Abstract
The Invention provides a knee prosthesis (10) comprising a femoral component (12), a tibial component (30) and a meniscal component (20) located between the femoral and tibial components (12, 30), interengaging means (26, 28, 36, 38) being provided between the tibial and meniscal components (30, 20) so as to allow limited anterior-posterior and rotational movement of the meniscal component (20) with respect to the tibial component (30), the interengaging means (26, 28, 36, 38) comprising anterior and posterior pegs (26, 28) and corresponding recesses (36, 38), wherein the pegs (26, 28) and corresponding recesses (36, 38) are configured so as to allow medial-lateral movement of the meniscal component (20) with respect to the tibial component (30) in addition to the anterior-posterior and rotational movement.
Description
KNEE PROSTHESIS
The invention relates to a knee prosthesis, particularly to a three-part knee prosthesis.
Strenuous efforts have been made to produce three-part knee prostheses which are capable of mimicking very closely the action of the natural knee joint. Inevitably, restraints acting between the tibial and meniscal components are normally required in order to provide the prosthesis with adequate stability. However, the restraints previously proposed are restrictive in that they prevent the prosthesis from moving in such a way that the action of the natural knee joint is very closely mimicked over the entire range of flexion.
An object of the invention is to provide a three-part knee prosthesis which mimics the action of the natural knee joint more closely than previously proposed arrangements.
The invention provides a knee prosthesis as set out in claim 1. Advantageous features are set out in the subsidiary claims.
The freedom of movement allowed by the prosthesis of the invention is greatly suited to the task of a knee
prosthesis. Anterior-posterior movement is allowed, together with medial-lateral movement and rotational movement about a wide range of different superior-inferior axes. In each case, the amount of movement allowed is restricted. With a particularly advantageous embodiment of the invention, rotation about a posterior axis is allowed at the extremes of medial-lateral movement. This is particularly advantageous in achieving a knee prosthesis which closely mimics the action of the natural knee joint.
An embodiment of the present invention will now be described with reference to the accompanying drawings wherein: '
Figure 1 is a side sectional view of a knee prosthesis according to the invention;
Figures 2(a) to 2(c) are, respectively, underneath, front, and side views of the meniscal component of the prosthesis of Figure 1; and
Figures 3(a) to 3(c) are, respectively, plan, front and side views of the tibial component of the prosthesis of Figure 1.
The prosthesis 10 shown in Figure 1 consists of a femoral component 12, a meniscal component 20 and a tibial component 30. The femoral component 12 is of standard design and shape, having fixing devices such as a femoral stem 14 or alternatively pegs or screws, and condyles 16. The femoral component 12 forms no part of
the present invention and will therefore be described no further here.
The meniscal component 20 is located between the femoral and tibial components 12, 30 and is shown in detail in Figures 2(a) to 2(c). The overall shape of the meniscal component 20 is of standard design, having a generally planar lower surface 22 and an upper surface 24 comprising condylar surfaces. The lower surface 22 of the meniscal component 20 also incorporates an anterior recess 26 and a posterior recess 28. The anterior recess 26 is substantially part-circular in plan or underneath view, having a part-cylindrical inner wall 26a and a relatively wide opening 26b in the side wall of the meniscal component 20. The posterior recess 28 has the general shape of an inverted U in the plan or underneath view, having an arcuate end wall 28a and substantially parallel side walls 28b. The distance between the parallel side walls 28b of the posterior recess 28 is less than the overall width of the anterior recess 26.
The tibial component 30 which supports the meniscal component 20 is illustrated in detail in Figures 3(a) to 3(c). Again, the overall design of the tibial component 30 is standard in that it has a planar upper surface or plateau 32, beneath which are located fixing devices such as pegs or stems 34. However, on the upper surface 32 are located an anterior peg or bollard 36 and a
posterior peg or bollard 38. Each peg 36, 38 is substantially cylindrical in shape and upstands from the plateau 32 by no more than the depth of the corresponding anterior and posterior recesses 26, 28 in the meniscal component 20. The anterior peg 36 is smaller in diameter than the posterior peg 38.
When the prosthesis is assembled, the tibial component 30 supports the meniscal component 20 which in turn supports the femoral component 12. The pegs 36, 38 engage in the recesses 26, 28 so that relative movement between the meniscal and tibial components 20, 30 is allowed but is restricted by the abutment of the pegs 36, 38 against the walls of the recesses 26, 28. The relative dimensions and spacings of the pegs 36, 38 and the recesses 26, 28 are such that a maximum anterior-posterior movement of substantially 4mm is allowed and a maximum medial-lateral movement of substantially 7mm is allowed.
The fact that the anterior recess 26 is wider than the posterior recess 28, and also that the anterior peg 36 is smaller in diameter than the posterior peg 38, means that, when the posterior peg 38 abuts against a side wall 28b of the posterior recess 28 at an extreme of medial-lateral movement, rotational movement of the meniscal component 20 with respect to the tibial component 30 about an axis passing through or close to the posterior peg 38 is still possible. This is advantageous in helping to mimic very closely the action
of a natural knee joint.
The femoral and tibial components 12, 30 are manufactured from an implant-grade material, normally a colbalt-chrome alloy. The upper surface 32 of the tibial component 30 is high polished so as to allow the planar lower surface 22 of the meniscal component 20, which is made from ultra high molecular weight polethylene or other suitable plastics material, to glide easily thereacrosε. Bone-contacting surfaces of the femoral and tibial components 12, 30 may have appropriately textured areas or coatings of materials to encourage bone-ingrowth such as Hydroxyappatite.
The invention is not limited to the precise embodiment described above. Various adaptations and modifications will be apparent to the skilled reader. For example, the relative sizes and spacings of the pegs and recesses may be altered to allow greater or less freedom of movement in any desired direction. Also, the pegs could be re-positioned on the meniscal component whilst the recesses could be formed in the tibial component. Means for achieving posterior stabilisation of the prosthesis could additionally be provided between the meniscal and femoral components.
Claims
1. A knee prosthesis comprising a femoral component, a tibial component and a meniscal component located between the femoral and tibial components, interengaging means being provided between the tibial and meniscal components so as to allow limited anterior-posterior and rotational movement of the mensical component with respect to the tibial component, the interengaging means comprising anterior and posterior pegs and corresponding recesses, wherein the pegs and corresponding recesses are configured so as to allow medial-lateral movement of the meniscal component with respect to the tibial component in addition to the anterior-posterior and rotational movement.
2. A knee prosthesis as claimed in claim 1, wherein the medial-lateral movement allowed by the anterior peg and recess is greater than the medial-lateral movement allowed by the posterior peg and recess.
3. A knee prosthesis as claimed in claim 1 or 2, wherein the anterior recess is generally part-circular in plan view.
4. A knee prosthesis as claimed in any one of claims 1 to 3, wherein the posterior recess has an arcuate anterior end and substantially parallel sides.
5. A knee prosthesis as claimed in any one of the preceding claims, wherein both the anterior and posterior pegs are cylindrical in plan view.
6. A knee prosthesis as claimed in claim 5, wherein the diameter of the anterior peg is smaller than the e diameter of the posterior peg.
7. A knee prosthesis as claimed in any one of the preceding claims wherein the meniscal component is movable between 2mm and 6mm with respect to the tibial component in the anterior-posterior direction.
8. A knee prosthesis as claimed in claim 7, wherein the meniscal component is movable substantially 4mm with respect to the tibial component in the anterior-posterior direction.
9. A knee prosthesis as claimed in any one of the preceding claims, wherein the meniscal component is movable between 0.5mm and 10mm with respect to the tibial component in the medial-lateral direction.
10. A knee prosthesis as claimed in claim 9, wherein the meniscal component is movable substantially 7mm with respect to the tibial component in the medial-lateral direction.
11. A knee prosthesis as claimed in any one of the preceding claims, wherein the recesses are formed in the meniscal component and the pegs are located on the superior surface of the tibial component.

12. A knee prosthesis as claimed in any one of the preceding claims, wherein the femoral and tibial components are made of a colbalt-chrome alloy.
13. A knee prosthesis as claimed in any one of the preceding claims, wherein the meniscal component is made of ultra-high molecular weight polyethylene.
14. A knee prosthesis substantially as hereinbefore described with reference to the accompanying drawings.
