Description

Die Erfindung betrifft eine Kniegelenkendoprothese nach dem Oberbegriff des An­ spruchs 1.
Aus der WO 95/23567 ist eine Kniegelenkendoprothese mit einem im Femur veranker­ ten metallischen Femurteil bekannt, welches mit einem im tibialen Knochen veranker­ ten Tibiateil aus Polyethylen artikuliert. Die artikulierende Fläche des Femurteils bildet ein keramisches Bauteil, welches mit dem Femurteil mechanisch stabil verbunden ist.
Der Erfindung liegt die Aufgabe zugrunde, eine Kniegelenkendoprothese nach dem Oberbegriff des Anspruchs 1 so zu verbessern, daß die Befestigung des keramischen Bauteils am Femurteil optimiert ist.
In einer erfindungsgemäßen Ausführungsform ist das keramische Bauteil über eine konische Klemmung auf dem Femurteil verankert. Hierzu ist sinnvollerweise im Fe­ murteil eine konische Nut eingebracht, in die das Bauteil mit seinen konischen Seiten­ flächen eingesetzt ist. Der Winkel der konischen Klemmung liegt bevorzugt zwischen 5° und 20°. Je größer der Winkel ist, desto leichter lassen sich die Bauteile auch nach einer Implantation während einer Reoperation wieder entfernen.
In einer bevorzugten Ausführungsform ist die konische Klemmung nur im Bereich oberhalb der Belastungszone bei gestrecktem Knie angeordnet.
In einer anderen bevorzugten Ausführungsform erstreckt sich die konische Klemmung über den gesamten Bereich der artikulierenden Fläche.
In einer alternativen Ausführungsform ist das Bauteil über mindestens eine Fixations­ schraube auf dem Femurteil verankert. Eine konische Klemmung würde in diesem Fall entfallen.
Bevorzugt werden zwei Bauteile je Endoprothese verwendet. Es kann aber auch nur ein Bauteil verwendet werden, welches dann zwei artikulierende Flächen besitzt.
In einer weiteren alternativen Ausführungsform ist das keramische Bauteil mit dem Femurteil über Schrumpfen, Löten oder Schweißen unlösbar verbunden. Löten schließt auch Aktivlöten mit ein.
Verbessert werden kann diese Befestigung, wenn das keramische Bauteil mit dem Femurteil zusätzlich über eine kongruente Oberflächengestaltung verbunden ist. Dies können z. B. ineinandergreifende Nuten und Rippen sein. In vorteilhafter Ausgestal­ tung ist das keramische Bauteil mit dem Femurteil zusätzlich über eine Nut-Feder- Verbindung verbunden. Diese ist beim Schrumpfen besonders zweckmäßig. Bei einer Nut-Feder-Verbindung ragt z. B. eine Rippe auf dem Bauteil in eine Nut des Femur­ teils oder umgekehrt. Es sind hier jedoch viele Varianten der kongruenten Oberflä­ chengestaltung möglich.
In einer weiteren alternativen Ausführungsform ist das keramische Bauteil schichtför­ mig ausgebildet und durch Aufdampfen, Aufspritzen oder dergleichen fixiert bzw. auf­ gebracht. Das Bauteil ist hier also eine Schicht. Natürlich kann diese Schicht auch aus mehreren einzelnen Schichten bestehen. Ein keramischer Stoff für die Schicht oder die anderen Bauteile wäre z. B. Aluminiumoxid.
Weitere Merkmale der Erfindung ergeben sich aus den Figuren, die nachfolgend be­ schrieben sind. Es zeigt:

Fig. 1b denselben Schnitt wie Fig. 1a jedoch um 180° gedreht,
Fig. 2 einen seitlichen Schnitt durch ein Femurteil mit einem über eine konische Klemmung aufgesetzten Bauteil, wobei die konische Klemmung nur oberhalb der Belastungszone angeordnet ist,
Fig. 3 die Befestigung des Bauteils am Femurteil über eine Fixationsschraube und
Fig. 4 die Befestigung des Bauteils am Femurteil über eine Schrumpfung, wobei Fig. 4a einen Schnitt wie Fig. 1b und Fig. 4b einen Schnitt wie Fig. 2, 3 darstellt.
In den Fig. 1a und 1b ist ein metallisches Femurteil 2 in verschiedenen Schnitten ge­ zeigt. Im Femurteil 2 ist im Bereich der artikulierenden Fläche eine Nut 5 bzw. zwei Nuten 5 eingebracht. Diese Nuten 5 haben an ihren Seiten konische Flächen mit Win­ keln zwischen 5° und 20°. In die Nuten 5 sind keramische Bauteile 3 eingefaßt, die entsprechende konische Seitenflächen besitzen, so daß eine konische Klemmung 4 gebildet ist. Es ist nicht gezeigt, daß die Bauteile 3 mit einem im tibialen Knochen ver­ ankerten Tibiateil artikulieren. Die Nut 5 erstreckt sich in dieser Ausführungsform ent­ lang der gesamten artikulierenden Fläche. Es können jedoch auch mehrere Nuten 5 oder einzelne Einkerbungen vorgesehen sein, so daß das Bauteil 3 nur an einzelnen Stellen mit dem Femurteil 2 über eine konische Klemmung 4 verankert ist.
Fig. 2 zeigt einen seitlichen Schnitt durch ein Femurteil 2 mit ebenfalls einem über eine konische Klemmung 4 aufgesetzten Bauteil 3. Die konische Klemmung 4 ist hier im Unterschied zu der Ausführungsform von Fig. 1 nur oberhalb der Belastungszone bei gestrecktem Knie angeordnet. Im Femurteil 2 ist hierzu eine weniger große Klemmfläche als in Fig. 1 eingebracht.
Fig. 3 zeigt die Befestigung des keramischen Bauteils 3 auf dem Femurteil 2 über eine Fixationsschraube 1. Hierzu ist im Bauteil 3 eine Bohrung 7 und im Femurteil 2 ein Gewinde 6 angeordnet. Die Fixationsschraube 1 ragt durch die Bohrung 7 in das Ge­ winde 6 und verankert so das Bauteil 3 am Femurteil 2.
Falls erforderlich, können auch mehrere Fixationsschrauben verwendet werden, die vorteilhafterweise im Bauteil 3 versenkt angeordnet sind.
Fig. 4 zeigt ein Femurteil 2, welches an seiner tibialen Seite eine Nut 10 aufweist. Das zugehörige keramische Bauteil 3 hat eine zur Nut kongruente Rippe 11. Fig. 4a zeigt das Femurteil 2 im Querschnitt und Fig. 4b im Längsschnitt. Das Bauteil 3 wird durch Aufschrumpfen befestigt. Es kann auch aufgelötet oder angeschweißt werden. Hierbei kann die Nut-Feder-Verbindung wahlweise entfallen.

