Description
Foreliggende oppfinnelse vedrører farmasøytisk og organisk kjemi og tilveiebringer nye forbindelser og farmasøytiske sammensetninger som er nyttige for å fremme ekspresjonen av insulin fra bukspyttkjertel B-type øyceller fra pattedyr. Opprinnelsen vedrører spesielt polypeptider, GLP-1-analoger og GLP-1 -molekylkomplekser, samt fremgangsmåter for fremstilling, farmasøytiske formuleringer og anvendelser av slike.
Endokrine utskillelser fra bukspyttkjerteløyer er under omfattende kontroll, ikke bare ved blod-bårede metabolitter (glukose, aminosyrer, katecholaminer, osv.), men også ved lokal parakrin innvirkning. De viktigste bukspyttkjerteløyhormoner (glukagon, insulin og somatostatin) reagerer blant deres spesifikke celletyper (A, B og D celler) for å modulere sekretoriske responser formidlet av ovennevnte metabolitter. Til tross for at insulinutskillelsen hovedsakelig blir kontrollert av blodnivåene til glukose, inhiberer somatostatin glukose-formidlet insulinsekretoriske responser. I tillegg til den foreslåtte paracrine "interislet" reguleringen av insulinsekresjonen er det bevis for å understøtte eksistensen av insulinotropiske faktorer i tarmen. Dette konseptet har opphav fra observasjoner om at glukose tatt oralt er et mye mer potent stimulerende middel for insulinsekresjon enn en sammenlignbar mengde glukose gitt intravenøst.
Det humane hormonet glukagon er et 29-aminosyrepeptidhormon produsert i A-cellene til bukspyttkjertelen. Hormonet hører til en multi-genfamilie av strukturelt relaterte peptider som innbefatter sekretin, gastrisk inhibitorisk peptid, vasoaktivt intestinalt peptid og glicentin. Disse peptidene regulerer karbohydratmetabolismen, mage-tarm bevegeligheten og den sekretoriske prosesseringen, forskjellig. De viktigste anerkjente virkningene til bukspyttkjertelglukagon er å fremme hepatisk glykogenolyse og glykoneogenese, som resulterer i en økning av blodsukkernivåene. I dette henseendet er virkningene til glukagon motsatt i forhold til insulin og kan bidra til hyperglykemi som ledsager diabetes mellitus [(Lund, P.K., et al., Proe. Nati. Acad. Sei, USA, 79:345-349
(1982)].
Glukoagon er blitt funnet å kunne bli bundet til spesifikke reseptorer som ligger på overflaten av insulinproduserende celler. Når glukoagon er bundet til disse reseptorene, stimulerer den hurtige syntesen av cAMP til disse cellene. cAMP er blitt funnet å stimulere insulinekspresjonen [Korman, L.Y., et al., Diabetes, 34: 717-722 (1985)]. Insulin inhiberer glukoagonsyntesen [Ganong, W.F., Review of Medical Physology, Lange Publications, Los Altos, California, s. 273 (1979)]. Ekspresjon av glukagon blir nøye regulert av insulin og også av serumglukosenivået.
Glukagongenet blir opprinnelig translatert fra en 360 basepar forløper for å danne polypeptidet, preproglukagon [Lund, et al., Proe. Nati. Acad. Sei. U.S.A. 79:345-349
(1982) ]. Dette polypeptidet blir deretter prosessert for å danne proglukagon. Patzelt, C, et al., Nature, 282:260-266 (1979), demonstrerte at proglukagon deretter ble spaltet til glukagon og et annet polypeptid. Senere arbeider til Lund, P.K., et al., Lopez L.C., et al., Proe. Nati. Acad. Sei. U.S.A., 80:5485-5489 (1983) og Bell, G.I., et al., Nature 302:716-718 (1983) demonstrerte at proglukagonmolekylet ble spaltet umiddelbart etter lysin-arginindipeptid residiene. Studier av proglukagon produsert av kanal-steinbit (Ictalurus punctata) indikerte at glukagon fra dette dyret også ble proteolytisk spaltet nabosittende lysin-arginin dipeptidresidier [Andrews P.C., et al., J. Biol. Chem., 260:3910-3914 (1985), Lopez, L.C., et al., Proe. Nati. Acad. Sei. U.S.A., 80:5485-5489
(1983) ]. Bell, G.I., et al., supra oppdaget at pattedyr proglukagon ble spaltet ved lysin-arginin eller arginin-arginin dipeptider og demonstrerte at proglukagon molekylet inneholdt tre adskilte og meget homologe peptidmolekyler som ble betegnet glukagon, glukagon-lignende peptid 1 (GLP-1) og glukagon-lignende peptid 2 (GLP-2). Lopez, et al., konkluderte at glukagon-lignende peptid 1 hadde en lengde på 37 aminosyreresidier og at glukagon-lignende peptid 2 hadde en lengde på 34 aminosyreresidier. Analoge studier på strukturen av rotte preproglukagon viste et lignende mønster på den proteolytiske spaltningen mellom nabosittende lysin-arginin eller arginin-arginin dipeptidresidier, hvilket resulterer i dannelsen av glukagon, GLP-1 og GLP-2 [Heinrich, G., et al., Endocrinol., 115:2176-2181 (1984)]. Humane, rotte, bovine og hamstersekvenser til GLP-1 er blitt funnet å være identiske [Ghiglione, M., et al., Diabetologia, 27:599-600 (1984)].
Konklusjonen til Lopez et al., med hensyn på størrelsen til GLP-1 ble bekreftet av arbeidet til Uttenthal, L.O., et al., J. Clin, Endocrinol. Metabol., 61:472-479 (1985). Uttentahl, et al., undersøkte de molekylære formene til GLP-1 som er til stede i human bukspyttkjertel. Deres forskning viste at GLP-1 og GLP-2 er til stede i bukspyttkjertelen som peptider med 37 aminosyrer og 34 aminosyrer.
Likheten mellom GLP-1 og glukagon foreslått av andre forskere tyder på at GLP-1 kan ha biologisk aktivitet. Til tross for at noen forskere oppdaget at GLP-1 kunne indusere rottehjerneceller til å syntetisere cAMP [Hoosein, N.M., et al., Febs Lett. 178:83-86
(1984) ], har andre forskere ikke funnet noen fysiologisk rolle for GLP-1 (Lopez, L.C., et al.). Mangelen på å identifisere en fysiologisk rolle for GLP-1 gjorde at noen forskere satte spørsmålstegn ved om GLP-1 var et hormon og om slektskapet mellom glukagon og GLP-1 kan være tilfeldig. Varianter av GLP-1 (7-37) og analoger derav, er også blitt beskrevet. Disse variantene og analogene innbefatter for eksempel Gln<9->GLP-1 (7-37), D-Gln<9->GLP-1 (7-37), acetyl-Lys^-GLP-l (7-37), Thr<1>6-Lys18-GLP-1 )7-37), Lys<18->GLP-1 (7-37) og lignende, og derivater derav innbefattende for eksempel syreaddisjonssalter, karboksylatsalter, lavere alkylestere og amider (se for eksempel WO 91/11457]. De forskjellige beskrevne formene av GLP-1 er kjent for å stimulere insulinutskillelsen (insulinotropisk virkning) og cAMP dannelsen (se for eksempel Mojsov, S., Int. J. Peptide Protein Research, 40:333-343 (1992)].
Mer viktig er det at flere forfattere har demonstrert forbindelsen mellom laboratorisk eksperimentering og pattedyr, spesielt humane, insulinotropiske responser overfor eksogen administrering av GLP-1, spesielt GLP-1 (7-36)NH2°8 GLP-1 (7-37) [se, for eksempel Nauck, M.A., et al., Diabetologica, 36:741-744 (1993); Gutniak, M., et al., New England J. of Medicine, 326(20): 1316-1322 (1992); Nauck, M.A., et al., J. Clin, Invest, 91:301-307 (1993); og Thorens, B., et al., Diabetes, 42:1219-1225 (1993)].
De fundamentale defektene identifisert å forårsake hyperglykemi ved begynnende diabetes hos voksne ("maturity onset diabetes") er redusert utskillelse av endogen insulin og motstand mot virkningene til insulin i muskel og lever [Galloway, J.S., Diabetes Care, 13:1209-1239, (1990)]. Sistnevnte defekt resulterer i omfattende produksjon av glukose fra leveren. Et normalt individ frigjør glukose i en rate på omtrent 2 mg/kg/min. utgjør denne mengden i pasienter med diabetes i voksne vanligvis mer enn 2.5 mg/kg/min. som resulterer i et netto overskudd på minst 70 gram glukose pr. 24 timer. Det faktumet at det eksisterer omfattende høye korrelasjoner mellom hepatisk glukose produksjon, fastende blodglukose og total metabolsk kontroll som vist ved glykohemoglobin målinger [Galloway, J.A., supra; og Galloway, J.A., et al., Clin. Therap., 12:460-472 (1990)], er det tydelig at kontroll på fastende blodglukose er en sine quo non for oppnåelse av total normalisering av metabolismen tilstrekkelig for å forhindre komplikasjoner med hyperglycemi. I lys av det faktumet at tilstedeværende former av insulin sjelden normaliserer hepatisk glukoseproduksjon uten å produsere signifikant hyperinsulinemi og hypoglykemi (Galloway, J.A., og Galloway, J.A., et al., supra) er det nødvendig med alternative tilnærmelser.
Intravenøse infusjoner av GLP-1 (7-36)NH2 f°r & produsere to ganger normale serumkonsentrasjoner er blitt demonstrert å produsere virkningene indikert i tabellen nedenfor:
Langtidsstabiliteten til GLP-1, spesielt GLP-1 som en komponent av en farmasøytisk sammensetning for administrering til pattedyr, kan diskuteres. Når denne lagres ved lavtemperatur ved 4°C, er biprodukter av GLP-1 (7-37) blitt funnet så tidlig som 11 måneder etter prøvepreparering (Mojsov, S., supra). Det eksisterer derfor et behov for en mer stabil GLP-1 forbindelse som trygt kan bli administrert til pattedyr som trenger slik behandling.
Den biologiske halveringstiden til GLP-1 molekylene, spesielt de molekylene som blir påvirket av aktiviteten til dipeptidyl-peptidase IV (DDPIV) er meget kort. Den biologiske halveringstiden til GLP-1 (7-37) er for eksempel bare 3 til 5 minutter og blir videre påvirket av den hurtige absorpsjonen etter parenteral administrering til et pattedyr. Det eksisterer derfor et behov for en GLP-1 forbindelse som forsinker absorpsjonen etter slik administrering.
Foreliggende oppfinnelse løser dermed problemet med serum instabilitet og kort serum halveringstid assosiert med native GLP-1 molekyler. Forbindelsene ifølge oppfinnelsen tilveiebringer også forsinket absorpsjon etter parenteral administrering og bør derfor ha forlenget biologiske halveringstider. Det er også frembrakt farmasøytiske sammensetninger av forbindelsen ifølge oppfinnelsen, samt fremgangsmåter for fremstilling og anvendelse av slike forbindelser.
