Кроме того, во время оперативного вмешательства в ходе удаления металлоконструкций возможно возникновение значительных костных дефектов. При имеющемся дефекте большеберцовой кости расстояние между артикулирующей поверхностью и большеберцовой костью может быть значительным, составлять несколько сантиметров, в этом случае костный цемент будет «вытекать» из-под пресс-формы тибиального компонента. При артикуляции в предложенной системе цемент-цемент неизбежно будет возникать большое количество дебриза, что существенно увеличивает риск возникновения рецидива воспалительного процесса. Кроме того, отсутствие интрамедуллярного элемента крепления может привести к возникновению нестабильности спейсера уже в раннем послеоперационном периоде. Поставленный технический результат достигается за счет того, что в способе, включающем интраоперационное последовательное изготовление артикулирующей поверхности тибитального компонента из костного цемента с антибиотиком, прикладывание полученной пресс-формы к большеберцовой кости и фиксирование на костный цемент с антибиотиком, разгибая сустав, предварительно на основе данных компьютерной томографии и полученных объемных параметров дефекта большеберцовой кости изготавливают модель спейсера, с одного конца совпадающего с рельефом проксимального отдела большеберцовой кости и костномозгового канала, с другого конца имеющего два углубления в виде протяженных канавок, зеркально повторяющих форму бедренного компонента эндопротеза, по высоте соответствующего имеющемуся дефекту. На основе модели спейсера изготавливают форму с отверстием в корпусе, состоящую из двух частей. Затем интраоперационно собирают форму, через отверстие в корпусе заливают костный цемент с антибиотиком. После установки клинико-рентгенологического диагноза глубокого нагноения после тотального эндопротезирования коленного сустава с дефектом большеберцовой кости производят компьютерную томографию области сустава при сканировании с минимальным шагом для получения полного набора томограмм. Из массива томографических данных отбирают информацию для восстановления образа костных структур определенной плотности, производя отсеивание вокселей с информацией об имевшемся у пациента имплантате. После построения компьютерной триангулярной модели коленного сустава производят гибридное параметрическое моделирование области поврежденного сустава. Затем создают триангулярную модель спейсера, которая совпадает с рельефом проксимального конца большеберцовой кости и имеющегося интрамедуллярного канала, по высоте учитывает потерю костной ткани и размеры имеющегося вкладыша удаляемого эндопротеза, проксимальная часть зеркально повторяет особенности бедренного компонента эндопротеза. На основе модели спейсера создают форму для его отливки с отверстием в корпусе, которую для удобства делят на 2 части и отправляют дистанционно или через жесткий носитель на FDM 3D принтер для ее производства. Интраоперационно изготовленную форму для спейсера собирают в единое целое и через предварительно выполненное отверстие в корпусе заливают костный цемент с антибиотиком. После его отвердения форму демонтируют, сформированный спейсер устанавливают в области дефекта на костный цемент с антибиотиком. Бедренный компонент спейсера устанавливают в обычном порядке. За счет обоих компонентов спейсера, установленных в правильном положении, получают стабильность и осуществляют артикуляцию в области установленного спейсера.After a clinical and radiological diagnosis of deep suppuration is established, after total knee replacement with a tibial defect, computed tomography of the joint area is performed during scanning with a minimum step to obtain a complete set of tomograms. Based on the spacer model, they create a mold for casting it with an opening in the housing, which for convenience is divided into 2 parts and sent remotely or via a hard medium to an FDM 3D printer for its production. Intraoperatively made mold for the spacer is assembled as a whole and bone cement with antibiotic is poured through a pre-made hole in the housing. After hardening, the mold is dismantled, the formed spacer is installed in the area of the defect on bone cement with an antibiotic. The femoral component of the spacer is mounted in the usual manner. Due to both spacer components installed in the correct position, stability is obtained and articulation is carried out in the region of the installed spacer.
Клинический пример: Больная С., 71 года, диагноз: глубокое нагноение после тотального ревизионного эндопротезирования правого коленного сустава. Из анамнеза заболевания выявлено, что эндопротезирование выполнено в 2014 году. Боли, укорочение конечности, свищевой ход возникли в начале 2015 года. Учитывая наличие дефекта большеберцовой кости, в 2015 году выполнено эндопротезирование с использованием ревизионной системы. Нагноение ревизионного протеза с образованием свищевого хода в начале 2016 года. При обследовании в предоперационном периоде по данным рентгенологического обследования в предоперационном периоде выявлен значительный дефект большеберцовой кости. На основании выполненной компьютерной томографии на 3Д принтере отпечатана форма для изготовления тибиального компонента спейсера. Оперативное вмешательство проведено 12.04.2016 года. Интраоперационно с использованием формы изготовлен тибиальный компонент артикулирующего спейсера, который был установлен в дефект большеберцовой кости на костный цемент с антибиотиком. Бедренный компонент спейсера установлен по стандартной методике. Осуществлено вправление спейсера. Тенденции к вывиху не отмечено. Сустав стабилен. Артикуляция осуществляется в полном объеме. Пациентка выписана через 12 суток с момента оперативного вмешательства. Рана зажила первичным натяжением. Рецидива воспалительного процесса не отмечено. The femoral component of the spacer is installed according to standard methods. The spacer has been adjusted. No tendency to dislocation. The joint is stable. Articulation is carried out in full. The patient was discharged 12 days after surgery. The wound healed by primary intention. Relapse of the inflammatory process is not noted. In January 2017, the second stage of revision arthroplasty was successfully completed.
Способ ревизионного протезирования коленного сустава при дефекте большеберцовой кости позволяет создать полноценную артикуляцию в области сустава при дефиците костной ткани, снизить частоту рецидивов воспалительного процесса, восстановить функцию коленного сустава. Способ позволяет снизить риск возникновения послеоперационных осложнений, что улучшает результаты лечения, сокращает сроки пребывания больных в стационаре.
