本发明公开了一种药物组合物在制备治疗黑色素瘤产品中的应用,所述药物组合物包括LAG‑3单抗和赛帕利单抗,所述LAG‑3单抗和赛帕利单抗的质量比为1:0.9‑3;所述LAG‑3单抗为GLS‑012;所述GLS‑012具有如SEQ ID NO.1所示的重链区,SEQ ID NO.2所示的轻链区。本发明首次发现,与同靶点同类药物Relalimab+Nivolumab 1:3配比组合物比较,GLS‑012+赛帕利单抗(Zimberelimab)1:1配比组合物对黑色素瘤具有更强抑瘤作用,使用剂量更低,安全性更好。

技术领域
本发明属于医药领域,具体涉及一种包括LAG-3单抗与赛帕利单抗的药物组合物或含所述药物组合物的药物制剂在制备治疗黑色素瘤产品中的应用。
背景技术
晚期恶性肿瘤患者生存期短,生活质量差,采用药物治疗是大多数晚期肿瘤患者主要治疗方法。传统抗肿瘤药物是采用多种细胞毒类药物进行联合化疗,这些化疗药物靶向性差,副反应明显,在杀灭肿瘤细胞同时对于正常组织细胞也产生较大损伤,临床对于新型抗肿瘤药物存在巨大需求。
近年来,肿瘤免疫治疗逐渐成为研发热点,并不断获得巨大突破。与传统化疗药物直接杀灭或抑制肿瘤细胞作用机制不同,肿瘤免疫治疗是通过提高机体免疫***功能与活性,激活人体自身免疫***对肿瘤细胞进行识别和杀伤,因此其更符合人体本身功能,不良反应更小。肿瘤免疫治疗已成为当今肿瘤治疗领域热点,其中 T 细胞肿瘤免疫治疗又处于核心位置。
LAG-3属于免疫球蛋白超家族,由胞外区、跨膜区和胞质区三个部分组成,其主要表达在活化 T 淋巴细胞、部分自然杀伤细胞等表面,负向调控机体免疫功能。大量研究业已证明,通过抑制 LAG-3 与其配体分子结合,可促进T细胞活化及免疫***对肿瘤细胞杀伤,LAG-3 已成为潜在肿瘤免疫治疗靶点,并为肿瘤免疫治疗提供了除 CTLA-4,PD-1 之外又一可能途径。
LAG-3 和 PD-L1/PD-1 均能负向调节 T 细胞功能。多种肿瘤组织(包括乳腺癌,结肠癌,肺癌等 1400 余例)测序结果显示,PD-L1 基因高表达和与 LAG-3 基因呈正相关性,提示肿瘤患者中该两种基因有同时表达现象,此外也有研究发现,在非小细胞肺癌患者中TIL细胞表面高表达PD-1 蛋白,与 LAG-3 蛋白表达呈正相关性;这两种负向调节 T 细胞功能蛋白(LAG-3 和 PD-L1/PD-1)的共表达,可协同抑制 T 细胞功能,从而导致患者预后较差。以上研究清楚显示,同时靶向 PD-1/PD-L1 和 LAG-3 作为治疗手段的可行性及潜在协同疗效。
恶性黑素瘤是由皮肤和其他器官黑色素细胞产生的肿瘤,属高度恶性肿瘤,也是近年来发病率增长最快恶性肿瘤之一,恶性度高,转移发生早,死亡率高,因此临床存在没有被满足的治疗需求。
综上所述,针对黑色素瘤临床刚性治疗需求,开发安全、有效的抗LAG-3靶点的新型抗体药物,特别是联用 PD-1/PD-L1 抑制剂,不仅可进一步改善 PD-1/PD-L1 抑制剂低有效率和耐药性等问题,而且是进一步提高黑色素瘤疗效的重要途径之一。
发明内容
发明目的:本发明所要解决的技术问题是针对现有技术不足,提供一种药物组合物或含所述药物组合物的药物制剂在制备治疗黑色素瘤产品中的应用。
为了解决上述技术问题,本发明公开了如下技术方案:
一种药物组合物或含所述药物组合物的药物制剂在制备治疗黑色素瘤产品中的应用。
其中,所述药物组合物包括LAG-3单抗和赛帕利单抗;所述LAG-3单抗为全人源抗LAG-3单抗GLS-012,其是誉衡生物自主研发具有完整独立知识产权的创新抗肿瘤生物药,具有如SEQ ID NO.1所示的重链区,SEQ ID NO.2所示的轻链区,已公开于中国发明专利ZL201910146172.2“抗人LAG-3单克隆抗体及其制备方法和用途”的1.53.3-uAb-IgG4k。所述赛帕利单抗具有如SEQ ID NO.3所示的重链区,SEQ ID NO.4所示的轻链区。
在一些实施例中,所述LAG-3单抗和赛帕利单抗的质量比为1:0.9-3。
在一些实施例中,所述LAG-3单抗和赛帕利单抗的质量比为1:0.9-2。

