Description
Foreliggende oppfinnelse vedrører stabile vandige sammensetninger inneholdende a-interferon hybrid.
Interferoner er familier av induserbare sektretoriske proteiner produsert som respons på viral og andre stimuli, a-interferoner blir for tiden anvendt klinisk for behandling av et antall forskjellige sykdomstilstander som omfatter: leukemisk retikuloendoteliose, Karposis sarkom i AIDS, kronisk ikke-A, ikke-B hepatitt og basal cellekarcinom.
Rekombinante humane a-interferon B/D hybriddeler er blitt konstruert, som bærer forskjellige deler av sekvensen til de opprinnelige a-interferon B- og D-artene for å danne molekyler med fordelaktige egenskaper (A. Meister, G. Uz, K.E. Mogensen, J. Gen. Virol. 67, 1633, (1986)). Opphavsmolekylene blir spaltet for å utveksle DNA-segmenter som koder for peptidsekvensene 1-60,61-92,93-150 og 151-166. BDBB-hybridet er blitt funnet å utvise spesielt interessant preklinisk anti-viral og anti-proliferativ aktivitet (H.K. Hochkeppel, M. Gruetter, M.A. Horisberger, J.K. Lazdins, Drugs of the future, 17 (10), 899, (1992)), Proteiner, så som a-interferon BDBB, har omfattende kjemiske og fysiske egenskaper som forårsaker vanskeligheter ved rensing, separasjon, lagring og levering av disse formene. Formulering av proteinmedisinske midler er meget forskjellig fra de fra rigide små organiske molekyler. Flere klasser av eksipienter er blitt anvendt i parenterale formuleringer av proteiner og peptider. På grunn av at proteiner har dårlig oral biotilgjengelighet, er den mest vanlige metoden å presentere disse som initierbare produkter for å oppnå effektiv medikamentavlevering. På grunn av ustabilitet i oppløsning blir proteinene generelt formulert som lyofiliserte (frysetørkede) pulvere for øyeblikkelig rekonstituering før subkutan, intramuskulær eller intravenøs injeksjon.
Proteindegradering kan deles inn i to kategorier, dvs. kjemisk stabilitet og fysisk stabilitet. Kjemisk stabilitet refererer til alle prosesser hvor proteinet er kjemisk modifisert via bindingsspaltning eller dannelse. Fysisk stabilitet involverer ikke kovalent modifisering, men forandringer i høyere ordensstrukturer til proteinet, for eksempel via denaturering, aggregasjon eller presipitering. Det er ønskelig å opprettholde hybrid a-interferon BDBB i en disaggregert tilstand for å eliminere mulige negative, immunogene virkninger og/eller inkonsistent dosering i løpet av terapi.
Ved en ønskelig konsentrasjon for anvendelse som et initierbart terapeutisk middel, er hybrid a-interferon BDBB fysisk ustabil i vann. Oppløseligheten blir kompromitert og det eksisterer i en aggregert tilstand. Stabiliserende eksipienter må derfor anvendes. Det er oppdaget at hybrid a-interferon eksisterer i en aggregert tilstand når formulert i oppløsning ved nøytral pH (fosfatbufret). Oppløseligheten til proteinet ser ut til å bli kompromittert under slike betingelser, og meget lite blir oppløst.
Det er nå oppdaget at formulering av hybrid a-interferon i vandig oppløsning ved lav pH stabiliserer proteinet, og opprettholder det i ønsket monomer tilstand og muliggjør at ytterligere blir oppløst i oppløsningen.
WO 89 04177 vedrører en flytende formulering av y-interferon og dets varianter. Her blir det ikke angitt noen form for flytende formulering av hybrid a-intereferon.
JP 59 196 823 angir fremstilling som omfatter interferon for ekstern anvendelse. Denne referansen spesifiserer ikke en stabil vandig løsning av hybrid a-interferon som er egnet for injeksjon.
Foreliggende oppfinnelse tilveiebringer derfor en stabil vandig oppløsning av hybrid a-interferon som inneholder som stabiliserende middel en buffer ved en pH på fra 3,0 til 5,0 for anvendelse i subkutan, intramuskulær eller intravenøs injeksjon.
Oppløsningsens pH er fortrinnsvis fra 4,0 til 4,5.
