Сущность способа заключается в том, что под визуальным УЗИ контролем осуществляют пункцию периферическим катетером типа флексюля, диаметр которого должен быть менее или равен ½ диаметра вены. После пункции вены катетером типа флексюля, в 2-3 этапа меняются проводники и катетеры на больший диаметр, на последнем этапе ставится диализный катетер. С помощью аппарата УЗИ линейным датчиком, расположенным в поперечной плоскости сосудистому пучку, определяется вена наибольшего диаметра и наиболее удаленная от расположенной рядом артерии (Рис.2), после определения диаметра вены подбирают диализный катетер, диаметр которого должен быть менее или равен ½ диаметра вены, затем выбирают катетер типа флексюля №1 и проводят пункцию вены. Диаметр катетера типа флексюля для пункции должен быть меньше на 2 размера диаметра иглы в наборе выбранного диализного катетера. При диаметре иглы в наборе 21 G катетер-флексюля должен быть 22-24 G, а при игле 18 G катетер-флексюля - 20-22 G (обратная зависимость в шкале размеров, чем больше порядковый номер G тем меньше диаметр катетера). После обработки операционного поля антисептиками, под визуальным контролем УЗИ (Рис.3), проводят пункцию вены выбранным катетером типа флексюля №1 (Рис.4). Выбирают проводник по размеру и проводят его через катетер типа флексюля в вену (Рис.5). После появления крови из катетера-флексюли удаляют иглу-мандрен (мандрен выполняет роль стилета), а в катетер-флексюлю по методике Сельдингера вводят металлический проводник №1, диаметр которого зависит от диаметра выбранного катетера-флексюли. В конструкции катетера типа флексюли предусмотрена «игла-мандрен», которая на этапе пункции препятствует засасыванию воздуха в катетер. После удаления «иглы-мандрена» из катетера малый диаметр катетера-флексюли не позволяет допустить большую кровопотерю. Однократность применения стерильного катетера типа флексюли является лучшей профилактикой инфекционных осложнений. Если катетер типа флексюля №1 диаметром 22-24 G, то диаметр проводника должен быть 0,46 мм, при диаметре флексюли 22-20 G проводник - 0,63 мм. After removal of the “mandrel needle” from the catheter, the small diameter of the flexure catheter does not allow for large blood loss. The single use of a sterile flexule catheter is the best prevention of infectious complications. If a catheter of flexule type No. 1 with a diameter of 22-24 G, then the diameter of the conductor should be 0.46 mm, with a diameter of flexula 22-20 G the conductor should be 0.63 mm. После удаления дилататора, по проводнику в вену проводят катетер типа флексюля №2 с диаметром, большим по сравнению с катетером-флексюлей №1. Перед проведением проводника диализного катетера из катетера-флексюли удаляют проводник №1, а затем и сам катетер-флексюлю №2, оставив в вене только проводник для диализного катетера. Затем проводят дилатацию канала расширителем из набора диализного катетера. Далее по проводнику устанавливают сам диализный катетер. Для снижения риска осложнений, катетеризация центральных вен у детей до 10 лет выполняется под общей анестезией. Ребенок К., возраст 7 мес, 8 кг, поступил в тяжелом состоянии с диагнозом: 2-сторонняя плевропневмония. Септический шок. Учитывая клиническую картину септического шока и острой почечной недостаточности, показано проведение сеансов гемофильтрации и гемодиализа. Для проведения инфузионной, инотропной, антибактериальной и иммунозаместительной терапии у ребенка установлен 2-просветный ЦВК во внутреннюю яремную вену справа. Для проведения сеансов гемофильтрации и гемодиализа необходима была катетеризация бедренной вены. Выбор места пункции вены был обусловлен тем, что бассейны вводимых препаратов интенсивной терапии и места забора крови для продленной гемодиафильтрации должны быть разобщены. Для инфузионной терапии и введения лекарственных препаратов использовался бассейн верхней полой вены (яремная вена), а для процедуры гемодиафильтрации - бассейн нижней полой вены (бедренная вена).