下述实施例中所述实验方法,如无特殊说明,均为常规方法;所述试剂和材料,如无特殊说明,均可从商业途径获得。
实施例1 全人源抗LAG-3单克隆抗体GLS-012与全人源抗PD-1赛帕利单抗1:1组合物对黑色素瘤生长抑制作用的小鼠体内实验研究
1 目的
与现有技术抗LAG-3与抗PD-1靶点药物Relalimab+Nivolumab 1:3配比组合物比较,探索全人源抗LAG-3单克隆抗体GLS-012与全人源抗PD-1赛帕利单克隆抗体1:1组合物抗黑色素瘤作用及其配比优势。
2 方法
2.1 将PBS重悬的黑色素瘤细胞B16F10接种于雌性B6 - hLAG-3小鼠的右侧胁肋部皮下,在肿瘤生长至74mm3左右时分组给药,共4组,每组8只动物,Goup1(阴性对照组):同型对照hIgG4 20mg/kg;Goup2(阳性对照1:3配比组):Relalimab 20mg/kg+Nivoluamb60mg/kg;Goup3(实验组1:3配比组):全人源抗GLS-012 20mg/kg +Zimbereliamb 60mg/kg;Goup4(实验组1:1配比组):全人源抗GLS-012 20mg/kg +Zimbereliamb 20mg/kg。
给药体积按动物体重10 μL/g。分组当天记为D0,每周给药2次,共给药4次,对相对肿瘤生长抑制率(TGIRTV)进行评价。其中,阳性对照药类似物 Relatlimab和Nivoluamb分别为全球首个获批上市抗LAG-3和抗PD-1单克隆抗体。
2.2 相对肿瘤生长抑制率TGIRTV(%)=(1-T/C)× 100%,其中:T/C = 治疗组RTV均值/对照组RTV均值,RTV为相对肿瘤体积,即治疗后与治疗前的TV比值。
3 结果
3.1 安全性
小鼠在给药后一般状态可,正常摄食饮水,除Goup3小鼠体重有明显下降之外,其余各组体重大体稳定,无停药和死亡。提示除Goup3可能因不良反应导致小鼠体重明显降低之外,其余配比剂量组均无明显不良反应发生。各组小鼠体重变化曲线详见图1。
3.2 有效性
与阴性对照组比较,各配比组合物组均显示明显抑制黑色素瘤细胞生长的作用,其对肿瘤生长抑制率TGIRTV(%)分别为Goup2:61%、Goup3:88%、Goup4:84%,与阴性对照组比较P均<0.01。其次,与Goup2阳性药物1:3组合物比较,Goup3、Goup4均具有更强抑瘤活性,但Goup3与Goup4两组比较,抑瘤率无明显差异(P>0.05),详见图3。提示GLS-012+赛帕利单抗1:3与1:1配比组合物两组疗效接近,但1:1配比组合物安全性明显更好,使用剂量更低。相关数据实验详见表1和图2、3。本研究结果显示,GLS-012与赛帕利单抗不同配比组合物的疗效并未随剂量增大而线性增强,可能与一定剂量下T淋巴细胞受体占有率已达饱和有关。

