- a number of devices are known for monitoring a person's cardiac or pulmonary activity in order to detect the onset of an abnormal physiological condition such as sleep apnoea.
- a measured value is compared with a constant, predetermined value and an alarm triggered when the two values differ by more than an acceptable amount, thus indicating the existence of an abnormal physiological condition.
- the device is unable to provide any warning prior to the onset of an abnormal physiological condition so that appropriate preventative action may be taken.
- spurious isolated measurements may cause false triggering of the alarm.
- a device comprising means for obtaining measurements of cardiac or pulmonary activity at successive instants in time, memory means for storing said measurements and processing means for identifying in said stored measurements any trend indicative of an abnormal physiological condition.
- the processing means are arranged to identify in said stored measurements any progressive increase or decrease in the rate or magnitude of cardiac or pulmonary activity indicative of an impending abnormal physiological condition, for example cardiac or pulmonary failure due to sleep apnoea.
- processing means are also arranged to identify in said stored measurements any substantially constant deviation in the rate or magnitude of said cardiac or pulmonary activity from a preset value indicative of an existing abnormal physiological condition.
- the processing means are arranged to identify a trend in the stored measurements indicative of an abnormal physiological condition by fitting an equation to the stored measurements. Most preferably a linear equation is fitted to the stored measurements.
- a measure of the deviation of each of the stored measurements from the equation fitted thereto or from a mean value is used to estimate the likelihood of an existing or impending abnormal physiological condition being incorrectly identified.
- the device comprises alarm means for providing an output signal when it is estimated from the fitted equation that within a predetermined period of time the magnitude or rate of cardiac or pulmonary activity will exceed a predetermined threshold value.

- a sensor 2 arranged to be attached to or placed in close proximity to a person, provides output signals in response to variations in pressure caused by the beating of the person's heart and by the person inhaling and exhaling.
- Analogue output signals from the sensor 2 pass ln-turn through an amplifier 4, low-pass filter 6 (to remove high frequency noise) and digital sampling means 8.
- a finite impulse response (FIR) digital bandpass filter 10 separates the digital signal into its cardiac and pulmonary components.
- a first processing means 12 derives measurements from the respective signals, at successive instants of time, of the magnitude and rates of cardiac and pulmonary activity and stores these measurements m an electronic memory 14. The stored measurements are subsequently processed by a second processing means 16 to identify therein any trends which may indicate an abnormal physiological condition or the likelihood of such a condition arising.
- a progressive increase or decrease m the rate and/or magnitude of cardiac and/or pulmonary activity may be indicative of an impending abnormal physiological condition.
- a substantially constant deviation m the rate and/or magnitude of cardiac and/or pulmonary activity from respective preset values whilst not necessarily indicative of an impending abnormal physiological condition, might nevertheless indicate an existing abnormal physiological condition.
- the second processing means 16 are arranged to identify trends in the stored measurements indicative of an abnormal physiological condition by fitting respective lines to each set of stored measurements.
- a fitted line of zero gradient, but displaced from a mean value by more than a predetermined amount, will be indicative of an existing abnormal physiological condition, whereas a fitted line of non-zero gradient will instead indicate the possibility of an abnormal physiological condition arising at some later time.
- the displacement of the fitted line from a predetermined mean value may be used by the second processing means to estimate the period of time remaining before a safe threshold level for a particular measurement will be exceeded. Where an abnormal physiological condition exists or is deemed imminent, an audible alarm 18 is sounded.
- a measure of the deviation of each of the stored measurements from the line fitted thereto or from a mean value is used to estimate the likelihood of an abnormal physiological condition being incorrectly identified.
- the device is provided with a first interface 20 for communicating data to and from a personal computer 22 for display or analysis, a second interface 24 by which the device may be connected to a computer network e.g. the World Wide Web, for on-line monitoring of the person's cardiac and pulmonary activity, and built-in display means 26.
- a computer network e.g. the World Wide Web
- the built-in display means 26 and interfaces 20 and 24 may be used to provide a display of the raw signals acquired by the sensor 2, the output of the bandpass filter 10, the output of the first processing means 12 and/or various results generated the second processing means 16.
- the device thus described may be used to provide a reliable early warning of the possible onset of an abnormal physiological condition such as sleep apnoea.
