Abstract
Description
創傷治癒の促進作用を示すものとして、有効成分としてアロエ等の抽出物、抗生物質、抗炎症剤、カリクレイン、アデニン、ニコチン酸、アラントイン、ビタミンA、亜鉛、cAMP誘導体(特許文献1)、外因性のコラーゲン(特許文献2)等の薬剤が挙げることができる。従来技術では、これらの有効成分の吸収性改善のための製剤の改良等が盛んに行われていたが、最近では皮膚組織学上の様々な知見が明らかになるにつれて、上皮細胞成長因子(EGF)を術後の創傷治癒に使う試みがなされている(特許文献3)。
薬剤を内服投与した場合、患部に作用するのが投与薬剤の一部であり、十分な効果を得るのが難しく、更に副作用の発現も懸念される。そのため、治療には直接皮膚に作用する外用剤のような薬剤が望まれるが、外用剤の創傷治癒促進剤は少なく、これが皮膚の創傷治癒を困難にしている。
現在、外用の創傷治癒促進剤として汎用されているものとして、幼牛 血液抽出物を主成分とするソルコセリル軟膏、cAMPの誘導体であるブクラデシンナトリウムを主成分とするアクトシン軟膏、bFGF(塩基性線維芽細胞成長因子)を主成分とするフィブラストスプレーが知られているが、治療のためには長時間患部の塗布する必要があり、刺激感・痛み、発赤、かゆみ、浸出液の増加などの副作用も報告されている。本願明細書に記載の特定のポリフェノール、例えばプロアントシアニジンやクロロゲン酸は、それぞれブドウ及びコーヒーに多く含まれるなど、食経験が豊富なものが多く、経口摂取においても安全性が高いことが特長の一つに挙げられる。加えて、上記薬品では線維芽細胞に対する効果はある程度の時間を必要とし、我々が過去に行ったbFGFも含めた各種増殖因子(TGF-β, IL-1α、PDGF)の線維芽細胞創傷モデル(インビトロ)での作用も24時間処理で効果を検出しており(非特許文献1)、数分で効果が表れるような薬品等は報告されていない。
一方、ポリフェノールは、抗酸化作用し、炎症部位において好中球などの炎症性細胞が産生する活性酸素による局所組織の酸化的傷害の抑制作用を有することが知られており、皮膚外用薬や口腔用剤などに使用されている(特許文献4~9、非特許文献2、3)。しかし、特定のポリフェノールが創傷治癒を促進できることは、確認されていなかった。

一般的に創傷は次のようなステップを経て治癒される。まず創傷による組織障害が起こり、止血、炎症期に入る。その後、血管の新生を伴う肉芽組織が形成され、形成肉芽の上を表皮が再生・進展し、創収縮の期間を経て、成熟し、治癒する。肉芽組織が形成される期間(肉芽形成期)では、線維芽細胞の増殖が必須である。創傷治癒の促進に効果的な特定のポリフェノールは、これを線維芽細胞に接触させると、線維芽細胞の増殖が見られる。すなわち、当該ポリフェノールは創傷治癒を促進することができる。

この細胞懸濁液を100μlずつI型コラーゲンでコートした96-wellマイクロプレート(旭硝子製)の各wellに播種した。37℃、5%CO2条件下で28~30時間培養後、培地を除去し、プロアントシアニジン溶液(Leucoselect(商標)、Indeda社製)を100μl加えた。1分後にプロアントシアニジン溶液を除去し、新鮮培地で洗浄後、再度新鮮培地を加え、さら37℃、5%CO2条件下で培養を再開した。なお、光の影響を検討する目的で、プロアントシアニジン溶液を添加後、405nmのレーザー光(実験用InGaN半導体レーザー装置、リコー光学製)を出力300mW、照射強度930mW/cm2で1分間照射する群を設けた。この群も同様にレーザー照射後、同様に新鮮培地に交換し、37℃、5% CO2条件下で培養を再開した。培養24時間後にMTT法により細胞増殖の程度を測定した。具体的には、培養20時間後にMTT試薬(Trevigen社製)を各wellに10μlずつ加え、さらに37℃、5%CO2条件下で4時間培養した。培養後、Detergent試薬(Trevigen社) を各100μl加え、遮光下、室温で一晩放置した。そして595nmでの吸光度をマイクロプレートリーダー(FilterMax(商標) F5 Multi-Mode Microplate Reader,Molecular Devices社) を用いて測定した。滅菌生理食塩水処理を対照群とし、対照群の吸光度を100%とした時の各処理群の割合(%)を算出した。結果を図1に記載する。なお、試験1の対照群の吸光度は0.289±0.016であった。
