Abstract
Description
Изобретение относится к медицине, а именно к способам лечения катаракты у больных с псевдоэксфолиативным синдромом (ПЭС).The invention relates to medicine, namely to methods for treating cataracts in patients with pseudoexfoliation syndrome (PES).
It is known that pseudoexfoliation syndrome is characterized by deposition of a pseudoexfoliation substance on all structures of the anterior part of the eyeball, dystrophic changes in the iris and ciliary body; thinning and reduced elasticity of the lens capsule; a decrease in the strength of the zinc binder and phacodonesone; increased pigmentation of the anterior chamber angle; violation of the hydrodynamics of the eye; in cases of cataract development - a high density of the nucleus and cortical layers, the risk of postoperative decompensation of intraocular pressure.
- 2006. - Т.3, №3. - С.10-17).Cataract surgery for pseudoexfoliation syndrome is associated with significant intraoperative technical difficulties and a burdened postoperative period, due to the specific manifestations of this pathology. The treatment of postoperative fibrinoid syndrome has three directions: conservative, laser and surgical, the first being mandatory and often combined with other types of treatment, especially in severe cases (Chentsova O.B., Tretyak E.V. Exudative iridocyclitis, endophthalmitis with artifact // Ophthalmology. - 2006. - T.3, No. 3. - S.10-17).
A well-known and widely used technique for laser treatment of fibrinoid syndrome (Ivanov AN The results of neodymium YAG laser treatment with a pronounced exudative reaction after implantation of an intraocular lens // Herald of Ophthalmology. - 2002. - No. 3. - S.13-15). This intervention accelerates the lysis of the destroyed exudative formations in the anterior chamber, recovery and improves visual results in the patient.
- 2001. - Т.2, №3. - С.114-115).A known method of treating fibrinoid syndrome in the postoperative period after cataract extraction in patients with pseudoexfoliation syndrome by conducting several sessions of laser destruction of fibrinous masses in the projection of the pupil and on the IOL against the background of intensive anti-inflammatory and fibrinolytic therapy (Novoderezhkin V.V. Laser correction of postoperative fibrinoid syndrome after cataract extraction // Clinical Ophthalmology. - 2001. - T.2, No. 3. - S.114-115).
The disadvantages of this method are its low efficiency and the possibility of side complications due to the need for a large number of sessions of laser destruction of fibrinous masses, the number of which sometimes reaches 5-7, which is unsafe for the structures of the anterior segment of the eye.
Closest to the claimed method, the prototype, is a method of treating cataracts in patients with pseudoexfoliation syndrome, which consists in the fact that 1-2 days before cataract extraction with implantation of an intraocular lens, thrombovazim is administered in a dose of 2 tablets 2 times a day, and in the postoperative period thrombovazim intake is increased to 4-5 tablets 2 times a day with a duration of up to 10 days. Then, YAG laser destruction of fibrinous elements in the anterior chamber of the eye is performed for several sessions, as well as anti-inflammatory therapy (antibiotics and corticosteroids intravenously, in conjunctival injections and instillations, mydriatics in instillations and subconjunctival injections). After each laser intervention session, an eyeball is massaged in the projection of the posterior chamber of the eye (patent RU 2414194 C1, publ. March 20, 2011). The method provides the treatment of the developed fibrinoid syndrome in this category of patients due to the complex effect on the fibrinolysis process by its activation and acceleration.
The main disadvantage of this method is the duration of treatment, which is at least 10 days, which significantly lengthens the rehabilitation period. Moreover, the methods used for administering drugs do not provide optimal doses and the effectiveness of their action in the tissues of the eye. Sessions of laser destruction of fibrinous masses, the amount of which can sometimes reach 5-7, and palpation massage of the eyeball are unsafe for the structures of the anterior segment of the eye. In addition, this method involves only the treatment of complications, and not their prevention.
The objective of the invention is to reduce the time of treatment of patients and prevention of side complications.
The problem is achieved by the proposed method, which consists in the following.
27.12.2007.For a patient 1-2 days before cataract surgery with implantation of an intraocular lens and within 3 days after the operation, they undergo complex treatment consisting of lymphotherapy by performing pterygo-injection in the orbit microcirculation region using a drug mixture, followed by intravenous administration ozonized saline solution. Cataract extraction is carried out by phacoemulsification by any known method, for example, the method described in patent RU 2313320 C1, publ. 12/27/2007.
Pterygopalatine injections (SED) are performed on the affected eye side, daily, with a course of 4 injections (the first injection is performed before the operation, and three injections after the cataract extraction operation with the implantation of an intraocular lens), which ensures the rapid flow and accumulation of drugs in the lymphatic region.
The SOI is performed as follows: directly under the zygomatic arch, retreating one third of the distance between the ear tragus and the edge of the orbit, inject the needle with a diameter of 0.6-0.7 mm and a length of 35-45 mm, and then change the angle of the needle by 30 degrees, push the needle past the pterygoid processes of the main bone to a depth of 3.5-4.5 cm, falling into the pterygo-fossa and introducing 4.0-4.5 ml of the drug mixture.
The composition of the medicinal mixture for the conduct of SOI includes the following drugs: lidocaine - 20-40 mg, histochrome - 0.2-0.4 mg, dalargin 0.5-1.0 mg, lidase - 16-32 units.
Blood ozonation is carried out by intravenous infusion of an ozonized 0.9% sodium chloride solution with an ozone concentration of 2-4 mg / L.
Lymphotropic therapy provides a high regional exposure and concentration of drugs, a minimum drug load on the body, a nociceptive block causes a decrease in the afferent irritating factor, which provides improved microcirculation, lymphocirculation, lymphatic drainage of the orbit region, and normalization of eye hydrodynamics.
Озонотерапия обеспечивает коррекцию гормональной регуляции, уменьшение гипоксии и гипоксемии, повышение антиоксидантной активности крови, стимуляцию иммунной системы, улучшение микроциркуляции, нормализацию гемодинамики, торможение свободнорадикального перекисного окисления липидов (ПОЛ).Ozone therapy provides correction of hormonal regulation, reduction of hypoxia and hypoxemia, increase of antioxidant activity of blood, stimulation of the immune system, improvement of microcirculation, normalization of hemodynamics, inhibition of free radical lipid peroxidation (LPO).
The histochrome plays the role of a free radical interceptor and neutralizes the main initiators of the non-enzymatic process of peroxidation of membrane lipids that accumulate in the area of ischemic tissue damage. It has a pronounced resorptive effect, as a result of which it improves blood flow and enhances metabolic processes, performs the function of a membrane protector.
Lidase - an enzyme preparation isolated from the testes of cattle, breaks down the main component of the interstitial substance of the connective tissue - hyaluronic acid, reduces its viscosity, increases tissue and vascular permeability, facilitates the movement of fluids in interstitial spaces; reduces swelling of the tissue.
Lidocaine stabilizes cell membranes, blocks sodium channels, dilates blood vessels.
Dalargin has a stress-reducing effect, helps normalize microcirculation, increase resistance to hypoxic hypoxia, reduce enzyme activity, and sensitivity to catecholamines, significantly stimulate the antibody formation reaction, which in turn leads to an increase in the body's immune response to the inflammatory process and reduces the time of postoperative rehabilitation of patients.
Using these balanced medicinal mixtures allows you to quickly normalize the redox processes due to a wide range of anti-inflammatory, decongestant, lymphostimulating, hemoresorption and disaggregate effects of the mixture used and the topography of its introduction.
The defining difference of the proposed method from the prototype is that they carry out preoperative and postoperative lymphotropic therapy on the side of the affected eye by performing pterygopalatine injection using an experimentally selected treatment mixture, coupled with intravenous administration of ozonized physiological solution, which reduces the treatment time to 4-5 days due to increase the effectiveness of treatment and eliminate postoperative complications.