Claims (11)

1. Kniegelenkendoprothese mit einem im Femur verankerten metallischen Femur­ teil (2), welches mit einem im tibialen Knochen verankerten Tibiateil artikuliert wobei die artikulierende Fläche des Femurteils (2) ein keramisches Bauteil (3) ist, welches mit dem Femurteil (2) mechanisch stabil verbunden ist, dadurch ge­ kennzeichnet, daß das keramische Bauteil (3) über eine konische Klemmung (4) auf dem Femurteil (2) verankert ist.
2. Kniegelenkendoprothese nach Anspruch 1, dadurch gekennzeichnet, daß im Femurteil (2) eine konische Nut (5) eingebracht ist, in die das Bauteil (3) mit sei­ nen konischen Seitenflächen eingesetzt ist.
3. Kniegelenkendoprothese nach Anspruch 1 oder 2, dadurch gekennzeichnet, daß der Winkel der konischen Klemmung (4) zwischen 5° und 20° liegt.
4. Kniegelenkendoprothese nach einem der Ansprüche 1 bis 3, dadurch gekenn­ zeichnet, daß die konische Klemmung (4) nur im Bereich oberhalb der Bela­ stungszone bei gestrecktem Knie angeordnet ist.
5. Kniegelenkendoprothese nach einem der Ansprüche 1 bis 4, dadurch gekenn­ zeichnet, daß sich die konische Klemmung (4) über den gesamten Bereich der artikulierenden Fläche erstreckt.
6. Kniegelenkendoprothese mit einem im Femur verankerten metallischen Femur­ teil (2), welches mit einem im tibialen Knochen verankerten Tibiateil artikuliert, wobei die artikulierende Fläche des Femurteils (2) ein keramisches Bauteil (3) ist, welches mit dem Femurteil (2) mechanisch stabil verbunden ist, dadurch ge­ kennzeichnet, daß das Bauteil (3) über eine Fixationsschraube (1) auf dem Fe­ murteil (2) verankert ist.
7. Kniegelenkendoprothese mit einem im Femur verankerten metallischen Femur­ teil (2), welches mit einem im tibialen Knochen verankerten Tibiateil artikuliert, wobei die artikulierende Fläche des Femurteils (2) ein keramisches Bauteil (3) ist, welches mit dem Femurteil (2) mechanisch stabil verbunden ist, dadurch ge­ kennzeichnet, daß das keramische Bauteil (3) mit dem Femurteil (2) über Schrumpfen, Löten oder Schweißen unlösbar verbunden ist.
8. Kniegelenkendoprothese nach Anspruch 7, dadurch gekennzeichnet, daß das keramische Bauteil (3) mit dem Femurteil (2) zusätzlich über kongruente Ober­ flächengestaltungen verbunden ist.
9. Kniegelenkendoprothese nach Anspruch 8, dadurch gekennzeichnet, daß das keramische Bauteil (3) mit dem Femurteil (2) über eine Nut-Feder-Verbindung verbunden ist.
10. Kniegelenkendoprothese nach einem der Ansprüche 1 bis 9, dadurch gekenn­ zeichnet, daß zwei Bauteile (3) je Kniegelenkendoprothese verwendet werden.
11. Kniegelenkendoprothese mit einem im Femur verankerten metallischen Femur­ teil (2), welches mit einem im tibialen Knochen verankerten Tibiateil artikuliert, wobei die artikulierende Fläche des Femurteils (2) ein keramisches Bauteil (3) ist, welches mit dem Femurteil (2) mechanisch stabil verbunden ist, dadurch ge­ kennzeichnet, daß das keramische Bauteil (3) schichtförmig ausgebildet und durch Aufdampfen, Aufspritzen oder dergleichen fixiert ist.
