它们具有基本上扁平形状和使用合适的方式来放置在身体伤口的区域中。
特别地,当涉及大的伤口护理制品时,可能发生的是,受影响的身体区域是不规则的以及展现凹陷、弯曲或上升。基本上扁平的伤口护理制品没有很好地符合这样的解剖结构,使得折叠或折痕形成,这使得患者感到不舒适以及对伤口护理制品的功效可能具有有害的影响。
发明内容
本发明的目的是要提出能够紧密地符合人和动物的不同身体区域的解剖轮廓的伤口护理制品。
该目的通过呈现的主权利要求的特征来实现。
因此,本发明的主题是扁平的伤口护理制品,其具有基本上多边形或椭球底面以及在一侧上布置的至少一个凹槽。
已令人惊讶地发现,由于该相当简单的修改,根据本发明的伤口护理制品展现相当大的通用性以及,特别地,能够紧密地符合人和动物的各种身体区域的解剖轮廓。

因此,优选地该层包括按重量的多于26%、27%、28%、29%、30%、31%、32%、33%、34%、35%、36%、37%、38%、39%、40%、41%、42%、43%、44%、45%、46%、47%、48%、49%、50%、51%、52%、53%、54%、55%、56%、57%、58%、59%、60%、61%、62%、63%、64%、65%、66%、67%、68%、69%、70%、71%、72%、73%、74%、75%、76%、77%、78%、79%、80%、81%、82%、83%、84%、85%、86%、87%、88%、89%、90%、91%、92%、93%、94%、95%、96%、97%、98%的超吸收性聚合物。
术语“超吸收性聚合物(superabsorbent polymers,SAP)”指能够吸收相当于它们自己的重量的多倍(高达1000倍)的合成物。化学上而言,这些是丙烯酸(丙酸,C3H4O2)和丙烯酸钠(丙烯酸的钠盐,NaC3H3O2)的共聚物,其中两个单体彼此的比率可以变化。另外,所谓的芯交联剂(CXL)添加到单体溶液以及该芯交联剂通过化学桥在一些位置中将形成的长链聚合物分子彼此连接(“交联”)。这些桥使得聚合物不能溶于水。当水或者水盐溶液渗透聚合物颗粒,该聚合物在分子水平上膨胀以及加强了该网络,使得水能够不再独力逃逸。
作为另选方案,选择的超吸收剂可以基于甲基丙烯酸、聚乙烯醇-马来酸酐共聚物、多糖-马来酸酐共聚物、马来酸衍生物、丙烯酰胺基丙磺酸共聚物、淀粉-丙烯腈接枝聚合物、糊化淀粉衍生物、烷基或羟烷基纤维素、羧甲基纤维素、淀粉-丙烯酸接枝聚合物、乙酸乙烯酯-丙烯酸共聚物、丙烯腈或丙烯酰胺共聚物。
超吸收性颗粒可采用粉末的形式存在或作为具有100μm和大约1000μm之间颗粒大小的小粒。
同样地,以上提及的超吸收性聚合物也可以是具有端羟基甲基丙烯酸酯单体的水凝胶纳米颗粒,例如甲基丙烯酸2-羟基乙酯(HEMA)和/或甲基丙烯酸-2-羟丙酯(HPMA),这是商业上可得到的,例如,如Altrazeal公司。

在另一实施例中,伤口护理制品也可包括编织的织物,该编织的织物包括部分由凝胶形成的材料制成的以及部分由非凝胶材料制成的条。例如从欧洲专利EP2153807已知这样的编织的织物。
在另一实施例中,伤口护理制品也可以形成芯,该芯可选地具有由超吸收性聚合物制成的絮状的-纤维或细丝以及采用小粒形式的超吸收性聚合物,由此小粒在若干水平中粘附或结合到纤维或细丝,以及小粒分布在芯的至少一部分的总重量的50%以上,由此小粒核纤维的混合部分存在。在这里,超吸收性聚合物的重量分数可以优选地按重量在10%和25%之间的范围中。类似的结构从传统的失禁材料已知,以及如卫生巾,对它们的缓冲性质是已知的。
在另一实施例中,伤口护理制品可以包含超吸收性纤维、超吸收性颗粒、生物组分纤维和纤维素纤维的混合物。
在另一实施例中,伤口护理制品也可以包括具有由超吸收性聚合物制成的纤维或细丝的扁平层,该超吸收性聚合物采用小粒形式粘附到所述纤维或细丝。在优选的实施例中,这导致具有至少三层的主体结构,由此两个覆盖层环绕包含超吸收性聚合物的层。

术语“渗出物”指因为血浆的炎性过程而从伤口漏出的流体。同样地,血液负责运输营养物以及其他信息化学物质,由此供应到身体的各个部分,渗出物以非常类似的方式起作用以供应到伤口床以及在那里发生的康复过程。为了实现这些许多功能,渗出物包含大量组分,导致稍微高于水的重量的特定重量。因此,该渗出物也不同于漏出物,该漏出物来自非炎性过程以及具有非常低的特定重量以及低的细胞和蛋白含量。除了为纤维组织母细胞和上皮细胞提供营养物外,由于渗出物的生长因子和细胞素的高含量,渗出物的组分在时间和空间方面影响伤口康复的各种过程。它们主要地通过血小板、角化细胞、巨噬细胞和成纤维细胞来形成。它们影响在伤口康复中涉及的各种细胞的能动性、迁移和增生。因此,到伤口床中的细胞的迁移被促进以及通过血管生成的新形成的肉芽组织的供应也被促进。渗出物也有助于伤口清洁。它包含各种丝氨酸、半胱氨酸和天冬氨酸蛋白酶以及基质金属蛋白酶,该基质金属蛋白酶的活性在严格调整过程中使损害的组织不可逆地降级,由此为康复过程的随后阶段准备伤口床。一般来说,在生理和病理渗出物之间的这样的过程中做出区别。
生理渗出物的组分特别是盐、葡萄糖、细胞素和生长因子、血浆蛋白质、蛋白酶(特别是基质金属蛋白酶)、粒细胞和巨噬细胞。
此外,根据本发明的伤口护理制品具有强的抗菌性质,该强的抗菌性质一方面是由于用于使蛋白质和细菌结合的超吸收剂的性质,以及另一方面可归因于它们的水结合性质,所述水结合性质负责对于它们的活性通过细菌提取所需的流体。
此外,可以示出,各种超吸收性材料的组合允许促炎症因子(例如,基质金属蛋白酶(“MMP”,特别是胶原酶和弹性蛋白酶)、氧基(“ROS”)IL-1β、IL-6、IL-8和TNFα)的调制。该效果也可归因于超吸收性聚合物关于蛋白质的结合性质。
此外,这样的产品也具有涂覆-溶解效果。这对生物膜和纤维蛋白涂层是特别真的。
下表示出优选的扁平层的性质的示例。在本文中,值范围要被理解为也包括限制值范围的数值。
在这里,优选地,扁平层在边缘区域中是较薄的,也就是说,例如,在边缘处它的横截面成锥形地逐渐变细。这确保在伤口的边缘处存在较少的材料。
此外,优选地,扁平层要与一侧上的薄的非织造布形成一层或在该薄的非织造布的层之下。这可以是例如由聚丙烯、聚乙烯或聚酯制成的薄的水可渗透的网,所述网的每单位面积的重量优选地在5g/m2和20g/m2之间的范围中。这样的非织造布改进层的结构粘聚力,特别地在它具有吸收的流体后。
根据本发明的伤口护理制品也可以具有包含纤维素纤维、泡沫材料、改性纤维素和/或藻酸盐的至少一个扁平层。
