“抗体片段”包含全长抗体的一部分,该部分通常指所述全长抗体的抗原结合区或可变区。抗体片段的例子包括:Fab、Fab’、F(ab’)2和Fv片段;双抗体;线性抗体;由Fab表达文库生成的片段;抗独特型(抗Id)抗体;CDR(互补决定区);ECD(胞外域);和以免疫特异性方式结合癌细胞抗原、病毒抗原或微生物抗原的上述任意各项的表位结合片段;单链抗体分子;和由抗体片段形成的多特异性抗体。
Fab片段还包含轻链的恒定域和重链的第一恒定域(CH1)。Fab’片段与Fab片段的不同之处在于重链CH1结构域的羧基末端增加了少数残基,包括来自抗体铰链区的一个或多个半胱氨酸。Fab’-SH是本文中对其中恒定域半胱氨酸残基携带至少一个游离硫醇基的Fab’的称谓。F(ab’)2抗体片段最初是作为在成对Fab’片段之间有铰链半胱氨酸的成对Fab’片段生成的。还知道抗体片段的其它化学偶联形式。
“单克隆抗体”指从一群基本上同质的抗体获得的抗体,即构成群体的抗体个体是相同的,除了可以以极小量存在的可能的天然存在突变形式外。单克隆抗体是高度特异性的,针对单一抗原性位点。此外,与包括针对不同决定簇(表位)的不同抗体的多克隆抗体制备物不同,每种单克隆抗体针对抗原上的单一决定簇。在它们的特异性之外,单克隆抗体的优势在于它们可以在未受到其它抗体污染的情况中合成。修饰语“单克隆”表明抗体自基本上同质的抗体群获得的特征,且不应解释为要求通过任何特定方法来生成抗体。例如,可依照本发明使用的单克隆抗体可通过最初由Kohler et al(1975)Nature 256:495记载的杂交瘤方法来制备,或者可通过重组DNA方法来制备(参见US 4816567)。“单克隆抗体”也可使用例如Clackson et al(1991)Nature,352:624-628;Marks et al(1991)J.Mol.Biol.,222:581-597中记载的技术自噬菌体抗体库分离。
“半胱氨酸改造抗体”为自野生型或亲本抗体通过引入一个或多个游离半胱氨酸氨基酸而改造的抗体。“游离半胱氨酸氨基酸”为已改造入亲本抗体,具有硫醇官能团(-SH),且未作为分子内或分子间二硫桥配对或以其它方式作为分子内或分子间二硫桥一部分的半胱氨酸氨基酸残基。游离半胱氨酸氨基酸可在抗体的重链、轻链或Fc区中。改造的半胱氨酸残基(“游离半胱氨酸硫醇”)能与硫醇反应性标记试剂反应。半胱氨酸改造抗体包括FAB抗体片段(thioFab)和表达的全长IgG单克隆(thioMab)抗体(US 2007/0092940;WO 2008/141044,通过述及收录其内容)。已在新引入的半胱氨酸硫醇处经由接头将ThioFab和ThioMab抗体与硫醇反应性接头试剂和药物-接头试剂缀合以制备抗体-药物缀合物。
“PEG”指聚(乙二醇)的片段、氧化乙烯的聚合物,而且包括2个或更多个乙撑氧基单位(-CH2CH2O-)。
标记试剂的合成
本发明包括一种模式平台,其用于快速制备能够有效地将18F引入蛋白质的各种水溶性辅基(Gill et al(2009)Jour.Med.Chem.52(19):5816-5825)。这种平台的效用通过硫醇特异性的辅基和标记试剂,[18F]FPEGMA 5(其在两步一锅合成中快速生成)示范(图1)。此外,为了提高[18F]FPEGMA与蛋白质在水性条件下所得的缀合效率和水溶性,使用基于聚乙二醇(PEG)的“构件块”。用4D5ThioFab评估作为辅基的[18F]FPEGMA 5,所得缀合物(18F-4D5ThioFab)在体内在受Hsp90抑制剂调控的人肿瘤异种移植物鼠模型中作为HER2表达水平的显像剂得到验证(Smith-Jones et al(2006)J Nucl Med47:793-6;Smith-Jones et al(2004)Nat Biotechnol 22:701-6)。
例示性实施方式[18F]FPEGMA 5的效用示范了多种涵盖的18F标记的辅基和标记试剂(图3),包括硫醇特异性的溴乙酰胺化合物[18F]FPEGBA、氨基特异性的N-羟基琥珀酰亚胺(NHS)化合物[18F]FPEGNHS、叠氮化物特异性的化合物[18F]FPEG-炔丙基、和炔烃特异性的化合物[18F]FPEGN3。其它硫醇特异性的官能团可用于18F标记的辅基和标记试剂,诸如作为Michael受体的乙烯基砜、作为另外的硫烷基化/烃基化试剂的氯乙酰胺和碘乙酰胺。用于标记赖氨酸和蛋白质的其它氨基基团的氨基特异性官能团包括各种活化的羧酸酯,诸如四氟苯基酯、五氟苯基酯、N-羟基琥珀酰亚胺、和硫羟基琥珀酰亚胺(sulfohydroxysuccinimide)。
一种有用的合成规程是通过1,3-偶极环加成(有时称为“Click化学”)从叠氮化物和炔中间体制备标记试剂的铜(I)催化的叠氮化物-炔环加成(CuAAC)。CuAAC反应与大部分官能团(包括在生物分子诸如蛋白质上找到的那些)的正交性(orthogonality)和快速反应速率提高形成产物的1,2,3-***基的稳定性和高收率。如此,CuAAC提供了一种使含有叠氮化物和炔官能团的杂双官能前体偶联的有力方法。