Hybrid a-interferon er fortrinnsvis a-interferon BDBB (SEQ ID NO. 1). Buffersaltet som velges bør være farmasøytisk akseptabelt, for eksempel glycin/HCl eller natriumsitrat og utgjør fortrinnsvis en konsentrasjon på mellom 20-400 mM. Foretrukket buf-fersalt er glycin/HCl i en konsentrasjon på 50-150 mM.
En farmasøytisk akseptabel polyol, for eksempel mannitol, glukose eller sukkrose kan også anvendes, fortrinnsvis ved en konsentrasjon på 20-500 mM. Foretrukket polyol er mannitol i en konsentrasjon på 100-250 mM. Polyol blir hovedsakelig anvendt for å ju-stere tonisiteten av oppløsningene til fysiologiske verdier.
Hybrid a-interferon er fortrinnsvis til stede i en konsentrasjon på 0,1-1,5 mg/ml. Det mest foretrukne området er 0,2-0,4 mg/ml.
Hybrid a-interferon blir normalt isolert og renset ved hjelp av en flertrinnsprosedyre og lagret som en frossen bulkoppløsning i et buffersystem ved pH 7, som ikke er farma-søytisk akseptabelt. For å danne stabile oppløsninger ifølge oppfinnelsen er det nødven-dig å bytte dette buffersystemet med ett som er farmasøytisk akseptabelt ved ønsket pH. Dette kan utføres ved å tilsette den ønskede bufferen, utsette oppløsningen for ultrafil-trering, tilsetting av buffer og gjentagelse av syklusen flere ganger. Andre fremgangs-måter kan anvendes, så som gelfiltreringskromatografi.
Formuleringene ifølge oppfinnelsen er egnede for anvendelse som farmasøytiske midler for indikasjonene nevnt ovenfor ved subkutan, intramuskulær eller intravenøs injeksjon.
Oppfinnelsen vil bli illustrert i følgende eksempler.
Eksempel 1
Fremstilling av stabil 0,3 mg/ml hybrid a-interferon BDBB sitratoppløsningsformule-ring ved pH 4,0: Stambulkoppløsningen av hybrid a-interferon BDBB (2,6 mg/ml i 200 mM ammo-niumklorid/20 mM tris(hydroksymetyl)metylamin ved pH 7,0) blir gjort til 50 ml med 40 mM natriumsitrat/sitronsyrebuffer ved pH 4,0 i en ultrafiltreringscelle (Amicon) utstyrt med en lOKDa Mwt avspaltningsmembran (Amicon YM 10). Cellen blir trykkbe-lastet til 50-60 psi ved anvendelse av oksygenfritt nitrogen. Volumet blir redusert, men ikke i en slik grad at proteinet vil presipitere, ved kontinuerlig omrøring for å minimali-sere proteinkonsentrasjonen ved overflaten av membranen, og fortynnet tilbake til 50 ml med 40 mM natriumsitrat/sitronsyrebuffer ved pH 4,0. Denne syklusen gjentas 5 ganger. pH til sluttoppløsningen blir verifisert som 4,0. Proteininnholdet til sluttoppløsnin-gen blir verifisert som 0,3 mg/ml ved anvendelse av standardprosedyrer (Bradford assay, Bio-Rad).
Formuleringen beskrevet ovenfor blir sterilfiltrert og fylt aseptisk inn i 2 ml glassbehol-dere av type 1 med farmasøytisk kvalitet og forseglet med teflonbelagte rør septa/alumi-nium krympelokk. Beholderne blir lagret ved 4°C i mørke. Resultatene blir uttrykt som gjennomsnittet av replikatet (generelt 3-5) injeksjoner. Variasjonskoeffisienten (cv) for disse bestemmelsene er < 0,3%.
Degraderingen ser ut til å følge pseudo-første ordens kinetikk. Ved ekstensjon av best-fit rette linje gjennom dataene, kan en vurdering av oppbevaringstid ("shelf-life"), defi-nert som tiden det tar å degradere til 95% (t95) av det opprinnelige innholdet av hybrid a-interferon BDBB, utføres. Denne formuleringen har en antatt oppbevaringstid på minst 8 måneder ved 4°C.
Størrelseseksklusjonskromatografi (SEC) viser ingen detekterbar dannelse av aggregater etter lagring i 136 dager ved 4°C.
Eksempel 2
Eksempel 1 ble gjentatt med unntagelse av at oppløsningen også inneholder 200 mM mannitol. Stabilitetstesting ble utført som i eksempel 1, men med lagring ved forskjellige temperaturer, som vist nedenfor, for akselererte forsøk.
Degraderingen ser ut til å følge pseudo-første ordens kinetikk. Ved anvendelse av fremgangsmåten beskrevet i eksempel 1 har formuleringen en forutbestemt oppbevaringstid på over 6 uker ved 25°C.
Eksempel 3
Fremstilling av stabil 0,3 mg/ml hybrid a-interferon BDBB glycinoppløsningsformule-ring ved pH 4,0: ved anvendelse av en analog prosedyre som beskrevet i eksempel 1 blir stambulkopp-løsning av hybrid a-interferon BDBB (2,6 mg/ml i 200 mM ammoiiiumklorid/20mM tris(hydroksymetyl)metylamin ved pH 7,0) omdannet til en 0,3 mg/ml oppløsning av protein i 80 mM glycin/HCl buffer ved pH 4,0.
Stabilitetstesting av stabil hybrid a-interferon BDBB glysin oppløsningsformulering ved pH 4,0 blir utført ifølge prosedyrene beskrevet i eksempel 1. Resultatene er:

Eksempel 4
Eksempel 3 ble gjentatt med unntagelse av at oppløsningen også inneholder 200 mm mannitol. Stabilitetstesting ble utført som i eksempel 3, men med lagring ved forskjellige temperaturer som vist nedenfor.
Degraderingen ser ut til å følge pseudo-første ordens kinetikk. Ved anvendelse av fremgangsmåten beskrevet i eksempel 1 har formuleringen en forutbestemt oppbevaringstid på minst 5 år ved 4°C.
SEC viser ingen signifikant dannelse av aggregerte arter etter lagring ved 4°C i 20 uker eller etter lagring ved 40°C i 4 uker.
Eksempel 5
Eksempel 3 ble gjentatt med unntagelse av at konsentrasjonen av glycin/HCl buffer er 318 mM. Stabilitetstesting ble utført som i eksempel 3, men med lagring ved forskjellige temperaturer som vist nedenfor.
Degraderingen ser ut til å følge pseudo-første ordens kinetikk. Ved anvendelse av fremgangsmåten beskrevet i eksempel 1 har formuleringen en forutbestemt oppbevaringstid på minst 6 måneder ved 4°C.
Claims (12)
1.
Stabil vandig oppløsning av hybrid a-interferon, karakterisert ved at det inneholder som stabilisator en buffer ved en pH på fra 3,0 til 5,0 for anvendelse i subkutan, intramuskulær eller intravenøs injeksjon.
2.
Stabil oppløsning ifølge krav 1, karakterisert ved at den har en pH på fra 4,0 til 4,5.
3.
Stabil oppløsning ifølge krav 1 eller 2, karakterisert v e d at bufferen er glycin/HCl eller natriumsitrat.
4.
Stabil oppløsning ifølge et hvilket som helst foregående krav, karakterisert ved at bufferen er til stede ved en konsentrasjon på 20-400 mM.
5.
Stabil oppløsning ifølge et hvilket som helst foregående krav, karakterisert ved at bufferen er lysin/HCl i en konsentrasjon på 50-150 mM.
6.
Stabil oppløsning ifølge et hvilket som helst foregående krav, karakterisert ved at hybrid a-interferon er a-interferon BDBB (SEQ ID NO: 1).
7.
Stabil oppløsning ifølge et hvilket som helst foregående krav, karakterisert ved at hybrid a-interferon er ved en konsentrasjon på 0,1 til 1,5 mg/ml.

Stabil oppløsning ifølge krav 7, karakterisert ved at hybrid a-interferon er en konsentrasjon på 0,2 til 0,4 mg/ml.

Stabil oppløsning ifølge et hvilket som helst foregående krav, karakterisert ved at den også inneholder en farmasøytisk akseptabel polyol.
10.
Stabil oppløsning ifølge krav 9, karakterisert ved at polyolen er mannitol, glukose eller sukkrose.
11.
Stabil oppløsning ifølge krav 9 eller 10, karakterisert ved at polyolen er til stede i en konsentrasjon på 20 til 500 mM.
12.
Stabil oppløsning ifølge krav 9, karakterisert ved at polyolen er mannitol i en konsentrasjon på 100 til 250 mM.
