プロピオン酸非ステロイド系抗炎症剤のアミノ酸塩と
カフェインを含有した組成物

本発明は、開始を早めるために十分な量のカフェインと一緒にプロピオン酸非
ステロイド系抗炎症剤のあるアミノ酸塩を含んだ組成物の安全有効量を投与する
ことにより改善された鎮痛及び/又は抗炎症効果を与える組成物及び方法に関す
る。
発明の背景
炎症又は“炎症反応”は、血管透過性亢進、液体蓄積と、炎症領域への炎症細
胞の変化群の遊走を含めた、複雑な相互関連生理現象の結果である。炎症の臨床
発現には腫脹(浮腫)、局部温度上昇、紅斑及び痛みがある。炎症反応はある細
菌、照射、化学剤に対する過敏症、関節炎様症状等を含めたいくつかの原因ファ
クターのうちいずれかで誘発されうる。炎症反応は体内の一次防御メカニズムで
あると通常考えられるが、未制御のままだと、過度になって、機能障害を起こす
ことがある。
炎症及び付随する痛みに抗する上で、非ステロイド系抗炎症剤、解熱剤及び鎮
痛剤、特にアスピリン及びアスピリン誘導体を含めたサリチレート類の使用は、
容認されている医療行為である。非ステロイド系は、例えば滑液包炎、関節炎等
に伴う痛み及び炎症を軽減するために通常用いられる。
痛みはその基本的に主観的な性質のせいで正確に規定できないが、その用語は
特定神経終末の刺激に起因する窮迫又は苦痛の感覚に関するものであると通常言
える。非常に様々な薬物がヒト及び他の動物で痛みを減少させるために開発され
た;一部はその起点で痛みを除くことに向けられ、他は脳による痛みの知覚を遮
断することに向けられている。痛みの感覚を遮断するように考えられた後者の群
の薬物の中には、意識消失を起こさずに通常痛みを軽減する鎮痛剤がある。鎮痛
剤は2つの主要なカテゴリー:即ちモルヒネ、コデイン、レボルファノールと、
モルヒネ様鎮痛剤メペリジン及びメタドンを含めたオピオイド鎮痛剤;アスピリ
ン、イブプロフェン、フェナセチン、アセトアミノフェン、フェニルブタゾン及
びインドメタシンのような解熱鎮痛剤に更に分類できる。
これら鎮痛剤の正確な薬理作用は不明であるが、オピオイド鎮痛剤と解熱剤と
を容易に区別するある種の効果がある。特に、解熱剤は弱い鎮痛剤であって、末
梢神経系でそれらの効果が大きく、そのため行動変化は通常起きない。通常、こ
れらの鎮痛剤は筋肉、関節、腱及び筋膜から派生する痛みのみを軽減し、深部内
臓痛に対しては無効である。しかしながら、オピオイド鎮痛剤はすべてのタイプ
の痛みに対してかなり有効であり、中枢神経系で広域作用を有する。強力な鎮痛
剤の他に、麻酔薬としても知られるオピオイドは気分及び他の行動変化にしばし
ば影響を与える。おそらく、オピオイド鎮痛剤の最も顕著な副作用は、それらの
反復使用が耐性と精神的及び身体的依存性に関連しているという事実である。
痛み及び炎症から軽減させる上で有用なイブプロフェンのオルニチン、リジン
及びアルギニン塩は、例えば1981年7月21日付で発行されたBruzzeseらの
米国特許第4,279,926号明細書で開示されている。イブプロフェンリジ
ン錠剤の製造方法は、1992年3月19日付で公開されたEP第505,18
0号明細書で開示されている。
イブプロフェンのラセミ混合物とカフェインとの併用は、例えば1984年8
月7日付で発行されたSunshineらの米国特許第4,464,376号明細書で開
示されている。交感神経興奮アミン類を含有した咳/感冒医薬組成物の製造にお
ける、イブプロフェンと、他のそれより新しい非ステロイド系抗炎症剤(即ち、
アスピリン、アセトアミノフェン及びフェナセチンを除く)との使用は、例えば
1985年11月12日付で発行されたSunshineらの米国特許第4,552,8
99号明細書で開示されている。S(+)形のイブプロフェンの使用は、例えば
1989年7月25日付で発行されたSunshineらの米国特許第4,851,44
4号明細書で、1992年4月16日付で公開されたGates らのWO第9,20
5,783号明細書では抗ヒスタミン剤との併用について開示されている。
本発明者らは、カフェインと共にプロピオン酸NSAIDのあるアミノ酸塩を
含む選択された組成物が改善された鎮痛及び/又は抗炎症効果を更に与えること
を発見した。
したがって、本発明の目的は痛み及び/又は炎症の治療のためのこのような組
成物及び方法を提供することである。
発明の要旨
本発明は:
a.鎮痛及び抗炎症上有効な量のプロピオン酸NSAIDのアミノ酸塩、及び
