Abstract
The utility model relates to medicine, namely to dentistry, and can be used in operations of targeted tissue regeneration of the periodontium and alveolar ridge as a non-resorbable membrane, which allows to distinguish between tissue growth of the mucous membrane and bone tissue. The non-absorbable membrane is made in the form of a wire mesh with a titanium alloy wire with a titanium content of at least 80%. The membrane does not cut through the tissue. The mesh structure holds the augment well, allows you to cut with scissors and give the membrane the necessary shape and size. The membrane has high plasticity and the property of adhesion (light adhesion) to the tissues, which allows it to adhere tightly to the tissues and does not require fixing with pins and screws. The porous structure of the mesh allows vessels to grow into the transplantation zone, integrate completely with connective tissue, improving the gum biotype through the formation of an internal tender scar, and does not require removal.
Description
The utility model relates to medicine, namely to dentistry and can be used in operations of directed tissue regeneration of the periodontium and alveolar process as a non-resorbable membrane, which allows to distinguish between tissue growth of the mucous membrane and bone tissue.
Targeted tissue regeneration is based on setting the required volume of bone tissue restoration, by installing a barrier - a membrane between soft and hard tissues, which helps to prevent soft tissue overgrowth between the granules of augmented bone material, and therefore a negative effect on bone formation.
(1).For directed tissue regeneration, resorbable and non-resorbable membranes are used (1).
The composition of resorbable membranes includes collagen, polymers of glyceric or polylactic acid. The main advantages of the resorbable membranes is that they completely absorb, eliminating the need for repeated surgery to remove the membrane (2). The disadvantage of resorbable membranes is the likelihood of antigenic conflict in response to foreign material, the lack of the ability to maintain volume to obtain a significant increase in bone tissue, which greatly limits their osteoregenerative capabilities. The resorption process can be faster than the restoration of bone tissue, which leads to the disappearance of the barrier function.
Non-resorbable membranes allow bone tissue to be restored in a given volume and trajectory. A significant disadvantage of non-resorbable membranes is that they require repeated surgery to remove, are poorly modeled, have a tendency to cut through tissues, require fixation with pins and screws. In addition, the residence time of the membrane in place is determined by the period of onset of periodontal tissue regeneration. A longer than necessary time spent by the membrane on the alveolar process increases the risk of tissue suppuration, while early removal of the membrane does not contribute to the complete regeneration of the periodontium.
(3).Non-resorbable membranes made of polytetrafluoroethylene, silicone, titanium nickelide are known. The disadvantage of non-resorbable membranes from the above materials is the likelihood of antigenic conflict in response to foreign material (3).
The closest analogues of the claimed utility model are titanium membranes, which are divided into perforated and non-perforated. Non-perforated membranes are rarely used, which is associated with poor tissue integration.
Taken as a prototype, is a non-resorbable membrane "FRIOS BoneShields" manufactured by Dentsply Implants (Germany) (http://www.mfl.ru/index.php/produkciya/dentalnaya-implantologiya/produktsiya-frios-friadent-germaniya), made in the form perforated titanium plate of a special shape. The disadvantage of this device is the tendency to cut through the tissue, the need for fixing with pins, the need for repeated operations to remove, a limited number of sizes, limited modeling.
The problem to which the claimed utility model is directed is to improve the quality of treatment of diseases where directed tissue regeneration using biologically inert material is necessary. To solve this problem, the non-resorbable membrane is made in the form of a wire mesh with a wire with a diameter of 0.02-0.2 mm, of a biologically inert material - a titanium alloy with a titanium content of at least 80%. The inventive utility model has a cellular structure with a side size of cells from 0.1 mm to 0.7 mm and fully performs barrier functions, has high biocompatibility, and is not subject to any effect of biological enzymes. Due to the absence of sharp corners and ductility, the membrane does not cut through the tissue. The mesh structure holds the augment well, allows you to cut with scissors, giving the necessary shape and size. Due to the mutual mobility of the wires, the grid allows you to give the membrane the shape of an object that it fits. The membrane has high plasticity and the property of adhesion (light adhesion) to tissues, which allows it to adhere tightly to the bone and does not require fixing with pins and screws. Unlike the analogue, the porous structure of the mesh allows the vessels to grow into the transplantation zone, integrates completely with connective tissue, improving the gum biotype due to the formation of an internal tender scar. No removal required.
Installation of a non-resorbable membrane is done through open access. The incision is made along the crest of the alveolar ridge. Using a smoothing machine, the mucosal-periosteal flap is peeled off. A bone defect is freed from granulation and is filled with bone-replacing material. The membrane is cut out in size and shape of the defect and is laid on top of the bone-replacing material. The flap is fixed with a simple interrupted suture. Literature:
