Abstract
Description
The invention relates to medicine and can be used in traumatology and neurosurgery.
The most common way to treat fractures and dislocations of the cervical spine is constant traction. For this purpose, Glisson’s “loop”, proposed by him in 1950 (a specially cut fabric loop covering the lower jaw and the back of the head), is still used. Along with positive qualities, this old-fashioned method has significant disadvantages: insufficient traction; unpleasant, and often painful sensations associated with pressure loops on soft tissues; inconvenience of patient care; difficulty eating.
Skeletal traction for the bones of the skull began to be used in the 30s of our century both abroad and in our country. Currently, skeletal traction is used in several forms: in the form of special klesses and brackets, in the form of wire, silk or kapron ligatures fixed to the bones of the skull, as well as loops or hooks attached to the zygomatic arches.
The closest is a method of treating injuries of the cervical spine, including skeletal traction by fixing braces on the bones of the skull, to which a force is applied along the spine. In this case, the bracket is fixed to the skull using knitting needles. Under local anesthesia with a scalpel, small incisions are made to the bone in the area of the parietal tubercles. Through the parietal tubercles carry needles (1). The disadvantage of this method is its complexity, since it requires special tools, the method is traumatic, surface tissues are damaged, the staple may be skewed or displaced during stretching and not effective enough because the stretching force is limited by the possibility of damaging the surface tissues, while the efforts of pressing the knitting needles to the bones of the skull is also limited due to the possibility of damage to them.
The aim of the invention is to simplify the method through the use of more accessible tools and devices, increasing efficiency by reducing damage to surface tissues and the possibility of increasing exhaust force.
This goal is achieved by the fact that in the treatment of injuries of the cervical spine, including skeletal traction by fixing the staples on the bones of the skull, the bracket is fixed using a spoke drawn through the middle zone of the face in the transverse direction at the level of the zygomatic bones, the ends of which are fixed in the bracket. This embodiment of the method avoids the above disadvantages, reliably securing the spoke. The method is publicly available, since there are knitting needles in any medical institution with a traumatological profile. In this case, it is possible to change the direction of traction and the traction force.
The method is as follows. Through the middle zone of the face at the level of the zygomatic bones, a spoke is drawn in the transverse direction, to the ends of which a bracket is then attached, for example, in the form of a standard frame with a tensiator. Traction is carried out according to the generally accepted method, attaching the load to the bracket through the block.
The proposed variant of skeletal traction was used in the treatment of 12 victims, both forced and long traction (up to one month) was used. After stretching, three patients underwent surgical fixation of the cervical vertebrae. In case of unsuccessful skeletal traction, one victim from this group was repaired and secured the dislocation of the cervical vertebrae in an open way.
П р и м е р. PRI me R. Patient D. born in 1946, delivered from the accident site on 11/30/90. Diagnosis: mild brain injury. Fracture-dislocation anterior C VI . After examination, on the day of admission to the patient under local anesthesia, skeletal traction was applied according to the proposed technique with a load of up to 2 kg. A week after the start of skeletal traction, the patient was confined with a Shants collar, and the spoke was removed because the correct standing of the previously displaced vertebra was observed radiologically. The patient was discharged for outpatient treatment 21 days after surgery. No complications were observed during treatment.