没食子酸(東京化成工業株式会社製)、カフェイン酸(東京化成工業株式会社製)、(+)-カテキン(東京化成工業株式会社製)、クロロゲン酸(Sigma-Aldrich社製)を供試した。上記試験1と同様に細胞懸濁液を100μlずつI型コラーゲンでコートした96-wellマイクロプレートの各wellに播種した。37℃、5%CO2条件下で27~28時間培養後、培地を除去し、被験物質溶液を100μl加えた。1分後に被験物質溶液を除去し、新鮮培地で洗浄後、再度新鮮培地を加え、さらに37℃、5%CO2条件下で培養を再開し、培養24時間後の細胞増殖の程度をMTT法により測定した。結果を図2に示す。なお、試験2の対照群の吸光度は0.196±0.011であった。
(+)-カテキンを供試した。試験1と同様に細胞懸濁液を100μlずつI型コラーゲンでコートした96-wellマイクロプレートの各wellに播種した。37℃、5% CO2条件下で24時間培養後、被験物質含有培地に交換し、37℃、5%CO2 条件下24時間培養した。被験物質含有培地を新鮮培地に交換し、MTT試薬(Trevigen社)を各wellに10μlずつ加え、さらに37℃、5%CO2 条件下で4時間培養後、Detergent試薬(Trevigen社) を各100μl加え、他試験と同様に細胞増殖の程度を測定した。結果を図3に示す。なお、試験3の対照群の吸光度は0.163±0.014であった。
緑茶エキス(丸善製薬製株式会社製)を供試した。試験1と同様に細胞懸濁液を100μlずつI型コラーゲンでコートした96-wellマイクロプレートの各wellに播種した。37℃、5%CO2条件下で28時間培養後、培地を除去し、被験物質溶液を100μl加えた。1分後に被験物質溶液を除去し、新鮮培地で洗浄後、再度新鮮培地を加え、さら37℃、5%CO2条件下で培養を再開し、培養24時間後の細胞増殖の程度をMTT法により測定した。結果を図4に示す。なお、試験4の対照群の吸光度は0.276±0.010であった。
25cm2 フラスコで37℃、5%CO2 条件下で培養した細胞(ヒト歯肉線維芽細胞。株式会社プライマリーセルより購入し、5代継代以内のものを供試した。)を、0.25%Trypsin-EDTA液(Life Technologies Corp.製)で処理し、常法に従い 細胞密度が2×104 cells/ml になるよう培地にて細胞懸濁液を調製した。ここで培地は、2mlのglutamine含有Dulbecco’s Modified Eagle Medium (DMEM,Life Technologies Corp., Frederick,MD USA) に、ウシ血清(Life Technologies Corp.製) 、及びペニシリン/ストレプトマイシン溶液(10000 units/mlのペニシリン、及び10 mg/mlのストレプトマイシン、和光純薬工業製)をそれぞれ10% (v/v)、1% (v/v) になるよう添加したものを増殖用培地として用いた。
この細胞懸濁液を100μlずつ96-wellマイクロプレート(Corning Corp.,NY, USA)の各wellに播種した。37℃、5%CO2条件下で24時間培養後、培地を除去し、滅菌生理食塩水に溶解したプロアントシアニジン(Leucoselect(商標)、Indeda社製)を100μl加えた。1分後にプロアントシアニジン溶液を除去し、新鮮培地で洗浄後、再び新鮮培地を加え、さら37℃、5%CO2条件下で培養を再開した。培養24時間後にMTT法により細胞増殖の程度を測定した。滅菌生理食塩水処理を対照群とし、対照群の吸光度を100%とした時の各処理群の割合(%)を算出した。結果を図5に示す。なお、試験5の対照群の吸光度は0.073±0.007であった。
上記試験5と同様に細胞懸濁液を100μlずつ96-wellマイクロプレートの各wellに播種した。37℃、5%CO2条件下で24時間培養後、培地を除去し、0.25mg/ml及び1mg/mlになるよう滅菌生理食塩水に溶解したプロアントシアニジンを100μl加えた。1分後にプロアントシアニジン溶液を除去し、新鮮培地で洗浄後、再び新鮮培地を加え、さらに37℃、5%CO2条件下で培養を再開し、経時的に細胞増殖の程度をMTT法により測定した。対照群には滅菌生理食塩水を同様に処理した。結果を図6に示す。なお、試験6の対照群0時点の吸光度は0.088±0.013であった。
