背景技术 
1.技术领域
本发明整体上涉及植入件,并且特别地涉及制造具有回弹芯的矫形植入件。
2.背景技术
为了使刚性的金属基植入件有效,往往在两个刚性金属元件之间放置较软的弹性体基的部件。例如,对于用于脊椎或用于新一代基于减振盘的总盘置换物的后部动态稳定***,开发了这样的结构。在这种情况下,软弹性体设置于两个平行的金属板之间并且提供在所有的六个自由度中移动/转动的轴向减振。
然而,将软材料如弹性体结合到硬材料如金属需要细致的界面设计。
对于长期植入,需要在复杂的恒定机械应力状态下的良好粘结强度,此外,结合必须是化学稳定的(水解、氧化)。通常,以一定方法制备金属表面以改进其对聚合物的兼容性。标准方法包括使用试剂如铬酸或磷酸化学蚀刻以产生优化的粗糙度或者施加称为底胶(primer)的中间层。已知硅烷基的组合物为合适的底胶。
类金刚石碳(DLC)涂层主要用于降低磨损或避免移动部件之间的冷焊接。DLC是在其中具有或多或少的氢并且具有一些短程有序的石墨或四面体秩序的各种碳物质(sp2、sp3)的通常非晶态的混合物。由用于DLC合成的生长机制所致,这些涂层通常展现几个GPa范围内的高压缩应力。因而,金属基材上的DLC粘结往往限制其用于长期应用,并且可用合适的夹层选择使其稳定。一个实例是用于CoCrMo基材的钽(Ta)夹层。对于钛(Ti)合金,正在开发基于TaN或TaC夹层的更完善的夹层。可用表面处理如暴露于氧等离子体或暴露于紫外线调整(例如提高)DLC的表面能;使用这样的工艺,表面变成疏水或亲水的,因而可能改进其对另一种材料的亲和力(affinity)。
弹性体为可通过注射成型或挤压而制造的热塑性材料,其中在高于弹性体的熔化温度的温度下将原料粒料熔化并压入模具中。熔体在一定程度上粘合至金属表面。还可将单体注入模具中,以取代完全反应的聚合物,在这种情况下聚合在模具中发生并且单体具有粘合至存在的其它物质的额外机会。该方法称为反应注射成型(RIM)。
仍然需要改进的植入件涂层。
发明内容
本发明提供了包含至少第一基材例如具有至少1mm厚度、涂覆有由类金刚石碳(DLC)制得的第一层的金属基材的植入件。DLC层可具有所需的厚度,例如小于10微米。根据一个实施方案,DLC层可具有至少约500nm的厚度。可在DLC层上方放置另一聚合物材料层。聚合物材料可具有小于第一基材的硬度,并且可具有所需的厚度例如大于1mm。
聚合物材料可由所需的任何合适的材料制得。例如,聚合物材料可为热固性材料或热塑性材料,例如热塑性弹性体(TPE)。根据某些实施方案,聚合物材料可为聚酯-氨基甲酸酯、聚醚型聚氨酯(PEU)、聚碳酸酯-氨基甲酸酯(PCU)或含有聚二甲基硅氧烷(PDMS)的聚氨酯,其可具有极性基团,例如碳酸根基团。在一个实施方案中,聚合物材料为有机硅。有机硅材料比TPE材料软。
在这方面,相对薄的DLC层提供了使第一基材表面与聚合物材料 如热塑性材料粘合的粘结促进剂。例如,在制备第一基材后例如用第一反应性夹层通过等离子体活化化学气相沉积(PACVD)可将DLC层沉积至第一基材表面上。反应性夹层可由过渡金属(包括单质过渡金属和/或含有过渡金属的化合物或合金)制得。合适的过渡金属的实例包括钽、铬、铌和钨。作为补充或者作为替代,反应性夹层可由硅(包括单质硅和/或硅化合物)例如碳化硅、特别是非晶态的碳化硅制成。在这方面,应该理解在第一基材和DLC层之间可布置反应性夹层。
