Energiemenge kann ebenso als Profil eingestellt sein. - Über Rückkopplung können Impedanz oder Temperatur gemessen werden, und sie findet entweder am Controller
48 oder an der elektromagnetischen Energiequelle42 , z. B. RF- oder Mikrowellenenergie, statt, wenn diese einen Controller beinhaltet. Für die Messung der Impedanz kann dies durch Bereitstellen einer kleinen Menge an Nicht-Ablations-RF-Energie geschehen. Dann werden Spannung und Strom gemessen. - Schaltkreise, Software und Rückkopplung an den Controller
48 führen zu einer Prozeßsteuerung und werden eingesetzt, um (i) die Energie, einschließlich RF-Energie, Ultraschallenergie usw., (ii) den Arbeitszyklus (Ein-Aus und Wattleistung), (iii) die monopolare oder bipolare Zuführung von Energie, (iv) die Zuführung chemotherapeutischer und elektrolytischer Lösungen, die Flußrate und den Druck zu verändern und (v) anhand von Zeit, Temperatur und/oder Impedanz zu bestimmen, wann die Ablation abgeschlossen ist. Diese Prozeßvariablen können gesteuert und variiert werden auf Basis der an einer Vielzahl von Orten überwachten Temperatur und der gemessenen Impedanz zum Stromfluß, wobei während des Ablationsprozesses auftretende Veränderungen in der Fähigkeit des Gewebes, Strom zu leiten, angezeigt werden. - Es wurden Ausführungsformen von RF-Tumorbehandlungseinrichtungen beschrieben, die für minimal invasive Verfahren geeignet sind. Es wurden Ausführungsformen dargestellt, bei denen das Äußere des Tumors von Behandlungselektroden umgeben ist, die ein gesteuertes Ablationsvolumen bestimmen, und wobei die Elektroden eine geregelte Menge an RF-Energie zuführen. Es wurden Ausführungsformen mit der Möglichkeit der Zuführung von Infusionen während eines vor der Ablation stattfindenden Schrittes beschrieben, bei denen das umliegende Gewebe nach der Ablation mit elektromagnetischer ("EM") Energie bei Hyperthermie-Temperaturen von unter 45°C behandelt wird. Dies würde die synergistische Wirkung von Chemotherapie und der Zufuhr einer Vielzahl von Flüssigkeiten am Tumorort nach lokal erfolgter Ablation und Hyperthermie gewährleisten.
- Die vorstehende Beschreibung bevorzugter Ausführungsformen der vorliegenden Erfindung dient Zwecken der Darstellung und Beschreibung. Sie soll nicht erschöpfend sein oder die Erfindung auf die genau beschriebenen Formen beschränken. Viele Modifikationen und Variationen liegen für Fachleute auf diesem Gebiet auf der Hand. Die Ausführungsbeispiele wurden so gewählt und beschrieben, daß sie die Prinzipien der Erfindung und deren praktische Anwendbarkeit am besten erklären und es anderen Fachleuten dadurch ermöglichen, die Erfindung in Bezug auf zahlreiche Ausführungsformen und mit verschiedenen Modifikationen zu verstehen, wie sie dem speziellen gewünschten Gebrauch angemessen sind.
Claims (22)
- Ablationsvorrichtung, die eine längliche Zuführungseinrichtung (
12 ) einschließlich eines Lumens und eine Energiezuführungseinrichtung aufweist, wobei die Energiezuführungseinrichtung beinhaltet: eine Mehrzahl von Hochfrequenzelektroden (20 ), die jeweils einen Gewebe durchdringenden distalen Abschnitt haben, und ein Glied (22 ) für das Vorrücken der Hochfrequenzelektroden, das innerhalb des Lumens positionierbar ist und mit den Hochfrequenzelektroden verbunden ist, wobei das Glied zum Vorrücken der Hochfrequenzelektroden derart konfiguriert ist, daß es die Hochfrequenzelektroden durch das Gewebe vorrückt, wobei die Hochfrequenzelektroden in einem zurückgezogenen Zustand in der länglichen Zuführungseinrichtung positioniert sind und derart betreibbar sind, daß sie sich von dem distalsten Ende der länglichen Zuführungsvorrichtung mit einer Krümmung entfalten, wenn sie von dem distalsten Ende der länglichen Zuführungseinrichtung zu einem entfalteten Zustand vorgerückt werden, und wobei die entfalteten Elektroden derart betreibbar sind, daß sie ein definiertes Ablationsvolumen erzeugen, das eine Gewebemasse umgibt, die fern von dem distalen Ende der Zuführungseinrichtung lokalisiert ist. - Vorrichtung nach Anspruch 1, wobei zumindest eine der Mehrzahl der Hochfrequenzelektroden einen Isolator hat, der in einer Umgebungsbeziehung zu zumindest einem Teil dieser Elektrode positioniert ist.