术语“泡沫材料”指优选地由聚氨酯制成的开孔泡沫材料或闭孔泡沫材料。
改性纤维素优选地是以下的衍生物:纤维素(优选地是纳米纤维素)、磺化的和/或磺基烷基纤维素以及它们的衍生物,优选地,乙基纤维素磺酸盐、羧烷基纤维素,优选地羧甲基纤维素、羧乙基纤维素和/或羧甲基纤维素,更复杂的纤维素衍生物,例如磺基乙基羧甲基纤维素、羧甲基羟乙基纤维素、羟丙基甲基纤维素、和酰胺化的纤维素衍生物,例如羧甲基纤维素酰胺或羧纤维素酰胺。羧甲基纤维素特别地采用羧甲基羟乙基纤维素钠的形式存在,并且是在品牌名称“Hydrofiber”下可得到的。在卫生和伤口产品中,纤维被集成到扁平基体中。因为它们吸收来自伤口渗出物的流体,纤维被逐渐转换成保留流体和不会再次释放它的凝胶垫。在本文中,纤维采用以下这样的方式来构造:伤口渗出物仅在垂直方向上吸收。这意味着,只要容量足够,该渗出物不流过伤口的边缘。以这种方式,伤口边缘的浸渍可以有效地防止。在本文中,甲壳素、壳聚糖和它们的衍生物也应被理解为纤维素衍生物。
藻酸盐从褐藻获得并编织以形成纤维网。化学上而言,它们是多糖,特别是钙和/或藻酸的钠盐。藻酸盐可以吸收高达它们自身重量20倍的流体,在其过程中伤口渗出物被储存在腔中。在藻酸盐网中包含的Ca2+离子被交换成来自渗出物的Na+离子直到在藻酸盐中Na离子的饱和度已达到。在该过程中,创伤敷料溶胀和藻酸盐纤维由于纤维的膨胀而转化成凝胶体。
此外,优选地,伤口护理制品具有至少部分包括流体可渗透的材料的外壳。这样的外壳具有多方面的功能。除其他以外,它可以防止伤口护理制品粘附到伤口;它可以防止渗出物回流到伤口;它可以有低变应原性的效果和可以防止伤口边缘的浸渍。外壳优选地至少部分通过接缝封闭,例如,粘合剂接缝或超声波接缝,并且它可以包括箔或膜(例如,由聚乙烯制成)或非织造布(例如,由聚丙烯制成)或羊毛。
尤其是,可以规定外壳具有孔,平均而言,所述孔比超吸收性颗粒小。这能够防止颗粒从外壳流出。这可以特别地通过灭菌过程来干扰,即,如果颗粒进入他们可能导致泄漏的密封屏障区域。
外壳也可以被涂覆或混合有采用元素或离子形式的重金属,例如,银、锌或铜。外壳还可以涂覆有通过疏水交互作用(诸如例如,二烷基氨基甲酰氯(DACC))结合细菌的材料。
然而,孔尺寸也影响要被吸收的渗出物的流率。这尤其适用于程度不同的疏水性聚合物材料。合适的增柔处理过程一般可提高外壳的润湿性,由此确保合适的流率,即使在小孔的情况下。
外壳的孔或网优选具有0.05mm至1.0mm的大小,优选为0.20mm至0.50mm。此外,可以优选地要由细丝或纤维部分(所述细丝或纤维部分的横截面通过外壳)定界的孔或网大约为弧形以及其弧顶面向外。
在这里,优选地
(i)在俯视图中,扁平层具有表面积(F1),所述表面积在非浸湿的状态中比外壳提供的内部的表面积(F2)小3%到75%。
(ii)外壳至少在某些部分中具有柔性的材料。
在第一种情况中,称为由扁平层的外壳形成的所谓的膨胀空间。因此,在两种情况中,确保的是,外壳不提供对由流体的吸收而引起的扁平层的体积增加的任何阻力,使得该层能够充分发挥其吸收能力。柔性材料可以包含,例如,莱卡、氨纶面料、聚丙烯、橡胶、胶乳、尼龙或类似物。

这样的网具有广泛的功能。取决于孔的配置,它可以执行阀功能,从而防止渗出物流回(特别是如果孔被配置以便为漏斗形或套环形)。它可以防止伤口护理制品附着到伤口(优选地通过使用硅树脂材料)。在适当的布置的情况下,这样的网可以具有磨蚀性质,以及因此将生物膜转移到伤口或防止它们的形成(特别是如果这些孔被配置为漏斗形或套环形)。它也可具有抗出血性质,以及在某些情况下,它可以能够通过静态交互作用(特别是与具有正净电荷的材料或聚乙烯材料一起使用)固定或结合细菌。此外,该表面可以被功能化,例如用银或硅树脂涂层。
此外,规定外壳至少部分包括浸渍的或水不可渗透的材料,或者与所述浸渍的或水不可渗透的材料形成一层或在所述浸渍的或水不可渗透的材料的层之下。这可以是涂色的或显著设计的洗涤保护(底片)。
此外,还规定伤口护理制品包含采用元素或离子形式的至少一个重金属的一部分。在最精细分散的形式中,重金属有杀菌作用,这由于大的反应表面积而可以归因于可溶性重金属离子的足够形成。
与采用元素或离子形式的至少一个重金属的掺杂可以给主扎带抗菌作用,这可以减少在感染倾向的伤口(褥疮,小腿溃疡,烧伤等)的情况下的并发症和可以同时增加创伤敷料可以保留在适当位置期间的时间。
优选地采用元素或离子形式的至少一个重金属从包含铜、锌和/或银的组中选择。上述杀菌性质对于这三种金属是特别真的。
此外,优选地扁平层或外壳可以在至少一侧上与覆盖膜形成一层或由该覆盖膜替换。优选地,所述覆盖膜具有以下性质中的至少一个:
·粘合涂敷
·流体密封性
·水-蒸汽渗透性和/或
·柔性。
在本文中,优选地覆盖薄膜延伸超过伤口护理制品的周边,并且它要应用于伤口周围的皮肤。这产生所谓的边界或岛敷料。
作为另选方案,规定外壳本身具有至少一侧上的粘合涂层。在上述情况中,该粘合涂层优选是丙烯酸酯粘合剂、硅树脂粘合剂、淀粉粘合剂、水胶体粘合剂和/或任何其它合适的生理无害的粘合剂。
所述粘合膜或上述底片能够粘附到它下面的层。在上下文中,优选给出使用柔性粘合剂,以便允许产品吸收流体时产品的体积增加。作为另选方案,然而,也可以规定覆盖膜或底片不粘附到它下面的层。
此外,根据本发明优选地伤口护理制品具有至少一个紧固元件,以便将伤口护理制品固定到身体的一部分,这可以是例如一个或多个维克罗紧固件、一种或多个粘合紧固件、一个或多个绷带夹或类似物。在优选实施例中,这些紧固元件也可以配置为压缩元件,例如作为压缩带或条,从而以这种方式形成压缩绷带。

·透明质酸(优选为超吸收性聚合物的覆盖物)

此外,优选地伤口护理制品具有一个或多个折线,破裂线或折痕。这是特别方便的,如果伤口护理制品具有一定的最小尺寸。使用这些折线,破裂线或折痕,伤口护理制品可封装,灭菌,储存和在节省空间的折叠状态中运输。仅当伤口护理制品从封装中移除时,它展开到其全尺寸。
此外,根据前述权利要求之一的伤口护理制品的使用旨在用于患者的以下部位:
·在大腿或小腿上
·在肩部、胸部或颈部区域中
·在腋部区域中
·在上或下胸部区域中

Claims (23)
1.一种扁平的伤口护理制品,具有基本上多边形或椭球底面和布置在一侧上的至少一个凹槽。
2.根据权利要求1所述的伤口护理制品,其特征在于,所述底面是矩形或从矩形得到。
3.根据权利要求1所述的伤口护理制品,其特征在于,所述底面是圆形、椭圆或椭球,或者所述底面从圆形、椭圆或椭球得到。
