Abstract
The invention relates to a method for predicting cardiovascular events within one year after regression of lymphoproliferative diseases by which the enzyme immunoassay determines the level of VE-cadherin.
Description
The invention relates to medicine, namely hematology and cardiology, and can be used to predict the condition of patients, namely the probability of death, cardiac arrhythmias, cardiac ischemic events, stroke, chronic heart failure and hospitalizations related to cardiovascular causes after regression lymphoproliferative diseases.
Cardiovascular events are a limiting factor in the treatment of oncohematological diseases. The successes of modern therapy of chronic lymphoproliferative diseases (chronic lymphocytic leukemia, lymphomas, multiple myeloma) have led to a significant increase in the life expectancy of patients, which led to an increase in the importance of cardiovascular events as a cause of reduced quality of life, death of patients after regression of lymphoproliferative diseases. The problem of predicting the occurrence of cardiovascular events in patients after regression of lymphoproliferative diseases remains unsolved, which makes the development of new, more reliable methods of predicting such conditions urgent.
У крові в день звернення визначають коефіцієнт коагуляції і при величині його більше 2,8 прогнозують несприятливий перебіг післяїнфарктного періоду (післяїнфарктна стенокардія, повторний інфаркт міокарда, раптова смерть), при значенні коефіцієнта менше 2,7 прогнозують неускладнений перебіг реабілітаційного періоду протягом 1 року спостереження (Патент РФ 2115126, МПК СО1ТМ 33/86There is a known method of predicting cardiovascular events in patients after an acute myocardial infarction, which consists in examining venous blood in patients 2 months after an acute myocardial infarction at the outpatient rehabilitation stage. In the blood on the day of application, the coagulation coefficient is determined, and if its value is more than 2.8, an unfavorable course of the post-infarction period is predicted (post-infarction angina, repeated myocardial infarction, sudden death), if the coefficient value is less than 2.7, an uncomplicated course of the rehabilitation period is predicted during 1 year of observation ( Patent of the Russian Federation 2115126, IPC СО1ТМ 33/86
10.07.1998).A method of predicting the course of the postinfarction period. - Publ. 10.07.1998).
A common essential feature of the analogue and the claimed method is the performance of immunoenzymatic blood analysis for predicting the occurrence of adverse cardiovascular events.
This method is not efficient enough because, firstly, the coagulation factor is not a generally accepted study due to the large error in the research results and the low correlation between the coagulation factor and the frequency of unfavorable prognosis in the post-infarction period. Secondly, determining the indicator after 2 months increases the probability of dispersion of results due to the fact that myocardial infarction is not the only factor that
Zo can affect the coagulation factor during such a long period.
- 2005.12.01.)There is a well-known method of predicting atrial fibrillation, which consists in performing an enzyme-linked immunosorbent assay for the presence of Spiatuadia rpeshtopiae and determining the quantitative level of inflammatory markers: C-reactive protein, tumor necrosis factor alpha. In the case of a seropositive result, the level of C-reactive protein higher than 1.8 mg/l and tumor necrosis factor alpha higher than 2.4 pg/dL predict the occurrence of atrial fibrillation in patients with coronary heart disease (Patent Mo 2303266 RF IPC СО1М 33/68 A method of predicting atrial fibrillation in patients with ischemic heart disease // Izobreteniya. Useful models. - 2005.12.01.)
A common essential feature of the analogue and the claimed method is the performance of immunoenzymatic blood analysis.
This method is not effective enough, because the definition of Spiatuadia rpeitopiaye is not specific for ischemic heart disease (the basis of which is the process of atherosclerotic damage of coronary arteries). There are rather contradictory data regarding the etiological connection of S. rpeitopiae with atherosclerosis. So, firstly, S. rpeptopiae was found in the macroscopically unchanged aortic wall, and secondly, the insufficient effectiveness of antibiotics in the prevention of atherosclerosis cast some doubt on the idea of the role of S. rpeptopiae in the etiology of this disease.
39/395. When it increases to more than 600 pg/ml, the presence of a cardiovascular disease and/or the risk of a cardiovascular event (myocardial infarction, thromboembolic stroke, cardiovascular death) is established. (Rybiysayop pitbeg: 791908482 Siavviyysayop: AbIK 39/395. Meipoadz oi adiadpovzi5, rgodpovzib apa eeaitepi oi sagaiomazstiag disease. - Rybiysayop daie: 2011-04-05 // poru//rash. i5rio.dom).
A common essential feature of the prototype and the claimed method is the immunoenzymatic analysis of blood to determine the prognosis of the occurrence of cardiovascular events.
This method is not effective enough because, firstly, the macrophage inhibitory cytokine is not only in activated macrophages, but also in the kidneys, pancreas, prostate, and placenta. Therefore, the definition of macrophage inhibitory cytokine is not specific for cardiovascular events. Secondly, there are quite contradictory data regarding the connection of macrophage inhibitory cytokine with oncological diseases. Thus, high concentrations of macrophage inhibitory cytokines were detected in patients with breast, rectal, and prostate cancer, which certainly reduces the specificity of this method of diagnosis and prognosis of cardiovascular disease.
The invention is based on the task of improving the method of predicting cardiovascular events within one year after the regression of lymphoproliferative diseases by determining the plasma level of cadherin, which will increase the reliability of the forecast.
