Abstract
Description
The invention relates to medicine, in particular to gastroenterology, surgery and oncology, and is intended for the treatment of pancreatitis.
С., Bakker, O.J., Besselink, M.G., Bollen, T.L., Fischer, K., Nieuwenhuijs, V.B. et al. Prediction of common bile duct stones in the earliest stages of acute biliary pancreatitis. Endoscopy. 2011; 43: 8-13; Tse, F. and Yuan, Y. Early routine endoscopic retrograde cholangiopancreatography strategy versus early conservative management strategy in acute gallstone pancreatitis. Cochrane Database Systemic Review. 2012; Dubravcsik Z., et al. Preventive pancreatic stents in the management of acute biliary pancreatitis (PREP AST trial): Pre-study protocol for a multicenter, prospective, randomized, interventional, controlled trial. Pancreatology. 2015. Vol 15. Is. 2. P. 115-123). A stent device is known for draining the pancreatic duct in acute pancreatitis (Van Santvoort, N.C., Bakker, OJ, Besselink, MG, Bollen, TL, Fischer, K., Nieuwenhuijs, VB et al. Prediction of common bile duct stones in the earliest stages of acute biliary pancreatitis. Endoscopy. 2011; 43: 8-13; Tse, F. and Yuan, Y. Early routine endoscopic retrograde cholangiopancreatography strategy versus early conservative management strategy in acute gallstone pancreatitis. Cochrane Database Systemic Review. 2012; Dubravcsik Z., et al. Preventive pancreatic stents in the management of acute biliary pancreatitis (PREP AST trial): Pre-study protocol for a multicenter, prospective, randomized, interventional, controlled trial. Pancreatology. 2015. Vol 15. Is 2. P. 115-123). The use of this stent involves the removal of the alleged cause of obstruction of the pancreatic duct. The stones found in the ducts are removed, and then a stent is installed in the lumen of the pancreatic duct, thereby restoring its patency.
The disadvantage of this device is its invasiveness. The stent itself can cause the development of pancreatitis, cholangitis, obstructive jaundice. Subsequent mandatory removal of the structure must be performed with anesthetic aid, this operation can also cause the aforementioned complications. Performing endoscopic retrograde cholangiopancreatography in 3-10% of cases can cause acute pancreatitis, so all patients have to prescribe special preventive therapy.
In addition, at the time of papillosphincterotomy, a large vessel can be damaged, which will lead to bleeding. The use of this stent is indicated only for biliary etiology of pancreatitis, i.e. in the presence of an obstructive calculus.
Another disadvantage of using a stent is the need for a specialist who knows this technique.
(Al-Omran, M., Albalawi, Z.H., Tashkandi, M.F., and Al-Ansary, L.A. Enteral versus parenteral nutrition for acute pancreatitis. Cochrane Database Syst Rev. 2010; Petrov, M.S., Pylypchuk, R.D., and Uchugina, A.F. A systematic review on the timing of artificial nutrition in acute pancreatitis. Br J Nutr. 2009; 101: 787-793).Closest to the claimed device is a duodenal probe. This device is installed through the nose, while the setting process is controlled using an endoscope so that the probe passes through the pyloroduodenal zone. Pancreatitis is treated by remediating stagnant contents as well as prescribing early enteral nutrition (Al-Omran, M., Albalawi, ZH, Tashkandi, MF, and Al-Ansary, LA Enteral versus parenteral nutrition for acute pancreatitis. Cochrane Database Syst Rev. 2010 ; Petrov, MS, Pylypchuk, RD, and Uchugina, AF A systematic review on the timing of artificial nutrition in acute pancreatitis. Br J Nutr. 2009; 101: 787-793).
The disadvantage of the device selected as a prototype is that it can only be used for feeding or passive evacuation of contents, and enteral nutrition can be performed only in the absence of increased intra-abdominal pressure or compartment syndrome. Thus, this duodenal probe can be used only for mild pancreatitis, as well as in non-operated patients, and in a more severe form, it performs only a rehabilitation function. It should be noted that the design of the probe does not directly affect the outflow of pancreatic juice from the gland.
The objective of the present invention is to improve the treatment results of acute and chronic pancreatitis.
The technical result of the present invention is achieved by the fact that in the device for the treatment of pancreatitis, which is a probe for aspiration and feeding, containing a main channel having openings opening into the cavity of the stomach and duodenum, a fixing device is installed on the side surface of the probe, and an additional channel is made inside wherein the fixing device is a cylinder and is arranged to be placed above the large duodenal papilla, and the additional channel is open INDICATES in the bottom of the fixture and is connectable to the suction.
This device implements a new method of treating pancreatitis by creating a constant, controlled negative pressure around the large duodenal papilla. A locking device in the form of a cylinder provides an optimal fit to the wall of the duodenum, and also forms a cavity around the large duodenal papilla, which is necessary for normal function and to prevent damage to the latter. An additional channel passing inside the device, on the one hand opens at the bottom of the locking device, and on the other joins the suction that creates a controlled negative pressure. When the device is working around the large duodenal papilla and the mouth of the pancreatic duct, a constant negative pressure is created, which is equal to physiological (60-120 mm aq), which is normally created in the duodenum. These conditions ensure restoration of pancreatic juice evacuation from the pancreas, thereby preventing its further damage. The possibility of prolonged use of the device (1-3 days) prevents the risk of relapse.
Thus, the claimed device for the first time allows you to apply the method of treatment of pancreatitis with the help of constant negative, controlled pressure. A separate achievement is the fact that the treatment process is physiological and non-traumatic.
In FIG. 1 shows a side view of the device, on the right side of the circuit, the probe channels open in two different ways: additional 1 and main 2, which in the opposite left part, which is installed in the lumen of the intestine, merge into one. On the side surface there is an elevation of the
8.In FIG. 2 shows a part of the probe in the region of the
In FIG. 3 shows a portion of the probe in the area of the
The device is used as follows. A probe 130 cm long is installed under the control of the endoscope in the cavity of the stomach and duodenum so that the
2.After the doctor is convinced that he has placed
In the probe there is a
The inventive device can improve the treatment of both acute and chronic pancreatitis, as well as conduct enteral nutrition and / or sanitation of the stomach and duodenum.
An additional advantage of the invention is its diagnostic value: it is possible to investigate the biochemical composition of the extracted juice, as well as evaluate the excretory function of the organ.
