Description
L−ドーパ、(−)−3−(3,4−ジヒドロキ
シフエニル)−L−アラニンは公知の化合物であ
り、このものはパーキンソン症候群(Parkin−
sonism)の治療処置に重要である。
また、脱炭酸酵素抑制剤の多くが公知の化合物
である。例えば、N1−dl−セリル−N2−(2,
3,4−トリヒドロキシベンジル)−ヒドラジン
塩酸塩はヘゲダス(Hegedus)らによる米国特許
第3178476号に開示されており、そしてこのもの
は脱炭酸酵素抑制剤として有効である。
パーキンソン症候群の処置において、L−ドー
パ及び特定の脱炭酸酵素抑制剤であるN1−dl−
セリル−N2−(2,3,4−トリヒドロキシベン
ジル)−ヒドラジン塩酸塩を配合して用いること
がバートリニ(Bartholini)らによる米国特許第
3557292号に記載されている。
速やかに崩壊する錠剤中でL−ドーパを単独で
用いることはシエス(Sheth)らによる米国特許
第3632778号に開示されている。
従来、多量のL−ドーパがパーキンソン症候群
の処置に用いられてきたが、その理由は投与され
たL−ドーパの多くのものが、その治療活性が利
用される血液−脳関門(blood−brain barrier)
を通過する前に胃または血液中で酵素、即ち脱炭
酸酵素によつて代謝されるからである。
L−ドーパを公知の脱炭酸酵素抑制剤と配合し
て投与する場合、そこなわれずに血液−脳関門を
通過する経口投与されたL−ドーパの量は実質的
には増加させることができ、即ち抗パーキンソン
症候群活性は「高められる」。
しかしながら、L−ドーパ及び脱炭酸酵素抑制
剤を含む通常のカプセル剤及び錠剤組成物は、最
初は高いL−ドーパの血中濃度を示すが、L−ド
ーパを単独で投与する際に得られる傾向と同様の
最初のピークを越える血液濃度を持続するもので
はない。
L−ドーパを含む通常の制御された放出組成物
では所望の治療応答性、即ち高いL−ドーパの血
中濃度が得られない。
本発明によれば、L−ドーパ及び脱炭酸酵素抑
制剤の両方を含む流体力学的に平衡化されて
(hydrodynamically balanced)制御され制御さ
れた放出組成物は優れたL−ドーパの血中濃度を
与えることが見い出された。更に詳細には、L−
ドーパ及び脱炭酸酵素抑制剤の両方を含む流体力
学的に平衡化され制御された放出カプセル剤また
は錠剤の経口投与により、L−ドーパ及び脱炭酸
酵素の両方を含む通常のカプセル剤もしくは錠剤
のいずれかのもの、或いはL−ドーパ単独もしく
はL−ドーパ及び脱炭酸酵素抑制剤の配合物のい
ずれかを含む通常の制御された放出カプセル剤ま
たは錠剤と比較して血中のL−ドーパの濃度をか
なり増加させるばかりでなく、L−ドーパの血中
濃度を更に持続する結果が得られる。
制御された放出組成物からのL−ドーパ及び脱
炭酸酵素抑制剤の経口投与は流体力学的に平衡化
されたカプセル剤または錠剤の投与形態により改
善される。
本発明の実施において用いられる制御された放
出組成物において、カプセル剤または錠剤のいず
れかの投与形態物は浮遊した状態を保ち、そして
長期間胃液中を自由に浮遊し、その間にこの組成
物中に含まれる実質的にすべての活性成分が胃液
中に放出される。
おかされずに胃液中に浮遊しており、その間に
実質的にすべての薬物がこのものから放出される
組成物は当該分野で報告されている。例えば、米
国特許第4126672号;同第4140755及び同第
4167558号には、経口投与に対する制御された放
出カプセル剤及び錠剤の調製のための組成物が記
載されている。カプセル剤または錠剤は胃液と接
触する際に1より小さい嵩密度(比重)を有する
ように流体力学的に平衡化され、従つて1.004乃
至1.010間の比重を有する胃液中で浮遊した状態
を保つであろう。これらの制御された放出組成物
は活性成分と1種またはそれ以上の親水性ヒドロ
コロイドとの混合物である。
この制御された放出性カプセル剤または錠剤を
経口的に摂取すると、カプセル剤の殻(shell)
または錠剤の被覆(coating)は、存在していて
も、溶解し、そして組成物が胃液と接触するよう
になる。最も外側のヒドロコロイドは水和して事
実上外側障壁を形成し、カプセル剤または錠剤を
いくぶん拡大させ、そして水の浸透を防止する。
この投与形態における嵩容量の増加、及び上記の
障壁の結果としてカプセル剤または錠剤中に乾燥
したままの中心部の空隙が相俟つて浮力が生じ
る。柔軟なゲル状の物質が生じ、このものは1よ
り小さい嵩密度を保持する。水和した最外層が
徐々に溶解するため、L−ドーパ及び脱炭酸酵素
抑制剤は拡散及び/または侵食によりこのものか
ら徐々に放出される。ひき続き、新しく露出した
ヒドロコロイドから新たに水和した外側層が生
じ、かくして障壁はそのまま保持される。すべて
の薬剤が溶解するまで投与物は胃液中で浮力を保
ちつつこの過程をくり返す。未だ胃液中で浮力を
保持している残りのマトリツクスは徐々に分散す
るかまたは排除される。
これらの制御された放出組成物を用いて達成さ
れる放出パターン及びその結果生じる血中濃度は
通常の制御された放出薬剤と比較して利点を有し
ている。
脱炭酸酵素抑制剤は脱炭酸酵素の活性を抑制す
る作用があり、かくして投与されたL−ドーパの
血漿中での濃度が上昇する。多数のかかる脱炭酸
酵素抑制剤は公知である。これらの流体力学的に
平衡化され制御された放出組成物に用いる際に適
するこれらの公知の脱炭酸酵素抑制剤には次のも
のがある:
a N1−dl−セリル−N2−(2,3,4−トリヒ
ドロキシベンジル)−ヒドラジン塩酸塩、
b β−(3,4−ジヒドロキシフエニル)−α−
ヒドラジノ−α−メチルプロピオン酸、
c m−ヒドロキシベンジルヒドラジン、及び
d α−メチルドーパ。
N1−dl−セリル−N2−(2,3,4−トリヒド
ロキシベンジル)−ヒドラジン塩酸塩が本発明の
好適な脱炭酸酵素抑制剤である。
これらの制御された放出組成物に用いるのに適
するヒドロコロイドには1種もしくはそれ以上の
天然、半合成または全合成のアニオン性、または
好ましくは非イオン性の親水性ゴム、変性された
セルロース性物質、或いは蛋白性物質、例えばア
ラビアゴム、トラガカントゴム、ローカストビー
ン(locust bean)ゴム、グアゴム、インドゴム、
寒天、ペクチン、カラゲーン、可溶性及び不溶性
のアルギン塩酸、メチルセルロース、ナトリウム
カルボキシメチルセルロース、カルボキシポリメ
チレン(Carbopol−Cabot Corporation)、ゼラ
チン、カゼイン、ゼイン、ベントナイト、
Veegum(R.T.Vanderbilt Co.)などが含まれる。
好適なヒドロコロイドはヒドロキシプロピルメチ
ルセルロースである。また、かかる物質を製薬学
的化合物中に用いることは当該分野で公知であ
る。例えば、カプラン(Kaplan)らによる米国
特許第3555151号には、支持された(sustained)
放出用制酸剤調製物中におけるかかるヒドロコロ
イドの使用が示されている。
用いるヒドロコロイドは酸性媒質、即ち0.1N
塩酸(ほぼ1.2のPH値)に相当するPH値を有する
胃液中で水和しなければならない。更に、本発明
の制御された放出調製物、殊に錠剤中に含まれる
ある種の組成物の初期の嵩比重は1以上であつて
もよいが、胃液と接触する際に組成物が1より小
さい嵩比重を有し、浮遊するように流体力学的に
平衡化することが本質的なことである。制御され
た組成物からの薬剤の放出速度を調整し得る方法
は数多く存在する。第一に、特定なヒドロコロイ
ドまたはヒドロコロイドの混合物を選ぶことによ
