Изобретение относится к медицине, в частности к способам лечения длительно незаживающих ран. The invention relates to medicine, in particular to methods for treating nonhealing wounds.
Известны способы лечения длительно незаживающих ран с помощью ферментов, например, ируксола (см. М.А. Трунин, А.С. Емельянов "Применение ируксола в хирургической клинике". Ж. "Хирургия", 1982, N 8, стр. 88), трипсина (см. П. И. Толстых и др. "Влияние биологически активных перевязочных покрытий ран на течение раневого процесса". Ж. "Вестник хирургии", 1987, N 3, стр.57). В качестве прототипа выбран способ лечения длительно незаживающих ран, заключающийся в наложении повязок с мазями на водорастворимой основе (см. Ю. Р. Кашин и соавт. "Опыт применения композиций из водорастворимых мазей и парафинизированных покрытий при лечении ожогов IIIA степени". Материалы VII Всероссийской конференции по проблеме термических поражений. Поставленная задача решается за счет того, что в способе, включающем наложение повязок с водорастворимыми мазями, в первой фазе раневого процесса проводят сеансы аэроионотерапии с величиной потока ионов от 15 до 40 мкА в течение 10-25 дней, а во второй фазе перед наложением повязок с водорастворимыми мазями на рану трансплантируют культуру аллофибробластов на пленочном матриксе. Способ лечения длительно незаживающих ран осуществляют следующим образом. В первой фазе раневого процесса после туалета раны и окружающих кожных покровов проводят сеансы аэроионотерапии. Величину потока ионов устанавливают в пределах от 15 до 40 мкА в зависимости от характера раневого процесса. Сеансы аэроионотерапии проводят в течение 10-25 дней. После этого во второй фазе раневого процесса на рану после туалета ее и окружающих кожных покровов антисептическими растворами переносят аллофибробласты на пленочном матриксе, а сверху накладывают повязку с физиологическим раствором. Через день пленочный матрикс удаляют и на рану накладывают повязку с мазями на водорастворимой основе (типа "левомеколь"). Последующие перевязки выполняют через день. Сеансы аэроионотерапии могут быть продолжены после наложения на рану культуры аллофибробластов. A method for the treatment of nonhealing wounds is as follows. After that, in the second phase of the wound healing process, allofibroblasts on a film matrix are transferred to the wound after its toilet and surrounding skin with antiseptic solutions, and a bandage with physiological saline is applied on top. After a day, the film matrix is removed and a bandage with ointments on a water-soluble basis (such as “levomekol”) is applied to the wound. Subsequent dressings are performed every other day. Aeroionotherapy sessions can be continued after the application of an allofibroblast culture to the wound.
Клинический пример. Жихарева А.Ф. 64 лет, и.б. N 191744 поступила в ННИИТО с диагнозом: длительно незаживающая рана правой голени после механической травмы, бронхиальная астма. Локальный статус: в нижней трети правой голени на фоне измененных кожных покровов имеется рана размером 1,5 х 2см. Дном раны являются вялые, склерозированные, покрытые фибрином грануляции. Края раны рубцово изменены, признаки краевой эпителизации не выражены. Цитологически: разрушенные волокна, детрит, клеточные элементы представлены в виде единичных дистрофически измененных нейтрофилов (дистрофический характер клеточной реакции). Длительность существования раны - 3 месяца. На первом этапе лечения с целью очищения раны от некротически измененных тканей, стимуляции микроциркуляции в ране, проведен курс консервативной терапии, включающий аэроионотерапию в течение 22 дней. В результате проведенного лечения характер раны изменился: грануляции стали сочными, яркими, отделяемое скудным, налет отсутствует. Цитологически: детрит в небольшом количестве, единичные волокна, клеточные элементы представлены большим количеством нейтрофилов разной степени деградации и единичными полибластами (3-5%), лимфоцитами. Во второй фазе раневого процесса на раневую поверхность трансплантированы аллогенные фибробласты на пленочном матриксе, сверху наложена повязка с раствором гидрокортизона. Пленка с поверхности раны удалена через 1 сутки, далее перевязки проводили через день с водорастворимыми мазями. На 5-е сутки после трансплантации аллофибробластов размеры раны сократились до 0,5 х 1 см, отмечается выраженная краевая эпителизация, дном раны являются яркие сочные грануляции, налета нет, отделяемое скудное. Цитологически: клеточные элементы обильные, количество полибластов увеличилось в 10 раз и составляет 45-50%, нейтрофилы, лимфоциты, единичные макрофаги, детрит в небольшом количестве (регенераторный тип клеточной реакции). Полная эпителизация наступила через 12 дней после трансплантации аллофибробластов. Clinical example. The bottom of the wound is a flaccid, sclerotic, fibrin-coated granulation. The wound edges are scarred, signs of regional epithelization are not expressed. Cytologically: destroyed fibers, detritus, cellular elements are presented as single dystrophically altered neutrophils (dystrophic nature of the cellular reaction). The duration of the wound is 3 months. At the first stage of treatment, in order to cleanse the wound of necrotic tissue, stimulate microcirculation in the wound, a course of conservative therapy was carried out, including aeroionotherapy for 22 days. As a result of the treatment, the nature of the wound changed: granulation became juicy, bright, scanty, no plaque. Cytologically: detritus in a small amount, single fibers, cellular elements are represented by a large number of neutrophils of varying degrees of degradation and single polyblasts (3-5%), lymphocytes. In the second phase of the wound healing process, allogeneic fibroblasts on the film matrix are transplanted onto the wound surface, a dressing with a hydrocortisone solution is applied on top. The film from the surface of the wound was removed after 1 day, then the dressings were performed every other day with water-soluble ointments. On the 5th day after the transplantation of allofibroblasts, the size of the wound decreased to 0.5 x 1 cm, pronounced marginal epithelization is noted, the bottom of the wound is bright juicy granulation, there is no plaque, the discharge is scanty. Cytologically: cellular elements are abundant, the number of polyblasts increased by 10 times and amounts to 45-50%, neutrophils, lymphocytes, single macrophages, and detritus in a small amount (regenerative type of cellular reaction). Complete epithelization occurred 12 days after transplantation of allofibroblasts.
