Description
Insulin og insulinderivater blir som kjent benyttet i betydelige mengder til behandling av sykdommen Diabetes mellitus og blir av den grunn også produsert i stor skala. Til tross for det betydelige antall av allerede eksisterende insulin-tilberedelser og -anvendelser med forskjellige virkningprofiler er det på grunn av forskjellighetene i organismen med inter- og intra-individuelle fluktuasjoner ennå et behov for insulinprodukter med andre egenskaper og virkningskarakteristikker.
Insulinderivater med en forsinket virkning er for eksempel beskrevet i EP-B-132 769 og EP-B-132 770. Det handler spesielt om posisjon B31 i insulin-B-kjeden basisk modifisert derivat med følgende formel I:
der R<1> betyr H eller H-Phe,
R<30> står for en rest av en nøytral, genetisk kodbar L-aminosyre og
R<31> står for en fysiologisk ufarlig organisk gruppe av basisk karakter med inntil 50 C-atomer, i deres oppbygning tar 0 til 3 a-aminosyrer del og deres eventuelt foreliggende endestående frie karboksylfunksjon kan foreligge som esterfunksjon, som amidfunksjon, som lakton eller redusert til CH2OH.
For disse insulinderivatene er et isoelektrisk punkt mellom 5,8 og 8,5 (målt i isoelektrisk fokusering) karakteristisk. Det isoelektriske punkt har - i motsetning til isoelektriske punkter til umodifiserte native insuliner eller proinsuliner (ved pH = 5,4) forskjøvet i det nøytrale området - i tillegg, på overflaten molekyler der det befinner seg positiv(e) ladning(er) som følge av den basiske modifikasjonen. Dermed er dette basiske modifiserte insulinderivat mindre løselig i nøytralområdet i forhold til nativt insulin eller proinsulin, som på normal måte foreligger oppløst i nøytralområdet.
Forsinkelses- eller depot-virkningen av basiske modifiserte insulinderivater med formel I går tilbake på deres tung-oppløselighet ved isoelektrisk punkt. Videreoppløsningen av insulinderivater under fysiologiske betingelser blir oppnådd etter de begge forannevnte siterte skriftene ved avspalting av tilleggs-basiske grupper, som kommer i stand etter derivat ved tryptisk eller trypsin-1ignende og/eller karboksypeptidase B eller karboksypeptidase B-lignende og/eller esterase-aktivitet. De aktuelle avspaltende gruppene er enten rene fysiologiske metabolitter eller lette metaboliserbare, fysiologisk godtagbare substanser.
Det forannevnte depot-prinsipp ifølge basisk modifikasjon av insulin ble ennå videre benyttet ved fremstilling og tilsvarende anvendelse - hovedsakelig innenfor A- og B-kjeden-av basisk modifiserte insulinderivater; sml. for eksempel EP-Å-0194 864 og EP-A-0254 516.
I insulinderivatene ifølge EP-A-0194 864 er det i B27-posisjon innbygget en basisk aminosyre og/eller en nøytral aminosyre sitter i posisjon A4, A17, B13 og/eller B21; foruten er den C-terminale karboksylgruppen i B-kjeden blokkert gjennom en amid- eller esterrest.
Insulinderivatet ifølge EP-A-0254 516 er et som ligner meget på den foran nevnte EP-A; for å oppnå stabilitetsforhøyelse ved den svakt sure pH-verdien til tilsvarende farmasøytiske tilberedelse kan imidlertid aminosyren Asn i posisjon A21 bli erstattet med andre, i surere medium stabilere aminosyrer som f.eks. Asp. Asn (= asparagin) adskiller seg fra Asp (= asparaginsyre) betydelig ved blokkering av en av karboksylgruppene med amidgruppen:
Samtlige insulinderivater ifølge de tre siste nevnte artiklene er hovedsakelig modifisert innenfor A- og B-kjedene; deres fremstilling foregår på genteknologiske måter.
I forsøket på å forhøye stabiliteten til den C-terminale ende av B-kjede basisk modifiserte insulinderivatet ifølge de innledningsnevnte EP-patentskriftene EP-B-0132 769 og EP-B-0132 770 i surt medium og eventuelt også å forandre deres virkningsprofil, er det funnet at dette mål på fordelaktig måte kan bli nådd ved å erstatte Asn-^<21> med andre genetisk kodbare aminosyrer, som ikke inneholder noen amidgrupper, og eventuelt ved substitusjon av His<B1>° med andre genetisk kodbare aminosyrer.
Gjenstand for oppfinnelsen er dermed en fremgangsmåte for fremstilling av insulinderivater med formel I
der
R<1> er H eller H-Phe,
R<2> er Ala, Gly, Val, Leu, Ile, Pro, Phe, Met eller Trp,
R<30> er Thr,
R<31> er 1 til 3 aminosyrer fra gruppen His, Arg, Phe eller Gly,
X betyr His, Ile eller Asn, samt deres fysiologisk godtagbare salter,
kjennetegnet ved at man dyrker en egnet vertscelle transformert med en ekspresjonsvektor som er istand til å dirigere ekspresjon av et gen kodende for insulinderivat med formel (I) eller et fusjonsprotein, hvorpå insulinderivatet eller fusjonsproteinet isoleres, eventuelt hvorpå insulin frigjøres fra fusjonsproteinet.
De nye insulinderivatene og deres fysiologisk godtagbare salter er ved de svakt sure pH-verdiene som tilsvarer farmasøytiske tilberedelser også stabile over lengre tidsrom. De besitter - særlig når også His<B1>° er byttet ut med andre aminosyrer - i motsetning til kjente - uforandrede - basisk modifiserte insulinderivater med den i begynnelsen angitte formel I endret (forkortet) virkningsprofil.
Fremstilling av insulin-derivater med formel I foregår hovedsakelig genteknologisk ved hjelp av sete-rettet muta-genese etter standardmetoder.
Til det blir en for det ønskede insulin-derivat med formel I kodende genstruktur konstruert og uttrykt i en vertscelle-fortrinnsvis i en bakterie som E. coli eller en gjær, særlig Saccharomyces cerevisiae i det tilfelle at genstrukturen koder for et fusjonsprotein - frisetter fra fusjonsproteinet det insulin-derivatet med formel II; analoge metoder er f.eks. beskrevet i EP-A-0 211 299, EP-A-0 227 938, EP-A-0 220 998, EP-A-0 286 956 og DE-patentsøknad P 38 21 159.9 fra 23.6.1988 (HOE 188/F 158).
Avspalting av fusjonsprotein-andelen foregår etter celle-oppslutning enten kjemisk ved hjelp av halogencyan - sml. EP—A—0 180 920 - eller enzymatisk ved hjelp av lysostafin-sml. DE-A-37 39 347.
Insulinforløperen blir deretter underkastet oksidativ sufitolyse etter f.eks. E.C. Marshall og A.S. Inglis i "Practical Protein Chemistry - A Handbook" (Herausgeber A. Darbre) 1986, s. 49-53 beskrevne metode og deretter re-naturert i nærvær av en tiol under dannelse av de korrekte disulfid-broene, f.eks. etter G.H. Dixon og A.C. Wardlow i Nature (1960), s. 721-724 beskrevne metode. C-peptidet blir fjernet ved hjelp av tryptisk spalting-f.eks. ifølge metoden Kemmler et al., J.B.C. (1971), s. 6786-6791 og insulinderivatet med formel II ved hjelp av kjente teknikker som kromatografi - sml. f.eks. EP-A-0 305 760 - og krystalli sasjons-rensing.
Insulin-derivater ifølge oppfinnelsen med formel I og/eller deres fysiologisk godtagbare salter (som f.eks. alkali- eller ammoniumsalter) blir hovedsakelig anvendt som virkestoffer til en farmasøytisk tilberedning for behandling av Diabetes mellitus.
Den farmasøytiske tilberedningen er fortrinnsvis en løsning eller suspensjon til injeksjonsformål; som er kjennetegnet ved et innhold på minst et insulin-derivat med formel I og/eller minst et av deres fysiologisk godtagbare salter i oppløst, amorf og/eller krystallinsk - fortrinnsvis i oppløst - form.
Tilberedelsen oppviser fortrinnsvis en pE-verdi mellom ca. 2,5 og 8,5 særlig mellom ca. 4,0 og 8,5,
inneholder et egnet isotoneringsmiddel,
et egnet konserveringsmiddel,
og eventuelt en egnet buffer,
såvel som fortrinnsvis også en bestemt zinkione-konsentrasjon,
alt naturligvis i steril vandig oppløsning.
Egnede isotoneringsmidler er f.eks. glycerin, glukose, mannit, NaCl, kalsium- eller magnesium-forbindelser som CaCl2, MgCl2 etc.
Ved valg av isotoneringsmiddel og/eller konserveringsstoffer påvirker man løseligheten til insulin-derivatene h.h.v. deres fysiologisk godtagbare salter ved de svake sure pH-verdiene.
Egnede konserveringsmidler er f.eks. fenol, m-kresol, benzylalkohol og/eller p-hydroksybenzosyreester.
Som buffersubstanser, særlig til innstilling av en pH-verdi mellom ca. 4,0 og 8,5 kan f.eks. natriumacetat, natrium-citrat, natriumfosfat etc. bli anvendt. Ellers er til innstilling av pH-verdiene også egnet fysiologisk godtagbare fortynnede syrer (typisk HC1) h.hv. lut (typisk NaOH).
Når man utfører tilberedelsen av et zinkinnhold, er et slikt fra 1 pg til 2 mg, særlig fra 5 ug til 200 pg zink/ml foretrukket.
Hensiktsmessige variasjoner av virkningsprofilene for tilberedningene kan også tilblandes umodifisert insulin, fortrinnsvis fe-, svin- eller human-insulin, særlig human-insulin.
Foretrukne virkningsstoff-konsentrasjoner er slike som tilsvarer ca. 1-1.500, videre foretrukket ca. 5-1.000 og særlig ca. 40-400 internasjonale enheter/ml.
Oppfinnelsen blir nå nærmere forklart med de følgende eksemplene.
A) Genteknologisk fremstilling
Eksempel 1
Konstruksjon av et plasmid til fremstilling av Gly (A21)-humaninsulin Arg (B31-0H)
I DE-patentsøknad P 38 21 159.9 (HOE 88/F 158) ble plasmidet pSW3 beskrevet.
Plasmid-DNA blir omsatt med restriksjonsenzymet Pvu2 og Sali og deretter behandlet med alkalisk fe-fosfatase. De begge oppstående fragmentene blir adskilt gelelektroforetisk og det store fragmentet isolert. Dette fragmentet blir knyttet sammen i en T4-DNA-ligasereaksjon med følgende syntetiske DNA-sekvens:
Kompetente E. coli W3110-celler ble transformert med liga-sjons-tilsetningen. Transformasjonsblandingen blir utplettert på NA-plater som inneholder 20 jjg Ap (= ampicillin)/ml og inkubert over natten ved 37"C. Av enkelt-kolonier blir en overnatts-kultur utvunnet og fra denne blir plasmid-DNA utvunnet. Denne DNA blir karakterisert ved hjelp av restrik-sjonsanalyse og DNA-sekvensanalyse. Riktige plasmider, som koder for den forandrede A-kjeden, får betegnelsen pIKlOO. Ekspresjon foregår analogt til eks. 3 i den forannevnte DE-patentsøknad P 38 21 159.9. Fremstilling av modifiserte mono-Arg-insuliner foregår likeså analogt til denne DE-patent-søknad beskrevne fremstilling av umodifiserte mono-Arg-insuliner.
Eksempel 2
Konstruksjon av et plasmid til fremstilling av Ser(A21)-humaninsulin Arg (B31-0H)
Konstruksjonen tilsvarer den beskrevne vei i eksempelet over. Den syntetiske DNA-sekvens er derimot endret som følger:
Man får plasmidet pIKHO som er karakterisert ved en tilleggs BspHl-gj enkj enningssekvens.
Eksempel 3
Konstruksjon av et plasmid til fremstilling av Gly(A21)-Asn(B10 )-humaninsulin-Arg(B31-0H)
DNA fra plasmid pIKlOO blir spaltet med restriksjonsenzymene Hpal og Dra3 og behandlet med alkalisk fe-fosfatase. Begge fragmentene blir gelelektroforetisk adskilt og det største av fragmentene isolert. Fragmentet blir ligert med den syntetiske DNA-sekvensen og transformert med kompetente E. coli W3110-celler med ligasjonsblanding. Den videre karakterisering av plasmid pIKlOl foregår som beskrevet i eksempel 1.
Eksempel 4
Konstruksjon av et plasmid til fremstilling av Ser(A21)-Asn-(B10)-humaninsulin
Konstruksjonen tilsvarer den kloning som er beskrevet i eksempel 3, imidlertid utgår plasmidet pIKHO fra DNA.
Det nye konstruerte plasmid får betegnelsen pIKlll.
Eksempel 5
Konstruksjon av et ekspresjonsplasmid for ape-proinsulin
Ape-proinsulin adskiller seg fra human proinsulin bare ved utbytting av en enkel aminosyre i C-peptidet (B37-Pro istedenfor Leu i denne posisjon til human proinsulin).
Plasmidet pSW3 blir åpnet med Hpal og Sali og restplasmid-DNA blir isolert. Fra det i EP-A-0 229 998 beskrevne plasmid pK50 blir Dra3-Sall-ape-proinsulinfragmentet isolert. Begge fragmentene blir knyttet sammen med det syntetiske DNA-fragment
i en T4-DNA-ligasereaksjon. Man får plasmidet pSW2, med hvilken DNA i det følgende tjener som utgangsmateriale for konstruksjon av Di-Arg-humaninsulinderivat kodende eks-presj onsplasmid.
Eksempel 6
Konstruksjon av et plasmid til fremstilling av Gly(A21)-humaninsulin Arg(B31)-Arg(B32)-0H
DNA fra plasmid pSW2 blir spaltet på tilsvarende måte som i eksempel 1 med Pvu2 og Sali og ligert med den syntetiske DNA fra eksempel 1; det oppstår plasmidet pSW21.
Eksempel 7
Konstruksjon av et plasmid til fremstilling av Ser(A21)-humaninsulin-Arg(B31)-Arg(B32)-0H
Ved å utgå fra pSW2-DNA blir på analog måte som i eksempel 2 plasmidet pSW22 konstruert.
Eksempel 8
Konstruksjon av et plasmid til fremstilling av Gly(A21)-Asn(B10)-humaninsulin-Arg(B31)-Arg(B32)-0H
Ved å utgå fra pSW21-DNA blir det på analog måte til eksempel 3 konstruert plasmid pSW23.
Som syntetisk DNA-sekvens blir dermed følgende sekvens anvendt:

Konstruksjon av et plasmid til fremstilling av Ser(A21)-Asn(B10)-humaninsulin-B31(Arg)-B32(Arg)-0H
Ved å utgå fra pSW22-DNA blir på analog måte til eksempel 4 under anvendelse av den i eksempel 8 beskrevne syntetiske DNA-sekvens plasmidet pSW24 konstruert.
B) Fremstilling av AspA21- humaninsulin- ArgB3:1-- ArgB32- 0E fra
humaninsulin- ArgB31- ArgB32- 0H ved hjelp av hydrolyse
1 g humaninsulin-ArgB31-ArgB32-0H blir oppslammet i 100 ml H2O. Ved tilsats av HC1 blir pE-verdien innstilt på 2,5 og løsningen får være ved 37"C. Etter en uke er ca. halvparten av materialet omsatt til AspA2l-humaninsulin-ArgB3l-ArgB32-OH. Produktet blir adskilt på en i og for seg kjent måte ved en ionebytter fra utgangsmaterialet, feller ut av eluatet og krystalliserer i buffer, som inneholder 10,5 g sitronsyre, 1 g fenol og 5 ml eter i 1% zink-kloridløsning pr. liter med en proteinkonsentrasjon på 5 g/l ved pH 6,0. Utbytte utgjør 390 mg Asp<A2l->humaninsulin-ArgB31-ArgB32.
C) Fremstilling av en in. ieks. ionsløsning
Insulin-derivatet ifølge B blir løst med en konsentrasjon på 1,4 mg/ml i en steril bæreløsning med følgende sammensetning (pr. ml):
18 mg glycerin, 10 mg benzylalkohol, 80 jjg Zn<2+>, pH 4,0.
D) Virkningsprofil av en AspA21- humaninsul in- ArgB3:*-- ArgB32-0H- tilberedning fra hund sammenlignet med humaninsulin-ArgB3<1->ArgB3<2->0E og Basal-H-insulin Eoechst(R) = en NPH-(nøytral-protamin etter Hagedorn) tilberedning med ca. 10 pg Zn2+. ;Disse eksemplene viser at AspA2l-humaninsulin-ArgB3:*-ArgB32-OE viser samme fordelaktige basalprofil som humaninsulin-ArgB3<1->ArgB3<2->0H. I tillegg har AspA21-humaninsulin-ArgB31-Arg<B32->0E den fordelaktige egenskap at forbindelsen er langtids-stabil under de valgte betingelsene.

Virkningsprofil av insulintilberedninger
etter subkutan injeksjon i hund sammenlignet med human-insulin:
Eksemplet viser at man ved forlenging av B-kjeden ifølge oppfinnelsen med forskjellige aminosyrerester i humaninsulin, kan oppnå en forsinket og forlenget virkning. Effekten er sterkest utpreget når A21 i tillegg er substituert.