在施加另外层之前,使DLC层的外表面(即与第一基材相对的DLC层的表面)活化。可通过紫外线或通过暴露于合适的等离子体(氧、氩)使DLC层的外表面活化。随后,可用聚合物材料的注射成型或反应注射成型加工另外层。该界面设计允许适用于长期植入的牢固和生物稳定材料的组合。
在第一基材上使用DLC层,可获得与该另外层的聚合物材料的化学结合并且因而提高与第一金属基材的粘结强度。
常规的粘结促进技术通常包括非生物兼容物质如酸的使用,其需要部件清洁和工艺安全的仔细管理。另一方面,对于人类脊椎,标准底胶如硅烷的长期使用和效果是未知的。对于总盘置换物装置,在严峻条件下的粘结强度对于长期配置是需要的。界面通常经受水解/氧化侵蚀和高的动态机械应力。DLC层改进了粘结强度并且使植入件***生物稳定。
使用物理/化学气相沉积在受控的条件下进行包含第一基材和DLC层的植入件的加工,降低了错误加工的风险。通过原位诊断如质谱分析,工艺控制的进一步改进是可能的。
根据本公开的植入件提供了高连接密度、化学惰性和机械强度的优点。
第一金属基材可由CoCr合金、不锈钢、钛或钛合金制得。可如所需配置DLC层的氢含量和主要结合类型。例如,DLC可主要地四面结合有低的原子氢浓度,以提供对烃的致密化学连接、对金属的高粘结强度、整体形式的化学惰性和机械强度。或者,DLC可具有较高的氢 浓度,其可提供较高的弹性以适应所结合的弹性体的机械性质。例如,DLC层中的氢浓度可以在基本上为0到约35原子%之间改变。
根据一个实施方案,四面体的非晶态DLC具有所需的氢含量。例如,DLC可具有零或基本上为零的氢含量,例如小于约1原子%,由此提供特别硬的DLC层。可将四面体的非晶态DLC的氢含量提高至较高的量,这提供相对较软的DLC层。根据一个实施方案,氢含量可为至少约25原子%,并且在另一个实施方案中,氢含量可为约25-约35原子%。因而,随着氢含量提高,DLC层的硬度降低。此外,DLC层可具有在0到约35原子%之间任意值的氢含量,以便控制DLC层的硬度,并且沿第一基材和另外层之间的方向可具有可变的氢含量梯度。因此,DLC层的面向基材的部分可具有第一氢含量,并且DLC层的面向另外层的部分可具有不等于或不同于(例如大于或小于)第一氢含量的第二氢含量。因而,可将DLC层的各部分配置成具有各自的机械性质(例如硬度),其适应并利用界面处邻近元件的机械性质。邻近元件可为第一基材或另外层的聚合物材料、第一基材和DLC层之间的第一反应性夹层,或者作为替代,可为布置于DLC层和另外层之间的第一底胶夹层。底胶夹层用于增强DLC层和聚合物材料之间的结合特性。例如,硅烷基组合物作为底胶夹层为有用的。
应该理解,在从第一基材朝向另外层的方向上氢含量可逐渐提高。可使用标准程序如ERD(弹性回缩探测)测量氢含量。
还应该理解,可用氟或氮掺杂DLC层,由此影响DLC层的表面能。表面能可以由关于各种液体例如单体液体的润湿行为的几个因素决定,该单体液体包含掺杂后作为另外层施加至DLC层的聚合物材料的基本单元。因而,取决于所选的液体的特性,DLC层的掺杂可具有不同效果。因此,掺杂DLC层并且改变DLC层的表面能可以提供对所需的润湿行为的改进。例如,DLC层的氟掺杂可有效地降低界面处的表面能,在该界面处DLC层与极性液体单体相互作用,导致更疏水的单体,而在氮掺杂以及用氧和氮表面活化的存在下,相同的极性单体可变成更亲水的。根据一个实施方案,可以通过在乙炔沉积步骤过程中相关前 体气体的添加通过用元素如氮或硅掺杂DLC层来完成掺杂步骤。
