Abstract
Description
Изобретение относится к экспериментальной хирургии. Предложен способ моделирования синдрома Ашермана, отличающийся тем, что выскабливается и удаляется весь эндометрий маточных рогов. Операция проводится следующим образом: вскрытие маточных рогов вдоль по всей длине, от воронок до непарной части матки (в которой имплантации не происходит), проводится кюретаж всего эндометрия, затем выполняется гемостаз, и последующее ушивание разреза обвивным швом (также обладающим гемостатическими свойствами). Модель используется для изучения методов лечения синдрома Ашермана, позволяя полностью исключить фертильность при достаточно глубоком и тщательном кюретаже.The invention relates to experimental surgery. A method for modeling Asherman's syndrome is proposed, characterized in that the entire endometrium of the uterine horns is scraped and removed. The operation is carried out as follows: opening of the uterine horns along the entire length, from the funnels to the unpaired part of the uterus (in which implantation does not occur), curettage of the entire endometrium is performed, then hemostasis is performed, and subsequent suturing of the incision with a wrapping suture (also having hemostatic properties). Заболевание известно также как синдром Фритча, травматическая атрофия эндометрия или склероз эндометрия. Предлагаемая модель соотносится с тяжелой степенью синдрома Ашермана: сращения плотные, состоят из склерозированной соединительной ткани, заполняют более 75% полости матки, блокируют устья маточных труб, дно матки.Asherman's syndrome, or intrauterine synechiae, is a pathological condition characterized by the formation of adhesions and outgrowths of the endometrium with its sclerosis and fibrosis. The disease is also known as Fritsch syndrome, traumatic endometrial atrophy, or endometrial sclerosis. The proposed model correlates with a severe degree of Asherman's syndrome: the adhesions are dense, consist of sclerotic connective tissue, fill more than 75% of the uterine cavity, block the mouth of the fallopian tubes, and the fundus of the uterus.
Актуальность: синдром Ашермана возникает у женщин всех рас с одинаковой частотой. После выскабливания полости матки у родивших женщин риск возникновения заболевания составляет 25%. Неразвивающаяся, или замершая беременность, чаще приводит к возникновению данной патологии и составляет до 30% случаев.Background: Asherman syndrome occurs in women of all races with equal frequency. В связи с широкой распространенностью и недостаточной эффективностью лечения синдрома Ашермана в последние годы предлагались новые методы лечения этого синдрома, включая клеточную терапию (патент RU 2725006 С2). Однако эндометрий склонен быстро и, главное, полноценно регенерировать, причем даже наличие сформировавшихся спаек в большинстве случаев не приводит к бесплодию, так что создать полноценную модель ВМС у экспериментальных животных крайне трудно, бесплодия добиться удается только у части животных. У всех прочих животных (от 30 до 60% в зависимости от метода) рождается меньше детенышей (чаще всего от 2 до 10). Поэтому в последние годы имеется устойчивая тенденция к созданию сочетанных многокомпонентных способов моделирования ВМС. Для решения данной проблемы нами предложена чисто механическая модель синдрома Ашермана, включающая хирургическое удаление всего эндометрия маточных рогов, отличающаяся тем, что в маточных рогах просто не остается ни одного участка здорового эндометрия, для чего проводят вскрытие маточных рогов вдоль по всей длине, от воронок до непарной части матки (в которой имплантации не происходит), кюретаж всего эндометрия, гемостаз, и последующее ушивание разреза обвивным швом.To solve this problem, we have proposed a purely mechanical model of Asherman's syndrome, including surgical removal of the entire endometrium of the uterine horns, characterized in that there simply is not a single section of healthy endometrium left in the uterine horns, for which purpose the uterine horns are opened along the entire length, from the funnels to unpaired part of the uterus (in which implantation does not occur), curettage of the entire endometrium, hemostasis, and subsequent suturing of the incision with a blanket suture.

Предложенная модель синдрома Ашермана у крыс характеризуется повреждением эндометрия, снижением эндометриальных желез, развитием фиброза и формированием фиброзных спаек, что характерно для клинической картины данного заболевания у людей. Полученная модель синдрома Ашермана у крыс является состоятельной, так как в отличие от прочих разновидностей, дававших при проверке восстановление фертильности почти у трети прооперированных в сроки от четырех месяцев после операции обеспечивается стопроцентное бесплодие пожизненно. После наркотизации, нанесения офтальмогеля на роговицу и обработки передней брюшной стенки (сбривание волос, обработка антисептиком) (Фигура 1, 2) - на передней брюшной стенке производилось послойное рассечение кожи, ПЖК, белой линии живота (лапаротомия). Длина разреза - 5 см. Матка расположена по средней линии, правильной формы, патологические образования не обнаружены. Правый маточный рог - форма не изменена, диаметр равен 4 мм. Правый яичник - без особенностей. Левый маточный рог имеет правильную форму, диаметр - 4 мм. Левый яичник - без патологии (Фигура 7, 8).Next, the abdominal and pelvic cavities were examined. В рану выводились поочередно рога матки. При помощи скальпеля и микрохирургических ножниц, отступив на 3 мм от тела матки, рога вскрывались продольным разрезом, не доходя 3 мм до яйцевода. Микрокюреткой производилось выскабливание эндометрия по всей длине рога. Обеспечение гемостаза (Фигура 9, 10).The horns of the uterus were removed one by one into the wound. Края раны у базального отдела рогов матки фиксировали к брюшине передней брюшной стенки при помощи узловых швов атравматическим материалом 7.0 Prolene на колющей игле - формирование трубчатой стомы.The edges of the wound at the basal part of the uterine horns were fixed to the peritoneum of the anterior abdominal wall using interrupted sutures with atraumatic material 7.0 Prolene on a piercing needle - the formation of a tubular stoma.

С целью профилактики послеоперационных инфекционных осложнений внутрибрюшинно вводили антибиотик широкого спектра (Цефтриаксон 10 мг/кг). Контроль гемостаза. Послойное ушивание лапаротомной раны узловым швом. После прекращения отхождения отделяемого дренажные катетеры №1 удаляется, а выведенные через контрапертуру катетеры №2 обрезаются до длины примерно 2-3 сантиметра снаружи брюшной полости, сворачиваются и укладываются во временный подкожные карманы, также ушиваемый поверх катетеров, для предотвращения контаминации. Обработка послеоперационной раны метрогиловым гелем во избежание погрызов швов.