一个例子是在两步一锅合成(图1)中使用铜催化剂Cu(MeCN)4PF6快速生成的携带马来酰亚胺的辅基,[18F]FPEGMA 5(实施例5)。可采用多种其它铜催化剂来形成***,包括CuSO4、CuI、CuBr、CuOTf(US 7375234)。该铜催化剂可为水合物或溶剂化形式。所使用的铜试剂可为Cu0种类诸如铜线、CuI盐或CuII盐(在还原剂诸如抗坏血酸钠存在下)。将双官能叠氮化物-甲苯磺酸酯中间体N3-PEG-OTs(其中PEG是2至12个亚乙基氧基团(-CH2CH2O-)的聚亚乙基氧单元),诸如23-叠氮基-3,6,9,12,15,18,21-七氧杂二十三烷-1-基对甲苯磺酸酯3(实施例3)用18F阴离子氟化以生成放射化学中间体N3-PEG-18F,实例是23-叠氮基-1-氟-3,6,9,12,15,18,21-七氧杂二十三烷4(实施例4)。此外,为了提高所得[18F]FPEGMA 5与蛋白质在水性条件中的缀合效率和水溶性,使用基于聚乙二醇(PEG)的“构件块”。用4D5ThioFab评估作为辅基的[18F]FPEGMA 5,而且所得缀合物(18F-4D5ThioFab 6)在体内在受Hsp90抑制剂调控的人肿瘤异种移植物鼠模型中作为HER2表达水平的显像剂得到验证(Smith-Jones et al(2006)J Nucl Med 47:793-6;Smith-Jones et al(2004)Nat Biotechnol 22:701-6)。
合成[18F]FPEGMA的考虑因素包括使用具有PEG链的水溶性构件块、执行微波加速的加热、经CuAAC体系偶联反应物、和优化[18F]FPEGMA 5对蛋白质诸如4D5ThioFab的位点特异性反应及后续生物学确认。亲水性辅基可提供与4D5ThioFab和其它亲水性蛋白质的优异的缀合动力学和效率。因此,调查了带有PEG链的构件块,因为它们改善水溶性而不损害18F的亲核掺入所需要的在有机溶剂中的溶解度。本发明的辅基比自[18F]FBALD平台衍生的化合物水溶性更高。例如,为[18F]5测得的logP(-2.41±0.09)与为[18F]FBOM报导的值相差3个单位,与[18F]FBAM相差超过5个单位(Wuest et al(2009)AminoAcids 36:283-295)。水溶性辅基在缀合至4D5ThioFab和可能一般性的亲水性蛋白质时的优点通过[18F]5提供了在相当的条件下与[18F]FBAM相比更优的缀合效率的观察结果而得到验证。
微波加热与常规的加热条件相比提高了[18F]4的收率并缩短了[18F]5的总合成时间。对于方法A和B,在3的18F-氟化、从TBAHCO3或K222/K2CO3洗出液共沸除去水、和在氧化铝处理[18F]4之后蒸发乙腈的过程中观察到溶剂加热的显著加速。微波加热对于一般放射化学用途的这些优点是普遍的。然而,一条重要的告诫是当使用高微波功率(50W以上)或温度(100℃以上)时,在共沸除去水的过程中观察到K222/K2CO3降解。因此,需要温和的微波加热条件来从K222/K2CO3洗出液共沸除去水,这提供与常规的加热条件相比,相当的蒸发时间和相当至更优的18F氟化收率。
用来偶联叠氮化物和携带炔烃的前体试剂并产生标记试剂[18F]5的CuAAC方法是基于观察到的反应速率、转化效率和纯度选择的(实施例5)。使用方法A观察到的低纯度的[18F]5可归于基于马来酰亚胺的化合物在水性条件中在20分钟反应时间里的不稳定性。使用已经确定为用于改善CuAAC反应速率和与蛋白质缀合的正交性的两种二齿配体,BPDS和TBTA(Lewis etal(2004)J Am Chem Soc 126:9152-3;Rodionov et al(2007)J Am Chem Soc129:12705-12;Rodionov et al(2007)J Am Chem Soc 129:12696-704)。当将BPDS添加至方法A的反应体系时对于[18F]5观察到纯度的显著增加可源自方法B的短反应时间,其限制了[18F]5在水性条件中的降解。这通过在方法A和B之间[18F]5随时间降解一般来说是相当的观察结果而得到加强。对于方法A和B,TBAHCO3提供比K222/K2CO3显著更少的18F标记的降解产物,这归于马来酰亚胺在水性条件中对碱性试剂的敏感性。当在CuAAC之前引入氧化铝处理时观察到[18F]5的纯度增加进一步指示该产物对碱的敏感性。
尽管在方法B中观察到反应时间和纯度的改进,但是在有机溶剂中实施的CuAAC反应会提高[18F]5的稳定性。这促使了对方法C的调查,方法C在无水乙腈中实施,在无水乙腈中[18F]5是稳定的且甚至是容忍碱性试剂,诸如2,6-二甲基吡啶。然而,为了防止后续[18F]5降解,将CuAAC反应用含有0.01%三氟乙酸的水猝灭。在此例子中,方法C提供与方法A相比显著更高的纯度,且与方法B相比相当的纯度。此外,对于更加水不安定的辅基,诸如[18F]FPEGNHS,方法C提供与方法A和B相比显著更高的纯度。
使用[18F]4或[18F]FPEG4N3用方法B和C获得的相当的收率。对于[18F]FPEG2N3观察到的有限的水溶性区别出方法C。此外,方法C不需要18F氟化和CuAAC反应步骤之间的氧化铝处理。因此,方法C可较好地适合于一般放射化学应用,便于开发一致的放射化学工艺用于生产各种18F标记的辅基。这可具有对自动化来说强的含意,并且可消除将合成模块给予特定辅基的需要,正如[18F]SFB的情况。
方法B主要用于生产[18F]5,因为方法C的纯化发生于[18F]5的优化和对4D5ThioFab的确认之后。但是,方法C已经确认提供[18F]5和18F-4D5ThioFab,收率与方法B相当。如此,方法B和C都可用于使用[18F]5制备18F标记的蛋白质,并且可适合于使用其它放射性核素的更宽的放射化学应用。
CuAAC反应的正交性有助于从普通的反应平台开发各种18F标记的辅基。通过更改特定性质,诸如化学选择性和中间体链结构,数种辅基能够将18F引入蛋白质和潜在的肽(图5)。在[18F]5之外,使用方法C将来自商品化前体N3-PEG4-NHS的氨基特异性辅基[18F]FPEGNHS 13缀合至4D5ThioFab。在提高[18F]FPEGNHS的收率和确保稳定性之外,本发明的标记物可限制随机氨基特异性修饰的免疫反应性特征的降低。例如,蛋白质结合位点内的[18F]FPEGNHS的空间位阻可通过使用具有较小中间体链的构件块来减小。因此,对于实施氨基特异性缀合来说,[18F]FPEGNHS是[18F]SFB的一种替代。此外,硫醇特异性溴乙酰胺类似物[18F]FPEGBA可用于使用方法B标记蛋白质。
多种18F标记的辅基是可获得的,包括硫醇特异性的溴乙酰胺化合物[18F]FPEGBA、氨基特异性的N-羟基琥珀酰亚胺(NHS)化合物[18F]FPEGNHS、叠氮化物特异性的化合物[18F]FPEG-炔丙基、和炔烃特异性的化合物[18F]FPEGN3。
详细的合成方案和分析数据示于实施例中。图3和实施例概述了标记中间体TsO-PEG8-N3 3、TsO-PEG4-N37和TsO-PEG2-N38(生产18F标记的辅基的一般效用),和炔丙基-PEG6-马来酰亚胺2(合成[18F]5([18F]FPEGMA)特异性的前体)的制备。化合物3从商品化杂双官能HO-PEG8-N3和甲苯磺酰氯在吡啶中以25%的收率合成。化合物7和8分别以30%和19%的收率通过在DMF中加热等摩尔量的NaN3和适宜的PEG-二甲苯磺酸酯至110℃而合成。从六乙二醇合成2需要两个步骤;Williamson醚合成使用等摩尔量的NaH和炔丙基溴进行,并提供54%收率的炔丙基-PEG6-OH 1,接着与过量的马来酰亚胺进行PPh3/DIAD介导的Mitsunobu反应,其提供25%收率的2。
叠氮化物试剂,N-(20-叠氮基-3,6,9,12,15,18-六氧杂二十烷基)-2-溴乙酰胺9和N-(20-叠氮基-3,6,9,12,15,18-六氧杂二十烷基)-2-碘乙酰胺9a从20-叠氮基-3,6,9,12,15,18-六氧杂二十烷-1-胺(NH2-PEG7-N3)制备。9和9a的溴乙酰基和碘乙酰基分别与蛋白质的某些亲核官能团诸如半胱氨酸硫醇基团反应形成叠氮基-蛋白质中间体。该叠氮基-蛋白质中间体能与具有下式的18F-PEG-炔试剂在铜催化剂存在下反应形成经过标记的蛋白质:

微波加速的亲核18F-氟化。选择四丁基碳酸氢铵(TBAHCO3)(而不选择Kryptofix 222和K2CO3(K222/K2CO3))作为相转移催化剂用于[18F]FPEG2N3、[18F]FPEG4N3、和[18F]FPEG8N3([18F]4)的亲核氟化。由于TBAHCO3相对于K222/K2CO3展现出更高的热稳定性,高温微波加热加速了5分钟内共沸除去水。此外,对于[18F]4在乙腈中来说,微波加热提供高的在3分钟里的18F氟化效率(89.0±1.4%,n=5),具有与[18F]FPEG4N所观察到的相当的收率。然而,由于化合物在氟化和蒸发加热步骤期间所使用的温度的挥发性(一个通过使用可再密封的反应容器盖纠正的问题),观察到[18F]FPEG2N3的稍微降低的收率。
制备[18F]5使用CuAAC反应,对于[18F]4和2的缀合,评估三种催化体系(表1)。方法A(实施例5),其采用CuSO4.5H2O和抗坏血酸钠作为CuI的来源(Glaser et al(2007)Bioconjug Chem 18:989-93),在20分钟里以79.3%的转化效率和65.7%的纯度提供期望产物。观察到的长反应时间和有限纯度促进了对能够加速CuAAC反应的配体的调查。方法B将CuI配体,红菲绕啉二磺酸盐(BPDS)添加至方法A,其将反应时间显著缩短至1分钟,将转化效率提高至95.6%,并将纯度提高至88.7%。对于方法A和B,当[18F]4的合成用TBAHCO3催化时(与K222/K2CO3相比)和当[18F]4用Alumina N Light筒在CuAAC反应之前纯化时,18F标记的降解产物的形成降低了。
方法C采用乙腈相容试剂诸如Cu(MeCN)4PF6作为CuI的来源,三[(1-苄基-1H-1,2,3-***-4-基)甲基]胺(TBTA)作为CuI配体,和2,6-二甲基吡啶作为碱催化剂(Lewis et al(2004)J.Am.Chem.Soc.126:9152-9153;Chan et al(2004)Org.Lett.6:2853-2855)。使用方法C,反应时间(不到5分钟)、18F标记的降解产物的形成(10%)、和转化效率(100%)与方法B相当。此外,[18F]4在CuAAC反应之前的氧化铝纯化不影响[18F]5的纯度,并因此不包含在方法C中。
在SPE处理之后,方法B在47±1min(n=5)中,以59±4%的收率、94%的放射化学纯度、和5.1±1.5Ci/μmol的比活性(针对19F标记的5的HPLC标准曲线确定的)提供[18F]5。[18F]5的身份(identity)通过与19F标记的标准品的HPLC共洗脱和衰变产物的LC/MS分析来确认。[18F]5的亲脂性(logP)(-2.41±0.09)在pH 7.4依照已公开的方法(Rodionov et al(2007)J Am Chem Soc129:12705-12)来测量。

