Abstract
Description
The invention relates to medicine and can be used in orthopedics for reconstruction of the foot with polydactyly.
Polydactyly is one of the most common malformations of the forefoot and represents an increase in the number of fingers (metatarsal bones) of more than five. In addition to an increase in the number of fingers (metatarsal bones), polydactyly of the foot is characterized by the development of secondary deformations of the osteoarticular apparatus of the affected segments.
The objective of the invention is to restore the shape and function of the foot after removal of the second metatarsal bone and the second toe.
The prototype of the invention is patent No. 2150911 "Method for the reconstruction of the foot after borrowing the second metatarsal bone." In this method, after performing an osteotomy of the outer third of the first metatarsal bone, the resulting fragment, including part of the articular surface, is displaced to the outside. Compared with the articular surface of the main phalanx of the second finger and fixed with a spoke. The method is technically very traumatic and difficult to perform, there may be cases of delayed consolidation, and even nonunion.
The essence of the invention lies in the combination of essential features sufficient to achieve the desired technical result, consisting in the treatment of polydactyly of the foot in the presence of six normally developed rays of the foot in a less traumatic way.
The essence of the method of treating polydactyly in the presence of six normally developed rays of the foot includes the removal of part of the foot. The tendon of the long extensor of the second beam is isolated, the second ray of the foot is removed. The distinguished tendon is carried out under the tendon of the long extensor of the first finger, closer to the head of the metatarsal bone, they surround the tendon of the long extensor of the first finger. While pressing on the first ray of the foot, it is shifted towards the third ray, the tendon of the long extensor of the second ray is pulled and it is sutured to the extensor of the 3rd ray and the metatarsal bones are fixed with needles, sutures are made on the wound, plaster cast.
Isolation of the tendon of the long extensor of the second beam and removal of the second beam of the foot provides the ability to bring the foot to the anatomically correct ratio, with a narrowing of the tarsal space.
The extracted tendon of the long extensor of the distant beam is carried out under the tendon of the long extensor of the first finger, closer to the head of the metatarsal bone, and they bend around the tendon of the long extensor of the first finger to enable fixation of the first beam displaced to the third beam.
By pressing on the first beam of the foot, and shifting it toward the third beam, pulling the tendon of the long extensor of the second beam, narrowing of the tarsal space between the first and third rays of the foot is achieved.
The long extensor of the removed beam is sutured to the extensor of the 3rd ray and additionally fix the metatarsal bones with needles - to stabilize the bones of the foot in the anatomically correct position.
The application of a longy provides a favorable course of the postoperative period.
The method is illustrated in the illustrations, where:
На фиг. 1 приведена рентгенограмма стопы до операции;In FIG. 1 shows an x-ray of the foot before surgery;
На фиг. 2 - рентгенограмма стопы после операции;In FIG. 2 - radiograph of the foot after surgery;
In FIG. 3 - view of the foot after removing the locking spokes.
Our proposed method is as follows.
Under anesthesia, a semi-oval incision on the back of the foot, soft tissues are separated in the projection of the 2 metatarsal bone, and a tendon of the long extensor 2 ray is isolated. 2 ray removed. The distinguished tendon of the long extensor of the removed beam is carried out under the tendon of the long extensor of the first finger, closer to the head of the metatarsal bone, with the tendon around the long extensor of the first finger. By pressing on the first beam of the foot, 3 beams are shifted to the side, and the tendon of the long extensor of the second beam is pulled, thereby narrowing the intertarsal space between the first and third rays, the long extensor of the removed beam is sutured to the extensor of the 3rd finger and additionally fix the metatarsal bones with needles, seams on a wound, impose a plaster cast.