4 结论
由上清楚可见,全人源抗LAG-3单克隆抗体GLS-012联合抗PD-1赛帕利单抗1:3组合物与1:1配比组合物均具有显著抑制黑色素瘤生长的作用,其中,1:1配比组合物与1:3配比组合物疗效相似,但安全性更好,使用剂量更低,更具有临床应用优势。
实施例2 全人源抗LAG-3单克隆抗体GLS-012联合抗PD-1赛帕利单克隆抗体治疗黑色素瘤的安全性、有效性临床研究
1 目的
初步探索全人源抗LAG-3单克隆抗体GLS-012联合抗PD-1赛帕利单克隆抗体方案治疗黑色素瘤患者的安全性和有效性。
2 方法
2.1 患者一般情况:所有12例黑色素瘤患者,其中,男性7例、女性5例,平均年龄56.6±14.7周岁(28-73周岁),ECOG评分:0分8例、1分4例;诊断为肢端黑色素瘤1例、黏膜黑色素瘤3例、皮肤黑色素瘤5例、黑色素瘤原发灶不明3例;既往接受过肿瘤手术治疗12例、化疗7例、靶向药物治疗7例、免疫治疗9例、干扰素治疗3例、放疗2例。
2.2 给药方案:GLS-012 480mg +赛帕利单抗360mg 静脉滴注,每3周一次(q3w)。
2.3 评价方法:疗效评价采用RECIST v1.1,安全性评价采用CTCAE v5.0。
3 结果
3.1 安全性
所有12例患者治疗期间均未观察到剂量限制性毒性(DLT),且未发生严重不良事件和免疫相关不良事件,无患者死亡。其中,1例发生3级不良事件,其发生率仅为8.3%,其余均为1-2级不良事件,且均恢复至正常(详见表2)。由此提示GLS-012联合赛帕利单抗的临床安全性、耐受性均较好。

3.2 有效性
所有12例患者中,完全缓解(CR)2例、部分缓解(PR)6例,疾病稳定(SD)2例,疾病进展(PD)2例。提示GLS-012联合赛帕利单抗治疗黑色素瘤客观有效率(ORR)为66.7%,临床获益率(DCR)为83.3%,疗效显著。
4 结论
综上所述,本临床研究结果初步表明,LAG-3 抑制剂GLS-012 480mg联合抗PD-1抑制剂赛帕利单抗360mg治疗黑色素瘤客观有效率(ORR)为66.7%、临床获益率(DCR)为83.3%,3-4级严重不良事件发生率仅8.3%,其余均为1-2级不良事件。提示该组合物GLS-012联合赛帕利单抗抗肿瘤活性更强,且安全性、耐受性均好,值得进一步开发。

Claims (8)
1.一种药物组合物或含所述药物组合物的药物制剂在制备治疗黑色素瘤产品中的应用,其特征在于,所述药物组合物包括LAG-3单抗和赛帕利单抗,所述LAG-3单抗和赛帕利单抗的质量比为1:0.9-3;所述LAG-3单抗为GLS-012;所述GLS-012具有如SEQ ID NO.1所示的重链区,SEQ ID NO.2所示的轻链区。
2.根据权利要求1所述的应用,其特征在于,所述LAG-3单抗和赛帕利单抗的质量比为1:0.9-2。
3.根据权利要求1所述的应用,其特征在于,所述LAG-3单抗和赛帕利单抗的质量比为1:0.9-1.5。
4.根据权利要求1所述的应用,其特征在于,所述LAG-3单抗和赛帕利单抗的质量比为1:1。
5.根据权利要求1所述的应用,其特征在于,所述药物制剂还包括药学上可接受的载体。
6.根据权利要求1所述的应用,其特征在于,所述产品包括药物、试剂盒。
7.根据权利要求1所述的应用,其特征在于,所述药物组合物或药物制剂的治疗有效量为LAG-3单抗400-560mg/次和赛帕利单抗280-440mg/次。
8.根据权利要求1所述的应用,其特征在于,所述LAG-3单抗的给药频率为每2-4周1次。