在另一个实施方案中,植入件可包含可如上关于第一基材描述的那样而构造的第二基材。因而,可用第二DLC层涂覆第二基材,并且可以以如上关于第一基材和第一反应性夹层描述的那样的方式,将第二反应性夹层布置在第二基材和第二DLC层之间。还可将上述聚合物材料的另外层置于第二DLC层上方,使得另外层布置和连接在第一基材和第二基材之间。
根据某些实施方案,植入件是椎间植入件,其具有第一基材和第二基材,每个分别适合接触两个相邻椎骨的端板。在这方面,椎间植入件的第一基材和第二基材可提供以上述的方式涂覆的第一基材和第二基材。
椎间植入件沿中心轴延伸。例如,椎间植入件包含沿中心轴间隔开的第一骨接触板和第二骨接触板。椎间植入件还包含布置于所述第一骨接触板和第二骨接触板之间的回弹芯。因此,第一骨接触板可限定涂覆有第一DLC层的第一基材,第二骨接触板可限定涂覆有第二DLC层的第二基材,并且回弹芯可包含布置于第一DLC层和第二DLC层之间的聚合物材料另外层。回弹芯可包含单个层或多个层。例如,回弹芯可包含不同硬度(例如从硬到软)的多个层,以便例如缓和从金属基材到聚合物材料的过渡。回弹芯还可包含提供不同功能的各种添加剂,例如用于辐射不透明性的硫酸钡、用于抗微生物效应的银或银离子、和/或用于骨结合的羟基磷灰石。回弹芯的层的孔隙率也可改变,而增加的孔隙率能够将骨结合改进至骨骼可长入孔隙的程度。
根据一个实施方案,第一骨接触板限定第一内锁定零件并且所述第二骨接触板限定第二内锁定零件。第一内锁定零件和第二内锁定零件可形成互锁结构,该互锁结构允许第一骨接触板和第二骨接触板相对于彼此的有限移动和转动。应该理解,互锁结构增加了第一骨接触板和第二骨接触板以及回弹芯之间的接触表面。因此,当第一骨接触板和第二骨接触板在具有平行和垂直于植入件中心轴上均延伸的方向分量的方向上彼此发生位移时,互锁结构限制了回弹芯的变形。当第 一骨接触板和第二骨接触板相对于与中心轴成一定角度(例如垂直)的轴转动时,互锁结构也可限制回弹芯的变形。互锁结构防止回弹芯变形程度过大。
在植入件的另一个实施方案中,所述回弹芯可具有5-50MPa、优选10-20MPa的杨氏模量。
根据一个实施方案,制造植入件的方法可包括以下步骤:
a)例如在有机溶剂中超声清洁基材;
b)将清洁和干燥的基材放入真空***中,例如可将该真空***抽至约10-7毫巴的基压;
c)例如使用氩轰击来预清洁基材,该氩轰击例如可在约2·10-2毫巴下进行;
d)将DLC层沉积到预清洁的基材上。DLC层可具有所需的厚度,例如小于约10微米。可通过任何合适的沉积技术如由乙炔沉积将DLC层沉积到基材上;和
e)将一聚合物材料的另外层置于DLC层上。聚合物材料可具有所需的硬度,例如小于基材的硬度。该另外层可具有所需的厚度,例如大于1mm。
应该理解,可将各个层直接置于每个其它层上面或上方,或通过所需的中间层间接置于每个其它层上面或上方。
根据一个实施方案,例如可通过四甲基硅烷沉积或磁控溅射将硅或金属化合物的反应性夹层沉积到预清洁的基材上面或上方。随后可将DLC层沉积到反应性夹层上面或上方。在这方面,应该理解,通过作为反应性夹层而提供的中间层将DLC层沉积到预清洁的基材上面或上方。
在所述方法的另一个实施方案中,金属化合物为Ta、Cr、Nb、W、或Ti。
在另一个实施方案中,可任选地将底胶夹层布置在DLC层和回弹芯的界面处。
