Известен способ лечения ожогов, состоящий в том, что после выведения больных из шока проводят туалет обожженных поверхностей. Его выполняют в операционной после предварительного введения больному обезболивающих средств. Кожу вокруг ожога протирают салфетками, смоченными 0,5%-ным раствором аммиака или мыльной водой, осушают и обрабатывают спиртом. Затем снимают с обожженной поверхности инородные тела, обрывки эпидермиса. После туалета ожоги лечат с помощью влажно-высыхающих и мазевых повязок. После появления грануляций проводят аутодермопластику с последующим лечением ожоговых и донорских ран с помощью влажно- высыхающих и мазевых повязок (Большая медицинская энциклопедия, М.:Советская энциклопедия, 1981, т. 17, с. 231). При необходимости проводят дерматомную некрэктомию с помощью дискового электродерматома Мензула (патент СССР N 1817692, кл. A 61 B 17/322). Лечение ожоговых ран перфорированным пленочным перевязочным материалом DDB-M проводят до полного некролизиса и заживления ран, или образования мелкозернистой грануляции. Перевязки осуществляют через 24, 48 и 72 ч по мере необходимости с обязательным припудриванием ран биопрепаратом "Кератан". Перевязки безболезненны, не требуют применения наркоза. Затем при необходимости проводят щадящую эктомию грануляций до основания и появления кровотечения в виде росы с помощью электродерматома Мензула (глубина среза 0,1-0,15 мм) с последующим гемостазом и аутодермопластикой. Аутодермопластику проводят закрытием раны расщепленным перфорированным (1 : 2-4) или неперфорированным кожным лоскутом. Кожный лоскут также срезают электродерматомом Мензула. Затем раны (ожоговые, закрытые кожными лоскутами, и донорские) закрывают перфорированным пленочным перевязочным материалом DDB-M. Перевязки осуществляют по мере необходимости через 24, 48 и 72 ч. Приживление кожных лоскутов 100%. После приживления кожного лоскута для предупреждения роста рубцов и деформаций тела на зажившие поверхности шьется индивидуальное компрессионное белье, которое носят 22 ч в сутки в течение 1 -2 лет. The engraftment of skin flaps is 100%. After engraftment of the skin flap to prevent the growth of scars and deformations of the body, individual compression garments are sewn onto the healed surfaces, which are worn 22 hours a day for 1-2 years.
В результате лечения ожогов предлагаемым методом значительно снижается болезненность процесса лечения и перевязок, процессы заживления ускоряются на 60-70% по сравнению с традиционными методами и примерно на 20-30% по сравнению с методом лечения повязками DDB, появляется возможность обойтись без операции аутодермопластики. Menzu) не токсичен, не вызывает аллергии, предохраняет рану от инфицирования извне, не травмирует раневую поверхность, не вызывает вторичных некрозов, обеспечивает наличие влажной среды, по своему общему характеру близкой к составу внутренней среды организма, останавливает развитие микробного процесса, стимулирует регенераторные процессы в ране, миграцию эпителиальных клеток, способствует краевой эпителизации, выполнению раны мелкозернистой грануляционной тканью, безрубцовому заживлению ран. Самые тяжелые ожоговые раны располагались на левой половине лица, в области подбородка и передней и боковых поверхностей шеи. С первых дней начато местное лечение ран в условиях влажной среды под перфорированным пленочным перевязочным материалом DDB-M с предварительным припудриванием ран биопрепаратом "Кератан". Перевязки проводились по мере необходимости сначала через 24, затем через 48 и 72 ч, были безболезненны, применения наркоза не требовалось. Example 1. Лечение ожогов проводилось на фоне эписиндрома. В ожоговом отделении местное лечение ран проводилось в условиях влажной среды с помощью перфорированного пленочного перевязочного материала DDB-M с обязательным припудриванием ран биопрепаратом "Кератан" перед наложением повязки. Перевязки проводились по мере необходимости сначала через 24, затем через 48 и 72 ч, были безболезненны, применения наркоза не требовалось. В процессе лечения большинство ран закрылись. Осталось около 3-4% выполненных мелкозернистой грануляцией ран. Затем была проведена щадящая дерматомная эктомия грануляций с помощью дискового электродерматома Мензула и проведена аутодермопластика со 100%-ным приживлением пересаженного кожного лоскута. Больной был переведен в другую больницу для лечения неврологического заболевания. Example 2.