所述方法快速且有效,在82分钟里以11±3%的总收率和2.0±0.2Ci/μmol的比活性提供18F放射性标记的蛋白质(18F-4D5ThioFab)(Gill et al(2009)Jour.Med.Chem.52(19):5816-5825)。18F-4D5ThioFab保留天然蛋白质的生物学活性且用受Hsp90抑制剂17-AAG调控的携带异种移植物肿瘤的鼠模型中的HER2表达的microPET成像成功地获得了体内确认。作为蛋白质浓度、pH、和时间的函数测试了[18F]5和4D5ThioFab的缀合效率(表2)。随着蛋白质浓度和pH升高,观察到缩短的缀合时间和升高的效率。相对于pH 6.5,在pH 8没有观察到[18F]5降解或4D5ThioFab聚集的显著增加。

在NAP-5脱盐柱和Bio-Sep S-2000SEC-HPLC柱(System D)上实施18F-ThioFab的纯化。NAP-5方法提供未缀合的[18F]5与4D5ThioFab的显著共洗脱,特别是对于缀合效率低的反应。Bio-Sep S-2000SEC-HPLC将18F-4D5ThioFab与18F标记的聚集物和[18F]5(和18F标记的降解产物)分开。通过HPLC***A、***C、和TOF LC/MS来分析纯化后的终产物18F-4D5ThioFab。经过优化的缀合规程提供以26±7%收率、大于90%的发射化学纯度、和2.0±0.2Ci/μmol(n=5)比活性的18F-4D5ThioFab。总合成时间为自合成开始起82±4min且总衰变修正收率为11±3%。

在作为用于合成[18F]FPEGMA的前体(图1)之外,可经CuAAC反应将[18F]FPEG-炔丙基和[18F]FPEGN3直接缀合至蛋白质(图2)。此办法reduces 18F标记的蛋白质的合成两个放射化学步骤且为实施位点特异性缀合提供一种备用方法。将炔丙基-PEG-马来酰亚胺(2)预缀合至ThioFab,接着是使用BPDS/Cu(MeCN)4OTf***(3∶1,3mM BPDS),[18F]4和炔丙基-PEG-ThioFab之间的直接CuAAC,观察到高达15%的缀合效率。采用一种类似的方法来以高缀合效率将[18F]FPEG3-炔丙基偶联至叠氮化物-PEG修饰的交联的右旋糖酐氧化铁(CLIO)纳米颗粒,该高缀合效率源自存在众多叠氮化物位点(Devaraj et al(2009)″18F Labeled Nanoparticles for in Vivo PET-CT Imaging″Bioconjug Chem 10.1021/bc8004649)。此技术可提供高比活性的18F标记的蛋白质,因为[18F]4是稳定的且可将多个叠氮化物或炔基团选择性放置在蛋白质上而没有交叉反应性的潜力。然而,由于CuII在抗坏血酸盐存在下趋于降解蛋白质(Devaraj et al(2009)″18F Labeled Nanoparticles for in Vivo PET-CTImaging″Bioconjug Chem 10.1021/bc8004649),因此此方法要求严格排除氧、使用还原剂、或使用CuI稳定性配体诸如BPDS来限制蛋白质切割。最后,[18F]FPEG-炔丙基和[18F]FPEGN3是18F标记的辅基,用于开发18F标记的肽。事实上,最近与方法C类似,用叠氮化物修饰的肽评估了携带炔丙基的辅基[18F]FPy5yne(Inkster et al(2008)Journal of Labelled Compounds andRadiopharmaceuticals 51:444-452)。
在反应速率和反应物稳定性方面优化[18F]5对4D5ThioFab的位点特异性缀合。关于携带硫醇的蛋白质的稳定性(其一般随pH和蛋白质浓度升高而降低),没有观察到在用于生成表2的pH(6.5-8)和蛋白质浓度(1-2mg/mL)的范围4D5ThioFab二聚化和聚集的增加。然而,携带硫醇的蛋白质形成不想要的二硫键有可能取决于硫醇基团的可及性。对于4D5ThioFab,选择最佳半胱氨酸替代位置的PHESELECTOR方法可提升硫醇稳定性,从而能够将更高的pH和蛋白质浓度使用延长的持续时间。在表2中测试的范围上没有观察到缀合反应期间[18F]5的稳定性对pH的依赖性,暗示将外部试剂(诸如自HPLC柱或SPE筒沉积的二氧化硅)引入缀合反应***能够以浓度依赖性方式降解[18F]5。因此,在清洗玻璃器皿和选择适宜溶剂、HPLC柱、和SPE筒时应当小心。考虑到针对4D5ThioFab观察到的pH独立性和[18F]5稳定性,选择碱性反应条件(pH 8)来促进[18F]5完全降解发生前更高的缀合效率。
[18F]FPEGMA 5的稳定性对缀合效率和比活性具有显著影响。[18F]5对4D5ThioFab的缀合效率取决于完整5(18F和19F标记的)的总量对4D5ThioFab的总量(因为每个ThioFab具有一个可及的硫醇基团)。自[18F]5的总活性、比活性、和降解分数来计算完整5的总量。例如,假设在没有[18F]5降解的时刻,[18F]5的报告值5Ci/μmol和200mCi提供40nmol总5。因此,获得最大缀合效率需要40nmol硫醇基团(约2mg 4D5ThioFab);然而,因为在例行制备期间使用1mg 4D5ThioFab,所以预期缀合效率为约50%。26.7%的观察缀合效率指示只有约一半的总4D5ThioFab已缀合,这一般通过对通过质谱术为衰变的18F-4D5ThioFab获得的天然和缀合峰积分来确认。观察相对于预期缀合效率的偏差是与ThioFab缀合期间用作限制试剂的[18F]5降解的结果。缀合效率和比活性的进一步改善可源自使用更高的蛋白质浓度或更稳定的马来酰亚胺构建块。
18F标记的蛋白质的成像
本发明的经过标记的半胱氨酸改造的抗体作为成像生物标志物和探针是有用的,其通过生物医学和分子成像的各种方法和技术来进行,诸如:(i)MRI(磁共振成像);(ii)MicroCT(计算机化断层摄影术);(iii)SPECT(单光子发射计算机化断层摄影术);(iv)PET(正电子发射断层摄影术)(Chen etal(2004)Bioconjugate Chem.15:41-49);(v)生物发光;(vi)荧光;和(vii)超声。免疫闪烁成像术是一种成像规程,其中将用放射性物质标记的抗体施用于动物或人类患者并对身体中抗体定位的部位拍摄照片(US 6528624)。成像生物标志物可客观测量并作为正常生物学过程、致病过程、或对治疗性干预的药理学响应的指示剂来评估。生物标志物可以是数种类型的:0型是疾病的天然历史标志物且与已知的临床指标(例如类风湿性关节炎中滑膜炎症的MRI评估)纵向关联;I型标志物依照作用机制来捕捉干预的影响,即使机制可能与临床结果无关联;II型标志物发挥替代终点的功能,其中生物标志物的变化或来自生物标志物的信号预示临床益处以“确认”目标响应,诸如类风湿性关节炎中通过CT测量的骨质侵蚀。成像生物标志物如此可提供关于下述各项的药效学(PD)治疗信息:(i)靶蛋白的表达,(ii)治疗剂对靶蛋白的结合,即选择性,和(iii)清除和半衰期药动学数据。体内成像生物标志物相对于基于实验室的生物标志物的优势包括:非侵入性处理,可量化,全身评估,重复剂量给药和评估(即多个时间点),和自临床前(小型动物)至临床(人)结果的潜在可转换效应。对于一些应用,生物成像取代临床前研究中的动物实验或将临床前研究中动物实验的数目降至最低。
Hsp90靶向疗法和microPET成像。根据17-AAG处理前1天通过18F-4D5ThioFab测定的肿瘤摄取和体积,将小鼠分配入两个小组(n=4)(Gillet al(2009)Jour.Med.Chem.52(19):5816-5825)。在第0天将17-AAG施用于处理组,而对照组动物不做处理。在第1天(施用最后一剂17-AAG后14小时)、第5天、和第7天用18F-4D5ThioFab实施PET成像。穿过对照动物和用17-AAG处理的动物(治疗前一天和治疗后一天)的肿瘤的冠状切片显示于图5a。对照和处理组在一周时段里的平均肿瘤摄取显示于图5b,而选定组织和分泌器官中的摄取列于表3。第1天的PET成像揭示了与对照组(P=0.00046)和第-1天测量的摄取(P=0.00025)相比肿瘤摄取降低50%(表3)。18F-4D5ThioFab的肾清除占优势,膀胱中有显著的放射性积累且肾皮层中有高摄取(表3)。肝胆路径也有助于示踪剂***,导致放射性代谢物在大肠和胆囊中的积累,而放射性在肝中的保留相对较低。体重和肿瘤尺寸在成像的7天期间没有变化。