本文公开的结合技术可用于需要或要求将金属粘合至聚合物的应 用中。除了本文描述的特定实施方案,这样的用途还可包括由金属和弹性体部件制得的后部动态稳定杆、人造指关节、用于腰椎或用于小关节(facet joint)的熔合装置、和具有柔性软部件和金属硬质部件的仪器。本文描述的结合技术提供了例如提高的弹性、移动性和/或振动吸收,同时允许用于植入件所需的充分稳定性。

具体实施方式
参考图1-3,将矫形植入件1配置为椎间植入件1,并且其沿中心轴8延伸并且还沿垂直于中心轴8的方向延伸。植入件1包含第一基材17,第一基材17又包含第一骨接触板2和第一内锁定零件6。植入件1还包含第二基材21,第二基材21又包含第二骨接触板3和与第一内锁定零件6接合的第二内锁定零件7。植入件1还包含布置于第一骨接触板2和第二骨接触板3之间的回弹芯4。在植入件1工作期间回弹芯4可提供减振零件。第一骨接触板2和第二骨接触板3可由钛、钛合金如Ti6Al7Nb或任何备选的合适生物相容的材料如CoCr合金、不锈钢或任何所需的备选金属制得。还应该理解,第一接触板和第二接触板可限定提供第一基材17和第二基材21(在其上沉积DLC层)的区域。
第一骨接触板2限定第一外部骨接触表面11和第二面向内部的表 面9。配置外部骨接触表面11以接触邻近椎体的下端板。面向内部的表面9面向第二骨板3。第二骨接触板3同样具有第一外部骨接触表面12和第二面向内部的表面10。配置外部骨接触表面12以接触邻近椎体的上端板。内表面10面向第一骨接触板2。回弹芯4可由提供所需减振特性的任何合适的材料制得。例如,回弹芯4可包含如本文描述的另外层19。例如,回弹芯4的材料可选自嵌段聚氨酯共聚物,例如聚氨酯基的热塑性弹性体(TPE)或有机硅材料或有机硅。
第一内锁定零件6从内表面9延伸。同样地,第二内锁定零件7从内表面10延伸。根据一个实施方案,第一内锁定零件6可与内表面9为一个整体,并且第二内锁定零件7可与内表面10为一个整体。然而应该理解,作为替代,锁定零件6和7可分离地连接至各自的内表面9和10。可组合第一互锁零件6和第二互锁零件7以提供植入件1的互锁结构。
因此,椎间植入件1包含:
·第一基材17,其包含所述第一骨接触板2和所述第一内锁定零件6;
·具有小于10微米厚度的第一DLC层18,其置于所述第一骨接触板2的所述内表面9和所述第一内锁定零件6的表面上方;
·回弹芯4,其包含布置于所述第一骨接触板2和第二骨接触板3之间的聚合物材料另外层19;
·第二基材21,其包含所述第二骨接触板3和所述第二内锁定零件7;和
·具有小于10微米厚度的第二DLC层18,其置于所述第二骨接触板3的所述内表面10和所述第二内锁定零件7的表面上方。
在这方面,将第一DLC层18布置在回弹芯4和第一基材17之间,而将第二DLC层18布置在回弹芯4和第二基材21之间。此外,可将底胶夹层布置在第一DLC层18和另外层19之间以及第二DLC层18和另外层19之间。底胶夹层可促进DLC层18和分别的另外层19的聚 合物材料之间的结合(例如化学和/或机械)。例如可通过硅烷如γ甲基丙烯酰氧丙基三甲氧基硅烷(Primer A-174)形成底胶夹层。
将DLC层18附着至所述第一骨接触板2和第二骨接触板3的金属基材17、21促进了回弹芯4的聚合物材料以及所述第一骨接触板2和第二骨接触板3之间的化学结合,因而提高了回弹芯4与所述第一骨接触板2和第二骨接触板3的金属基材的粘结强度。
