Abstract
Предлагаемая полезная модель относится к медицине, а именно к хирургии и может быть использована для лечения верхнего медиастинита. Сущностью модели является то, что на внешней трубке установлена эластичная манжета с возможностью ее перемещения вдоль осевой линии трубки, снабженная приспособлением для нагнетания в нее воздуха. Выше манжеты посредством тройника к трубке подсоединены вакуумное устройство и моновакуумметр для контроля за отрицательным давлением, создаваемым в гнойной полости средостения. Над манжеткой на внешней трубке установлен перфорированный дренаж-кольцо, для эвакуации скапливающегося здесь патологического экссудата. Конец этого дренажа присоединен к вакуумному насосу. Устройство обеспечивает создание вакуума в полости средостения с постоянным аспирированием гнойного отделяемого, промыванием и контролем за отрицательным давлением в данной полости. Предлагаемое устройство позволяет более эффективно дренировать средостение по сравнению с другими дренажами и способствует быстрейшему выздоровлению больного, уменьшает летальность, повышает степень медицинской и социальной реабилитации.The proposed utility model relates to medicine, namely to surgery and can be used to treat upper mediastinitis. The device provides a vacuum in the cavity of the mediastinum with constant aspiration of purulent discharge, washing and monitoring negative pressure in this cavity. The proposed device allows more efficient drainage of the mediastinum compared with other drains and contributes to the faster recovery of the patient, reduces mortality, increases the degree of medical and social rehabilitation.

Предупреждение распространения гноя по средостению при верхних медиастинитах во многом зависит от эффективности удаления гноя из полости средостения и промывания ее антисептиками. Вакуумирование из замкнутой полости наиболее оптимально в условиях ее герметичности. Поэтому создание двухпросветного дренажа с герметизацией в области верхнего грудного отверстия с помощью манжетки, предупреждающей развитие пролежней мягких тканей и стенок сосудов, а также постоянное контролируемое вакуумирование, значительно повышает эффективность дренирования по сравнению с известными методами.Prevention of the spread of pus on the mediastinum with upper mediastinitis largely depends on the effectiveness of removing pus from the mediastinal cavity and washing it with antiseptics. Дренажную трубку вводят через верхнее грудное отверстие в гнойную полость средостения. При этом трубка подсоединена к аспиратору. За ближайший аналог предлагаемого устройства принят дренаж Н.Н.Каншина разработанный в 1971 году (И.И.Греков «Хирургический вестник», статья Н.Н.Каншина «Сравнительная оценка различных методов дренирования средостения при лечении заднего гнойного медиастинита», том №4, стр.7-11, 1978 г.). Дренирование заключается в том, что в полость средостения вводят 2-х просветную трубку с диаметрами каналов 1 и 6 мм. По каналу меньшего диаметра осуществляется вливание антисептиков в гнойную полость, по каналу большего диаметра производится постоянная аспирация. Герметичность достигается путем ушивания мягких тканей вокруг 2-х просветной трубки.The closest analogue of the proposed device is the drainage of N.N. Kanshin developed in 1971 (I.I. Tightness is achieved by suturing soft tissues around a 2-lumen tube.

Сущностью модели является то, что на внешней трубке установлена эластичная манжета с возможностью ее перемещения вдоль осевой линии трубки, снабженная приспособлением для нагнетания в нее воздуха. Выше манжеты посредством тройника к трубке подсоединены вакуумное насос и моновакуумметр для контроля за отрицательным давлением, создаваемым в гнойной полости средостения. Над манжеткой на внешней трубке установлен перфорированный дренаж-кольцо, для эвакуации скапливающегося здесь патологического экссудата. Предварительно манжетку (2) устанавливают соответственно верхнему грудному отверстию на таком расстоянии, чтобы дистальный конец трубки располагался на дне гнойной полости. В манжетку шприцом нагнетают воздух до создания надежной герметичности между дренажной трубкой и стенками верхнего грудного отверстия. К проксимальному концу трубки подсоединяют тройник (6), один конец которого идет к вакуумному насосу (3), второй к моновакуумметру (4). Над манжеткой устанавливают дренаж-кольцо (5), конец которого подсоединен к вакуумному насосу. По каналу большего диаметра осуществляется постоянное удаление гноя из средостения, по каналу меньшего диаметра гнойную полость периодически промывают антисептиками. Отрицательное давление постоянно контролировалось моновакуумметром и поддерживалось на уровне 70 мм.вд.ст. Герметичность манжетки не корригировалась и сохранялась в течение 2-х дней. Через 3-е суток в связи с уменьшением размеров гнойной полости манжетка была смещена книзу, а трубку расположили на уровне дна полости. В последующем давление в манжетке корригировалось 1 раз в сутки, смещения манжетки не требовалось.For the treatment of mediastinitis, the proposed device was used. К этому времени рана на шее покрылась грануляционной тканью и 13.09.2005 была произведена вторичная хирургическая обработка с полным закрытием раны кожными лоскутами. 19.09.2005 были сняты швы и произведена контрольная рентгенография грудной полости.