The task is solved by the fact that in the method that includes immunoenzyme analysis of blood, the new thing is that the level of ME-cadherin is determined, and if the indicator is 0.88 ng/ml or more, then the occurrence of cardiovascular events, namely sudden cardiac death, is predicted , fatal and non-fatal arrhythmias, fatal and non-fatal myocardial infarction, fatal and non-fatal stroke, heart failure within a year after achieving regression of lymphoproliferative disease.
The cause-and-effect relationship between the set of declared signs, according to the technical result, is as follows.
Over the past decades, the success of modern therapy of chronic lymphoproliferative diseases has led to a significant increase in the life expectancy of patients, which has led to an increase in the importance of cardiovascular events as causes of a decrease in the quality of life, death of patients.
Cardiovascular events after regression of lymphoproliferative diseases include sudden cardiac death, fatal and nonfatal arrhythmias, fatal and nonfatal myocardial infarction, fatal and nonfatal stroke, and heart failure.
As for the risk factors for the occurrence of cardiovascular diseases, in addition to age, history of heart diseases, hypertension, diabetes, smoking, obesity, dyslipidemia, which apply to the population as a whole, patients with lymphoproliferative diseases have additional risk factors, namely dysfunction of the cardiovascular system due to infiltration by a pathological clone of lymphocytes, damage
From endotheliocytes as a result of the impact on mitochondrial electron transport systems of free radicals, which are formed after chemotherapy and radiation therapy. Activation of signaling pathways, release of pro-inflammatory cytokines, disruption of angiogenesis processes are important.
Determination of prognostic factors for the occurrence of cardiovascular events in patients after achieving regression of lymphoproliferative diseases is an important and not fully resolved task. It is clear that to predict the occurrence of cardiovascular events in patients with lymphoproliferative diseases, it is necessary to use specific markers that are associated with all risk factors for the mechanisms of occurrence of cardiovascular events in this population. Such markers include ME-cadherin.
ME-cadherin is a molecule of intercellular adhesion of endothelial cells and plays an important role in the regulation of the endothelial barrier and inhibits the excessive growth of blood vessels. When involved in the inflammatory process, angiogenesis is stimulated by regulating the permeability of endothelial cells through the expression of ME-cadherin on their surface. It was established that in the smooth muscle cells of the arterial wall, ME-cadherin ensures the signaling effects of low-density lipoproteins.
The expression of ME-cadherin in endothelial cells, smooth muscle cells and pericytes of the arterial wall increases during atherosclerosis and neoangiogenesis, which indicates the role of ME-cadherin as one of the regulators of pathological angiogenesis in the development of cardiovascular events.
Extravasal infiltration by inflammatory cells and cells of a pathological clone in oncological diseases, changing vascular permeability, affects tumor-dependent angiogenesis. Although many different molecules are involved in the process of tumor-dependent angiogenesis, all of them are related to the key action of ME-cadherin, namely the effect on the stabilization of the endothelial barrier and homeostasis due to its adhesive and signaling properties.
The value of dividing the concentration of circulating ME-cadherin for use in prognostic models for the purpose of stratifying patients into high-risk groups for the occurrence of cardiovascular events in patients after regression of lymphoproliferative diseases has not been clarified.
Thus, the proposed method will allow to identify patients of high-risk groups of cardiovascular events in patients after regression of lymphoproliferative diseases within a year, which, in turn, will allow timely prevention of these events.
The method is carried out as follows. After reaching the regression of the lymphoproliferative disease, the patient takes blood from the ulnar vein on an empty stomach with a disposable needle (diameter 0.8-1.1 mm) in a disposable syringe with a volume of 5 ml. When drawing blood into a syringe, the blood from it is carefully (without foaming) transferred into a disposable test tube. Separate the plasma as quickly as possible to avoid hemolysis, only non-hemolyzed samples are used. The tube with blood is centrifuged for 15 minutes at 3000 revolutions per minute to separate the plasma. The plasma sample from the test tube is transferred directly into microplates for enzyme immunoassay. Determination of ME-cadherin is carried out by the enzyme-linked immunosorbent assay method at room temperature immediately after a blood sample has been taken. The obtained results are interpreted as follows: if the indicator and if the level of ME-cadherin is 0.88 ng/ml and more, then the occurrence of cardiovascular events is predicted, namely, sudden cardiac death, fatal and non-fatal arrhythmias, fatal and non-fatal myocardial infarction, fatal and non-fatal stroke, heart within a year after achieving regression of lymphoproliferative disease.
Example 1. The patient, P., born in 1947, was hospitalized at the "Zaporizhia Regional Clinical Hospital" ZOR, medical history No. 9412, in which regression of chronic lymphocytic leukemia was recorded based on the clinical picture, hemogram, and myelogram. Blood was taken from the patient's ulnar vein on an empty stomach with a disposable needle with a diameter of 1.1 mm in a disposable syringe with a volume of 5 ml. The blood in the syringe was carefully (without formation of foam) transferred to a disposable test tube. The plasma was separated, namely the tube with blood was centrifuged for 15 minutes at 3000 revolutions per minute to separate the plasma. The plasma sample from the test tube was transferred directly into microplates for enzyme immunoassay. ME-cadherin was determined by enzyme immunoassay at room temperature immediately after the blood sample was taken. The result was 0.15 ng/ml.