将第一内锁定零件6配置为第一突出物,形成从所述第一骨接触板2的内表面9凸出的栓(peg)13。栓13可限定沿垂直于中心轴8的方向的椭圆横截面积。该栓还与中心轴8同轴延伸。将第二内锁定零件7配置为从所述第二骨接触板3的内表面10延伸的第二突出物。第二突出物可限定与中心轴8同轴延伸的椭圆槽14。因而,第二内锁定零件7限定了环形壁15,该环形壁15限定了椭圆槽14。椭圆槽14沿垂直于中心轴8的方向具有椭圆横截面积。椭圆槽14的横截面积大于所述栓13的横截面积,使得当在所述椭圆槽14内接收所述栓13时保留了横断中心轴8的间隔。由于所述间隔,互锁结构允许第一骨接触板2和第二骨接触板3相对于彼此移动和转动。第一骨接触板2和第二骨接触板3之间可能的相对运动受互锁结构限制到约0mm-约2mm,例如约1mm。此外,互锁结构增加了第一骨接触板2和第二骨接触板3中的每个与回弹芯4之间的接触表面。此外,第二内锁定零件7的环形壁15可包含多个穿孔16,使得回弹芯4的聚合物材料可透过穿孔16进入栓13和所述内表面15之间的所述间隔,由此提供所述回弹芯4与第二骨接触板3改进的固定。
制造
与另外层19的聚合物材料结合的DLC层18的表面可经受活化处理,该活化处理可在将另外层19结合到DLC层18之前不久进行,由此避免损坏。活化处理从DLC层18表面移除污染物,并且在DLC层18表面上产生适用于与另外层19的聚合物材料连接的悬空键。可使用工艺如将DLC层18的表面暴露于大气压等离子体(例如氧、空气、氩等)或暴露于短波紫外线辐射,完成活化处理。可进行化活化处理 以避免表面的显著石墨化。另外层19的聚合物材料可为热塑性弹性体(例如聚二甲基硅氧烷基的嵌段聚氨酯)或热固性材料。在施加DLC层18之前,可使基材17、21表面结构化以获得因表面几何形状效应所致的额外的粘结提高。
实施例(弹性体减振元件)
因而应该理解,可提供生物稳定的金属-弹性体连接,例如当包含于椎间植入件中时其能够承受体内的动态加载。这样的连接对于将骨结合板(例如表面处理的TiAlNb合金)连接至用于关节置换物(脊椎盘、人工指关节等)的减振元件是需要的。
可使用乙炔等离子体通过等离子体辅助化学气相沉积(PACVD)制造DLC层18,从而将DLC层18沉积到基材17和21上。可通过在涂覆工艺中使用合适的反应性夹层***获得稳定的连接(TiAlNb基板-DLC)。适用作反应性夹层的组合物可包括硅如单质硅或硅化合物如硅化物。作为替代或者补充,反应性夹层可由钽(包括单质钽和钽化合物,如碳化钽和氮化钽)制得。将DLC层18沉积到基材上的该工艺可描述如下:
1.在丙酮/乙醇混合物中超声清洁板材;
2.将清洁和干燥的板材放入真空***中并且抽至例如10-7毫巴的基压。
3.通过在例如2·10-2毫巴下用-600V RF(射频)自偏压的氩轰击30分钟来预清洁板材;
4.通过在例如-600V RF自偏压下由四甲基硅烷沉积10分钟来沉积100nm硅夹层;和 
5.通过在例如-600V的RF自偏压下由乙炔沉积15分钟来沉积500nm厚的DLC层。
在将另外层附着到DLC层之前,可以以上述的方式使DLC层活化。活化移除表面污染物并且通过氢移除和氧附着而使DLC层的表面极化,因而提高DLC层的表面能。因此,DLC表面具有用于另外层的聚合物材料的随后附着的高密度停泊位点(docking site)。等离子体活化 工艺可描述如下:
1.通过在丙酮/乙醇混合物中超声5分钟来预清洁样品;
2.在惰性气体射流中干燥样品并且将它们放置在基压能够<1·10-4毫巴的真空腔室中;
3.通过ECR(电子回旋共振)、RF等将离子密度1·1010/cm3的等离子体(氩、氧或氮)施加至DLC层持续至少20分钟。样品表面处的离子能量应该不会引起显著的溅射,并且样品应该不会显著升温;和
