Abstract
Description
The present invention relates to medicine, namely to ophthalmology.
One of the main causes of vision loss in diabetes is macular edema. Long-existing diabetic edema of the macula leads to irreversible death of retinal photoreceptors, an ischemic variant of the course of this pathology is a particularly insoluble problem.
Annu Rev Public Health, 1996. P.137-158).A known method of surgical treatment of the disease, namely the introduction of intravitreal triamcinol, which in 19% of cases leads to a decrease in macular edema, but this effect is unstable and dangerous such complications as the development of glaucoma, intravitreal hemorrhages, retinal detachment (Klein R. Diabetic retinopathy. Annu Rev Public Health, 1996. P.137-158).
(Steven M. Bloom, Alexander S. Brucer Laser Surgery of the Posterior Segment, 1990. - P.112-113).The closest is a method of direct laser coagulation in the area of retinal edema. Lasers with different wavelengths from 497-810 nm are used, the parameters are selected individually depending on the transparency of the media, retinal pigmentation, edema, the presence of intraretinal hemorrhages, etc. As a result of this intervention, ischemic zones are “turned off”, the spread of macular edema is limited, multiple scars on the retina are formed in order to prevent the production of a vasoproliferative factor and the oxygen tension in the retina and vitreous increases due to the death of some oxygen consuming photoreceptors. However, this treatment is very aggressive and has many complications, including an increase in macular edema (Steven M. Bloom, Alexander S. Brucer Laser Surgery of the Posterior Segment, 1990. - P.112-113).
The technical result of the proposed method is to reduce macular edema, increase visual acuity and improve blood circulation in the optic nerve and indirectly in the retina, as well as reduce complications.
New in achieving the technical result is that they use laser infrared transpupillary thermotherapy, the effect is carried out directly on the optic nerve disk with a wavelength of 810 nm, a radiation power of 500-700 mW, a spot diameter of 2.0 mm, exposure for 60-90 seconds, the course of treatment is 1-2 sessions.
Laser infrared transpupillary vascular thermotherapy is a subthreshold method that uses a large area spot, low energy, long-term exposure of radiation. When used, a selective thermal effect is achieved without thermal damage to surrounding tissues, which leads to improved blood circulation in the optic nerve system.
Comparative analysis with the prototype shows that the inventive method is characterized in that they use laser infrared transpupillary thermotherapy, the effect is directly on the optic nerve disk with a wavelength of 810 nm, a radiation power of 500-700 mW, a spot diameter of 2.0 mm, exposure for 60 -90 seconds, the course of treatment - 1-2 sessions, which meets the criterion of "novelty."
A new set of features provides a reduction in macular edema, an increase in visual acuity and improved blood circulation, and also reduces the risk of complications due to the lack of a coagulating effect and performs surgery on a relatively narrow pupil, which meets the criterion of "industrial applicability".
The method is as follows.
The following ophthalmological examination is performed for all patients before treatment, as well as a month after treatment: visometry, biomicroscopy, optical coherence tomography of the retina and optic nerve, dopplerography in the optic nerve and retina system using Valuson 730 pro. Before the operation, installation anesthesia with S.Innocaini 1% solution is performed. The exposure is carried out by the IRIDEX laser directly on the optic nerve disk with a wavelength of 810 nm, the exposure power is 500-700 mW, the spot diameter is 2.0 mm, the cure exposure is a total of 60-90 seconds. Only 1-2 treatment sessions with an interval of 1-2 months.
The method is illustrated by the following clinical example.
