Предлагаемое изобретение относится к медицине, а именно к пластической хирургии, комбустиологии, травматологии и может быть использовано у больных, которым требуется выполнение пластического закрытия дефекта мягких тканей лоскутом на временной питающей ножке.The present invention relates to medicine, namely to plastic surgery, combustiology, traumatology, and can be used in patients who require plastic closure of a soft tissue defect with a flap on a temporary pedicle.
Пластика лоскутами на питающих ножках относится к основным приемам пластических операций (Казарезов М.В. и соавт. Контрактуры: Руководство для врачей. Новосибирск, 2002. - С. 106). Кожный покров, восстановленный с помощью лоскутов на питающей ножке, хорошо подвижен и достаточно устойчив к механической нагрузке, что особенно важно при устранении дефекта над обнаженной костью, сухожилиями (Ожоги: Руководство для врачей. Под ред. Б.С. Вихриева и В.М. Бурмистрова. Л.: Медицина, 1981. - С. 143.). При пересадке несвободных комплексов тканей из удаленных анатомических областей конечностям придают вынужденное положение (обеспечивающее связь дефекта с лоскутом) на срок, необходимый для образования достаточных сосудистых связей между пересаженными тканями и воспринимающим ложем, после чего питающую ножку отсекают (Белоусов А.Е. Помимо соответствующего планирования адекватность кровоснабжения лоскутов на ножке может быть достигнута хирургической тренировкой (Азолов В.В., Дмитриев Г.И. Аналогом предлагаемого изобретения нами принят способ комбинированной кожной пластики (Тычинкина А.К. Кожно-пластические операции. М.: Медицина, 1972. - С. 121-139.). По данному способу выкраивают и отслаивают лоскут на ножке, раневое ложе закрывают расщепленным кожным трансплантатом, после чего лоскут на ножке укладывают на прежнее место. Операцию заканчивают наложением узловых швов между краями лоскута и его ложа. Вторым этапом повторно выкраивают ранее сформированный лоскут и производят им пластику дефекта.Analogous to the proposed invention, we have adopted a method of combined skin plastics (Tychinkina A.K. Способ предусматривает формирование филатовского стебля, миграцию одной из его ножек, выкраивание и мобилизацию кожно-жирового лоскута по продолжению неотсеченной ножки стебля, закрытие раны под лоскутом свободным кожным трансплантатом, поверх которого снова укладывают мобилизованный лоскут и фиксацию его швами. Осуществление способа хирургической тренировки кожно-жирового лоскута на временной питающей ножке выполняют следующим образом. Первым этапом в условиях операционной после обработки операционного поля скальпелем рассекают кожу, формируя границы кожно-жирового лоскута. Далее подкожно-жировую клетчатку питающей ножки лоскута пересекают частично на глубину, необходимую для последующего моделирования кожно-жирового лоскута, после чего операционную рану зашивают редкими узловыми швами. Вторым этапом в условиях операционной снимают ранее наложенные швы. После обработки операционного поля края ушитой раны разводят, скальпелем производят мобилизацию краев питающей ножки, пересекая подкожно-жировую клетчатку на глубине, необходимой для последующего моделирования кожно-жирового лоскута. Производят гемостаз. Для закрытия раны швы не накладывают, а сближение краев и дренирование операционной раны осуществляют путем наложения вакуумной повязки с постоянным давлением 90 мм.рт.ст. Вакуумная повязка при этом позволяет следить за состоянием кожно-жирового лоскута после его частичного пересечения и обеспечить усиление кровообращения в краях послеоперационной раны. Следующим этапом в операционной снимают вакуумную повязку и после обработки операционного поля полностью пересекают временную питающую ножку. Выполняют пластическое закрытие кожно-жировым лоскутом. Донорскую рану частично ушивают, уменьшая ее площадь. По показаниям накладывают на донорскую рану вакуумную повязку для стимуляции грануляционного роста и последующего отсроченного закрытия расщепленным кожным трансплантатом.The implementation of the method of surgical training of the skin-fat flap on a temporary feeding leg is performed as follows. The next step in the operating room is to remove the vacuum bandage and, after processing the surgical field, the temporary feeding leg is completely crossed. Plastic closure is performed with a skin-fat flap. The donor wound is partially sutured, reducing its area. According to the indications, a vacuum dressing is applied to the donor wound to stimulate granulation growth and subsequent delayed closure with a split skin graft.