b.鎮痛応答の開始を早めてそれを高めるために十分な量のカフェイン
を含んだ組成物の安全有効量をヒト又はそれより下等の動物に投与することから
なる、治療の必要なヒト又はそれより下等の動物において持続性の、高い鎮痛応
答を発揮させる方法に関する。
本明細書で用いられるすべてのパーセンテージ及び比率は、他で指摘されない
かぎり、重量による。
発明の具体的な説明
本発明は、イブプロフェンリシネートと、鎮痛応答の開始を早めてそれを高め
るために十分な量のカフェインを含んだ組成物の安全有効量をヒト又はそれより
下等の動物に投与することからなる、治療の必要なヒト又はそれより下等の動物
において持続性の、高い鎮痛応答を発揮する組成物及び方法に関する。
“アミノ酸塩”という用語は、一級、二級、三級及び四級アミンの薬学上許容
される有機無毒性塩基、天然置換アミンを含めた置換アミン、環状アミン及び塩
基性イオン交換樹脂、例えばトリエチルアミン、トリプロピルアミン、2‐ジメ
チルアミノエタノール、2‐ジエチルアミノエタノール、リジン、オルニチン、
アルギニン、ヒスチジン、カフェイン、プロカイン、N‐エチルピペリジン、ヒ
ドラバミン、コリン、ベタイン、エチレンジアミン、グルコサミン、メチルグリ
カミン、テオブロミン、プリン類、ピペラジン、ピペリジン、ポリアミン樹脂等
から誘導された塩に関する。
本発明の組成物で有用な非ステロイド系抗炎症剤のプロピオン酸誘導体は当業
者に周知であり、例えば1985年11月12日付で発行されたSunshineらの米
国特許第4,552,899号明細書で開示されており、これは参考のため本明
細書に組み込まれる。非ステロイド系抗炎症剤の化学構造、合成、副作用等の詳
細な開示については、Anti-inflammatory and Anti-Rheumatic Drugs,K.D.Rains
ford,Vol.I-III,CRC Press,Boca Raton(1985) 及びAnti-inflammatory Agents,
Chemistry and Pharmacology,I.R.A.Scherrerら,Academic Press,New York(1974
)を含めた標準テキストが参照され、それら双方とも参考のため本明細書に組み
込まれる。
本発明の組成物で有用な好ましい非ステロイド系抗炎症剤には、イブプロフェ
ン、ナプロキセン、ベノキサプロフェン、フルルビプロフェン、ケトプロフェン
、フェノプロフェン、フェンブフェン、インドプロフェン、ピルプロフェン、カ
ルプロフェン、オキサプロジン、プラノプロフェン、ミロプロフェン、チオキサ
プロフェン、スプロフェン、アルミノプロフェン及びチアプロフェニックのよう
なプロピオン酸誘導体のアミノ酸塩がある。これら非ステロイド系抗炎症剤の混
合物も用いてよい。これらのプロピオン酸NSAIDの中では、イブプロフェン
、ナプロキセン及びケトプロフェンが最も好ましい。
本発明で使用上最も好ましいのはこれらNSAID塩のS(+)異性体である
。本発明の鎮痛剤に適用される“S(+)”という用語は、本発明のアミノ酸塩
誘導体の右旋性又はS(+)異性体を包含した意味である。用語“S(+)”と
共に用いられる“R(−)対掌体を実質上含まない”という表現は、S(+)エ
ナンチオマーが望ましい開始早期化及び高い鎮痛効果を発揮する上でそのR(−
)対掌体を実質上含まないことを意味する。現実的に言えば、これは活性成分が
少くとも90重量%のS(+)エナンチオマーと10重量%以下のR(−)エナ
ンチオマーを含有していることを意味する。好ましくは、S(+)エナンチオマ
ー対R(−)エナンチオマーの重量比は20:1以上、更に好ましくは97:3
以上である。最も好ましくは、S(+)エナンチオマーは99重量%以上であっ
て、R(−)エナンチオマーを含まず、即ちS対Rの重量比は大体99:1又は
それ以上である。
イブプロフェン、ナプロキセン、ベノキサプロフェン、フルルビプロフェン、
ケトプロフェン、フェノプロフェン、フェンブフェン、インドプロフェン、ピル
プロフェン、カルプロフェン、オキサプロジン、プラノプロフェン、ミロプロフ
ェン、チオキサプロフェン、スプロフェン、アルミノプロフェン及びチアプロフ
ェニックのアミノ酸塩の安全有効量は、通常約7.5〜約1000mgの範囲であ
り、それらの酸誘導体対応部分と通常同様である。これら薬剤の有用な投与量は

