Корисна модель належить до медичної та хіміко-фармацевтичної галузі, а саме до лікарських засобів природного походження на основі аміноцукрів, що є похідними глюкозаміну (ГА), і може бути використана у моно- чи комплексній терапії гострих та хронічних захворювань нирок різної етіології, у якості нефропротекторного та гіпоазотемічного засобу.The useful model belongs to the medical and chemical-pharmaceutical field, namely to drugs of natural origin based on amino sugars, which are derivatives of glucosamine (HA), and can be used in mono- or complex therapy of acute and chronic kidney diseases of various etiologies, as nephroprotective and hypoazotemic agent.
Патогенетична терапія хворих із порушеннями ниркової функції є важливою проблемою сучасної медицини, що пов'язано з відсутністю у клінічній практиці ефективних препаратів нефропротекторної дії. Ураження нирок незалежно від етіології з часом призводять до виникнення хронічної хвороби нирок (ХХН), що займає особливе місце серед захворювань сечовидільної системи, її перебіг призводить до неминучого розвитку важких ускладнень, таких як гостра та хронічна ниркова недостатність, в результаті чого пацієнти швидко піддаються інвалідизації і втрачають свою соціальну активність (1| Згідно даних світової статистики поширеність ХХН становить 10-16 95 від загальної чисельності населення і сягає 30 95 в осіб похилого віку (2). В Україні на сьогоднішній день налічується біля 500 тис. хворих на ХХН, більшість з яких не може отримати спеціалізовану медичну допомогу (|З). Нефропротекторні властивості похідних ГА - ГА гідрохлориду (ГА г/х) та його активного метаболіту НАГА було вивчено при в/ш та в/м шляхах введення у щурів за умов розвитку мембранозного ураження нирок. Нефропатію відтворювали шляхом внутрішньоочеревинного (в/о) введення доксорубіцину в дозі 10 мг/кг (10). Усі тест-зразки вводились протягом трьох тижнів у дозі 50 мг/кг на добу (максимальна ефективна доза ГА - ЕДго за протизапальною дієюNephroprotective properties of HA derivatives - HA hydrochloride (HA g/x) and its active metabolite NAGA were studied by i.v. and i.m. routes of administration in rats under conditions of development of membranous kidney damage. Результати дослідження свідчать, що серед вивчених похідних ГА неф-ропротекторні властивості найбільш виражені у НАГА при обох шляхах введення, але у більшій мірі за умов в/м застосування. Це проявлялось у вірогідному відносно контрольної патології зниженні протеїнурії у 4,1 разу й збільшенні ШКФ у 2,4 разу до інтактного рівня, що говорить про нормалізацію функціонального стану нирок. Також даний тест-зразок сприяв зниженню деструктивних та вільнорадикальних процесів у нирках, про що свідчить вірогідне збільшення вмісту ендогенного НАГА у 2,3 разу до рівня інтакту та зниження вмісту ТБК-Р у 1,6 разу (табл. 1). При цьому даний препарат вірогідно перевершував активність ГА г/х при в/м введенні за показниками протеїнурії та ТБК-Р нирок, а також ГА г/х при в/ш введенні за всіма дослідженими параметрами. Таким чином, НАГА є найперспективнішим похідним ГА для застосування у якості засобу лікування захворювань нирок, який може з успіхом використовуватись як при в/м, так й при в/ш шляху введення.The results of the study indicate that among the studied GA derivatives, the nephroprotective properties are most pronounced in NAGA by both routes of administration, but to a greater extent under the conditions of intravenous use. At the same time, this drug probably surpassed the activity of HA g/x when administered intravenously according to the indicators of proteinuria and TBC-R of the kidneys, as well as HA g/x when administered intravenously according to all the studied parameters. Thus, NAGA is the most promising derivative of HA for use as a means of treating kidney diseases, which can be successfully used both by intramuscular and intravenous route of administration.

