Description

【発明の詳細な説明】 本発明は人工心臓弁に関する。本発明の目的
は、生体に補形することができ、自然の僧帽弁と
ほぼ同等の性能を有する人工心臓弁、例えば二尖
僧帽弁又は三尖僧帽弁の代用品を提供することで
ある。 本発明によれば、生体に補形できる二尖僧帽弁
又は三尖僧帽弁の代用品は、リング形状の基部
と、少なくとも１対の直立する支持部とを有する
可撓性のフレームを具備する。支持部は、ベース
をたわみ度の異なる少なくとも２つの部分に分割
する。僧帽弁代用品は動物組織片をさらに具備
し、この動物組織片の周囲は、支持部の先端の間
に張り渡された可動部分と、支持部の対応する側
面及び基部の隣接する部分に固着され、密封され
た固定部分とから構成される。 この二尖僧帽弁又は三尖僧帽弁の代用品を心臓
内において使用する場合には、リング形状の基部
のたわみ度の大きい部分を大動脈弁に隣接させた
状態で基部を心耳―心室口に（好ましくは中間縫
合リングを介して）装着する。基部は、自然の僧
帽弁の場合と同様に、ほぼ円形の形態からほぼＤ
字形状に変形することができる。 二尖僧帽弁の代用品の場合、一対の支持部は変
形する前のベースの直径の両端に配設するか、又
は自然の僧帽弁のように大きさの異なる組織片を
収容するために、直径の両端の位置からたわみ度
の小さい部分に片寄つた位置に配設すればよい。
三尖僧帽弁代用品においては、支持部は変形する
前の、基部に規則的に離間して配設されるか、又
は、たとえば大動脈弁代用品における冠状口に必
要とされる解剖学的条件に応じた間隔をおいて配
設される。 可撓性のフレームはDelrin（登録商標）製であ
り、Dacron（登録商標）布で被覆されているの
が好ましく、支持部の両側でフレーム材料の厚さ
を変えることにより、たわみ度の異なる部分が得
られる。支持部は、両側でリング形状の基部の
各々の弓形の部分と接合されているのが好まし
く、この接合は、一方の支持部の丸くなつた先端
から他方の支持部の丸くなつた先端に至る連続的
な湾曲部分を介して行なわれているのが好まし
い。 弁の動物組織片は、完全に固定された（すなわ
ち、グルテルアルデヒドで処理した）動物組織
（たとえば牛又は小牛の心嚢）の平坦なシートか
ら切取つたものであるのが好ましいが各片の形状
は円錐の表面の一部であるのが好ましいこの円錐
の表面の一部分は、円錐の表面においてリング形
状の基部の周囲長さ及びフレームの支持部の先端
の間の距離に夫々対応する周囲長さを有する２つ
の平行な平面と円錐の表面との２本の交線と、円
錐の頂点に向かつて凹形状に湾曲し且つ円錐の両
側において２つの平行な平面と交差する湾曲面と
円錐の表面との交線である第３の交線とにより規
定される。平行な平面の間隔及び湾曲面お湾曲
は、円錐の表面における湾曲面の広がりが、長さ
及び湾曲度の点で、リング形状に基部の一方の弓
形部分と、支持部の隣接する側面とが連続的に混
じり合う湾曲と一致するように定められているた
め、動物組織片材料の成形又は応力固定は不要で
ある。 以下、添付の図面を参照して本発明の実施例に
ついて説明する。 第１図から第３図に示されるように、生体に補
形できる二尖僧帽弁の代用品は、リング形状の基
部２を基部の直径の両端から直立し、基部をたわ
み度の異なる２つの部分２Ａ及び２Ｂに分割する
１対の支持部３とを有する可撓性のプラスチツク
フレーム１（第３図）を具備する。２つの動物組
織片４（第１図及び第２図）の周囲は、夫々、支
持部３の先端３Ｘの間に張り渡された可動部分４
Ａと、支持部の対応する側面及びベースの隣接す
る部分に固着され、密封された固定部分４Ｂとか
ら構成されている。 可撓性のフレーム１はDelrin製であり、
Dacronの布５（第１図）により被覆されてい
る。基部２の各々の部分２Ａ，２ＢのDelrinの厚
さを変えることにより、これらの部分のたわみ度
に変化をつけている。支持部３はその両側におい
て、一方の支持部３の丸くなつた先端３Ｘから他
方の支持部３の丸くなつた先端３Ｘに至る連続す
る湾曲部６を介して基部２と接合している。 第１図及び第２図は、動物組織片がDacronの
布で被覆されたフレーム１に装着された後の自然
な位置にある状態を示す。フレームは縫合リング
７に縫いつけられており、この縫合リングを介し
てフレームを心臓（図示せず）の心耳―心室口の
周囲に装着することができる。この場合、基部２
のたわみ度の大きい部分２Ｂが大動脈弁に隣接し
て固定されるので、基部は自然の僧帽弁の場合と
同様にほぼＤ字形状に変形することができる。 弁の動物組織片４は、第４図に示されるように
完全に固定された（すなわち、グルテルアルデヒ
トで処理した）動物組織（たとえば牛又は小牛の
心嚢）の平坦なシートから切取られたものであ
る。各動物組織片４の形状は円錐８の表面の一部
である。この円錐の表面の一部分は、円錐の表面
においてリング形状の基部２の周囲長さ及びフレ
ーム１の支持部３の先端３Ｘの間の距離に夫々対
応する周囲長さを有する２つの平行な平面９，１
０と円錐の表面との２本の交線と、円錐８の頂点
１２に向かつて凹形状に湾曲し且つ円錐の両側に
おいて２つの平行な平面９，１０と交差する湾曲
面１１と円錐の表面との交線である第３の交線と
により規定される。平行な平面の間隔及び湾曲面
の湾曲（その長さ全体にわたつて変化する）は、
円錐の表面における湾曲面１１の交線４Ｂが長さ
及び湾曲度の点で、リング形状の基部２の一方の
弓形部分２Ａ又は２Ｂと、支持部３の隣接する側
面とが連続的に混じり合う湾曲面６の湾曲と一致
するように定められているため、動物組織片４の
成形又は応力固定は不要である。第３図及び第４
図のＤ，ｄ，Ｈ及びｈは夫々関連する直径と高さ
を示す。 第３ａ図及び第３ｂ図の破線４A′及び４
A″は、夫々、動物組織片４の可動部分４Ａの縁
の開放位置と閉鎖（すなわち創縁接合）位置とを
示している。 前述のように、また、第３図から明らかである
ように、支持部３は変形する前の基部２の直径の
両端に配設される。これに対し、第５図に示され
ている支持部３は、自然の僧帽弁の場合のように
大きさの異なる動物組織片４Ｘ，４Ｙを収容する
ために、たわみ度の小さい部分２Ａに片寄つた位
置に配設されている。この場合、動物組織片は第
４図のように平坦なシートから切取られるが、各
片について円錐の角度は異なり、適切な角度が採
用されている。 第６図及び第７図に示されるように、生体に補
形できる三尖僧帽弁の代用品は、リング形状の基
部２と、変形する前の基部に規則的な間隔をおい
て配設された３つの直立した支持部３とを有する
可撓性のプラスチツクフレーム１を具備する。基
部２は、たわみ度の異なる２つの部分２Ａ及び２
Ｂを有し、部分２Ｂより部分２Ａを厚くすること
により、たわみ度に変化をつけている。すなわ
ち、部分２Ｂにより大きくたわむので、ベースは
第３ｂ図に関して説明したように変形することが
できる。３つの動物組織片４の周囲は、夫々、支
持部の先端３Ｘの間に張り渡された可動部分４Ａ
と、（フレーム１をDacron布５で被覆した後に）
支持部３の対応する側面及びベース２の隣接する
部分に固着された固定部分４Ｂとから構成され
る。図示されている三尖僧帽弁の代用品において
は、動物組織片４は、Dacron布で被覆されたフ
レーム１に取付けられた後の自然な位置にある。
フレーム１は縫合リング７に縫いつけられてお
り、この縫合リングを介してフレームを心臓（図
示せず）の心耳―心室口の周囲に装着することが
できる。これ以降の機能は第１図から第３図の二
尖僧帽弁の代用品と同様である。三尖僧帽弁の代
