Группа изобретений относится к стоматологии. Устройство для атравматичного удаления зубов, включает активный проволочный элемент из материала с эффектом памяти формы, фиксатор приложения усилия, выполненный с возможностью закрепления на выдвигаемом из челюсти зубе и фиксации среднего участка активного проволочного элемента и опорные фиксаторы. Опорные фиксаторы выполнены с возможностью фиксации крайних участков активного проволочного элемента на опорных зубах одного зубного ряда, между которыми в этом же зубном ряду расположен зуб, выдвигаемый из челюсти. Активный проволочный элемент выполнен с возможностью изгиба на среднем участке из первоначальной формы на величину не менее величины требуемого вертикального перемещения зуба. Показание к применению способа сохранения и подготовки альвеолярной костной ткани к имплантации без применения остеопластических материалов осуществляется также за счет возможности применения данного способа в зубах с субгингивальным разрушением. Осуществление изобретения:Implementation of the invention:
Показаниями для применения предлагаемого изобретения является наличие тотального субгингивального дефекта коронковой части зуба в следствии кариозного поражения, травмы, резорбции в переднем отделе верхней и нижней челюсти. Перед осуществлением способа сохранения альвеолярной костной ткани при помощи трансдентальной экструзии зуба производят постановку местной инфильтрационной анестезии. Затем осуществляют прямым методом снятие слепка для изготовления временной пластмассовой конструкции, являющейся горизонтальной опорой для проведения экструзии зуба. После чего осуществляют удаление подвижной коронковой части при наличии у пациента трещины или перелома коронковой части. Далее производят механическую обработку измененных тканей зуба турбинным наконечником и осуществляют изоляцию мягких тканей зуба способом растяжения лентой из политетрафторэтилена, описанном в Способе протезирования зуба по патенту RU №2800252 (опубл. 19.07.2023). Пломбируют полость зуба жидкотекучим композитным материалом для фиксации стекловолоконного штифта и в последующем для создания культи зуба. Изготавливают временную пластмассовую конструкцию прямым методом с использованием ранее снятого слепка, которая является горизонтальной опорой. Формируют трансдентальный канал для позиционирования стекловолоконного штифта. Устанавливают стекловолоконный штифт и временную пластмассовую конструкцию на соседние опорные зубы в подготовленную площадку, фиксируют их жидкотекучим композитным материалом. Затем фиксируют резиновые тяги и фиксируют зуб к временной пластмассовой конструкции. Спустя 2 дня достигается выдвижение зуба до заданных параметров и производится фиксация зуба к временной конструкции жидкотекучим композитным материалом. Через 2 недели проводится сегментация коронковой части зуба и создание повторного трансдентального доступа апикальнее предыдущей позиции с припасовкой стекловолоконного штифта. Проводится повторная фиксация резиновых тяг. Данные этапы повторяются до момента полного атравматичного выведения корня зуба из альвеолы.Indications for the use of the present invention is the presence of a total subgingival defect of the crown of the tooth as a result of carious lesions, trauma, resorption in the anterior part of the upper and lower jaw. Before implementing the method of preserving alveolar bone tissue using transdental tooth extrusion, local infiltration anesthesia is administered. Then, a direct impression is taken to make a temporary plastic structure, which serves as a horizontal support for tooth extrusion. After that, the movable coronal part is removed if the patient has a crack or fracture of the coronal part. Next, the modified tooth tissues are mechanically processed with a turbine tip and the soft tissues of the tooth are isolated by stretching with a polytetrafluoroethylene tape, described in the Method of tooth prosthetics according to patent RU No. 2800252 (published on July 19, 2023). The tooth cavity is filled with a fluid-flowing composite material to fix the fiberglass pin and subsequently to create a tooth stump. A temporary plastic structure is made using the direct method using a previously taken impression, which serves as a horizontal support. A transdental canal is formed to position the fiberglass pin. A fiberglass pin and a temporary plastic structure are installed on the adjacent abutment teeth in the prepared area, and they are fixed with a fluid-flowing composite material. Then rubber rods are fixed and the tooth is fixed to a temporary plastic structure. After 2 days, the tooth is advanced to the specified parameters and the tooth is fixed to the temporary structure with a fluid composite material. After 2 weeks, the coronal part of the tooth is segmented and a second transdental access is created apical to the previous position with the fitting of a fiberglass pin. The rubber rods are re-fixed. These steps are repeated until the tooth root is completely atraumatically removed from the alveolus.
Способ заключается в сохранении природных параметров альвеолярной костной ткани при подготовке к имплантации. Данный способ позволяет снизить степень физиологической деструкции костной ткани в области постэкстрационной лунки, исключив применение дополнительных методов стимуляции остеогенеза, путем внесения в лунку удаленного зуба костных графтов различного происхождения. С этой целью проводится экструзия причинного зуба в несколько этапов, с трансдентальной фиксацией стекловолоконного штифта, что позволяет расширить показания применения данной методики, в частности в клинических случаях с субгингивальными дефектами, а также трещинами корневой части зубов.The method consists in preserving the natural parameters of the alveolar bone tissue in preparation for implantation. This method makes it possible to reduce the degree of physiological destruction of bone tissue in the area of the post-extraction socket, eliminating the use of additional methods of stimulating osteogenesis, by introducing bone grafts of various origins into the socket of the extracted tooth. For this purpose, extrusion of the causative tooth is carried out in several stages, with transdental fixation of a fiberglass pin, which allows expanding the indications for the use of this technique, in particular in clinical cases with subgingival defects, as well as cracks in the roots of the teeth.