对18F-4D5ThioFab观察到的来自[18F]5的高比活性能够降低4D5ThioFab的总注射剂量,这可证明对于受体饱和和靶物摄取(特别是对于具有低细胞表面受体表达水平的细胞)是重要的。然而,对于下文讨论的17-AAG调控研究,肿瘤摄取不依赖于总4D5ThioFab剂量(30-120μg/动物)。18F-4D5ThioFab的剂量不依赖性肿瘤摄取可能源自高HER2表达水平的BT474M1细胞系,这限制受体饱和在所应用的剂量的潜力。
优化[18F]5对4D5ThioFab的缀合后,通过Scatchard分析确认了所得18F-4D5ThioFab的生物学活性,其中没有观察到与天然4D5ThioFab相比缀合物的亲和力的显著变化。另外,通过PHESELECTOR方法设计且用作模型硫醇-蛋白质的4D5ThioFab保留了亲本4D5Fab的亲和力(Kd=0.1nM)。别的生物学确认包括受HSP90抑制剂17-AAG调控的鼠模型中HER2过表达肿瘤异种移植物的处理响应研究。
HER2是跨膜受体的表皮生长因子家族的一种受体酪氨酸激酶且是用于开发乳腺癌治疗剂的一种重要靶物。17-AAG靶向分子伴侣Hsp90,其负责广泛癌蛋白(包括HER2)的正确折叠、稳定性和功能。17-AAG对HER2阳性肿瘤的影响已有记载(Solit et al(2002)Clin Cancer Res 8:986-93)。用68Ga-DOTA缀合的trastuzumab-F(ab′)2(68Ga-DCHF)调查了17-AAG对乳腺癌异种移植物(BT474)鼠模型中HER2表达水平的影响(Smith-Jones et al(2006)J Nucl Med 47:793-6;Smith-Jones et al(2004)Nat Biotechnol 22:701-6)。观察到HER2水平降低70%,持续5天,接着在12天后回到处理前水平,提示68Ga-DCHF能够比18FDG PET要更早地监测肿瘤对17-AAG处理的响应。然而,68Ga的短半衰期(68min)未能理想地匹配F(ab’)2相对较长的血浆半衰期且显著的信号减弱在PET获取前所要求的3小时时段内是内在的。单价Fab片段相对较短的血浆半衰期联合18F(110min)可提供改良的成像特性(Williams et al(2001)Cancer Biother Radiopharm 16:25-35)。
观察到的HER2表达降低(图5)与Smith-Jones用基于F(ab’)2的成像剂观察到的广泛可比。17-AAG治疗后一天18F-4D5ThioFab肿瘤摄取与Smith-Jones等人使用68Ga-DCHF观察到的降低(70%)相比更低的降低(50%)(图5)可归于数个因素。虽然18F-4D5ThioFab的快速肾***提供改良的肿瘤对背景比,但是单价Fab片段具有与二价F(ab’)2相比具有更低的基线肿瘤摄取。假设Fab和F(ab’)2有相似的处理后肿瘤摄取,由于图5a(右上)所示示踪剂的非特异性摄取和脉管***分布,示踪剂摄取的百分比差异可偏倚背离在处理前具有更低信号量的化合物。17-AAG处理后(第1天)平均肿瘤摄取0.65±0.09%ID/g与皮肤摄取0.50±0.05%ID/g粗略可比的观察结果加强了这一点,如表3所示。另外,由于17-AAG在水中具有有限的溶解度,因此配制剂(由通过超声处理制备的基于鸡蛋磷脂的脂质体的乳状液组成)可能不会再现所报告的效力。这可解释实验中HER2表达的更快反跳(第5天100%)。虽然17-AAG处理后的信号衰减程度比先前观察到的要略微更低(Smith-Jones et al(2006)JNucl Med 47:793-6;Smith-Jones et al(2004)Nat Biotechnol 22:701-6),但是用18F-4D5ThioFab获得的图像质量(图5)优于用68Ga-DCHF获得的。Fab片段的快速肾清除及所致肿瘤对背景比增强克服了与F(ab’)2片段相比降低的肿瘤摄取绝对量。这便于示踪剂施用后仅仅2小时或约一个半衰期的数据获取,这提供了比用68Ga-DCHF成像前几乎三个半衰期时段更多的信号。
药物组合物
本发明的药物组合物或配制剂包括本发明的经过标记的蛋白质和一种或多种药学可接受载体、助流剂、稀释剂、或赋形剂。
药物组合物涵盖散装组合物(bulk compostion)和个体剂量单位二者,其包含超过一种(例如两种)药学活性剂(包括经过标记的蛋白质)以及任何药学无活性赋形剂、稀释剂、载体、或助流剂。散装组合物和每个单独的剂量单位(individual dosage unit)可含有固定量的上述药学活性剂。散装组合物指尚未形成单个剂量单位的物质。一种例示性的剂量单位是口服剂量单位,诸如片剂、丸剂、胶囊剂、等等。类似地,对受试者施用本发明的药物组合物的方法也意图涵盖施用散装组合物和单个剂量单位。