Foreliggende oppfinnelse tilveiebringer et polypeptid, kjennetegnet ved at det har følgende formel:

Også tilveiebrakt ifølge foreliggende oppfinnelse er en analog av GLP-1 (7-37)OH eller GLP-1 (7-36)NH2, kjennetegnet ved at den omfatter L-histidin i posisjon 7 og Val i posisjon 8; nevnte analog har et isoelektrisk punkt i området fra 6,0 til 9,0.
Foreliggende oppfinnelse tilveiebringer videre et GLP-1 -molekylkompleks, kjennetegnet ved at det består av et divalent metallkation assosiert med og i stand til å kopresitere med et polypeptid ifølge hvilket som helst av kravene 1 til 8.
Foreliggende opprinnelse tilveiebringer videre en farmasøytisk formulering, kjennetegnet ved at den omfatter polypeptidet ifølge oppfinnelsen eller farmasøytisk akseptabelt salt derav, assosiert med én eller flere farmasøytisk akseptable bærere, diluenter eller eksipienter derfor.
Foreliggende oppfinnelse tilveiebringer videre en farmasøytisk formulering, kjenntegnet ved at den omfatter komplekset ifølge opprinnelsen, assosiert med én eller flere farmasøytisk akseptable bærere, diluenter eller eksipienter derfor.
Foreliggende oppfinnelse tilveiebringer videre en fremgangsmåte for fremstilling av komplekset ifølge oppfinnelsen, kjennetegnet ved at den omfatter å blande et GLP-1 - molekyl med et divalent metallkation.
Foreliggende oppfinnelse tilveiebringer videre anvendelse av en effektiv mengde av komplekset ifølge oppfinnelsen, for fremstilling av et medikament for å behandle diabetes mellitus som starter i moden alder hos et pattedyr.

Som anvendt i foreliggende oppfinnelse angir betegnelsen "GLP-1 molekyl" naturlig forekommende GLP-1 (7-36)NH2, GLP-1 (7-37), naturlig og unaturlige funksjonelle analoger og derivater derav, og salter derav. Aminosyresekvensen til GLP-1 (7-36)NH2 er velkjent innenfor fagområdet, men er presentert nedenfor:
For GLP-1 (7-37) er den karboksy-terminale amidfunksjonaliteten til Arg<36> erstattet med Gly i 37 posisjonen til GLP-1 (7-36)NK2 molekylet.
I tillegg er eksistensen og preparering av et flertall beskyttede, ubeskyttede og spesielt beskyttede naturlige og unaturlige funksjonelle analoger og derivater av GLP-1 (7-36)NH2°S GLP-1 (5-37) molekyler blitt beskrevet innenfor fagområdet (se for eksempel US-PS 5.120.712 og 5.118.666, som er inkorporert heri som referanse og Orskov, C, et al., J. Biol. Chem., 264(22): 12826-12829 (1989) og WO 91/11457 (Buckley, D.I., et al., publisert august 8,1991)].
Som kjent innenfor fagområdet kan aminosyreresidier være i beskyttet form hvor både amino- og karboksygruppene har hensiktsmessige beskyttelsesgrupper, i delvis-beskyttet form hvor enten amino- eller karboksygruppene har hensiktsmessige beskyttelsesgrupper, eller i ubeskyttet form hvor verken amino- eller karboksygruppene har en hensiktsmessig beskyttelsesgruppe. Et flertall av reaksjoner for dannelse og fjerning av slike beskyttelsesgrupper er beskrevet i et antall standardarbeider som inkluderer for eksempel "Protective Groups in Organic Chemistry", Plenum Press (London og New York, 1973); Green, T.H., "Protective Groups in Organic Synthesis", Wiley (New York, 1981); og "The Peptides", Vol. I, Schroder og Lubke, Academic Press (London og New York, 1965).
Representative aminobeskyttelsesgrupper innbefatter for eksempel formyl, acetyl, isopropyl, butoksykarbonyl, fluorenylmetoksykarbonyl, karbobenzyloksy og lignende.

I tillegg til de beskyttede formene hvor både amino- og karboksygruppene har
hensiktsmessige beskyttelsesgrupper refererer betegnelsen "beskyttet" også til de GLP-1 molekylene hvor aktiviteten til dipeptidyl-peptidase IV er hemmet eller inhibert (se for eksempel, Mentlein, R., et al., Eur. J. Biochem., 214:829-835 (1993)]. I tillegg til GLP-1 (7-36)NH2, er molekyler som er beskyttet fra aktiviteten til DPPIV foretrukket, og Gly<8->GLP-1(7-36)NH2, Val<8->GLP-l(7-37)OH, a-metyl-Ala<8->GLP-l(7-36)NH2, og Gly8-Gln<21->GLP-l(7-37)OH er mer foretrukket.
Derivater av naturlig forekommende GLP-1 molekyler er de peptider som blir oppnådd ved fragmentering av en naturlig-forekommende sekvens, eller blir syntetisert basert på en kunnskap om sekvensen til den naturlig forekommende aminosyresekvensen til det genetiske materialet (DNA eller RNA) som koder for denne sekvensen. Betegnelsen "derivater" innbefatter kjemisk modifikasjon av naturlige eller unaturlige GLP-1 molekyler. Fremgangsmåte for fremstilling av disse derivatene er velkjent for organiske og peptidkjemikere innenfor dette fagområdet (se for eksempel WO 91/11457, supra).