Пациентка Н. обратилась с жалобами на наличие отека и боль в области центрального зуба на верхней челюсти слева (Фиг.1 и 1а). Пациентка была направлена на компьютерную томографию, оценка которой продемонстрировала наличие перелома коронковой части зуба, деструктивные изменения в периапикальной области (Фиг. 2).Patient N. complained of swelling and pain in the area of the central tooth on the left upper jaw (Figures 1 and 1a). The patient was sent for a computed tomography scan, the evaluation of which demonstrated the presence of a fracture of the coronal part of the tooth and destructive changes in the periapical area (Fig. 2).
Затем пациентке была проведена механическая обработка коронковой части зуба 21, удаление подвижных и измененных тканей и изоляция мягких тканей (Фиг.3). Клинический осмотр, а также диагностика с применением кариес- маркера, выявили трещину медиальной и дистальной поверхности корня зуба на глубину 4 мм (Фиг.4).Then the patient underwent mechanical treatment of the coronal part of tooth 21, removal of mobile and changed tissues and isolation of soft tissues (Figure 3). Clinical examination, as well as diagnostics using a caries marker, revealed a crack in the medial and distal surface of the tooth root to a depth of 4 mm (Figure 4).

После чего произведена фиксация опорного стекловолоконного штифта на соседние зубы (Фиг.6). Изготовлена временная пластмассовая конструкция методом силиконового штифта и произведена ее фиксация на опорные зубы ( Фиг.7 и 7а). Произведено создание трансдентального доступа для позиционирования опорного стекловолоконного штифта и изготовлены композитные фиксаторы временной пластмассовой конструкции (Фиг.8 и 8а). Процедура повторяется в несколько этапов до достижения максимальной экструзии зуба. Еще через 2 недели произведена повторная сегментация коронковой части зуба и произведено повторное создание трансдентального доступа апикальнее предыдущей позиции, достижение зубом заданной высоты, создание и фиксация временной коронки (Фиг. 13, 13а, 13б и 13в).The procedure is repeated in several stages until maximum tooth extrusion is achieved. Анализ рентгенологических данных, полученных методом конусно-лучевой томографии, в сравнительном аспекте со снимком до начала осуществления способа сохранения альвеолярной костной ткани при помощи трансдентальной экструзии зуба для подготовки альвеолярной костной ткани к имплантации, продемонстрировал сохранение вестибулярной стенки, а также альвеолярной костной ткани максимально приближенной к исходным значениям, что позволило провести одномоментную имплантацию после удаления корня зуба 21. Клинически так же было достигнуто сохранение мягкотканого профиля и межзубных сосочков.Analysis of radiological data obtained by cone-beam tomography, in a comparative aspect with an image before the implementation of the method of preserving alveolar bone tissue using transdental extrusion of a tooth to prepare alveolar bone tissue for implantation, demonstrated the preservation of the vestibular wall, as well as alveolar bone tissue as close as possible to initial values, which made it possible to carry out one-stage implantation after removal of the root of tooth 21. Clinically, the preservation of the soft tissue profile and interdental papillae was also achieved.

Осуществление способа заключается в отсутствии специальных материалов и оборудования, также в его малоинвазивности. Для достижения поставленных целей в качестве вертикальной опоры используют стекловолоконный штифт и временную пластмассовую конструкцию, зафиксированную на соседние зубы. Изготовление временной пластмассовой конструкции позволяет пациентам максимально комфортно пройти все этапы лечения, а также сформировать у пациента мягкотканный контур и сохранить естественную анатомию межзубных сосочков.The implementation of the method consists in the absence of special materials and equipment, as well as in its minimal invasiveness. To achieve these goals, a fiberglass pin and a temporary plastic structure fixed to adjacent teeth are used as a vertical support. The production of a temporary plastic structure allows patients to go through all stages of treatment as comfortably as possible, as well as to form a soft tissue contour for the patient and preserve the natural anatomy of the interdental papillae.
Этапность лечения состоит в постепенной трансдентальной экструзии зуба для стимуляции биологического потенциала организма и достижения необходимого объема и качества нативной костной ткани в лунке причинного зуба.The stage of treatment consists of gradual transdental extrusion of the tooth to stimulate the biological potential of the body and achieve the required volume and quality of native bone tissue in the socket of the causative tooth.
За счет отсутствия дополнительных этапов хирургического лечения, предлагаемый способ сохранения альвеолярной костной ткани при помощи трансдентальной экструзии зуба позволяет сократить временной промежуток планируемого лечения, а также достигнуть природной структуры костной ткани в области планируемой имплантации, что снижает в дальнейшем риски и осложнения.Due to the absence of additional stages of surgical treatment, the proposed method of preserving alveolar bone tissue using transdental tooth extrusion allows us to reduce the time period of the planned treatment, as well as achieve the natural structure of the bone tissue in the area of planned implantation, which further reduces risks and complications.