По данному способу в клинических условиях оперировано двое больных, которым была произведена пластика лоскутом на временной питающей ножке по поводу посттравматических дефектов кистей.According to this method, two patients were operated on in a clinical setting, who underwent plastic surgery with a flap on a temporary pedicle due to post-traumatic defects of the hands.
Клинический пример. Пациент Г., 46 лет, поступил в ожоговое отделение ГБУЗ-НИИ ККБ №1 им. проф. С.В. Очаповского с диагнозом: посттравматический рубцовый дефект мягких тканей правой кисти, состояние после пластики кожно-жировым лоскутом на временной питающей ножке (рис. 1). Первым этапом в условиях операционной после обработки операционного поля скальпелем рассекли кожу, формируя границы кожно-жирового лоскута. Далее подкожно-жировую клетчатку питающей ножки лоскута пересекли частично на глубину, необходимую для последующего моделирования кожно-жирового лоскута, после чего операционную рану зашили редкими узловыми швами (рис. 2). Вторым этапом в условиях операционной сняли ранее наложенные швы. После обработки операционного поля края ушитой раны развели, скальпелем произвели мобилизацию краев питающей ножки (рис. 3), пересекая подкожно-жировую клетчатку на глубине, необходимой для последующего моделирования кожно-жирового лоскута. Произвели гемостаз (рис. 4). Для закрытия раны швы не накладывали, а сближение краев и дренирование операционной раны осуществили путем наложения вакуумной повязки с постоянным давлением 90 мм.рт.ст. (рис. 5). Вакуумная повязка при этом позволяла следить за состоянием кожно-жирового лоскута после его частичного пересечения и обеспечить усиление кровообращения в краях послеоперационной раны. Следующим этапом в операционной сняли вакуумную повязку (рис. 6-7) и после обработки операционного поля полностью пересекли временную питающую ножку (рис. 8). Выполнили пластическое закрытие кожно-жировым лоскутом (рис. 9-10). Донорскую рану частично ушили, уменьшая ее площадь (рис. 11), затем наложили на нее вакуумную повязку (рис. 12) для стимуляции грануляционного роста и последующего отсроченного закрытия расщепленным кожным трансплантатом. В послеоперационном периоде трофических расстройств тканей кожно-жирового лоскута не наблюдали.Clinical example. Patient G., aged 46, was admitted to the burn department of the GBUZ-NII KKB No. prof. After processing the surgical field, the edges of the sutured wound were parted, the edges of the feeding pedicle were mobilized with a scalpel (Fig. 3), crossing the subcutaneous fat at a depth necessary for the subsequent modeling of the skin-fat flap. Hemostasis was performed (Fig. 4). To close the wound, sutures were not applied, and the edges were brought together and the surgical wound was drained by applying a vacuum bandage with a constant pressure of 90 mm Hg. (Fig. 5). At the same time, the vacuum dressing made it possible to monitor the condition of the skin-fat flap after its partial intersection and to ensure increased blood circulation in the edges of the postoperative wound. At the next stage, the vacuum bandage was removed in the operating room (Fig. 6-7) and, after processing the surgical field, the temporary feeding leg was completely crossed (Fig. 8). Plastic closure was performed with a skin-fat flap (Fig. 9-10). The donor wound was partially sutured, reducing its area (Fig. 11), then a vacuum dressing was applied (Fig. 12) to stimulate granulation growth and subsequent delayed closure with a split skin graft. In the postoperative period, trophic disorders of the tissues of the skin-fat flap were not observed.
