膝手術におけるドリルジグ用位置決め探針
技術分野
本発明は、膝の手術に関し、特に、以下ACLと呼ぶ前十字靱帯の交換に関す
る。
背景技術
編組ジグザグ構造を成して炭素やポリエステル繊維を含むサージクラフト社の
商標ABCにより知られるような合成スカフォード型のACL交換は成功した。
電子顕微鏡検査を使用しての失敗分析方式は、多く失敗(過去6年間で9%乃至
12%)が脛骨ドリル孔の内側出口で生じることを示した。
外科的に回避できる作用、例えば顆間切れ込みの頂部とドリル孔の延長部の縁
部との間の衝撃による繊維の破砕、及び脛骨ドリル孔の尖った縁部の反復応力に
よる疲労損傷等は、解明されてきた。
破砕及び疲労損傷は、脛骨ドリル孔に略ラジアスカッタを使用することにより
防ぐことができる。
切れ込みの衝撃による失敗は、膝を一杯に延ばしたときに脛骨ドリル孔を顆間
切れ込み頂部の後ろに位置決めすることにより防ぐことができる。
発明の開示
従って、本発明の目的は、直接の視覚化(関節切開)又は間接の視覚化(関節
鏡検査)を行うことなく、脛骨ドリル孔の正確な位置決めを容易にするドリルジ
グ探針を提供することである。
本発明によれば、ACLドリルジグ探針は、ACLドリルジグ内に固定される
定位部と、前記定位部から延びて膝蓋鍵の一方の側を通過する軸部と、前記探針
を使用するACLドリルジグの解剖基準点として、顆間切れ込みの後部に定位す
るために、軸部に対して鈍
角を成して接合部から延びる角状突起部とを備えている。
軸部と角状突起部は、例えばステンレス鋼製の直径が4.0mmで一定の長さ
の丸棒から一体的に形成されることが望ましいが、棒は、半円形その他の断面形
を有してもよい。軸部と角状突起部との間の接合部は屈曲部(僅かに湾曲しても
よい)を成し、角状突起部の自由端部は、好ましくは丸みを付けられている。軸
部の他方の端部(即ち、角状突起部から離れた方の端部)は、定位部の一方の端
部の孔に固定されることが望ましい。この定位部は、好ましくは、例えば横方向

曲部(それぞれ僅かに湾曲してもよい)を有する。これにより、脛骨ドリル孔は
、探針の角状突起部の軸線に対して三次元的に傾斜した軸線を有し、その出口端
部は、接合部の上側の角状突起部上の点の下側で、軸部と軸方向に整合する。こ

探針は、軸部内の屈曲部のために左又は右の膝に対して「向きを持つ」ように
なる。また、本発明は、一つは左膝用でもう一つは右膝用の少なくとも二つの互
換性のあるACLドリルジグ探針と、共面ACLドリルジグとを備えた一連の器
具を提供する。しかしながら、患者によって膝の寸法が変化するため、前記一連
の器具は、一
定範囲の左膝と一定範囲の右膝のために複数の互換性のある探針を備える。
本発明に係るACLジグ探針の実施例及び該探針を備えた一連の器具について

発明を実施するための好適な態様
図面の図1乃至図3に示したACLドリルジグ用探針は、ACLドリルジグ(
すぐに図5及び図6を参照して説明する)内に固定するための定位部10と、膝
蓋鍵21(図7も参照)の一方の側を通過するように定位部から延びた軸部11
と、探針を使用するACLドリルジグの解剖基準点として、顆間切れ込みの後部
22(図8参照)に定位するために、軸部に対して鈍角を成して接合部13から
延びる角状突起部12とを備えている。
軸部11と角状突起部12は、一定の長さで直径4.0mmのス
テンレス鋼丸棒から一体的に形成されている。接合部13では、鋭角の屈曲部を
成し、角状突起部の自由端部14は丸みを付けられている。軸部11の他方の端
部は、定位部10の一方の端部の孔24(図6参照)に、例えばピンやセメント
を介して、固定されている。定位部10は、一定の長さで横方向寸法が8.0m
mのステンレス鋼角棒から成り、角部と端部は斜めに切るか丸みを付けられてお
り、また一方の側には半円形のノッチ15を備えて、探針を用いるACLドリル
ジグ内に堅固に取り付けられるようにしている。
探針を既存の共面ACLドリルジグに使用するとき内側大腿骨顆の側面25及
び膝蓋鍵21の周囲に軸部11を通すのを容易にするために、軸部は反対方向を
向いた二つの屈曲部16、17を有する。これにより、脛骨ドリル孔26(図8
参照)は、探針の角状突起部12の軸線に対して三次元的に傾斜した軸線を有し
、その出口端部(図2に符号18で示す)は、接合部13の上側の角状突起部1

軸部11の屈曲部16、17により探針は「向きを持つ」ので、本発明に係る
一連の器具は、(既存の共面ACLドリルジグの他に)少なくとも一つの図示し
たような探針と、屈曲部16、17がそれぞれ逆向きになった少なくとも一つの
他の探針とを含む。しかし、異なる患者の膝の解剖寸法の変化に対応するために