4.从真空腔室移除样品并且在等离子活化处理后将它们模制不迟于20分钟。
一旦DLC层的表面被活化,例如就通过注射成型将另外层附着到DLC层。根据一个实施方案,可通过反应注射成型(RIM)将另外层附着到DLC层。
在上述等离子体活化的一个替代性实施方案中,使用紫外线或电子束活化也可使DLC层活化。在这样的紫外线活化中,工艺步骤3可描述如下:
3.将具有大于4.5eV能量的高强度紫外线辐射施加至DLC层持续至少10分钟。
在这样的电子束活化中,工艺步骤3可描述如下:

Claims (23)

1.一种矫形植入件,其包含具有至少1mm厚度的第一金属基材,该第一金属基材涂覆有
具有小于10微米厚度的第一类金刚石碳(DLC)层;和
置于所述DLC层上方的聚合物材料的另外层,所述聚合物材料不如所述基材硬并且具有大于1mm的厚度,
置于第二基材上方的具有小于10微米厚度的第二DLC层,所述第二基材具有至少1mm的厚度;
使得该另外层在第一DLC层和第二DLC层之间。
2.根据权利要求1的植入件,其中所述基材由CoCr合金、不锈钢、钛或钛合金制得。
3.根据在前权利要求中任一项的植入件,其中所述DLC层具有至少500nm的厚度。
4.根据权利要求1或2的植入件,其中所述DLC层由非晶态DLC制得。
5.根据权利要求4的植入件,其中所述DLC层由四面体的非晶态DLC制得。

11.根据权利要求1-2、5和7-9中任一项的植入件,其中所述DLC层掺杂有氟或氮。
12.根据权利要求1-2、5和7-9中任一项的植入件,其中所述聚合物材料为热塑性弹性体(TPE)。
13.根据权利要求1-2、5和7-9中任一项的植入件,其中所述聚合物材料为聚酯-氨基甲酸酯、聚醚聚氨酯(PEU)、聚碳酸酯-氨基甲酸酯(PCU)或含有聚二甲基硅氧烷(PDMS)的聚氨酯。
14.根据权利要求13的植入件,其中所述含有聚二甲基硅氧烷(PDMS)的聚氨酯具有极性基团。
15.根据权利要求1-2、5、7-9和14中任一项的植入件,其中所述聚合物材料为有机硅材料。
16.根据权利要求1-2、5、7-9和14的任一项的植入件,其中所述植入件为椎间植入件,其包含适合接触两个邻近椎骨端板的第一基材和第二基材。
17.根据权利要求16的植入件,其包含中心轴、限定涂覆有第一DLC层的第一基材的第一骨接触板、限定涂覆有第二DLC层的第二基材的第二骨接触板以及布置于所述第一骨接触板和第二骨接触板之间包含所述另外层的回弹芯。
18.根据权利要求17的植入件,其中所述第一骨接触板包含第一内锁定零件并且所述第二骨接触板包含第二内锁定零件,并且其中所述第一内锁定零件和第二内锁定零件形成互锁结构,该互锁结构配置成允许第一骨接触板和第二骨接触板相对于彼此的有限位移和转动。
19.根据权利要求17的植入件,其中所述回弹芯具有5-50MPa的杨氏模量。
20.根据权利要求19的植入件,其中回弹芯具有10-20MPa的杨氏模量。
21.用于制造根据在前权利要求中任一项的植入件的方法,其包括:
a)预清洁基材;
b)在基材上沉积DLC层;和
c)在DLC层上方施加另外层。
22.根据权利要求21的方法,还包括在步骤a)和b)或者b)和c)之间沉积反应性夹层,其中反应性夹层为含有硅或金属的化合物。
23.根据权利要求22的方法,其中含有金属的化合物选自Ta、Nb、W、Ti及其混合物和合金。