4.根据权利要求1至3中任一项所述的伤口护理制品,其特征在于,具有圆形的或成一定角度的角。
5.根据权利要求1至4中任一项所述的伤口护理制品,其特征在于,所述凹槽布置在一个纵向侧上。
6.根据权利要求1至5中任一项所述的伤口护理制品,其特征在于,所述凹槽的形状为圆形的一部分或椭球的一部分,优选地,所述凹槽是半圆或半椭球。
7.根据权利要求1至5中任一项所述的伤口护理制品,其特征在于,所述凹槽是有角的,优选地,所述凹槽是V形的。
8.根据前述权利要求中任一项所述的伤口护理制品,其特征在于,具有至少一个扁平层,所述至少一个扁平层包括吸收材料。
9.根据前述权利要求中任一项所述的伤口护理制品,其特征在于,包括至少一个超吸收性聚合物。
10.根据前述权利要求中任一项所述的伤口护理制品,其特征在于,还具有至少一个扁平层,所述至少一个扁平层包含纤维素纤维、泡沫材料、改性纤维素和/或藻酸盐。
11.根据前述权利要求中任一项所述的伤口护理制品,其特征在于,还具有外壳,所述外壳至少部分包括流体可渗透的材料。
12.根据权利要求8所述的伤口护理制品,其特征在于,
(i)在俯视图中,所述扁平层具有表面积(F1),在非浸湿的状态中所述表面积(F1)比所述外壳提供的内部的表面积(F2)小3%至75%,和/或
(ii)所述外壳具有至少在一些部分中是柔性的材料。
13.根据权利要求11至13所述的伤口护理制品,其特征在于,所述外壳至少部分由三维伤口隔离网组成,或所述外壳与所述三维伤口隔离网形成一层或在所述三维伤口隔离网的层之下。
14.根据权利要求11至14所述的伤口护理制品,其特征在于,所述外壳至少部分由浸渍材料或水不可渗透的材料组成,或所述外壳与所述浸渍的或水不可渗透的材料形成一层或在所述浸渍的或水不可渗透的材料的层之下。
15.根据前述权利要求中任一项所述的伤口护理制品,其特征在于,具有采用元素或离子形式的至少一个重金属的一部分。
16.根据权利要求15所述的伤口护理制品,其特征在于,所述采用元素或离子形式的至少一个重金属选自铜、锌和/或银。
17.根据前述权利要求中任一项所述的伤口护理制品,其特征在于,所述扁平层或所述外壳由至少一侧上的覆盖膜形成一层或由所述覆盖膜替换。
18.根据权利要求17所述的伤口护理制品,其特征在于,所述覆盖膜延伸超过所述伤口护理制品的外周,所述覆盖膜能够被施加于伤口周围的皮肤。
19.根据前述权利要求中任一项所述的伤口护理制品,其特征在于,所述扁平层或所述外壳在至少一侧上具有粘合涂层。
20.根据前述权利要求中任一项所述的伤口护理制品,其特征在于,还具有至少一个紧固元件,用于将所述伤口护理制品附着到身体的一部分。
21.根据前述权利要求中任一项所述的伤口护理制品,其特征在于,还具有一个或多个折叠线、破裂线或褶痕。
22.根据前述权利要求中任一项所述的伤口护理制品放置在患者的以下部位的应用:
·大腿或小腿肚上
·肩部、胸部或颈部区域中
·腋部区域中
·上或下胸部区域中
·胸骨下区域中
·上臂或前臂区域中
·眼睛以下的区域中
·骶骨区域中
·背部上
·膝盖的腔中
·腹部区域中
·耳后面或耳上方的区域中
·指间区域中,和/或
·脚区域中。
23.根据前述权利要求中任一项所述的伤口护理制品在负压伤口护理***的应用。