药物组合物还涵盖同位素标记的本发明化合物,它们与本文所述化合物相同,只是一个或多个原子被具有与自然界通常找到的原子质量或质量数不同的原子质量或质量数的原子替换。规定的任何特定原子或元素的所有同位素涵盖在本发明化合物及其用途的范围内。能掺入本发明化合物的例示性同位素包括氢、碳、氮、氧、磷、硫、氟、氯和碘的同位素,诸如2H、3H、11C、13C、14C、13N、15N、15O、17O、18O、32P、33P、35S、18F、36Cl、123I和125I。某些同位素标记的本发明化合物(例如那些用3H和14C标记的)在化合物和/或底物组织分布测定法中是有用的。氚(3H)和碳-14(14C)同位素因它们易于制备和可检测性而有用。另外,用更重的同位素诸如氘(2H)替代可提供某些诊断优势,其源自更大的代谢稳定性(例如体内半衰期延长或剂量需求降低),并因此在有些情况中是优选的。发射正电子的同位素,诸如15O、13N、11C和18F,可用于正电子发射断层摄影术(PET)研究以检查底物受体占据。同位素标记的本发明化合物一般能通过遵循与实施例中公开的规程类似的规程,通过用同位素标记的试剂替代非同位素标记的试剂来制备。
合适的载体、稀释剂、添加剂、和赋形剂是本领域熟练技术人员公知的,而且包括下述物质,诸如碳水化合物、蜡、水溶性和/或可膨胀聚合物、亲水性或疏水性物质、明胶、油、溶剂、水等等。所使用的具体载体、稀释剂或赋形剂会取决于应用本发明化合物的手段和目的。一般基于本领域技术人员公认对哺乳动物施用安全(GRAS)的溶剂来选择溶剂。一般而言,安全的溶剂是无毒的水性溶剂,诸如水和其它在水中可溶或易混合的无毒溶剂。合适的水性溶剂包括水、乙醇、丙二醇、聚乙二醇(例如PEG 400,PEG 300)、二甲亚砜(DMSO)、Cremophor(例如CREMOPHORBASF)及其混合物。配制剂还可以包括一种或多种缓冲剂、稳定剂、表面活性剂、湿润剂、润滑剂、乳化剂、悬浮剂、防腐剂、抗氧化剂、不透明剂、助流剂、操作助剂、着色剂、增甜剂、芳香剂、矫味剂和其它已知添加剂以提供药物(即本发明的化合物或其药物组合物)有效的呈现或帮助制造药学产品(即药物)。
可以施用常规溶解和混合规程来制备配制剂。例如,在存在一种或多种上文所述赋形剂下在合适的溶剂中溶解散装药物物质(即本发明的化合物或化合物的稳定化形式(例如与环糊精衍生物或其它已知的络合剂复合)。本发明的化合物通常配制成药学剂量形式,以提供可容易控制剂量的药物和使得患者顺从处方方案。
可以以多种方式包装供应用的药物组合物(或配制剂),这取决于用于施用药物的模式。一般而言,分发的物品包括容器,其中沉积有适宜形式的药物配制剂。合适的容器是本领域技术人员公知的,而且包括诸如瓶(塑料的和玻璃的)、囊、安瓿、塑料袋、金属筒、等等材料。容器还可以包括防干扰装置以防止不慎接触到包装的内容物。另外,容器上可贴有描述容器中所含内容物的标签。标签还可以包括适宜的警告。
可以制备本发明的化合物的药物配制剂,供各种路径和类型的施用。例如,任选可以以冻干配制剂、碾碎的粉末、或水溶液的形式将具有期望纯度的经过标记的蛋白质与药学可接受稀释剂、载体、赋形剂或稳定剂混合(Remington′s Pharmaceutical Sciences(1995)18th edition,Mack Publ.Co.,Easton,PA)。可以通过于环境温度与适宜的pH,及于期望的纯度,与生理学可接受载体(即在所采用的剂量和浓度对接受者无毒的载体)混合来进行配制。配制剂的pH主要取决于具体用途和化合物的浓度,但是范围可以是约3至约8。药物配制剂优选是无菌的。具体而言,用于体内施用的配制剂必须是无菌的。这种无菌易于通过无菌滤膜过滤来实现。药物配制剂通常能作为固体组合物、冻干配制剂或作为水溶液来贮存。
本发明的药物配制剂会以与优良医学实践一致的方式即量、浓度、时间表、疗程、媒介和施用路径来进行剂量给药和施用。在此语境中要考虑的因素包括所诊断或治疗的具体病症、所治疗的具体哺乳动物、患者个体的临床状况、病症的起因、药剂的递送部位、施用方法、施用时间表、和医学从业人员知道的其它因素。要施用的化合物的“治疗有效量”会由此类考虑来决定,而且是诊断、预防、改善、或治疗病症所必需的最小量。
每剂口服或胃肠外施用的经过标记的蛋白质的初始药学有效量会在约0.01-1000mg/kg的范围内,即约0.1至20mg/kg患者体重每天,所使用的化合物的典型初始范围为0.3-15mg/kg/天。要施用的经过标记的蛋白质的剂量和化疗剂的剂量各自的范围可以为约1mg至约1000mg每单位剂量形式或约10mg至约100mg每单位剂量形式。经过标记的蛋白质和化疗剂的剂量可以以按重量计约1∶50至约50∶1的比或以按重量计约1∶10至约10∶1的比施用。

