Abstract
Description
The invention relates to medicine, in particular to cardiology, and can be used to predict refractivity to antihypertensive therapy in men with severe arterial hypertension.
A known method for predicting refractoriness to antihypertensive therapy in patients with arterial hypertension based on an analysis of a number of clinical parameters (M. Fried, S. Greens (ed.). Cardiology in tables and diagrams - M .: Practice, 1996, p. 38-39), which consists in the fact that, according to a number of clinical and anamnestic signs characterizing the condition of a patient with arterial hypertension, a conclusion is made about the possible refractoriness to therapy.
The disadvantage of this method is the low accuracy of the forecast, in addition, this method does not take into account the gender of the patient and the severity of arterial hypertension.
There is a method adopted by us for the prototype (Schekotov V.V. Clinical and pathogenetic and therapeutic aspects of refractoriness of arterial hypertension. Abstract of thesis ... Doct. Medical Sciences. - Ekaterinburg, 1994, p. 11, 36-37), consisting in that blood pressure (BP) is measured upon admission to the hospital and a study of central hemodynamics is performed using the Impekard-2-Vitaton software and hardware complex, while refractoriness to treatment is predicted when a certain level of diastolic blood pressure is reached or not achieved, or erdechnogo index, or total peripheral resistance.
The disadvantage of this method is the low prevalence and high cost of the hardware-software complex "Imecard-2-Vitaton", as well as the lack of accuracy of the forecast.
Y=0,014•A1+2,79•А2,
The essence of the claimed invention lies in the fact that, according to the method for predicting refractoriness in severe arterial hypertension, which includes determining physiological parameters before starting a course of treatment, average blood pressure and the degree of lengthening of the QT interval with electrocardiography in lead V 5 are used as physiological parameters, then calculate the prognostic coefficient according to the formula
Y = 0.014 • A 1 + 2.79 • A 2 ,
where Y is the prognostic coefficient,
And 1 - average blood pressure, determined by the formula
where AD∂ - diastolic blood pressure, mm Hg,
ADS - systolic blood pressure, mmHg,
And 2 - the degree of lengthening of the QT interval, determined by the formula
where QTu is the QT interval measured in lead V 5 ,
QTp - QT interval calculated by the Bazetta formula,
and with a value of Y greater than 2.265, a possible refractoriness to antihypertensive therapy is predicted, and with a value of Y less than or equal to 2.265, its absence is predicted.
The application of the proposed method allows to increase the accuracy of the prognosis of refractoriness to antihypertensive therapy in patients with severe arterial hypertension, using non-invasive, readily available screening studies, which makes it possible to schedule an adequate antihypertensive treatment on time.
Y=0,014•A1+2,79•А2,
The method is as follows. Directly during the initial examination, the patient is measured blood pressure and electrocardiography is performed according to the standard method, during which the duration of the QT interval in lead V 5 is measured and the proper duration of the QT interval is calculated according to the well-known Bazett formula (Cardiology Manual, in 4 volumes / Ed. EI Chazova. M., 1982, volume 2, p. 214.), After which the prognostic coefficient is calculated by the formula:
Y = 0.014 • A 1 + 2.79 • A 2 ,
where Y is the prognostic coefficient,
And 1 - average blood pressure, determined by the formula
where AD∂ - diastolic blood pressure, mm Hg,
ADS - systolic blood pressure, mmHg,
And 2 - the degree of lengthening of the QT interval, determined by the formula
where QTu is the QT interval measured in lead V 5 ,
QTp - QT interval calculated by the Bazetta formula,
and with a value of Y greater than 2.265, a possible refractoriness to antihypertensive therapy is predicted, and with a value of Y less than or equal to 2.265, its absence is predicted.