- Vorrichtung nach Anspruch 2, wobei der Isolator verschiebbar auf der Elektrode positionierbar ist, um eine auswählbare Energiezuführungsoberfläche zu erzeugen.
- Vorrichtung nach einem der vorherigen Ansprüche, die weiterhin einen thermischen Sensor aufweist, der mit zumindest einer der Hochfrequenzelektroden verbunden ist.
- Vorrichtung nach einem der vorherigen Ansprüche, wobei die Vorrichtung derart konfiguriert ist, daß sie in einem bipolaren Modus betreibbar ist.
- Vorrichtung nach Anspruch 5, wobei zumindest eine der Mehrzahl von Hochfrequenzelektroden als eine positive Elektrode konfiguriert ist.
- Vorrichtung nach einem der Ansprüche 1 bis 4, wobei die Vorrichtung derart konfiguriert ist, daß sie in einem monopolaren Modus betreibbar ist.
- Vorrichtung nach einem der vorherigen Ansprüche, die weiterhin ein entfernbares Absperrorgan (
30 ) mit einem distalen Ende (32 ) aufweist, das ausreichend scharf ist, um Gewebe zu durchdringen. - Vorrichtung nach Anspruch 8, wobei das Absperrorgan in dem Lumen der länglichen Zuführungseinrichtung positionierbar ist.
- Vorrichtung nach einem der vorherigen Ansprüche, wobei zumindest eine der RF-Elektroden aus einer Formgedächtnislegierung gebildet ist.
- Vorrichtung nach einem der vorherigen Ansprüche, wobei zumindest eine der Hochfrequenzelektroden aus nicht rostendem Stahl gebildet ist.
- Vorrichtung nach einem der vorherigen Ansprüche, die weiterhin einen thermischen Sensor aufweist, der mit der länglichen Zuführungseinrichtung verbunden ist.
- Vorrichtung nach einem der vorherigen Ansprüche, die weiterhin eine Hochfrequenzquelle aufweist, die mit der Mehrzahl von Hochfrequenzelektroden gekoppelt ist, für das zur Verfügung stellen von Hochfrequenzenergie zu den Hochfrequenzelektroden.
- Vorrichtung nach Anspruch 13, die weiterhin eine Steuereinrichtung (
46 ,48 ) aufweist, die in Wirkverbindung mit den Hochfrequenzelektroden und der Hochfrequenzquelle stehen, um das Anlegen von hochfrequentem Strom an die Elektroden zu steuern. - Vorrichtung nach Anspruch 14, wobei die Steuereinrichtung eine Rückkopplungssteuerung ist, die in Wirkverbindung mit einem thermischen Sensor auf jeder der Hochfrequenzelektroden und der Hochfrequenzquelle steht.
- Vorrichtung nach Anspruch 15, wobei die Rückkopplungssteuerung die Hochfrequenzenergie einstellt, die von der Hochfrequenzquelle zu den Hochfrequenzelektroden geliefert wird.
- Vorrichtung nach Anspruch 14, wobei die Steuereinrichtung eine Rückkopplungssteuerung ist, die die Hochfrequenzenergie einstellt, die zu den Hochfrequenzelektroden von der Hochfrequenzquelle geliefert wird in Antwort auf eine Impedanz, die an einer oder mehreren der Hochfrequenzelektroden gemessen wird.
- Vorrichtung nach einem der vorherigen Ansprüche, die weiterhin eine Steuereinrichtung (
46 ,48 ) aufweist, die mit der Energiezuführungseinrichtung verbunden ist, für das Steuern von Prozeßvariablen. - Vorrichtung nach Anspruch 18, wobei die Prozeßvariablen zumindest eine der folgenden Variablen sind: Leistungsniveau, Arbeitszyklus oder Energiezuführung.
- Vorrichtung nach Anspruch 18 oder Anspruch 19, wobei die Steuereinrichtung zumindest eines der folgenden Elemente aufweist: digitaler Controller, analoger Controller und programmierter Computer.
- Vorrichtung nach einem der vorherigen Ansprüche, die weiterhin eine Infusionsquelle aufweist für das Zuführen eines Infusionsmediums, wobei jede der Elektroden hohl ist und eine Mehrzahl von Fluidverteilungsanschlüssen zur Verfügung stellt, durch die Fluid infundiert werden kann.
- Vorrichtung nach Anspruch 21, die weiterhin eine Steuereinrichtung (
46 ,48 ) aufweist, die mit der Energiezuführungseinrichtung verbunden ist, für das Steuern von Prozeßvariablen, wobei die Prozeßvariablen zumindest die Fluidzuführungsflußrate oder einen Fluiddruck beinhalten.