GLP-1 molekylene ifølge foreliggende oppfinnelse innbefatter også analoger av GLP-1 (7-36)NH2 og GLP-1 (7-37) hvor en eller flere aminosyrer som er til stede i den opprinnelige sekvensen blir tilsatt eller deletert, og derivater derav. His og desamino-histidin er foretrukket for R\, dersom det totale isoelektriske punktet til molekylet er i området på fra 6 til 9. Ala, Gly og Val er foretrukket i "X" posisjonen, dersom det totale isoelektriske punktet til molekylet er i området på fra 6 til 9. Likeledes er Glu og Gin foretrukket i "Y" posisjonen dersom det totale isoelektriske punktet til molekylet er i området på fra 6 til 9. Glu og Gin er foretrukket i "Z" posisjonen dersom det totale isoelektriske punktet til molekylet er i området på fra 6 til 9. Til slutt er Gly-OH foretrukket for R2 dersom det totale isoelektriske punktet til molekylet er i området på fra 6 til 9.
Foreliggende oppfinnelse innbefatter videre en saltform av et GLP-1 molekyl. En GLP-1 ifølge denne oppfinnelsen kan ha tilstrekkelig sure, en tilstrekkelig basiske, eller begge funksjonelle gruppene, og reagerer dermed med hvilke som helst av et antall uorganiske baser, og uorganiske og organiske syrer, for å danne et salt. Syrer som vanligvis blir anvendt for å danne syreaddisjonssalter er uorganiske syrer så som saltsyre, hydrobromsyre, hydrojodsyre, svovelsyre, fosforsyre og lignende, og organiske syrer så som p-toluensulfonsyre, metansulfonsyre, oksalsyre, p-bromfenylsulfonsyre, karbonsyre, ravsyre, sitronsyre, benzosyre, eddiksyre og lignende. Eksempler på slike salter innbefatter sulfat, pyrosulfat, bisulfat, sulfitt, bisulfitt, fosfat, monohydrogenfosfat, dihydrogenfosfat, metafosfat, pyrofosfat, klorid, bromid, iod, acetat, propionat, dekanoat, kaprylat, akrylat, format, isobutyrat, kaproat, heptanoat, propiolat, oksalat, malonat, succinat, suberat, sebacat, fumarat, maleat, butyn-l,4-dioat, heksyn-l,6-dioat, benzoat, klorbenzoat, metylbenzoat, dinitrobenzoat, hydroksybenzoat, metoksybenzoat, fthalat, sulfonat, xylensulfonat, fenylacetat, fenylpropionat, fenylbutyrat, citrat, laktat, gamma-hydroksybutyrat, glykolat, tartrat, metansulfonat, propansulfonat, naftalen-1-sulfonat, naftalen-2-sulfonat, mandelat og lignende. Foretrukne syreaddisjonssalter er de som blir dannet med mineralsyrer så som saltsyre og hydrobromsyre, og spesielt saltsyre.
Baseaddisjonssalter innbefatter de som er avledet fra uorganiske baser, så som ammonium eller alkali eller alkaliske jordmetallhydroksider, karbonater, bikarbonater og lignende. Baser nyttige for dannelse av saltene ifølge denne oppfinnelsen innbefatter natriumhydroksid, kaliumhydroksid, ammoniurnhydroksid, kaliumkarbonat og lignende. Saltformene er spesielt foretrukket.
Når forbindelsene ifølge denne oppfinnelsen blir anvendt for farmakoterapeutiske formål kan forbindelsene også være i form av et salt, men saltet må være farmasøytisk akseptabelt.
GLP-1 molekylene ifølge foreliggende oppfinnelse innbefatter dermed, blant annet, de GLP-1 molekylene som funksjonelt demonstrerer insulinotropisk aktivitet. Betegnelsen "insulinotropisk aktivitet" vedrører evnen som en forbindelse har til å stimulere, eller forårsake stimulering av, syntesen eller ekspresjonen av hormonet insulin.
Insulinotropiske egenskaper til en forbindelse kan bli bestemt ved å gi forbindelsen til
dyreceller, eller injisere forbindelsen i dyr og registrere frigjøringen av immunoreaktivt insulin (IRI) til mediet eller det sirkulatoriske systemet til dyret. Tilstedeværelse av IRI blir detektert ved anvendelse av en radioimmunanalyse som kan spesifikt kan detektere insulin.
Til tross for at en hvilke som helst radioimmunanalyse som kan detektere tilstedeværelse av IRI kan bli anvendt er det foretrukket å anvende en modifikasjon av analysemetoden til Albanom J.D.M., et al., Acta Endocrinol., 70:487-509 (1972). I denne modifikasjonen blir en fosfat/albuminbuffer med en pH på 7.4 anvendt. Inkubasjonen tilberedes ved etterfølgende tilsetning av 500 ul fosfatbuffer, 50 ul perfusatprøve eller rotteinsulinstandard i perfusat, 100 (il anti-insulin antiserum (Wellcome Laboratories; 1:40.000 fortynning) og 100 ul t^5f) insulin, gitt et totalt volum på 750 ul i et 10x75 mm engangs glassrør. Etter inkubasjon i 2-3 dager ved 4°C blir fritt insulin separert fra antistoff-bundet insulin ved trekullseparering. Analysesensitiviteten er 1-2 uU/ml. For å måle frigjøring av IRI inn i cellekulturmediet fra celler dyrket i vevskultur inkorporerer man fortrinnsvis radioaktiv markør inn i proinsulin. Til tross for at en hvilken som helst radioaktiv markør med evne for å merke et polypeptid kan bli anvendt, er det foretrukket å anvende ^H leucin for å oppnå merket proinsulin. Merkingen kan bli utført i en hvilke som helst tidsperiode tilstrekkelig for å muliggjøre dannelsen av en detekterbar merket blanding av proinsulinmolekyler, men det er foretrukket å inkubere cellene i nærvær av radioaktiv markør i en periode på 60 minutter.