2日付で発行されたSunshineらの米国特許第4,552,899号明細書でみる
ことができ、それら双方が参考のため本明細書に組み込まれる。
例えば、本発明の組成物で用いられるイブプロフェンのアミノ酸塩の安全有効
量は、通常約50〜約800mg、好ましくは約50〜約400mg、更に好ましく
は約50〜約200mg、最も好ましくは約50〜約100mgの範囲である。本発
明の組成物で用いられるフルルビプロフェンのアミノ酸塩の安全有効量は、通常
約12.5〜約300mg、好ましくは約12.5〜約200mg、更に好ましくは
約12.5〜約100mg、最も好ましくは約12.5〜約50mgの範囲である。
本発明の組成物で有用なケトプロフェンのアミノ酸塩の安全有効量は、通常約5
〜約100mg、好ましくは約5〜約75mg、更に好ましくは約5〜約50mg、最
も好ましくは約5〜約25mgの範囲である。通常、これら薬剤のS(+)異性体
の量はラセミ混合物の量の約半分である。
好ましくは、本発明の医薬組成物は、約10:1〜約1:10、好ましくは約
5:1〜約1:5、最も好ましくは約2:1〜約1:5の比率で、鎮痛剤及びカ
フェインを含んでいる。
錠剤、ゲルカップ、カプセル、顆粒、ロゼンジ及びバルク粉末のような固体形
と、シロップ及び懸濁液のような液体形を含めた様々な経口剤形が使用できる。
これらの経口形は安全有効量、通常少くとも約5%の活性成分を含有している。
固体経口剤形は、好ましくは約5〜約95%、更に好ましくは約10〜約95%
、最も好ましくは約25〜約95%の活性成分を含有している。液体経口剤形は
、好ましくは約1〜約50%、更に好ましくは約1〜約25%、最も好ましくは
約3〜約10%の活性成分を含有している。
錠剤は、適切な結合剤、滑沢剤、希釈剤、崩壊剤、着色剤、香味剤、保存剤及
び流動誘導剤を含有させて、圧縮、すりこみ、腸溶性コート、糖衣化、フィルム
コート又はマルチ圧縮することができる。
液体経口剤形には、適切な溶媒、保存剤、乳化剤、懸濁剤、希釈剤、甘味剤、
味覚遮断剤、着色剤及び香味剤を含有した、水性及び非水性溶液、エマルジョン
、懸濁液と、非沸騰性顆粒から再調製される溶液及び/又は懸濁液がある。経口
剤形を処方するために用いられる、薬学上許容されるキャリヤ及び賦形剤の具体
例は、1975年9月2日付で発行されたRobertの米国特許第3,903,29
7号明細書で記載されており、これは参考のため本明細書に組み込まれる。固体