用品の３つの動物組織片４ま第４図の場合と同様
に平坦なシートから切取つたものであるが、可動
部分４Ａ及び固定部分４Ｂの長さが異なるので、
夫々、適切な円錐角度と直径をもつて切取られて
いる。 第８図において、Ｍ及びＡは、導入流体貯槽１
４に有する密閉された試験チエンバ１３内におけ
る僧帽弁位置と、大動脈弁位置とを夫々示す。マ
イクロコンピユータ１７のデジタル／アナログ
（Ｄ／Ａ）変換器に結合された速度サーボ機構１
６を有する。ピストン１５によりポンプ動作が行
なわれる。流体は矢印の方向に、大動脈弁位置Ａ
から一群の後負荷パラメータモデル１８及びター
ビン流量計１９を介して僧帽弁位置Ｍへ流れる。
Ｖp及びＡpは、先端にカテーテルを装着した圧力
測定用の装置により測定される心室模型内圧力と
心室模型内圧力である。Ｐd及びＰvは、任意の瞬
間におけるピストンの吐出量と速度でありＶcは
マイクロコンピユータ１７からそのデジタル／ア
ナログ（Ｄ／Ａ）変換器を介して取出されるピス
トン制御信号である。 第１図及び第２図に示される二尖僧帽弁の代用
品と、これに対応する大きさの現在使用されてい
る２種類の三尖僧帽弁の代用品を、僧帽弁位置Ｍ
において圧力降下及び逆流の２点について試験し
た。試験の結果を添付した表にまとめて示す。こ
の表からわかるように、本発明による新規な二尖
僧帽弁の代用品は現在使用されている２種類の三
尖僧帽弁の代用品に比べて非常にすぐれた性能を
有する。特にイオネスクーシリー（Ionescu―
Shiley）の僧帽弁の代用品と比較した場合、本発
明による二尖僧帽弁又は三尖僧帽弁の代用品では
圧力低下がほぼ同じでも逆流はかなり少なくなる
ことがわかる。 【表】

３ｃ図に対応し、基部がＤ字形状に変形した状態
を示す底面図、第４図は、第１図及び第２図の二
尖僧帽弁の代用品の各々の動物組織片の形状をい
かに定めるかを示す図、第５図は第３ａ図に対応
し、支持部が基部の直径の両端からたわみ度の小
さい部分の方の片寄つて配設されている二尖僧帽
弁の代用品のフレームを示す側面図第６図は、本
発明による三尖僧帽弁の代用品の斜視図、第７図
は、第６図の三尖僧帽弁の代用品の流入部の底面
図、そして第８図は、僧帽弁の代用品を試験する

Claims (1)

    【特許請求の範囲】
  1. １ リング形状の基部と少なくとも１対の直立す
    る支持部とを有するフレームと、複数の弁片とを
    備え、前記支持部は、一方の支持部の先端から他
    方の支持部の先端に至る連続する湾曲部を介して
    前記基部に接合され、前記各弁片は、前記支持部
    の先端の間に張り渡された可動部分と、前記支持
    部の対応する側面及び前記基部の隣接する部分に
    固着され密封された固定部分とを有し、且つ前記
    弁片は、完全に固定された動物組織の平坦なシー
    トから切取られたものであり、各弁片の形状は円
    錐の表面の一部分であり、前記円錐の表面の一部
    分は、円錐の表面において前記リング形状の基部
    の周囲長さ及び前記プラスチツクフレームの支持
    部の間の距離に夫々対応する周囲長さを有する２
    つの平行な平面と円錐の表面との２本の交線と、
    円錐の頂点に向かつて凹形状に湾曲し且つ円錐の
    両側において２つの平行な平面と交差する湾曲面
    と円錐の表面との交線である第３の交線とにより
    規定され、２つの平行な平面の間隔及び湾曲面の
    湾曲は、円錐の表面における湾曲面の広がり長さ
    及び湾曲度の点で、前記リング形状の基部の一方
    の弓形部分と、前記支持部の隣接する側面とが連
    続的に混じり合う湾曲部分の湾曲と一致するよう
    に定められていることを特徴とする人工心臓弁。
