Abstract
Description
The invention relates to medicine, namely to neurosurgery, and can be used to predict the preservation of visual functions and correct tactics of surgery during surgical interventions in the chiasmal-sellar region.
The prognosis of the preservation of visual functions in patients during manipulations in the optic nerve during interventions in the pathological processes of the chiasm-sellar region is always an extremely important issue for the surgeon.
A known method for predicting the safety of the functions of the optic nerve during surgery by assessing evoked potentials in the surgical treatment of tumors of the chiasmal-sellar region (Olyushin V.E., Tigliev G.S., Weinstein L.G. et al. "Monitor visual evoked potentials in the surgical treatment of tumors of the chiasmosellar region. "- Questions of Neurosurgery, No. 3, 1990, p.22-24). During the operation, the evoked visual potentials are fixed and the functional state of the optic nerve is judged by assessing the latent period of the P100 peak.
This method is chosen by us as a prototype.
The disadvantage of the prototype is the lack of accuracy and information of the method, due to the fact that it reflects the functional state of nerve tissue, possibly already damaged before surgery by a tumor. Thus, with significant preoperative damage to the optic nerve, the indicators will be initially sharply changed, which can be regarded as a poor prognostic criterion, while there will be no real visual impairment after surgery.
The invention is aimed at creating a method for predicting the safety of the functions of the optic nerve during surgery in the chiasmal-sellar region, which improves the accuracy and information content of the method.
The specified technical result during the implementation of the invention is achieved by the fact that in the known method for predicting the safety of the functions of the optic nerve during surgery in the chiasmal-sellar region by examining during the operation of the optic nerve, the feature is that the blood flow in the artery of the optic nerve is evaluated by a microdroplerographic sensor, and the value of the pulse index PI≥1.0 and / or the value of the resistive index RI≥0.6 the prognosis of the preservation of the functions of the optic nerve is unfavorable.
The method is as follows. During the operation, the patient with a 25 MHz sensor performs a dopplerographic ultrasound examination of the optic nerve artery under visual control. Standard parameters of arterial blood flow are determined, including pulse, resistive indices. Indexes are automatically calculated by the equipment, which facilitates the use of the method. With a pulse index value (PI) ≥1.0 and / or a resistive index value (RI) ≥0.6, the prognosis of the preservation of the functions of the optic nerve is unfavorable.
The inventive method is developed and passed clinical trials and RNHI them. prof. A.L. Polenova during operations of 12 patients with tumors of the chiasmal-sellar region.
We give examples - extracts from the medical history.
1.Example 1
Patient Sh., 55 years old, and / b No. 2291-2003 with a diagnosis of pituitary adenoma. The patient underwent surgery - bifrontal osteoplastic trepanation, removal of the pituitary adenoma.
According to the claimed method, during the operation, an ultrasound examination of the optic nerve arteries was carried out with an ultrasonic sensor of 25 MHz. The following indicators were obtained: artery of the left optic nerve: cf. linear blood flow velocity - 24 cm / s, PI - 1.3; RI - 0.8; artery of the right optic nerve: cf. linear blood flow velocity of 37 cm / s; PI - 0.7; RI - 0.48. Given the high rates of PI and RI of the artery of the left optic nerve, visual impairment was predicted in the left eye and it was recommended to limit the scope of the operation so as not to cause vascular disorders in the right optic nerve. After the operation, a sharp decrease in visual acuity was revealed in the left eye (from 0.7 to 0.07), visual acuity in the right eye remained at the preoperative level.
Example 2. Patient D., and / b No. 2562-03, with a diagnosis of pituitary adenoma. The patient underwent the same operation - bifrontal osteoplastic trepanation, removal of the pituitary adenoma.
According to the claimed method, during the operation, an ultrasound examination of the optic nerve arteries was carried out with an ultrasonic sensor of 25 MHz. The following indicators were obtained: artery of the right optic nerve - cf. linear blood flow velocity - 24 cm / s, PI - 0.62; RI - 0.44. The preservation of the visual functions of the patient after surgery was predicted. After surgery, visual acuity remained at the preoperative level.
Using the claimed method allows to improve the results of surgical treatment due to the preservation of vision.
