Известен катетер VANSCHIE3 (Beacon Tip Angiographic Catheter, Cook). Этот катетер используется для катетеризации сосудов и представляет собой полую трубку с загнутым концом. Недостатком аналога является резкий боковой ход первой части (четверть полукруга) загнутого конца катетера (исходный круг диаметром всего 9 мм), что увеличивает риск диссекции артерий при манипуляциях. Следующим недостатком является небольшой угол в центре загнутого конца (место перехода плавного изгиба в прямую линию), который явится причиной недостаточной поддержки и местом вероятного залома катетера при попытке инициации реканализации хронической устьевой окклюзии передней большеберцовой артерии. Концевой загиб в виде прямой линии также является минусом катетера. Прямая форма не поглощает излишнее напряжение, оказываемое на катетер, что, с одной стороны, повышает риск диссекции при манипуляциях, а с другой - увеличивает вероятность залома катетера при попытке реканализации плотной окклюзии передней большеберцовой артерии от устья. Длина катетера 125 см является избыточной. Устройство применяется следующим образом. По ранее заведенному в тибиоперонеальный ствол проводнику под рентгеновским контролем катетер доставляется слегка дистальнее устья передней большеберцовой артерии. Проводник извлекается, после чего оператор ориентирует загнутый конец 1 в сторону отхождения передней большеберцовой артерии и подтягивает катетер. Кривизна катетера при этом сформирована не до конца (концевой загиб 2 разогнут) в связи с небольшим диаметром тибиоперонеального ствола. На уровне отхождения передней большеберцовой артерии концевой загиб 2 полностью принимает форму и катетер попадает в переднюю большеберцовую артерию. В случае окклюзии сосуда оператор делает упор катетером в сторону устья и инициирует реканализацию проводником. В случае окклюзии тибиоперонеального ствола извлечение проводника и поворот катетера осуществляются на уровне устья передней большеберцовой артерии.The device is used as follows. Применен заявляемый катетер, который по проводнику доставлен чуть дистальнее предполагаемого места отхождения передней большеберцовой артерии. Проводник удален для придания катетеру формы. Катетер развернут в сторону отхождения искомого сосуда, подтянут проксимально и его кончик уперся в устье. Инициация реканализации окклюзии проводником при поддержке заявляемым катетером удается без технических сложностей. После успешной инициации реканализации произведена замена катетера на баллон и реваскуляризация артерии выполнена в стандартной манере. Восстановлен магистральный кровоток к зоне некроза на стопе. Интраоперационных и послеоперационных осложнений, в том числе связанных с применением заявляемого устройства, не наблюдалось. Восстановление кровоснабжения стопы создало необходимые условия для заживления раны после некрэктомии, которая была выполнена после ангиопластики. Послеоперационная рана на стопе зажила, пациент в удовлетворительном состоянии выписан.The inventive catheter is applied, which is delivered along the guide wire slightly distal to the intended place of origin of the anterior tibial artery. The postoperative wound on the foot healed, the patient was discharged in satisfactory condition.
