Abstract
Description
The invention relates to medicine and can be used in clinical practice for the treatment of patients using blood purification, in particular for diseases such as renal failure and impaired purine metabolism.
Известны способы гемофильтрации, в которых для очистки крови предварительно отделенную плазму крови пропускают через колонку с сорбентом, например, с гепарин-сефарорзой (Morgani S., lupier P.J., Award J. "Extracorporal removal of plasma lipoproteins by affinity binding to heparin-agarose", Glin. Asta, 1977, v.77, 1, p. 21-30); декстран-сульфатом (Yokojama S., Mayashi R., Satani M., Yamamoto A. "Selective removal of low density lipoprotein by plasmaphoresis in familial hyperchotesterolaemia", Arteriosclerosis, 1985, v.5, p.613-622). Hemofiltration methods are known in which, for blood purification, previously separated blood plasma is passed through a column with an sorbent, for example, heparin-sepharza (Morgani S., lupier PJ, Award J. "Extracorporal removal of plasma lipoproteins by affinity binding to heparin-agarose" Glin. Asta, 1977, v. 77, 1, p. 21-30); dextran sulfate (Yokojama S., Mayashi R., Satani M., Yamamoto A. "Selective removal of low density lipoprotein by plasmaphoresis in familial hyperchotesterolaemia", Arteriosclerosis, 1985, v.5, p.513-622).
These methods are quite effective in use for impaired lipid and lipoprotein metabolism, but are not effective for renal failure, impaired purine metabolism and other diseases associated with an increased content of uric acid and creatinine in the blood.
A known method of purifying blood from uric acid and creatinine, including filtration by adsorption using activated carbon (patent RU 2086264, A 61 M 1/38, 08/10/1997).
However, activated carbon has poor biocompatibility and hemocompatibility, because when it comes into contact with blood, it causes depletion of red blood cells, platelets, white blood cells, fibrinogen and other biological substances. In addition, in the circulation, it releases microemboli, which cause chills, fever and accumulation of coal in the reticuloendotedial system of patients. The measures taken in this method to protect against the listed phenomena — the use of an additional filter, for example, from gauze or a coal hemoperfusion cartridge — are ineffective and insufficient.
An object of the invention is to eliminate the undesirable consequences of blood filtration by increasing the biocompatibility and hemocompatibility of the adsorbent while maintaining high adsorption properties.
10%. The problem is solved in that in a method for purifying blood from uric acid and creatinine, which includes filtration using a carbon-based adsorbent, blood plasma is preliminarily separated, the separated plasma is filtered, and a carbon mixture consisting of expanded graphite and carbon nanocrystals is used as an adsorbent, moreover, the content of carbon nanocrystals in the mixture is not less than 10%.
5.In this case, nanocrystals are nanotubes 1-10 nm in size, with C, C 2 , C 3 , C 4 , C 5 free radicals attached to them, and / or radicals in the form of one or more connected hexagons and / or hexagons with attached radicals of the form C, C 2 , C 3 , C 4 and C 5 .
A method of purifying blood is as follows.
Previously, the blood of patients is divided into a plasma separator, i.e. carry out a plasma exchange procedure. The separated plasma is then passed through the carbon mixture.
е. не впитывает воду и не соединяется с водой (краевой угол смачивания больше 90o).The blood purification method is based on the use of a carbon mixture as an adsorbent, which has great sorption ability with respect to uric acid and creatinine. Such properties of the carbon mixture are due to the presence of carbon nanocrystals in the form of nanotubes 1-10 nm in size with a large disorder and the fact that in the manufacture of a mixture of natural flake graphite, or graphite in the form of a powder, or other graphite-containing raw material, not only crystallites separate into separate packets basal planes, as with the known methods of manufacturing expanded graphite, but also the breaking of interhexagonal covalent bonds. This leads to the formation of energetically stressed atomic carbon compounds. In addition, the resulting carbon mixture is a hydrophobic material, that is, does not absorb water and does not combine with water (contact angle greater than 90 o ).
10.03.2001. The carbon mixture can be obtained by one of the known methods described in patent RU 2163883, publ. 03/10/2001.
In the manufacture of a carbon mixture, the chemical treatment of the initial graphite-containing raw materials (natural flake graphite or graphite in the form of a powder) is carried out by halogen-oxygen compounds of the general formula MXOn, where M is one of the chemicals of the series H, NH 4 , Na, K; X is one of the chemicals of the series Cl, Br, J; and n = 1-4, with the formation of initiating complexes capable of exothermic explosive decomposition, resulting in the initiation of an autocatalytic decomposition process, as a result of photochemical, mechanical, thermochemical, sonochemical or direct chemical exposure. The initiating complexes are introduced into the interlayer spaces of graphite, their explosive decomposition is initiated, and not only the van der Waals bonds are broken, but also the covalent bonds, which leads to the formation of a carbon mixture. When carrying out the process under conditions close to normal atmospheric pressure (760 mm Hg) and room temperature (20 o C), carbon nanotubes are formed in a ratio sufficient for efficient sorption (at least 10%). The conversion process is carried out in any capacity (vessel, etc.), including the possible without oxygen.
The process of graphite transformation (breaking of van der Waals bonds) is carried out under the influence of microexplosions of explosives introduced into the interlayer spaces of graphite, in this case called initiating complexes. Explosive is in the interlayer space at the molecular level and is chemically initiated before the explosion. As a result of the energies released by the microexplosion, not only the van der Waals bonds, but also the interatomic bonds are broken with the formation of not only nanotubes, but also free radicals C, C 2 , C 3 , C 4 , C 5 and radicals in the form of hexagons (one or more) with radicals of the form C, C 2 , C 3 , C 4 , C 5 attached to them, which together provide a high reactivity of the resulting carbon mixture.
The carbon mixture is a substance in the form of fluff and / or dust with a mass content of carbon of 99.4%, bulk density from 0.002 g / cm 3 to 0.01 g / cm 3 , pore sizes up to 40 microns.
The microstructure of the carbon mixture is a granule having an elongated fibrous structure (similar to a bast) on the surface with a fiber diameter of the order of units or even fractions of a micron.
The carbon mixture has high biocompatibility and hemocompatibility.
The method of blood purification was tested in the laboratory of hemodialysis and plasmapheresis of the Russian Cardiology Research and Production Complex of the Ministry of Health of the Russian Federation. The tests were carried out in vitro using a Gambro roller pump and a slit nozzle. Patients' blood was divided on a PF-05 plasma separator. Of the 13 parameters analyzed, significant changes were observed in a decrease in the level of uric acid (the level of decrease exceeded 50%) and creatinine (a metabolite of nitrogen metabolism). The test results suggest the effectiveness of this method in the treatment of gout (impaired purine metabolism).
