Description
The invention relates to medical technology and can be used in the treatment of patients with osteo-articular pathology.
The most promising elements of the structures of external fixation devices in the treatment of patients with osteo-articular pathology are the Kirschner spokes, which ensure the immobile fixation of the bone processes in the process of osteogenesis.
However, often under the action of an aggressive biological environment, due to the absence of physicomechanical conditions that ensure effective integration (at the micro and nano level) interaction of the surface of the spokes with adjacent bone structures, processes of inflammation of adjacent tissues and rejection of temporarily installed structures occur.
Therefore, the problem of increasing the efficiency of using Kirchner spokes is highly relevant and can be solved by imparting bacteriostatic properties to the surface provided with a bioactive coating.
It is known that copper and copper-containing medicines have an effective bacteriostatic ability and are often used for various medical purposes. Therefore, the development and implementation of implant structures with a biocompatible copper-containing coating with bacteriostatic properties will reduce the number of postoperative complications, shorten engraftment times and improve treatment and rehabilitation indicators due to the bacteriostatic effect of copper in the coating composition.
с. 1983].Known used in the apparatus for external fixation of the needle Kirchner, made in the form of a smooth rod, having at the end of a faceted sharpening [A. USSR №1055499, publ. 1983].
The disadvantage of this design of the spokes is the lack of bioactive coating and bacteriostatic properties, which leads to a violation of the stability of osteosynthesis, causes the development of postoperative complications and increases the duration of treatment.
[А.с. СССР №1428361, опубл.: 07.10.1988.].Known needle for osteosynthesis, made in the form of a rod having at the end of a faceted sharpening and a plot of threaded cutting in the middle part. [A. USSR №1428361, publ .: 10/07/1988.].
The disadvantage of this design of the needles for osteosynthesis is the lack of bioactive coating and bacteriostatic properties, which leads to disruption of the stability of osteosynthesis, causes the development of postoperative complications and increases the duration of treatment.
The closest to the technical nature of the proposed utility model is the design of the spokes for osteosynthesis [RF Patent 130494 IPC А61Б 17/58 (2006.01), published on July 27, 2013]. The needle for osteosynthesis is made in the form of a rod with threaded cutting and faceted sharpening, provided with a bioactive coating, the cutting is performed throughout, and the faceted sharpening - at both ends. The needle can be used as an independent stabilizing element, for example when performing arthrodesis.
The disadvantage of this design is the lack of microhardness of the surface of the structure and the absence of the bacteriostatic properties of the bioactive coating, which leads to a violation of the stability of osteosynthesis, causes the development of postoperative complications and increases the duration of treatment.
The task of the utility model is the creation of needles for osteosynthesis with a bioactive coating, which has a high surface microhardness and bacteriostatic properties.
(CO2).The technical result of the utility model is to strengthen, in particular, increase the microhardness of the bioactive spokes coating for osteosynthesis and impart bacteriostatic properties due to the synthesis of a copper-containing diamond-like carbon-like film on their surface in a vacuum environment of carbon dioxide (CO 2 ).
The problem is solved due to the fact that the proposed needle for osteosynthesis, made in the form of a rod having a threaded slit all over the outer surface, cut triangular sharpening at both ends and a bioactive hydroxyapatite coating, according to a new hydroxyapatite threaded coating parts of the spokes there is a copper-containing carbon diamond-like film synthesized in the process of ion-beam processing by a beam of copper ions (Cu + ) in a vacuum environment of carbon dioxide.
Production of the proposed needles for osteosynthesis can be carried out by casting, drawing, machining, gas-plasma processing (applying a bioactive coating from hydroxyapatite), ion-beam processing (synthesis on the coating surface of a threaded part from hydroxyapatite with a copper-containing carbon diamond-like film obtained in the process of ion-beam processing a beam of copper ions (Cu + ) in a vacuum environment of carbon dioxide).
Description of the design.
FIG. The proposed design shows the needles for osteosynthesis, where the positions indicate: 1 - rod, 2 - threaded cutting along the core, 3 - faceted (triangular) sharpening at both ends, 4 - biologically active coating of hydroxyapatite, 5 - copper-containing carbon diamond-like film.
The design of the needle for osteosynthesis (Fig.) Consists of a
To install the proposed needles for osteosynthesis using a drill without prior reaming of the channel, the
The proposed design of the spokes for osteosynthesis has a high microhardness due to the threaded part of the hydroxyapatite copper-containing carbon diamond-like film formed on the surface of the bioactive coating, the synthesis mechanism of which ensures high adhesion due to ionic mixing of the copper-containing carbon diamond-like film fragments with the surface and surface layers of the surface patterns of surface and surface layers. (Perinskaya, IV Synthesis of electroplasma hydroxyapatite coating on the surface of titanium using the ion-beam processing method / IV Perinskaya, VN Lyasnikov, VV Perinsky, OD Muktarov // Perspective materials, 2013. - № 8. - p. 52-57.). The results of the study of the microhardness of the surface of the fabricated spokes for osteosynthesis are the values of which are 28-30 GPa, while the microhardness of the hydroxyapatite coating is 13-15 GPa. The needle for osteosynthesis also has bacteriostatic properties due to a copper-containing diamond-like carbon film synthesized in the process of ion-beam processing with a beam of copper ions (Cu + ) in a carbon dioxide vacuum environment, which is confirmed by the experimentally obtained results of the study, which showed that the optimal doses of copper ions, necessary to impart bacteriostatic properties to the coating are 1.8⋅10 16 -2.4⋅10 16 ion / cm 2 with an accelerating voltage of 75 kV. At doses of copper ions less than 1.8⋅10 16 ion / cm 2 and more than 2.4⋅10 16 ion / cm 2, bacteriostatic properties do not appear. The bacteriostatic ability of the coating is due to the complex of therapeutic properties inherent in copper-containing coatings and medical preparations against pathogenic microflora, which delay the complete reproduction of bacteria and other microorganisms, that is, cause bacteriostasis, which is especially important for bio-engineering structures of temporary residence in the body.
Thus, the proposed design of the spokes for osteosynthesis, which has an increased surface microhardness, creates the best conditions for effective integration interaction with bone tissue due to synthesis on the surface of the bioactive coating of the threaded part of the hydroxyapatite copper-containing carbon diamond-like film, which has improved biocompatibility and pronounced bacteriostatic properties that help eliminate the risk of occurrence and development of complications th after the operating period by minimizing the development of pathogenic microflora in the zone of contact with biological tissue.