Til tross for at mange cellelinjer med evne for å uttrykke insulin kan bli anvendt for å bestemme om en forbindelse har en insulinotropisk effekt, er det foretrukket å anvende rotte insulinomceller og spesielt RJN-38 rotteinsulinomceller. Slike celler kan bli dyrket i et hvilke som helst egnet medium, men det er foretrukket å anvende DME medium inneholdende 0,1% BSA og 25 mM glukose.
Den insulinotropiske egenskapen til en forbindelse kan bli bestemt ved bukspyttkjertelinfusjon. In situ isolert perfusert rottebukspyttkjertelpreparering er en modifikasjon av metoden til Penhos, J.C., et al., Diabetes, 18:733.738 (1969). Fastende hann Charles River stamme albinorotter med vekt på 350-600 g blir bedøvet med en intraperetoneal injeksjon av Amytal Sodium (Eli Lilly and Co., 160 ng/kg). Nyre, adrenal, mage og lavere tarmblodårer blir ligert. Hele tarmen blir operert med unntagelse av omtrent 4 cm tolvfingertarm og nedstigende tykktarm og rektum. Derfor blir bare en liten del av tarmen perfusert hvilket minimaliserer mulig innvirkning av enteriske forbindelser med glukagon-lignende immunreaktivitet. Perfusatet er en modifisert Krebs-Ringer bikarbonatbuffer med 4% dekstran T70 og 0.2% bovin serum-albumuin (fraksjon V) og blir boblet med 95% 02 og 5% C02. En ikke-pulserende strøm, 4-kanals rullébærende pumpe (Buchler polystatic, Buchler Instruments Division, Nuclear-Chicago Corp.) blir anvendt og et skifte fra en perfusatkilde til en annen blir oppnådd ved dreining på en 3-veis stoppekran. Måten hvorved perfusjon blir utført på, registrert og analysert følger fremgangsmåten til Weir, G.C., et al., J. Clin. Inestigat. 54:1403-1412 (1974), som er inkorporert heri som referanse.
GLP-1 molekylene ifølge foreliggende oppfinnelse er nødvendig for å inneha en histidin funksjonalitet ved aminoterminal ende. GLP-1 molekylene ifølge foreliggende oppfinnelse kan også ha en modifisert histidinfunksjonalitet i lys av den nødvendige histidinfunksjonaliteten.
Betegnelsen "modifisert histidin" refererer til til en histidinfunksjonalitet som er blitt kjemisk eller biologisk endret eller en endret histidin funksjonalitet som er blitt syntetisert de novo, men som har beholdt de metallbindende egenskapene.
Mange slike modifiserte histidinfunksjonaliteter og prepareringer derav er kjent innenfor fagområdet og innbefatter for eksempel D-histidin (WO 91/11457), desamino-histidin (WO 92/18531), 2-amino-histidin [Levine-Pinto, H., et al., Biochem. GLP-1 molekylene ifølge foreliggende oppfinnelse må videre ha et isoelektrisk punkt i området fra 6.0 til 9.0. Mange GLP-1 molekyler har et isoelektrisk punkt i dette området og er blitt beskrevet og innbefatter for eksempel:

I tillegg har GLP-1 molekylene ifølge foreliggende oppfinnelse, når de innehar hver av de ovenfor refererte modifiserte histidinfunksjonalitetene i lys av histidinfunksjonaliteten, isoelektriske punkt som faller innenfor ovennevnte område. Fremgangmåte for å beregne eller eksperimentelt bestemme det isoelektriske punktet til andre GLP-1 molekyler er kjent for fagfolk innenfor dette området.
Fremgangsmåter for fremstilling av GLP-1 molekylene ifølge foreliggende oppfinnelse er også velkjent for peptidkjemikere.
I en fremgangsmåte blir GLP-1 molekyler fremstilt ved hjelp av velkjente fastfase peptidsyntese skjemaer beskrevet av Merrifield, J.M., Chem. Soc., 85:2149 (1962), og Stewart og Young, Solid Phase Peptide Synthesis, s. 24-66, Freeman (San Francisco, 1969). Det er også mulig å oppnå fragmenter med proglukagonpolypeptid eller GLP-1 (1-37) ved fragmentering av naturlig forekommende aminosyresekvens ved anvendelse av for eksempel proteolytisk enzym. Det er videre mulig å oppnå de ønskede fragmentene av proglukagonpeptidet eller GLP-1 (1-37) ved anvendelse av rekombinant DNA teknologi som beskrevet av Maniatis, T., et al., Molecular Biology: A Laboratory Manual, CSH (Cold Spring Harbor, 1982).
Teknikkens stand innen molekylærbiologi gir fagfolk innenfor dette området andre metoder hvorved forbindelsene ifølge oppfinnelsen kan bli oppnådd. Til tross for at de kan bli produsert ved fastfase peptidsyntese eller rekombinante metoder, kan rekombinante metoder være foretrukket på grunn av at høyere utbytter er mulig.
Grunnleggende trinn i rekombinant produksjon er:
a) isolering av en naturlig DNA sekvens kodende for et GLP-1 molekyl eller konstruering av en syntetisk eller semi-syntetisk DNA kodende sekvens for
et GLP-1 molekyl,
b) plassering av den kodende sekvensen i en ekspresjonsvektor på en måte som er egnet for ekspresjon av proteiner enten alene eller som et fusjonsprotein, c) tranformering av en hensiktsmessig eukaryot eller prokaryot vertscelle med ekspresj onsvektor, d) dyrking av den transformerte vertscellen under betingelser som vil muliggjøre ekspresjon av et GLP-1 molekyl og e) isolering og rensning av det rekombinant produserte GLP-1 molekylet
Som tidligere angitt kan de kodende sekvensene være fullstendig syntetiske eller
resultatet av modifikasjoner til større, nativt glukagon-kodende DNA. En DNA sekvens som koder for preproglukagon er presentert i Lund et al, Proe. Nati. Acad. Sei. U.S.A. 79:345-349 (1982) og kan bli anvendt som utgangsmateriale for semisyntetisk produksjon av forbindelsene ifølge foreliggende oppfinnelse ved å endre den native sekvensen for å oppnå de ønskede resultatene.
Syntetiske gener, hvor in vitro eller in vivo transkripsjon og translasjon resulterer i produksjon av et GLP-1 molekyl, kan bli konstruert ved hjelp av teknikker som er velkjente innenfor dette fagområdet. På grunn av den naturlige degenerasjonen til den genetiske koden vil fagfolk erkjenne at et defuierbart antall DNA sekvenser kan bli konstruert og disse koder for GLP-1 molekylene.
Metoder for syntetisk genkonstruksjon er velkjent innenfor fagområdet. Se Brown et al.,
(1979) Methods i Enzymology, Academic Press, N.Y., Vol. 68, s. 109-151. DNA sekvenser som koder for et GLP-1 molekyl kan bli konstruert basert på aminosyresekvensene beskrevet heri. Når konstruert kan selve sekvensen bli dannet ved anvendelse av konvensjonell DNA syntese apparatur så som modell 380A eller 380B DNA synthesizers (PE-Applied Biosystems, Inc., 850 Lincoln Center Drive, Foster City, CA 94404).
For å oppnå ekspresjon av et GLP-1 molekyl blir den omkonstruerte syntetiske DNA sekvensen skutt inn i hvilke som helst av mange hensiktsmessige, rekombinante DNA ekspresjonsvektorer ved anvendelse av hensiktsmessige restriksjonsendonukleaser. Se generelt Maniatis et al. (1989) Molecular Cloning; A Laboratory Manual, Cold Springs Harbor Laboratory Press, N.Y., Vol. 1-3. Restriksjonsendonuldeasespaltriingssetet blir omkonstruert i hvilke som helst ende av GLP-1 molekyl-kodende DNA for å lette isolering fra, og integrering inn i, kjente amplifikasjons- og ekspresjonsvektorer. Den bestemte endonukleasen som blir anvendt vil bli bestemt av restriksjonsendonuklease-spaltningsmønsteret til den opprinnelige ekspresjonsvektoren som blir anvendt. Valg av restriksjonsseter blir valgt for å orientere den kodende sekvensen riktig med kontrollsekvenser for å oppnå riktig i-rammeavlesning og ekspresjon av protein av interesse. Den kodende sekvensen må bli posisjonert slik at den er i riktig leseramme med promoter og ribosombindingssetet til ekspresjonsvektoren, i det begge er funksjonelle i vertscellen hvor proteinet skal bli uttrykt.
For å oppnå effektiv transkripsjon av det syntetiske genet må det bli operasjonelt forbundet med en promoter-operator region. Promoter-operatorregionen til det syntetiske genet blir derfor plassert i samme sekvensielle orientering med hensyn på ATG startkodonet til det syntetiske genet.
Forskjellige ekspresjonsvektorer nyttige for transformering av prokaryote og eukaryote celler er velkjente innenfor fagområdet. Se The Promega Biological Research Products Catalogue (1992) (Promega Corp., 2800 Woods Hollow Road, Madison, WI, 53711-5399); og The Strategene Cloning Systems Catalogue (1992) (Stratagene Corp., 11011 North Torrey Pines Road, La Jolla, CA, 92037). Også US-PS 4.710.473 beskriver videre sirkulære DNA plasmidtransformasjonsvektorer nyttige for ekspresjon av eksogene gener i høye nivåer i E. coli. Disse plasmidene er nyttige som transformasjonsvektorer i rekombinante DNA prosedyrer og
a) gir plasmidet evnen for autonom replikasjon i en vertscelle; b) kontrollerer autonom plasmid replikasjon i relasjon til temperaturen hvorved vertscellekulturer blir opprettholdt; c) stabilisere opprettholdelsen av plasmidet i vertscellepopulasjoner; d) leder syntesen av et protein prod. indikerende plasmid opprettholdelse i en vertscellepopulasjon; e) tilveiebringer i serier restriksjonsendonuklease-gjenkjenningsseter som er unike for plasmidet og
f) terminerer mRNA transkripsjonen.
Disse sirkulære DNA plasmidene er nyttige som vektorer i rekombinante DNA
prosedyrer for å sikre høye nivåer av ekspresjon av eksogene gener.
Når det er blitt konstruert en ekspresjonsvektor for et GLP-1 molekyl, utgjør det neste trinnet å plassere vektoren inn i en egnet celle og derved konstruere en rekombinant vertscelle nyttig for uttrykking av polypeptidet. Teknikker for transformering av celler med rekombinante DNA vektorer er velkjente innenfor dette området og kan finnes i slike generelle referanser som Maniatis et al. supra. Vertsceller kan bli konstruert fra enten eukaryote eller prokaryote celler.
Prokaryote vertsceller produserer generelt proteinet i høyere rater og er lettere å dyrke. Proteiner som blir uttrykt i bakterielle ekspresjonssystemer med høye nivåer aggregerer karakteristisk i granuler eller inklusjonslegemer som inneholder høye nivåer av overuttrykt protein. Slike proteinaggregater må vanligvis bli oppløst, denaturert og foldet på nytt ved anvendelse av teknikker som er velkjente innenfor fagområdet. Se Kreuger et al. Når det ønskede GLP-1 molekylet er fremstilt, forutsatt at det har et isoelektrisk punkt i området fra 6.0 til 9.0, blir komplekser av foreliggende oppfinnelse fremstilt ved kompleksbinding av et ønsket GLP-1 molekyl med et divalent metallkation via velkjente fremgangsmåter innenfor fagområdet. Slike metallkationer innbefatter for eksempel Zn<++>, Mn<44>", Fe4"1", Co<44>, Cd<44>, Ni<44> og lignende. Av metallkationene er Zn<44> foretrukket.
Generelt blir et ønsket GLP-1 molekyl som har nødvendig isoelektrisk punkt, kombinert med en blanding av en hensiktsmessig buffer og en hensiktsmessig form av et metallkation.
Hensiktsmessige buffere er de som vil opprettholde blandingen ved et pH område fra 6.0 til 9.0, men som ikke vil interferere med reaksjonen. Foretrukne buffere innbefatter Goodes buffer, spesielt HEPES og Tris og Tris acetat.
Hensiktsmessige former for metallkationer er en hvilken som helst form av et divalent metallkation som er tilgjengelig for å danne et kompleks med et GLP-1 molekyl ifølge foreliggende oppfinnelse. Et divalent metallkationisk salt så som sinkklorid er tilveiebragt i overskudd for å tilveiebringe et molart forhold på opp til omtrent 50 molekyler av et divalent metallkation for hvert molekyl av GLP-1 substrat. Temperaturen anvendt i dette trinnet er det som er tilstrekkelig for å tilveiebringe endt reaksjon. Vanligvis blir reaksjonen utført ved romtemperatur. Produktet ifølge foreliggende oppfinnelse, en krystallinsk eller amorf suspensjon, blir isolert og renset ved anvendelse av standardteknikker.
Foreliggende oppfinnelse tilveiebringer også farmasøytiske sammensetninger som omfatter en forbindelse ifølge foreliggende oppfinnelse i kombinasjon med en farmasøytisk akseptabel bærer, fortynningsmiddel eller eksipient. Slike farmasøytiske sammensetninger blir fremstilt på en måte som er velkjent innenfor det farmasøytiske området, og blir administrert individuelt eller i kombinasjon med andre terapeutiske midler, fortrinnsvis via parenterale veier. Spesielt foretrukne veier innbefatter intramuskulært og subkutan administrering.
Parenterale daglige doseringer, fortrinnsvis en enkel, daglig dose, er i området fra omtrent 1 pg/kg til omtrent 1.000 ug/kg kroppsvekt, til tross for at lavere eller høyere doseringer kan bli administrert. Den nødvendige doseringen vil avhenge av alvorlighetsgraden til pasientens tilstand og av slike kriterier som pasientens høyde, vekt, kjønn, alder og medisinsk bakgrunn.
Ved fremstilling av sammensetningene ifølge foreliggende oppfinnelse blir det aktive ingredienset, som omfatter minst en forbindelse ifølge foreliggende oppfinnelse, vanligvis blandet med en eksipiens eller fortynnet med en eksipiens. Når en eksipiens blir anvendt som et fortynningsmiddel, kan det være et fast, halvfast eller flytende materiale som virker som en bærer, eller et medium for den aktive ingrediensen.
Ved fremstilling av en formulering, kan det være nødvendig å male den aktive forbindelsen for å tilveiebringe hensiktsmessig partikkelstørrelse før kombinering med andre ingredienser. Dersom den aktive forbindelsen er vesentlig uoppløselig bør det vanligvis bli malt til partikkelstørrelse som er mindre enn omtrent 200 mesh. Dersom den aktive forbindelsen er vesentlig vannoppløselig blir partikkelstørrelsen normalt justert ved maling for å tilveiebringe en vesentlig jevn fordeling av formuleringen, for eksempel omtrent 40 mesh.
Noen eksempler på egnede eksipienter innbefatter laktose, dekstrose, sukrose, trehalose, sorbitol og mannitol. Sammensetningene ifølge oppfinnelsen kan bli formulert for å tilveiebringe hurtig, vedvarende eller forsinket frigjøring av den aktive ingrediensen etter administrering til pasienten ved å anvende prosedyrer som er velkjente innenfor
dette fagområdet.
Sammensetningene blir fortrinnsvis formulert i en enhetsdoseringsform i det hver dosering normalt inneholder fra omtrent 50 ug til omtrent 100 mg, mer vanlig er fra omtrent 1 mg til omtrent 10 mg av den aktive ingrediensen. Betegnelsen "enhetsdoseringsform" refererer til fysiske adskilte enheter egnede som enhetsdoseringer for humane individer og andre pattedyr, hvor hver enhet inneholder en forutbestemt mengde av det aktive materialet beregnet å produsere den ønskede terapeutiske effekten i assosiasjon med en egnet farmasøytisk eksipiens.
For parenteral administrering blir sammensetninger inneholdende en forbindelse ifølge foreliggende oppfinnelse fortrinnsvis kombinert med destillert vann og pH blir justert til i området fra 6.0 til 9.0.
Ytterligere farmasøytiske metoder kan bli anvendt for å kontrollere virkningsvarigheten. Preparater med kontrollert frigjøring kan bli oppnådd ved anvendelse av polymerer for å kompleksbinde eller absorbere en forbindelse ifølge foreliggende oppfinnelse. Den kontrollerte leveringen kan bli utført å velge hensiktsmessige makromolekyler (for eksempel polyestere, polyaminosyrer, polyvinylpyrrolidon, etylenvinylacetat, metylcellulose, karboksymetylcellulose og protaminsulfat) og konsentrasjonen av makromolekylene, samt fremgangsmåter for inkorporering for å kontrollere fri-gjøringen.
En annen mulig fremgangsmåte for å kontrollere virkningsvarigheten til preparater med kontrollert frigjøring er å inkorporere en forbindelse ifølge foreliggende oppfinnelse i partikler til et polymerisk materiale så som polyestere, polyaminosyrer, hydrogeler, poly(melkesyre) eller etylenvinylacetatkopolymerer.
Alternativt, i stedet for inkorporering av en forbindelse til disse polymere partiklene, er det mulig å inneslutte en forbindelse ifølge foreliggende oppfinnelse i dannede mikrokapsler, for eksempel ved koacervasjonsteknikker eller ved interfacialpolymerisering, for eksempel, hydroksymetylcellulose eller gelatin-mikrokapsler, eller i colloidale medikamentleveirngssystemer, for eksempel, liposomer, albuminmikrosfærer, mikroemulsjoner, nanopartikler og nanokapsler, eller i mikroemulsjoner. Slike teknikker er beskrevet i Remingtons Pharmaceutical Sciences
(1980).
Forbindelsene ifølge foreliggende oppfinnelse har insulinotropisk aktivitet. Et annet aspekt av foreliggende oppfinnelse tilveibringer videre en fremgangsmåte for å fremme ekspresjonen av insulin omfattende tilveiebringing til en pattedyrbukspyttkjertel B-type øycelle en effektiv mengde av en forbindelse ifølge foreliggende oppfinnelse.
Forbindelsene ifølge oppfinnelsen kan benyttes til å behandle diabetes mellitus hos voksne pattedyr, fortrinnsvis menneske, ved administrering av en effektiv mengde av en forbindelse eller sammensetning ifølge foreliggende oppfinnelse.
Eksempel 1
Individuelle alikvoter av fem forskjellige GLP-1 molekyler ble fremstilt ved hjelp av velkjent, fastfase peptidsyntese og ble lyofilisert i små beholdere. Porsjoner av 0.1M HEPES (N-[2-hydroksyetyl]piperazin-N<*->[2-etansulfonsyre]) buffere ved pH 7.4 inneholdende forskjellige nivåer av sinkklorid ble tilsatt til alikvotene for å oppnå en proteinkonsentrasjon på ca. 0,1 mg/ml. Prøvene ble blandet og lagret ved romtemperatur (22°C) i omtrent 18 timer. Blandingene ble deretter sentrifugert (Fisher Model 235C mikro-sentrifuge) i 5 minutter. De klare supernatantene ble pipettert fra rørene. Proteininnholdet til supernatantene ble vurdert ved måling av absorbansen ved 280 nm i et spektrofotometer (Gilford 260). Den teoretiske absorbanseverdien til en 0,1 mg/ml oppløsning av GLP-1 molekylene med denne bølgelengden i 1 cm kuverter er 0,207. Resultatene av dette eksperimentet er vist i tabell 1.
Dette eksempelet viser at bare en liten mengde sink er nødvendig for å kompleksbinde med og presipitere en signifikant del av GLP-1 molekylene fra disse fortynnede opp-løsningene.
Eksempel 2
5 mg GLP-1 (7-36)NH2 ble fullstendig løst opp i 2.5 ml pH 7.4, sink-fri 0.1M HEPES buffer. Ytterligere 2.5 ml pH 7.4, 0.1M HEPES buffer inneholdende 0.6 mM sinkklorid ble hurtig tilsatt. Hensiktsmessig molart forhold av sink og GLP-1 (7-36)NH2 i denne prøven er 1 til 1. Oppløsningen ble øyeblikkelig sløret og presipiteringen ble raskt dannet. Blandingen ble lagret ved romtemperatur (22°C) i 18 timer.
Presipitatet ble fast forankret i bunnen av glassbeholderen. Supernatanten ble fullstendig dekantert med pipette. Presipitatet ble deretter fullstendig oppløst i 5.0 ml 0.01N saltsyre. Absorbansen ved 280 nm ble bestemt for både supernatant og på ny oppløste presipitatoppløsninger. Sinknivåene i disse oppløsningene ble kvantifisert ved atomisk absorpsjonsspektrofotometri. Resultatene av dette eksperimentet er vist i tabell 2.
Dette eksemplet viser at det meste av GLP-1 (7-36)NH2 presipiterte fra oppløsningen når den sink-inneholdende HEPES oppløsningen ble tilsatt. 280 nm absorbanseverdi på 1.932 indikerer at GLP-1 (7-36)NH2 konsentrasjonen av det på ny oppløste presipitatet er 0.933 mg/ml eller 283 nm. Sinkkonsentrasjonen til den samme oppløsningen, 13.3 ppm, er ekvivalent med en sinkkonsentrasjon på 203 um. Det molare forholdet mellom sink og GLP-1 (7-36)NH2 i presipitatet var 0.717 til 1.

Polypeptid, karakterisert ved at det har følgende formel: Rj-X-Glu-Gly-Thr-Phe-Thr-Ser-Asp-Val-Ser-Ser-Tyr-Leu-Y-Gly-Gln-Ala-Ala- Lys-Z-Phe-Ile-Ala-Trp-Leu-Val-Lys-Gly-Arg-R2
hvor: Ri er valgt fra gruppen bestående av L-histidin, D-histin, desamino-histidin, 2-
amino-histid, B-hydroksy-histidin, homohistidin, alfa-fluormetyl-histidin og alfa-metylhistidin; X er valgt fra guppen bestående av Val, Ile og alfa-metyl-Ala; Y er valgt fra gruppen bestående av Glu, Gin, Ala, Thr, Ser og Gly, Z er valgt fra gruppen bestående av Glu, Gin, Ala, Thr, Ser og Gly; R2er valgt fra gruppen bestående av NH2 og Gly-OH; eller et farmasøytisk akseptabelt salt derav;
hvor nevnte polypeptid har et isoelektrisk punkt i området fra 6,0 til 9,0.
