Abstract
Description
The invention relates to medicine, namely to endoscopic surgery, and can be used for debridement of the bile ducts in an endoscopic manner.
All diseases accompanied by a violation of the outflow of bile from the liver to the duodenum (duodenum) (calculus of the common bile duct, cicatricial narrowing, compression from the outside, tumor lesions, etc.) cause inflammatory changes in the wall of the bile ducts (cholangitis). Marked inflammatory changes in the wall of the bile ducts (purulent cholangitis) significantly aggravate the general condition of patients, causing a septic state in patients with the possible development and progression of multiple organ failure phenomena. At the same time, surgical interventions against the background of severe cholangitis significantly increase the percentage of complications, such as the failure of biliary anastomoses, the development of liver failure, septic conditions and associated fatal outcomes.
Currently, endoscopic interventions through the large duodenal nipple (BDS) of the duodenum (duodenum) are actively used in biliary surgery, and endoscopic decompression and debridement of the gall tree plays an important role in the treatment and preoperative preparation of patients.
Не контролируемое введение растворов в просвет холедоха повышает давление в билиарной системе и способствует проникновению элементов гнойной желчи в кровеносное русло (билио-венозный шунт).As an analogue, you can take nazobiliary drainage (NDB) of COOK-medical company, presented in the catalog of the company on page 91 (or www.cookmedical.com). This device is a drainage tube 250 cm long, at the distal end of which there are side holes (5 or 9). This drainage tube using an endoscope is installed in the bile ducts above the site of obstruction. Due to the increased pressure in the bile ducts through the drainage tube, outflow of bile occurs. In addition, medications (antibiotics) can be periodically administered through the proximal end of the tube into the lumen of the bile ducts for local therapy of the inflammatory process. The introduction of these drugs is carried out manually, using a syringe. Moreover, this procedure takes some time from the medical staff, and most importantly, the doctor cannot control the pressure that is created in the bile ducts. Uncontrolled introduction of solutions into the lumen of the common bile duct increases the pressure in the biliary system and promotes the penetration of purulent bile into the bloodstream (biliary-venous shunt).
The task is to create a device that is installed using the endoscope into the bile ducts, and in an automated mode, without spending a lot of time on the medical staff, controlling the pressure in the bile ducts, reorganizes the bile ducts through dosed administration of drugs (LP) and excretion (E ) from the lumen of the bile ducts.
The essence of the proposed device consists in the following: a catheter installed in an endoscope designed to aspirate exudate (E), which contains a catheter for delivering drugs (PL) into the lumen of the bile ducts, catheters are connected to the control handles, and a cylinder for supplying PL from the reservoir to lumen of the bile ducts associated with an arm equipped with an adjustable weight, to ensure LP pressure, an aspiration cylinder is connected to the drive cylinder, connected to a ruler equipped with a reg adjustable stops, which, acting on the lever of the tap with flats, carry out automatic switching of the supply of drugs and aspiration, for which one flat connecting the supply of pressurized water to the drive cylinder, and the second with a different position of the tap lever connecting the water drain, freeing the load on the LP cylinder and the ruler, and the movement speeds of the drive cylinder, LP cylinder, aspiration cylinder E are regulated by cranes, valves for the intake of E from the lumen of the bile ducts and the supply of E to the container are provided.
The device will provide:
