Проведенные электроэнцефалографические исследования в сочетании с длительной регистрацией электрокардиограммы (ЭКГ) выявляют различные виды нарушений ритма и проводимости сердца у 40-44% пациентов с эпилепсией. В ходе наблюдения в течение 12-19 месяцев общее количество приступов составило 1477. Нарушения ритма и проводимости были зафиксированы у 42% больных во время развития генерализованных приступов. Активация записи подкожного петлевого регистратора ЭКГ возможна двумя способами:There are two ways to activate recording of a subcutaneous loop ECG recorder:
1) автоматически по 6 программируемым триггерам - асистолия (паузы), брадикардия, желудочковая тахикардия, быстрая желудочковая тахикардия, предсердные тахиаритмии/фибрилляция предсердий. Продолжительность эпизодов сохраненных в памяти составляет 30 секунд до и 30 секунд после момента активации, для предсердных тахиаритмии - 2 минуты, предшествующих автоактивации.1) automatically according to 6 programmable triggers - asystole (pauses), bradycardia, ventricular tachycardia, fast ventricular tachycardia, atrial tachyarrhythmias / atrial fibrillation. Таким образом, в случае возникновения эпилептического приступа пациент или его родственник/опекун выполняют активацию подкожного петлевого регистратора ЭКГ при помощи наружного активатора. Необходимо отметить, что в течение всего срока наблюдения за пациентом осуществляется индивидуальный выбор настроек для активации записи в зависимости от частоты возникновения и вида приступов. Пациенту А., 44 лет, с диагнозом симптоматическая фокальная эпилепсия со сложными парциальными, простыми парциальными и вторично-генерализованными приступами, у которого несмотря на прием противоэпилептической терапии сохраняются приступы с периодичностью до двух раз в месяц, был имплантирован подкожный петлевой регистратор ЭКГ. Устройство было запрограммировано для автоматической детекции аритмий, кроме того, пациент был обучен самостоятельному использованию пульта-помощника в зависимости от типа, возникающих у него эпилептических приступов.Patient A., 44 years old, with a diagnosis of symptomatic focal epilepsy with complex partial, simple partial and secondary-generalized seizures, in which, despite taking antiepileptic therapy, seizures with a frequency of up to two times a month persist, a subcutaneous loop ECG recorder was implanted. При плановом визите пациента в клинику осуществлялось «считывание» информации с кардиомонитора. Активация записи пациентом осуществлялась трижды сразу после окончания приступа - во всех случаях имело место развитие генерализованного приступа, сопровождающееся потерей сознания, без предшествующей ауры.During a planned patient visit to the clinic, information was read from a cardiomonitor. На ЭКГ зафиксированы артефакты, свойственные сокращениям скелетной мускулатуры в момент развития тонической, а затем клонической фаз судорог, в последующем сменяющиеся частой наджелудочковой экстрасистолией и наконец развитием устойчивого эпизода фибрилляции предсердий с ЧСЖ до 150 ударов в минуту (Фиг. 2).On the ECG, artifacts were recorded that were characteristic of skeletal muscle contractions at the moment of development of the tonic and then clonic phases of seizures, subsequently followed by frequent supraventricular extrasystole and finally the development of a stable episode of atrial fibrillation with CSF up to 150 beats per minute (Fig. 2).