細胞を所定時間純水に暴露する浸透圧ストレスを与え、それに対するプロアントシアニジンの影響を検討した。上記試験5と同様に細胞懸濁液を100μlずつ96-wellマイクロプレートの各wellに播種した。37℃、5%CO2条件下で15~16時間培養後、培地を除去し、滅菌純水あるいは滅菌純水に1mg/mlになるよう溶解したプロアントシアニジンを100μl加えた。1、4及び16分後に純水あるいはプロアントシアニジン純水溶液を除去し、新鮮培地で洗浄後、再び新鮮培地を加え、さら37℃、5%CO2条件下で培養を再開した。培養24時間後にMTT法により細胞増殖の程度を測定した。無処理(培地のみ)で40時間培養したものを対照群とした。結果を図7に示す。なお、試験7の対照群(無処理)の吸光度は0.228±0.030であった。
細胞を1、2、及び3時間滅菌生理食塩水に暴露する飢餓ストレスを与え、それに対するプロアントシアニジンの影響を検討した。上記試験5と同様に細胞懸濁液を100μlずつ96-wellマイクロプレートの各wellに播種した。37℃、5%CO2条件下で19時間培養後、培地を除去し、滅菌生理食塩水あるいは滅菌生理食塩水に1mg/mlになるよう溶解したプロアントシアニジンを100μl加えた。1、2、及び3時間後に滅菌生理食塩水あるいはプロアントシアニジン生理食塩水溶液を除去し、新鮮培地で洗浄後、再び新鮮培地を加え、さら37℃、5%CO2条件下で培養を再開した。培養22~24時間後にMTT法により細胞増殖の程度を測定した。無処理(培地のみ)で44時間培養したものを対照群とした。なお、試験8の対照群(無処理)の吸光度は0.137±0.004であった。結果を図8に示す。
上記試験5と同様に細胞懸濁液を100μlずつ96-wellマイクロプレートの各wellに播種した。37 ℃、5%CO2 条件下で20時間培養後、培地を除去し、1mg/mlになるよう滅菌生理食塩水に溶解したプロアントシアニジンを100 μl加えた。1分後にプロアントシアニジン溶液を除去し、新鮮培地で洗浄後、大腸菌由来のlipopolysaccharide(LPS、和光純薬工業)を2.5μg/mlの割合で含有する培地を加え、さら37℃、5%CO2条件下で培養を再開した。培養24時間後にMTT法により細胞増殖の程度を測定した。無処理(培地のみ)で44時間培養したものを対照群とした。なお、試験9の対照群の吸光度は0.109±0.006であった。対照群の平均値との有意差は、Dunnettの多重比較解析法で検定した。結果を図9に示す。

- 一般式:
(式中、R1~R3はそれぞれ独立して、それぞれ置換又は非置換の、水素原子、ハロゲン原子、ニトロ基、シアノ基、チオ基、アルコキシ基、アリールオキシ基、ヘテロアリールオキシ基、カルボニル基、アミノ基、C1-10のアルキルアミノ基、スルホンアミド基、イミノ基、スルホニル基、スルフィニル基、C1-10のアルキル基、C3-12のシクロアルキル基、C3-12のヘテロシクロアルキル基、C9-12のビシクロアルキル基、C3-12のヘテロビシクロアルキル基、C1-10のアリールアルキル基、C1-5のヘテロアリールアルキル基、C1-10のパーハロアルキル基、C1-3のカルボニルアルキル基、C1-3のチオカルボニルアルキル基、C1-3のスルホニルアルキル基、C1-3のスルフィニルアルキル基、C1-10のアミノアルキル基、C1-3のイミノアルキル基、アリール基、ヘテロアリール基、C9-12のビシクロアリール基及びC4-12のヘテロビシクロアリール基からなる群から選択されるか、或いはR1、R2、及びR3の任意の二つが一緒になって置換又は非置換の環を形成する。Xは置換又は非置換のC1-20の脂肪族炭化水素基、C3-20の芳香族炭化水素基、C3-20の多環芳香族炭化水素基を表す。)で表わされるポリフェノール、その重合体及び医薬上許容できるその塩の少なくとも1種を含む、創傷治癒用医薬組成物。 - 上記R2及びR3が、いずれも水素原子である、請求項1に記載の創傷治癒用医薬組成物。
- 上記R1が、水素原子である請求項1又は2に記載の創傷治癒用医薬組成物。
- 前記ポリフェノールが、カフェイン酸、(+)-カテキン酸及びクロロゲン酸からなる群から選ばれる請求項1~3のいずれか一項に記載の創傷治癒用医薬組成物。
- 一般式:

- 1秒から10分間接触させる、請求項14に記載の方法。
- 接触後、接触部位を洗浄することをさらに含む、請求項10~15のいずれか一項に記載の方法。
- 該創傷が口腔内にある、請求項10~16に記載のいずれか一項に記載の方法。
- 創傷治癒促進剤の製造のための、一般式:
