Известен способ лечения кератоконуса (см. Предлагаемое изобретение решает задачу разработки нового способа лечения кератоконуса. Получаемый при этом технический результат состоит в улучшении процесса насыщения роговицы рибофлавином и, в целом, в повышении эффективности предлагаемого способа лечения кератоконуса за счет строгой дозированной по глубине и площади эксимерлазерной абляции поверхностных слоев эпителия с сохранением его герминативного слоя. При этом улучшаются условия для качественной и быстрой эпителизации, ускорения процесса заживления и снижения до минимума риска возникновения послеоперационных осложнений. Предлагаемый способ позволяет предотвратить повреждения боуменовой мембраны и стромы роговицы. Способ прост в исполнении и не представляет сложности для специалистов при его использовании.The present invention solves the problem of developing a new method of treating keratoconus. At the same time, conditions are improved for high-quality and fast epithelization, acceleration of the healing process and minimization of the risk of postoperative complications. The proposed method allows to prevent damage to the bowman membrane and stroma of the cornea. The method is simple to implement and is not difficult for specialists when using it.

Предварительно проводят полное предоперационное обследование, в том числе многоточечное измерение толщины роговицы. Определяют толщину эпителиального слоя роговицы. Устанавливают параметры для эксимерлазерной абляции эпителиального слоя. В условиях стерильной операционной проводят дозированную эксимерлазерную абляцию эпителиального слоя на расчетную глубину, составляющую не более 2/3 толщины эпителиального слоя роговицы, и диаметр, соответствующий зоне ультрафиолетового облучения. После этого проводят УЗ-пахиметрию для определения толщины роговицы. Насыщают роговицу 0,1%-ным раствором рибофлавина путем многократных инстилляций каждые 2-3 минуты в течение 30 минут до диффузного прокрашивания стромы роговицы. Непосредственно перед ультрафиолетовым облучением осуществляют пахиметрический контроль толщины роговицы. Проводят ультрафиолетовое облучение роговицы длиной волны 365 нм мощностью 3 мВт/см2 в течение 30 мин, которое сопровождают дополнительными инстилляциями 0,1%-ного раствора рибофлавина на роговицу каждые 2-3 минуты для поддержания его концентрации. Еще раз проводят пахиметрический контроль толщины роговицы. В течение 1-2 суток происходит полная равномерная и быстрая эпителизация роговицы, что позволяет значительно снизить выраженность послеоперационного болевого роговичного синдрома, ускорить процесс реабилитации пациента и свести к минимуму риск возникновения послеоперационных осложнений.A preliminary preoperative examination, including multi-point measurement of the thickness of the cornea, is preliminarily carried out. Пациент А., 25 лет. Жалобы на прогрессирующее снижение зрения на левом глазу (OS). Оптические средства коррекции пациент не использовал. Было проведено полное офтальмологическое обследование, включающее визометрию, рефрактометрию в обычных условиях и в условиях циклоплегии, многоточечную пахиметрию, измерение диаметра роговицы, определение плотности эндотелиальных клеток роговицы, кератотопографию, осмотр глазного дна с линзой Гольдмана.Patient A., 25 years old. Результаты обследования: VOD=1,0, ROD=sph - 0,25 Дптр cyl - 0,5 Дптр ax 10, VOS=0,1 cyl - 6,0 Дптр ax 130=0,3, ROS=sph - 1,75 Дптр cyl -9,0 Дптр ах 137, пахиметрия в центральной оптической зоне OD=481 мкм, пахиметрия в центральной оптической зоне OS=422 мкм. Толщина эпителиального слоя - 40 мкм. Плотность эндотелиальных клеток OD=2902 CD/мм2, плотность эндотелиальных клеток OS=2750 CD/мм2, диаметр роговицы OD=10,5 мм, OS=11,0 мм. Кератотопографически на OS картина кератоконуса. Поставлен клинический диагноз «Кератоконус OS II степени». Пациенту рекомендовано лечение кератоконуса. Установлены параметры для эксимерлазерной абляции эпителиального слоя. Произведена операция по предлагаемому способу лечения кератоконуса. Под инстилляционной местной анестезией выполнена абляция на эксимерлазерной установке Швинд Амарис эпителиального слоя роговицы на расчетную глубину 20 мкм, что составляет не более 2/3 толщины эпителиального слоя роговицы. Диаметр зоны абляции составил 8,0 мм. Контрольное измерение толщины роговицы после лазерной абляции - 402 мкм. Провели насыщение роговицы 0,1%-ным раствором рибофлавина, для чего его закапывали каждые 2-3 минуты в течение 30 минут до диффузного прокрашивания роговицы. Непосредственно перед ультрафиолетовым облучением осуществили пахиметрический контроль толщины роговицы - 405 мкм. Провели ультрафиолетовое облучение роговицы ультрафиолетом с длиной волны 365 нм мощностью 3,0 мВт/см2 с расстояния 50 мм в течение 30 мин. Облучение роговицы сопровождалось дополнительными инстилляциями 0,1%-ного раствора рибофлавина на роговицу каждые 2-3 минуты. Толщина роговицы после облучения составила 415 мкм. После операции пациенту в оперированный левый глаз был назначен Тобрекс по 1 капле 6 раз в день и Корнерегель - 3-4 раза в день Полная эпителизация была достигнута через 42 часа после операции.
