Способ реконструктивно-пластической операции у пациентов с местно-распространенным раком гортаноглотки T4A, с распространением опухоли на шейный отдел пищевода, включающий выполнение нижней трахеостомии, лимфаденэктомии и фаринготомии по Троттеру, слизистую оболочку задней стенки глотки и шейный отдел пищевода отсепаровывают от предпозвоночной фасции, отличающийся тем, что отсепаровывают слизистую оболочку задней стенки глотки и шейного отдела пищевода от предпозвоночной фасции, после вскрытия глотки и шейного отдела пищевода выполняют удаление опухоли единым блоком с включением верхнего пищеводного сфинктера, отступя 1 см от видимой границы, производят измерение поперечного и продольного размеров дефекта и производят разметку участка кожи в проекции большой грудной мышцы на стороне операции, мобилизуют размеченный кожный лоскут вместе с участком большой грудной мышцы и перемещают через подкожный тоннель в область операционной раны на шее, после чего осуществляют тщательный гемостаз в области раны на передней грудной стенке и ушивают ее с оставлением аспирационного дренажа, далее перемещенный кожный-мышечный лоскут ротируют на 180 градусов и с помощью него формируют внутреннюю выстилку нижней части глотки и шейного отдела пищевода, а также верхний пищеводный сфинктер, кожную порцию перемещенного лоскута герметично подшивают к краям дефекта слизистой оболочки глотки и шейного отдела пищевода по кругу узловыми швами из рассасывающего материала, вводят носо-пищеводный зонд, мышечную порцию лоскута фиксируют к местным тканям, создавая дополнительную герметизацию и восполняя объем дефекта мягких тканей шеи, рану послойно ушивают с оставлением аспирационного дренажа.A method for reconstructive plastic surgery in patients with locally advanced laryngopharyngeal cancer T4A, with tumor spread to the cervical esophagus, including lower tracheostomy, lymphadenectomy and pharyngotomy according to Trotter, the mucous membrane of the posterior pharyngeal wall and the cervical esophagus are separated from the prevertebral fascia, characterized in that that separate the mucous membrane of the posterior pharyngeal wall and cervical esophagus from the prevertebral fascia, after opening the pharynx and cervical esophagus, the tumor is removed in a single block with the inclusion of the upper esophageal sphincter, 1 cm away from the visible border, the transverse and longitudinal dimensions of the defect are measured and the marking of the skin area in the projection of the pectoralis major muscle on the side of the operation, the marked skin flap is mobilized together with the area of the pectoralis major muscle and moved through the subcutaneous tunnel to the area of the surgical wound on the neck, after which a thorough hemostasis is performed in the area of the wound on the anterior chest wall and sutured leaving aspiration drainage, then the displaced skin-muscle flap is rotated 180 degrees and with it the inner lining of the lower part of the pharynx and cervical esophagus is formed, as well as the upper esophageal sphincter, the skin portion of the displaced flap is sealed sutured to the edges of the defect in the mucous membrane of the pharynx and cervical esophagus in a circle with interrupted sutures made of absorbable material, a nasoesophageal probe is inserted, the muscle portion of the flap is fixed to local tissues, creating additional sealing and replenishing the volume of the defect in the soft tissues of the neck, the wound is sutured in layers leaving aspiration drainage.
