Abstract
Description
The invention relates to medicine, namely to resuscitation, surgery, therapy, and can be used as a salt electrolyte solution (SER) for conducting enteral infusions in resuscitation, surgical, trauma, burn, neurological and therapeutic departments.
Enteral infusion - the introduction into the gastrointestinal tract in the natural direction of a salt electrolyte solution of SER. Patients drink the solution (according to the indications, patients can also administer the solution through a nasogastric or nasoduodenal probe), then the solution is excreted naturally. When the injected volume exceeds 1-2 liters, depending on the individual characteristics of the body, the bowel is emptied without disturbing the electrolyte composition of the blood and tissues, and the detoxification effect is manifested.
The use of saline solutions for infusion therapy for septic diseases is a standard practice for medical institutions. They are used to fill the deficiency of BCC (circulating blood volume), restore tissue perfusion, correct homeostasis and detoxify the body.
The vast majority of these solutions (Acesol, Disol, Trisol, Isosol, Quintasol, Ringer's solution, etc.) are almost identical in composition, are administered intravenously and do not contain all the necessary trace elements in the human body. However, they are often used to correct the water-electrolyte balance and detoxify the body. Moreover, due to the unbalanced composition and method of administration to the human body, they have the same disadvantages: 1) the risk of water-electrolyte disturbances, 2) the inability to detoxify the small intestine, which is often the focus of septic processes in 70% of cases, 3) the risk of introducing nosocomial infections.
The use of the enteral route of administration of saline solutions is hampered by the fact that all widely known solutions (not only saline) for enteral administration have a serious drawback - they are not balanced by the macroelement composition with the small intestine chyme directly involved in the process of water-salt metabolism. Currently, there is no understanding in world medical practice that, if the regulation of water-electrolyte metabolism takes place with the participation of the small intestine chyme, then solutions for enteric infusions should be as close as possible to the composition of the chyme in terms of macroelement composition. In addition, none of the known saline solutions for infusion therapy has both a detoxification effect and the ability to influence the restoration of the intestinal barrier.
Показаниями к применению Регидрона, согласно инструкции лекарственного препарата, являются исключительно: «Диарея у детей, лечение и профилактика тепловых поражений, связанных с нарушением водно-электролитного обмена (интенсивное потоотделение)».The closest analogue of the proposed salt electrolyte solution, both in composition and method of application (enteral) is the well-known drug "Regidron", intended for enteral infusion to correct energy and electrolyte balance, restore water-electrolyte balance, and correct acidosis [Mashkovsky M. D. Medicines In two parts. Part II - 12th ed. M: Medicine, 1993. S. 134], soluble in water, which includes: sodium chloride, potassium chloride, sodium citrate and glucose. Indications for the use of Regidron, according to the instructions of the drug, are exclusively: "Diarrhea in children, treatment and prevention of heat injuries associated with impaired water-electrolyte metabolism (intense sweating)."
Regidron" does not allow you to effectively control the water-electrolyte balance of macrocells due to the lack of macroelements important for regulating the water-electrolyte balance of it, namely magnesium, calcium, phosphorus and sulfur. In addition, Regidron does not allow correcting uncompensated acidosis (if any), positively affecting the qualitative and quantitative composition of the intestinal microflora, normalizing intestinal permeability, and effectively detoxifying the enteral and parenteral environments of the body. Uncontrolled administration of "Rehydron" in resuscitation patients can cause hyperhydration, accompanied by severe complications, hypernatremia, hyperkalemia.
The invention solves the problem of creating salt electrolyte solutions with effective regulation of the water-electrolyte balance of macronutrients during enteral infusions, which can significantly reduce the level of endogenous intoxication and help restore the intestinal barrier.
%):The specified technical result is achieved in that a saline electrolyte aqueous solution for enteral infusion includes sodium chloride, sodium acetate, potassium chloride, sodium phosphate, magnesium sulfate, calcium chloride and water, in the following ratio of components (in wt.%):
%):The specified technical result is also achieved by the fact that the salt electrolyte aqueous solution additionally contains pectin, inulin, glutamine and citric acid, in the following ratio of components (in wt.%):
To stabilize the finished SR, citric acid can be additionally used in an amount of 0.2 to 1.4 g per liter of solution.
A ready-to-use SER solution is prepared by dissolving the contents of a kit, including: component A containing a mixture of sodium chloride, sodium acetate, sodium phosphate, potassium chloride (dry or solution); component B - a solution of magnesium sulfate or dry magnesium sulfate; C) a solution of calcium chloride.
The solution is prepared immediately before use by adding components A, B, C to a container with distilled water heated to a temperature of 35-40 ° C, slowly and thoroughly mixing the mixture until it is completely dissolved. To stabilize the finished SR, citric acid can be used in an amount of 0.2 to 1.4 g per 1 liter of solution.
Other components that have a therapeutic effect can be added during production in the factory, or at the time of mixing and diluting the components of the solution before use.
The introduction of the solution occurs, depending on the patient’s condition, through naso-gastric, naso-intestinal probes or by siping (oral intake in small sips). The rate of administration of the SER solution is from 1.5 to 25 ml per minute.

Patient N., 32 years old, was admitted to the ICU department with a diagnosis of “Cerebral edema, cryptococcal meningoencephalitis. HIV infection". The patient was on a ventilator. Conducted resuscitation therapy allowed to stabilize the patient's condition, at the same time there was a slight deterioration in clinical parameters. In order to restore the propulsive function of the intestine (from the moment of admission to the hospital, i.e. for four days, the patient had no stools), and due to the ineffectiveness of detoxification therapy, it was decided to conduct enteral infusion with the use of SER in the amount of 1 , 5 liters for 2 hours. 5 hours after the start of the procedure, the first portion of a stool of dense consistency, greenish-brown, was obtained. Subsequent batches of intestine became more fluid. Eight hours after the start of the procedure, the use of SER was observed to stabilize the parameters of systemic hemodynamics, which allowed to stop the continuous infusion of norepinephrine. Enteral nutrition is started 12 hours after the start of the administration of SR.
Dynamics of septic state markers within 12 hours from the start of an enteric infusion of SERs: procalcitonin - decrease from 2.21 ng / ml to 1.67 ng / ml, presepsin - decrease from 868 pg / ml to 652 pg / ml, decrease white blood cells from 11.2 * 10 9 / l to 7.4 * 10 9 / l Change in the rate of restoration of the intestinal barrier (fecal calprotectin decreased from 205 μg / g to 47 μg / g).
For the preparation of 1 liter of the prepared solution of SER, the following were used: sodium chloride 4.52 g, sodium acetate 4.2 g, potassium chloride 2.33 g, sodium phosphate 4.75 g, magnesium sulfate 3.20 g, calcium chloride 3.55 g, pectin 10.0 g, inulin 10.0 g, glutamine 20.0, distilled water 937.45 g.
Thus, the use of the claimed solution can reduce markers of endogenous intoxication, contribute to the restoration of the intestinal barrier, which leads to an improvement in the condition of patients and a reduction in the time of their rehabilitation.
The proposed SER is intended for conducting enteral infusions in resuscitation, surgery, neurology, therapy for septic conditions, intestinal paresis, for the early onset (start) of enteral nutrition, cerebral edema, pulmonary edema, for the prevention of infectious complications in resuscitation, surgical, neurological, therapeutic patients both independently and as a base solution, allowing you to add additional components to it, allowing you to improve the properties of the solution or the effectiveness of its use in patients.