药物配制剂包括那些适合于本文详述的施用路径的。配制剂可以方便地以单位剂量形式存在,而且可以通过药学领域公知的任何方法来制备。技术和配方一般见Remington′s Pharmaceutical Sciences 18th Ed.(1995)MackPublishing Co.,Easton,PA。此类方法包括使活性组分与构成一种或多种辅助组分的载体联合的步骤。一般而言,通过均匀且紧密地使活性组分与液体载体或粉碎的固体载体或二者联合,然后在必要时使产物定形来制备配制剂。

可以与载体材料联合以产生单个剂量形式的活性组分的量会根据宿主和具体的施用模式而变化。例如,预定用于静脉内输注的水溶液可以含有约3至500μg的活性组分每毫升溶液,以便能以约30mL/hr的速率输注合适的体积。

经过标记的蛋白质的代谢物
本文中所描述的经过标记的蛋白质的体内代谢产物也落在本发明的范围内。此类产物可源自例如所施用化合物的氧化、还原、水解、酰胺化、脱酰胺、酯化、脱脂、酶促切割、等等。因而,本发明包括经过标记的蛋白质的代谢产物,包括由如下方法生成的化合物,该方法包括使本发明的化合物接触哺乳动物一段时间,该时间足以产生本发明化合物的代谢产物。代谢物结构可以以常规方式来测定,例如通过MS、LC/MS或NMR分析。一般而言,对代谢物的分析是以与本领域技术人员公知的常规药物代谢研究相同的方式进行的。代谢产物在用于本发明化合物的治疗性剂量给药的诊断测定法中是有用的,只要没有在体内在其它情况中找到它们。

实施例1:3,6,9,12,15,18-六氧杂二十一碳-20-炔-1-醇1

实施例2:1-(3,6,9,12,15,18-六氧杂二十一碳-20-炔-1-基)-2,5-吡咯二酮(pyrroledione)2

实施例4:23-叠氮基-1-氟-3,6,9,12,15,18,21-七氧杂二十三烷4

方法A。或者,为了制备[18F]5,使[18F]23-叠氮基-1-氟-3,6,9,12,15,18,21-七氧杂二十三烷4通过Sep-Pak Alumina N light筒,用乙腈(0.5mL)冲洗,蒸发至接近干燥,并冷却至30℃。将溶于乙腈(0.2mL)中的化合物2(4mg 10.0μmol)添加到[18F]4,接着添加新鲜制备的抗坏血酸钠(7.5mg,40μmol)在0.1mL Tris缓冲液pH 8中的溶液和CuSO4.5H2O(2.2mg,8μmol)在Tris缓冲液pH 8(0.2mL)中的溶液,如图4中。将得到的棕-橙色溶液在室温搅拌20分钟,接着添加具有0.1%TFA的H2O(1mL),并递送至HPLC回路用于纯化。

方法C。或者,为了制备[18F]5,使[18F]23-叠氮基-1-氟-3,6,9,12,15,18,21-七氧杂二十三烷4蒸发至接近干燥(不需要氧化铝处理),然后添加溶于乙腈(0.1mL)中的化合物2(4mg,10.0μmol)。接着,添加新鲜制备的Cu(MeCN)4PF6(3mg,8μmol)、TBTA(4.3mg,8μmol)和25μL 2,6-二甲基吡啶在乙腈(0.2mL)中的混合物。将得到的浅黄色溶液在室温搅拌10分钟,浓缩至100μL,用具有0.1%TFA的水(1mL)稀释,并递送至HPLC回路用于纯化。
将该粗产物[18F]5通过半制备性HPLC(***B)纯化,保留时间为15-18分钟。迅速用水将收集到的级分稀释至20mL,加载到Phenomenex Strata-X 60mg SPE筒上,捕集在该筒上的产物用H2O(10mL)冲洗,用空气冲洗,用乙腈(0.5mL)洗脱到1mL v-小瓶中,并在30℃使用600ccm氩气蒸发至接近干燥。MS ESI(m/z):对于C35H62FO15N7的[M+H]+的计算值797.4;实测值797.4;HPLC (***A)保留时间2.98min。
为了限制[18F]5在水性条件下的降解,设计放射化学方法的优化来最小化CuAAC反应和从固相提取(SPE)的回收之间的合成时间。当CuAAC反应体系迅速用0.1%TFA水猝灭,HPLC以高流速(8mL/min)进行,和迅速加载[18F]5并从SPE筒洗脱时,观察到[18F]5的改善的收率。从[18F]5形成的18F标记的降解产物随着SPE树脂床体积和当产物保留在该树脂床上延长的时间时而增加。因此,将[18F]5快速从小体积的Phenomenex Strata-X 60mg筒洗脱,其提供良好的回收率和可接受的杂质水平。
实施例6:[18F]FPEGMA-Thio4D5Fab 6
与先前描述的规程类似地制备4D5-ThioFab(Junutula,J.R.et al(2008)JImmunol Methods 332:41-52;US 2007/0092940)。通过定点诱变在轻链位置Val 110处将半胱氨酸替代引入Trastuzumab(4D5,Herceptin)抗体构建物。将表达并纯化的4D5-ThioMab在25mM Tris pH 8.0中稀释至1mg/mL,并以1∶1000(重量:重量)的酶对抗体的比使用Lys-C(Wako)于37℃酶促消化1小时。用5μM蛋白酶抑制剂甲苯磺酰基-L-赖氨酸氯甲基酮(TLCK)停止消化并使用50mM乙酸钠缓冲液和10个CV的0-300mM NaCl梯度在5mL Hi-Trap SP FF柱(GE Healthcare)上通过阳离子交换层析纯化。然后通过还原和氧化规程以除去结合至Cys110的二硫化物加合物使分离的ThioFab准备好进行缀合。首先,通过添加含有25mM MES pH 5.8、300mL NaCl、和5mM EDTA的缓冲液中的2mM三(2-羧乙基)膦(TECP)(Pierce)将蛋白质还原24小时。还原后,通过添加5mM脱氢抗坏血酸(Sigma)氧化蛋白质,并使用S200柱(GE Healthcare)和含有25mM MES pH 5.8、300mM NaCl、和5mM EDTA的缓冲液通过凝胶过滤纯化蛋白质。通过SDS-PAGE和质谱术分析分离的蛋白质以确保蛋白质得到正确还原和氧化。
将100mM磷酸钠pH 8中的4D5ThioFab(1.0mg,12.6nmol)(2mg/mL)添加至[18F]5,并在试管摇动器上摇动10分钟。通过半制备性SEC-HPLC柱(***D,保留时间10-12分钟)纯化粗制产物,并在必要时用Amicon 10kDa膜浓缩,供注射用。HPLC(***A)保留时间3.3分钟;SEC HPLC(***C)保留体积3.0mL,得到6。
实施例7:5-叠氮基-3-氧杂-戊基对甲苯磺酸酯7

