Abstract
본 발명은 제형 100ml 당 약 0.01 내지 약 5.0g의 라파마이신, 및 약 0.05 내지 약 10용적%의 계면활성제 및 약 75 내지 약 99.95용적%의 인지질 또는 레시틴 용액(여기서, 인지질 또는 레시틴은 40 내지 75중량%이다)을 포함하는 신규한 경구용 라파마이신 제형을 제공한다.
Description
본 발명은 면역억제를 유도하고 이식 거부, 숙주 대 조직이식 질환, 자가면역 질환, 염증 질환, 딱딱한 종양, 진균 감염, 성인 T-세포 백혈병/임파종 및 과증식성 혈관 장애를 치료하기 위한 경구투여에 유용한 라파마이신 또는 약제학적으로 허용되는 라파마이신 염을 함유하는 제형 또는 조성물에 관한 것이다.
라파마이신은 항진균제로서의 이의 특성이 먼저 밝혀진 스트렙토마이세스 하이그로스코피커스(Streptomyces hygroscopicus)에 의해 제조된 마크롤라이드 항생제이다. 이는 칸디다 알비칸스(Candida albicans) 및 마이크로스포룸 집세움(Microsporum gypseum) 같은 균류의 성장을 억제한다. 라파마이신, 이의 제법 및 이의 항생 활성은 미국 특허 제3,929,992호 [1975년 12월 30일자로 서렌드라 세갈(Surendra Sehgal) 등에게 허여됨]에 기술되어 있다. 1977년 마텔(Martel, R.R) 등은 문헌에 실험적인 알레르기성 뇌염 및 애주번트성 관절염(adjuvant arthritis)에 대한 라파마이신의 면역억제 특성에 관하여 발표했다[참조: the Canadian Journal of Physiological Pharmacology, 55, 48-51(1977)]. 1989년에 칼른(Calne, R.Y) 등은 문헌에 동종이식 생체내 거부 반응의 억제에 대한 라파마이신의 효과에 관하여 발표하였다 [참조: Calne, R. M. in Medicinal Science Research, 1989, No. 17, P, 609-10]. 라파마이신의 면역억제 특성 및 거부반응 억제 특성에 관한 많은 논문들이 발표되었으며 사람에게 있어 이식에서 거부 반응을 억제하는 라파마이신의 용도에 관한 임상연구가 시작되었다.
라파마이신은 단독(미국 특허 제4,885,171호) 또는 피시바닐(미국 특허 제4,401,653호)과 병용할 경우 항암 작용을 갖는 것으로 나타났다. 문헌[참조: R. Martel et al. Can. J. Physiol. Pharmacol. 55, 48(1977)]에는 라파마이신이 실험적 알레르기성 뇌척수염 모델, 다발성 경화증에 대한 모델; 애주번트성 관절염 모델, 류머토이드성 관절염에 대한 모델에서 유효하며 IgE-유사 항체의 형성을 유효하게 억제한다고 기술하고 있다.
라파마이신의 면역억제 효과는 문헌[참조: FASEB 3,3411(1989)]에 기술되어 있다. 사이클로스포린 A 및 FK- 506, 다른 매크로사이클릭(macrocyclic) 분자가 또한 면역억제제로서 효과적이므로, 이식 거부를 예방하는데 유용하다[참조: FASEB 3, 3411(1989); FASEB 3,5256(1989); R. Y. Calne et al., Lancet 1183(1978); and 미국 특허 제5,100,899호].
라파마이신은 또한 전신성 홍반성 낭창[미국 특허 제5,078,999호], 폐렴[미국 특허 제5,080,899호], 인슐린 의존성 당뇨병[참조: Fifth Int Conf. Inflamm. Res. Assoc. 121(Abstract), (1990)] 및 평활근 세포 증식 및 혈관 손상에 따른 혈관내막 비대화[참조: Morris, R.J. Heart Lung Transplant 11(pt. 2): 197(1992)]를 예방하거나 치료하는데 유용하다.

미국 특허 제5,100,899호(Calne)에는 라파 마이신 및 이의 유도체 및 프로드럭을 사용하여 포유 동물에서 이식 거부를 억제하는 방법이 기술되어 있다. 라파마이신과 함께 사용하기 위해 나열된 다른 화학요법제는 아자티오프린, 코르티 코스테로이드, 사이클로스포린(및 사이클로스포린 A) 및 FK-506 또는 이들의 배합물이다.

- 제1항에 있어서, 조성물 25ml당 라파마이신 0.125g, 계면활성제 0.25ml 및 조성물이 총 25ml가 되게 하는 양의 50% 인지질 용액을 포함하는 조성물.
- 제1항에 있어서, 조성물 100ml당 라파마이신 1g, 계면활성제 1ml 및 조성물이 총 100ml가 되게 하는 양의 50% 인지질 용액을 포함하는 조성물.
- 제1항에 있어서, 약제학적으로 허용되는 전분 캡슐 내에 함유되어 있는 조성물.
- 제1항에 있어서, 약제학적으로 허용되는 젤라틴 캡슐 내에 함유되어 있는 조성물.
- 조성물 100ml당 0.01 내지 5g의 라파마이신 및 조성물이 총 100ml가 되게 하는 양의 용매 시스템을 포함하고, 이때 용매 시스템은 a) 0.05 내지 10용적%의 계면활성제, b) 0.1 내지 50용적%의 무수 에탄올 및 c) 40 내지 99.85용적%의 인지질 용액(여기서, 인지질 용액은 적합한 용매 중 40 내지 75중량%의 인지질을 포함한다)을 포함하는 조성물.
- 제11항에 있어서, 인지질 용액이 레시틴 용액인 조성물.
- 제11항에 있어서, 조성물 100ml당 2 내지 3g의 라파마이신 및 조성물이 총 100ml가 되게 하는 양의 용매 시스템을 포함하고, 이때 용매 시스템은 a) 3 내지 7.5용적%의 계면활성제, b) 5 내지 20용적%의 무수 에탄올 및 c) 72.5 내지 92용적%의 인지질 용액(여기서, 인지질 용액은 적합한 용매 중 40 내지 60중량%의 인지질을 포함한다)을 포함하는 조성물.
- 제11항에 있어서, 조성물 100ml당 라파마이신 2.5g, 계면활성제 5ml, 무수 에탄올 12.67ml 및 조성물이 총 100ml가 되게 하는 양의 50% 인지질 용액을 포함하는 조성물.
