Abstract

Цель устройства - обеспечить быстрый стабильный и малотравматичный остеосинтез, позволяющий удерживать костные отломки в состоянии репозиции без дополнительной иммобилизации. Устройство может быть использовано при оперативном лечении переломов дистального метаэпифиза лучевой кости, внутрисуставных переломов голени, ключицы, плечевой кости и др. Устройство имеет пластины, на одном из концов которой расположены моделируемые лапки, второй выполнен скругленным. На лапках имеются шипы, которые загибаются во время операции под необходимым углом и погружаются в дистальный отломок метаэпифиза лучевой кости, обеспечивая надежное удержание костных фрагментов. Дополнительной иммобилизации не требуется, к разработке движений в суставах можно приступить в раннем послеоперационном периоде. Малые размеры устройства делают остеосинтез менее травматичным.The purpose of the device is to provide fast stable and slightly traumatic osteosynthesis, which allows to keep bone fragments in a state of reposition without additional immobilization. The device can be used in the surgical treatment of fractures of the distal metaepiphysis of the radius, intraarticular fractures of the lower leg, clavicle, humerus, etc. The device has plates, on one end of which there are modeled legs, the second is rounded. On the legs there are spikes that are bent during the operation at the required angle and immersed in the distal fragment of the radial bone metaepiphysis, ensuring reliable retention of bone fragments. Additional immobilization is not required, the development of movements in the joints can begin in the early postoperative period. The small size of the device makes osteosynthesis less traumatic.

Технический результат достигается за счет следующих особенностей устройства. Оно имеет вид лапчатой пластины и состоит из основания (1, Фиг.1), расширенного лапчатого (2, Фиг.1) и свободного (3, Фиг.1) концов. На лапчатом конце расположены не менее двух лапок (4, Фиг.1) с заостренными шипами (5, Фиг.1), причем одна лапка длиннее остальных (6, Фиг.1). В основании имеются отверстия (7, Фиг.1). Устройство изготовлено из биологически инертного сплава (нержавеющей стали или титана). Длина, ширина устройства и количество лапок в зависимости от сустава и типа перелома могут быть различными. К проксимальному отломку пластина фиксируется кортикальными винтами АО, и если имеется такая возможность, дополнительный винт проводится в дистальный отломок. Рана послойно зашивается. Устройство позволяет восстановить ось конечности, ее длину, волярный и радиоульнарный угол в суставе. Особенностью внутрисуставных переломов являются малые размеры дистального отломка, что затрудняет его стабильную фиксацию. Загнутые и погруженные в дистальный отломок шипы противодействуют смещающим его силам и обеспечивают надежное удержание отломков в достигнутом положении. Необходимость в применении дополнительной иммобилизации при таком остеосинтезе отсутствует, что делает возможным раннее восстановление функции суставов, например при переломах лучезапястного и пальцев кисти. Небольшие размеры устройства позволяют снизить травматичность операции при сохранении стабильности фиксации отломков. При оскольчатых переломах устройство также позволяет удерживать фрагменты дистального метаэпифиза кости в положении репозиции за счет увеличения точек фиксации. There is no need to use additional immobilization in this osteosynthesis, which makes possible early restoration of joint function, for example, with fractures of the wrist and fingers. The small dimensions of the device can reduce the invasiveness of the operation while maintaining the stability of fixation of fragments. With comminuted fractures, the device also allows you to hold fragments of the distal metaepiphysis of the bone in position of reposition by increasing the fixation points. The use of the device is possible with intraarticular fractures of the lower leg, clavicle, humerus, etc.

Больная Г., 22 лет, поступила в отделение травматологии ВОКБ №1 в плановом порядке 19 июня 2001 года, история болезни №7952/3, с жалобами на боли в области правого предплечья. Травму получила 11 июня 2001 года в быту. Первая помощь была оказана в БСМП - гипсовая лонгета. На рентгенограмме правого предплечья с захватом локтевого и лучезапястного суставов в прямой и боковой проекции от 11.06.2001 г. имеются многооскольчатый перелом дистального метаэпифиза лучевой кости и оскольчатый перелом нижней 1/3 локтевой костей со смещением отломков. Диагноз: Закрытый многооскольчатый перелом дистального метаэпифиза лучевой кости со смещением отломков. Закрытый оскольчатый перелом нижней 1/3 локтевой кости со смещением отломков. 22 июня 2001 года произведена операция: открытая репозиция и металостеосинтез костей предплечья. После репозиции произведен остеосинтез перелома дистального метаэпифиза лучевой кости, состоящего из множества фрагментов, устройством для остеосинтеза переломов дистального метаэпифиза лучевой кости и одним винтом, локтевая кость дополнительно фиксирована тремя спицами. На контрольных рентгенограммах стояние отломков хорошее. Послойно швы, асептические повязки, гипсовая лонгета. На 10-ый день после операции сняты швы, гипсовая лонгета снята. Больная Г. приступила к реабилитации: массаж правого плеча и предплечья, ЛФК. Через один месяц после операции на рентгенограммах правого предплечья в прямой и боковой проекции сросшиеся в условиях металостеосинтеза переломы обеих костей правого предплечья. Спицы удалены. В течении последующей недели объем движений в лучезапястном суставе и лучелоктевом суставе восстановился полностью. Трудоспособность восстановлена. After reposition, osteosynthesis of a distal radial metaepiphysis fracture consisting of many fragments, a device for osteosynthesis of distal radial metaepiphysis fractures and one screw was performed, the ulna was additionally fixed with three knitting needles. On the control radiographs, the standing of the fragments is good. Seams in layers, aseptic dressings, plaster casts. On the 10th day after the operation, the sutures were removed, the plaster cast was removed. Patient G. started rehabilitation: massage of the right shoulder and forearm, exercise therapy. One month after the operation, on radiographs of the right forearm in direct and lateral projection, fractures of both bones of the right forearm, fused under the conditions of metallosteosynthesis, are fused. Knitting needles removed. Over the next week, the range of motion in the wrist joint and the elbow joint was fully restored. Disability restored. Typically, with such fractures, the recovery period is much longer.
