Изобретение относится к области медицины, а именно: к анестезиологии-реаниматологии, наркологии и психиатрии, может быть использовано при лечении опиоидной зависимости (наркомании). Известен способ сверхбыстрой детоксикации опиоидов под наркозом введением высокой дозы налоксона (5). Суть метода состоит в сокращении фазы детоксикации до 6 - 8 ч. Налоксон в течение указанного времени полностью вытесняет опиоиды из рецепторов нервных клеток. При этом провоцируется значительное усиление симптоматики "отказа", что несет значительную угрозу для пациентов (вегетативная нестабильность, гипертонические или гипотонические кризы, нарушения сердечного ритма, тахипноэ, диарея, отек легких, почечная недостаточность и другие симптомы на основе избыточного высвобождения норадреналина и усиления симпато-адреналовой активности) (7). Не случайно в связи с опасностью метода данная терапия проводится в отделениях интенсивной терапии, а наркоз используется для устранения тяжелых субъективных ощущений и стабилизации вегетативного состояния. Серьезным недостатком способа и его модификаций является недостаточная надежность препаратов для наркоза по стабилизации нейровегетативного состояния пациентов, по предупреждению и ликвидации излишней симпатикотонии и выброса стрессовых гормонов. Более того, некоторые препараты (катамин, калипсол, оксибутират натрия) сами могут стимулировать симпато-адреналовую систему. Известны и побочные действия налоксона - гипертонические кризы, отек легких, мерцание желудочков сердца и остановка сердца (3). Поставленной задачи достигают за счет того, что дополнительно используют стресс-протекторное действие адреноганглиолитиков: за 40 мин до наркоза больному наряду с седуксеном и атропином вводят внутримышечно пентамин - 25 мг, обзидан - 1 мг, пирроксан - 10 мг; во время наркоза стресс-протекцию поддерживают дополнительным введением пентамина - в общей дозе 50-100 мг, обзидана 1-2 мг, пирроксана 10-20 мг; после выхода из наркоза в течение 2 дней больному продолжают введение пентамина - 25 мг, обзидана - 1 мг, пирроксана - 10 мг внутримышечно 3 раза в сутки. Способ осуществляют следующим образом: включают в премедикацию, наркоз и последующую терапию стресс-протекторные препараты: ганглиоблокаторы, альфа- и бета-адреноблокаторы. За 40 мин до наркоза внутримышечно проводят премедикацию:
р-р Седуксена 0,5% - 10 мг
р-р Пентамина 5% - 25 мг
р-р Обзидана 0,1% - 1 мг
р-р Пирроксана 1% - 10 мг
р-р Атропина 0,1% - 0,5 мг

Стресс-протекцию во время наркоза поддерживают дополнительным введением пентамина (методом тахифилаксии в дозе 50-100 мг), пирроксана (10-20 мг) и обзидана (1-2 мг). Во время наркоза по общим правилам анестезиологии осуществляют мониторирование функций внешнего дыхания, кровообращения, диуреза (контроль ЭКГ, АД, ЧСС, количества мочи, пульсооксиметрия), инфузионную терапию кристаллоидными растворами под контролем лабораторных анализов электролитного обмена. Сравнение результатов лечения опытной (с пролонгированной стресс-протекцией) и контрольной групп (по известному способу) показывает, что дополнительное включение стресс-протекторов (адреноганглиолитиков) в ускоренную опиоидную детоксикацию под наркозом позволяет успешно коррегировать отрицательные нейрогуморальные и вегетативные симптомы "отмены", свести до минимума опасности метода ускоренной детоксикации. В проведенных лабораторных и клинических исследованиях значительных сдвигов не определяется. АД - 115/80 мм рт.ст., пульс 86 уд. мин. За 40 мин до наркоза проведена внутримышечно премедикация: атропин 0,1% - 0,5 мг, седуксен 0,5% - 10 мг, пентамин 5% - 25 мг, обзидан - 0,1% - 1 мг, пирроксан - 1% -10 мг. Вводный наркоз тиопенталом натрия 1% - 400 мг + седуксен 15 мг + оксибутират натрия 4 г. Основной наркоз в течение 6 ч поддерживался дополнительным введением тиопентала натрия (суммарно 1 г), седуксена (40 мг), оксибутирата натрия (8 г). Дыхание самостоятельное, без интубации трахеи. За время наркоза фракционно вводился налоксон по 0,4-0,8 мг в общей дозе 8 мг. Инфузионная терапия раствором Рингера, физиологическим раствором и 5% раствором глюкозы в количестве 3,5 л. Колебания АД не выходили за пределы физиологических - 120/80 - 135/85 мм рт. ст., ЧСС от 64 до 92, состояние микроциркуляции на протяжении всей анестезии хорошее, пульсооксиметрия 97-98%. Диурез со стимуляцией лазиксом (20 мг) - 2,5 л. Выход из наркоза спокойный, плавный. Через 10 мин после окончания наркоза больной в сознании, отвечает на вопросы, принял через рот 50 мг антаксона. В ближайшие 12 ч состояние стабильное. Через сутки выписан из стационара для дальнейшего долечивания в амбулаторных условиях. Наблюдается психиатром и наркологом в течение года, наркотики не употребляет. Clinical example: 1. Patient A., 19 years old, has been using heroin for 4 years. Two years ago he suffered hepatitis B. Independent breathing, without tracheal intubation. During anesthesia, 0.4-0.8 mg naloxone was fractionally administered in a total dose of 8 mg. Infusion therapy with Ringer's solution, physiological saline and 5% glucose solution in an amount of 3.5 l. Fluctuations in blood pressure did not go beyond the physiological range of 120/80 - 135/85 mm Hg. Art., heart rate from 64 to 92, the state of microcirculation throughout the anesthesia is good, pulse oximetry is 97-98%. Diuresis with stimulation with lasix (20 mg) - 2.5 l. The exit from anesthesia is calm, smooth. 10 minutes after the end of anesthesia, the patient is conscious, answers questions, and receives 50 mg of antaxon by mouth. In the next 12 hours, the condition is stable. A day later, was discharged from the hospital for further follow-up care on an outpatient basis. Observed by a psychiatrist and narcologist throughout the year, does not use drugs.