З метою підтвердження ефективності НАГА при гострій нирковій недостатності проводили вивчення його впливу за умов парентерального введення на моделі міоглобінуричної нефропатії у щурів, яку відтворювали шляхом в/м ведення 50 95 розчину гліцеролу у дозі 10 мл/кг (10)Ї. НАГА вивчали у дозі 50 мг/кг (згідно з прикладом 1) при щоденному в/м та в/в введенні протягом 7 діб у порівнянні з препаратом Корвітин, який вводили в/о у дозі 34 мг/кг (ЕДоо за нефропротекторною активністю (7/1). Отримані дані свідчать, що НАГА як при в/м, так і при в/в шляхах введення чинить виражену позитивну дію на перебіг гострої ниркової недостатності у щурів без статистичних відмінностей один від одного. При цьому спостерігається вірогідне порівняно з нелікованими тваринами зниження летальності з 50 95 до 10 95, збільшення діурезу у 1,5-1,6 разу, зниження протеїнурії у 2,0-2,2 разу, підвищення екскреції креатиніну - у 1,3 разу й сечовини - у 1,7 разу, що в цілому говорить про нормалізацію функціонального стану нирок й азотистого обміну (табл. 2). Слід відмітити, що за рівнем активності НАГА при обох шляхах введення вірогідно перевершив дію референс-препарату Корвітину за більшістю показників. Як у прикладі 2 НАГА вивчали у дозі 50 мг/кг при щоденному в/м та в/ш введенні протягом трьох тижнів у порівнянні з препаратом Корвітин, що вводився в/о у дозі 34 мг/кг. Результати, наведені у таблиці З свідчать, що НАГА є ефективним засобом для лікування хронічної ниркової недостатності при будь-якому шляху введення, але більше при в/м застосуванні. Так, під впливом аміноцукру при в/м введенні повністю зникали випадки летальності у тварин, спостерігалось вірогідне відносно групи контрольної патології збільшення діурезу у 1,7 разу, ШКФ - у 4,7 разу та канальцевої реабсорбції - на 3,2 95, що загалом говорить про покращення видільної функції нирок. Вагомим є те, що НАГА при в/м введенні за умов хронічної ниркової недостатності за деякими показниками був вірогідно краще, ніж при в/ш застосуванні. До того ж за ступенем активності він достовірно перевершив дію препарату порівняння Корвітину за більшістю вивчених показників. Отже, НАГА є ефективним засобом корекції ниркової недостатності різного походження і може розглядатись як перспективний препарат для комплексної терапії ХХН.The results shown in Table C indicate that NAGA is an effective agent for the treatment of chronic renal failure by any route of administration, but more so when administered intravenously. В результаті дослідження було доведено, що НАГА проявляє виражену гіпоазотемічну дію та сприяє виведенню азотистих сполук з організму щурів із сечею. При цьому за умов в/м введення даний гексозамін є вірогідно кращим, ніж при в/ш застосуванні. Так, під його впливом спостерігалось достовірне порівняно з нелікованими тваринами зниження вмісту у крові креатиніну у 2,3 разу та сечовини у 2,5 разу, а також відповідне підвищення їх сечової екскреції: креатиніну - у 2,0 рази, а сечовини - у 1,5 разу (табл. 4). З метою оцінки антиоксидантних властивостей НАГА при нирковій недостатності було проведено вивчення його впливу на показники ПОЛ щурів з сулемовою нефропатією за схемою, використаною у прикладі 3. Через З тижні після відтворення патології щурів виводили з досліду і у крові та тканині нирок визначали вміст продуктів ПОЛ: первинних - дієнових кон'югатів (ДК) та вторинних - ТБК-Р. Це свідчить про пригнічення інтенсивності ліпопероксидації у нирках та крові тварин під впливом дослідного аміноцукру. До того ж за ступенем антиоксидантної дії НАГА при в/м введенні вірогідно перевершував активність при в/ш застосуванні за показниками ДК крові та ТБК-Р нирок.