口剤形を作るための技術及び組成物は、Marshall,′Solid Oral Dosage Forms′

載されており、これは参考のため本明細書に組み込まれる。錠剤(圧縮及び成形
)、カプセル(硬質及び軟質ゼラチン)及び丸剤を作るための技術及び組成物は
、Remington's Pharmaceutical Sciences(Arthur Osol,editor),1553-1593(1980
)で記載されており、これは参考のため本明細書に組み込まれる。
液体経口剤形を製造する上で、活性成分は慣用的調剤実務に従い水性ベースで
経口上許容される製剤キャリヤ中に配合される。“水性ベースで経口上許容され
る製剤キャリヤ”とは、全部又は主要溶媒分が水であるものをいう。典型的キャ
リヤには単純な水性溶液、シロップ、分散液及び懸濁液と、水中油型のような水
性ベースエマルジョンがある。最も好ましいキャリヤは、適切な懸濁剤を含有し
た水性ビヒクル中における医薬組成物の懸濁液である。適切な懸濁剤にはアビセ
ル(Avicel)RC‐591(FMC市販の微結晶セルロース/カルボキシメチルセ
ルロースナトリウム混合物)、グアーガム等がある。このような懸濁剤は当業者
に周知である。本発明の組成物中における水の量は活性成分の全重量及び容量と
他の任意非活性成分に応じてかなり広範囲にわたるが、最終組成物の重量に基づ
く全水分含有率は通常約20〜約75重量/容量%、好ましくは約20〜約40
%の範囲である。
水自体でキャリヤ全体を構成してもよいが、典型的液体処方物では、組成物中
への香油等のような非水溶性成分の溶解及び配合を助けるために、共溶媒、例え
ばプロピレングリコール、グリセリン、ソルビトール溶液等を含有していること
が好ましい。したがって、一般的には、本発明の組成物は、好ましくは約5〜約
25容量/容量%、最も好ましくは約10〜約20容量/容量%の共溶媒を含有
している。
本発明の組成物は、場合により1種以上の他の公知の治療剤、特に咳/感冒製
剤で常用されるもの、例えばデキストロメトルファン、クロフェジアノール、カ
ルベタペンタン、カラミフェン、ノスカピン、ジフェンヒドラミン、コデイン、
ヒドロコドン、ヒドロモルホン、ホミノベン、それらの薬学上許容される塩のよ
うな咳抑制剤;グアヤコール酸グリセリル、テルピン、塩化アンモニウム、N‐
アセチルシステイン、ブロムヘキシン、アンブロキソル、それらの薬学上許容さ
れる塩のような去痰剤又は粘液溶解剤;クロルフェニラミン、ブロムフェニラミ
ン、d‐クロルフェニラミン、d‐ブロムフェニラミン、トリプロリジン、ドキ
シルアミン、トリペレナミン、シプロヘプタジン、カルビノキサミン、ブロモジ
フェンヒドラミン、フェニンダミン、ピリラミン、アザタジン、それらの薬学上
許容される塩のような抗ヒスタミン剤と、アクリバスチン、AHR‐11325
、フェニンダミン、アステミゾール、アゼラスチン、セチリジン、エバスチン、
ケトチフェン、ロドキサミド、ロラチジン、レボカバスチン、メキタジン、オキ
サトミド、セタスチン、タジフィリン、テメラスチン、テルフェナジン、それら
の薬学上許容される塩を含めた非鎮静抗ヒスタミン剤を含有していてもよい:こ
れらすべての成分とそれらの許容される投与量範囲は以下で記載されている:1
988年11月8日付で発行されたSunshineらの米国特許第4,783,465
号、1986年10月28日付で発行されたSunshineらの米国特許第4,619
,934号明細書;これらは参考のため本明細書に組み込まれる。テオフィリン
及びアルブテロールのような気管支拡張剤と、アセトアミノフェン及びアスピリ
ンのような他の鎮痛剤も有用である。高度に好ましい任意成分はカフェインであ
り、これは約10〜約50%のレベルで存在することが好ましい。
薬剤師業界で周知の他の任意成分、例えば嗜好に合う快適な外観の最終製品を
提供するための天然又は人工甘味剤、香味剤、着色剤等と、貯蔵寿命を伸ばして
高めるための酸化防止剤、例えばブチル化ヒドロキシアニソール又はブチル化ヒ
ドロキシトルエン、及び保存剤、例えばメチルもしくはプロピルパラベン又は安
息香酸ナトリウムも、これらの成分について通常知られる量で含有させてよい。
治療方法
投与される医薬組成物の量はその処方物内にある活性成分の%に依存しており
、1回分当たりに要求されるイブプロフェン及びカフェインといずれか任意の成
分、例えばうっ血除去剤、去痰剤及び/又は抗ヒスタミン剤の量、安定剤、放出
特性と他の薬学的パラメーターの関数である。
通常約1〜約50mg/kg/日、好ましくは約2〜約30mg/kg/日、最も好ましく
は約3〜約20mg/kg/日の医薬組成物が本明細書で記載されたように投与される
。この量は1回で、又は好ましくは複数回(2〜6回)に分けて反復して投与し
ても、あるいは治療クールにわたり徐放的に投与してもよい。通常、本発明の医
薬組成物の各個別投与量は約1〜約25mg/kg、好ましくは約2〜約15mg/kg、
最も好ましくは約3〜約10mg/kgの範囲である。経口投与用の典型的な単位剤
形は、通常約50〜約2000mg、好ましくは約100〜約600mg、最も好ま
しくは約100〜約400mgのイブプロフェンと、約25〜約200mg、好まし
くは約50〜約200mg、最も好ましくは約50〜約100mgのカフェインを含
んでいる。上記より多い投与量も鎮痛軽減を与える上で有効であるが、有害な副
作用を防止するためにいかなる薬物に関しても一部の個体で注意が払われねばな
らない。
下記例は必須及び任意双方の成分が混合された本発明の態様について示してい
る。

経口投与用の硬質ゼラチンカプセル組成物は、下記成分を混合することにより

トを撹拌しながらアルコールに加える。プロピレングリコール及びフレーバーを

Non-steroidal systems are commonly used to reduce pain and inflammation associated with, for example, bursitis, arthritis and the like.