实施例14:通过CuAAC对炔丙基修饰的4D5ThioFab的直接18F标记
将5倍过量的1-(3,6,9,12,15,18-六氧杂二十一碳-20-炔-1-基)-2,5-吡咯二酮2(0.4mg,1μmol)添加到4D5ThioFab(10mg,0.2μmol,5mg/mL,50mM磷酸钠pH 7.2)并在37℃培育1小时。用10%乙酸将溶液调节至pH 4-5,并通过阳离子交换层析在HiTrap SP FF柱(GE Healthcare)上用50mM Tris缓冲液pH8(具有100mM氯化钠)纯化。将一等份回收的炔丙基-经修饰4D5ThioFab(100ug)添加到经干燥的[18F]23-叠氮基-1-氟-3,6,9,12,15,18,21-七氧杂二十三烷4,接着添加1.2mg BPDS在脱气的缓冲液(250uL Tris pH 8)中的溶液和0.26mg Cu(MeCN)4PF6在20uL乙腈中的溶液。用氩气从反应体系中排除大气中的氧。然而,反应经10分钟从暗棕色变成无色,指示反应被氧猝灭了,需要更加严格的脱气条件。
实施例15:动物模型
自Harlan Sprague Dawley(Livermore,CA)获得6-8周龄米色裸XID小鼠。细胞接种前3天,给小鼠植入(s.c,左侧)0.36mg 60天持续释放17β-***团粒(Innovative Research of America)以维持血清***水平。给小鼠在***脂肪垫中接种50%不含酚红的Matrigel中的5x 106个BT474M1细胞,即BT474(得自California Pacific Medical Center)的亚克隆。动物护理和处理遵循得到Genentech Institutioned动物护理和使用委员会批准的方案,其被实验室动物护理评估和鉴定联合会(AAALAC)接受。
实施例16:17-AAG配制和施用
将DCM(5mL)中的鸡蛋磷脂(Egg-phospholipid)(0.5g)转移入500mL圆底烧瓶,蒸发溶剂并将残渣于减压保存16小时。将5%(m/v)右旋糖溶液(25mL)添加至干燥后的磷脂并将混合物于室温超声处理60分钟以获得乳状乳状液。将17-AAG(25mg)溶于DMSO(1mL),将其中一小份(0.25mL)与鸡蛋磷脂乳状液(4.8mL)混合并于室温超声处理15分钟。在制备后的30分钟内,如先前所述(Smith-Jones et al(2006)J Nucl Med 47:793-6;Smith-Jones et al(2004)Nat Biotechnol 22:701-6)在24小时时段里以三个1mL剂量(各50mg/kg)通过腹膜内注射施用所得乳状液。17-AAG为17-N-烯丙基氨基-17-脱甲氧基格尔德霉素,一种处在癌症治疗研究中、对具有某些类型白血病或实体瘤的年轻患者是特异性的物质。17-AAG 也称作:[(3S,5S,6R,7S,8E,10R,11S,12E,14E)-21-(烯丙基氨基)-6-羟基-5,11-二甲氧基-3,7,9,15-四甲基-16,20,22-三氧-17-氮杂双环[16.3.1]二十二-8,12,14,18,21-五烯-10-基]氨基甲酸酯/盐(CAS登录号75747-14-7)。
实施例17:MicroPET成像
用3%七氟醚麻醉小鼠并经尾静脉导管i.v接种等张溶液(50-100μL)中的0.3-0.4mCi 18F-Thio4D5Fab。容许动物在加热毯上恢复,直至能走动,然后回笼。有意识的摄取2小时后,用3%七氟醚麻醉动物,并将其以头前俯卧位置于扫描床上。通过直肠探头来测量体温并通过温暖空气来维持。获取动态60分钟扫描。使用最大后验算法(MAP)获得全身图像重建并用ASIPro软件(CTI Molecular Imaging)创建最大强度投影(MIP)。使用ASIPro软件(CTIMolecular Imaging),使用MAP重建图像来获得每个感兴趣器官中的定量活性水平。
统计分析:用R软件2.4.1版(R Foundation for Statistical Computing,Vienna,Austria)构建图像。使用双尾Student氏t检验来确定统计显著性并将小于0.05的P值认定为显著的;数据表述为均值±s.e.m.,除非另有说明。