り放出速度に影響を及ぼすことができ、例えば高
粘度のヒドロコロイド、例えば4000cpsを有する
ヒドロキシプロピルメチルセルロースは低粘度の
ヒドロコロイド、例えば10cpsを有するヒドロキ
シプロピルメチルセルロースと比較してより徐々
に水和し、そして長い時間柔軟な物質の状態を保
持する。更に、1より小さい比重を有する食用の
製薬学的に不活性な脂肪物質を組成物に加え、組
成物の親水特性を減少させ、従つて浮力を増加さ
せることができる。かかる物質の例には精製され
た等級のみつろう;脂肪酸;長鎖の脂肪アルコー
ル例えばセチルアルコール、ステアリルアルコー
ル;脂肪酸のエステル例えばモノステアリン酸グ
リセリル;水素添加されたひまし油などが含まれ
る。
また、これらの制御された放出組成物中に製薬
学的調剤の分野で認められる付加的な食用の無毒
性の成分、例えば賦形剤、即ち緩衝剤、防腐剤、
安定剤、錠剤潤滑剤などを配合することができ
る。
これらの制御された放出組成物中に存在するヒ
ドロコロイド成分の量は広い範囲、即ち約5〜約
80重量%の範囲内で変えることができる。ヒドロ
コロイドの量は活性成分及び用いる不活性な製薬
学的補助薬の量並びに特性に関係して変えること
ができる。一般に、ヒドロコロイドの量は約15乃
至約60重量%間であろう。
脂肪物質または脂肪物質の混合物が制御された
放出組成物中に存在する場合、かかる物質は全体
の組成物の約60重量%までを構成する。一般に、
組成物に脂肪物質が含まれる場合、かかる物質は
約5〜約30重量%の量で存在する。脂肪物質の量
は活性成分、及び流体力学的に平衡化された組成
物、即ち胃液中で1より小さい嵩密度を獲得する
処方物を得る目的のヒドロコロイドの両方の量及
び物理的特性により制御される。
また制御された放出組成物中に存在し得る食用
の不活性な製薬学的補助物質は他の成分の量及び
物理的特性に従つて変わるであろう。それ自体が
1より小さい嵩密度を有するような物質、例えば
エチルセルロースまたは他の増量剤は組成物の浮
力を高めるであろう。投薬状態の密度がそれ程高
くない場合は浮力を高めるために少量の二酸化炭
素発生剤を組成物中に用いることができる。かか
る物質が二酸化炭素を発生すると、発泡物が水和
した外側層に捕捉され、かくして処方物の浮力が
高まる。更に重要なことに、組成物からの活性成
分の放出速度を改質するために不活性な製薬学的
補助物質を選んで用いることができる。例えば、
可溶性の賦形剤、例えばラクトース、マンニトー
ルなどは放出の速度を増加させ、一方不溶性の賦
形剤、例えばリン酸二カルシウム、白陶土などは
放出速度を減少させるであろう。かかる製薬学的
補助物質を処方物中に含ませる場合、これらのも
のは最終処方物の約80重量%まで存在することが
できる。一般に、かかる通常の製薬学的補助剤は
処方物の約5〜約60重量%の量で存在する。かか
る物質を含むこと、そしてこのものを選ぶことは
当該分野の範囲内のことと考えられる。
流体力学的に平衡化されて制御された放出カプ
セル剤または錠剤組成物は約10〜約70重量%、好
ましくは約20〜約50重量%のL−ドーパを含むこ
とができる。
流体力学的に平衡化されて制御されて放出カプ
セル剤または錠剤組成物中の脱炭酸酵素抑制剤の
量は用いる特定の抑制剤に依存する。有効な範囲
は約2〜約20重量%、好ましくは約4〜約10重量
%である。
本発明を実施する際の好適な脱炭酸酵素抑制剤
にはN1−dl−セリル−N2−(2,3,4−トリヒ
ドロキシベンジル)−ヒドラジン塩酸塩がある。
流体力学的に平衡化されて制御された放出カプセ
ル剤または錠剤組成物中のN1−dl−セリル−N2
−(2,3,4−トリヒドロキシベンジル)−ヒド
ラジン塩酸塩の量は約3〜約15重量%、好ましく
は約4〜約10重量%の範囲であることができる。
これらの制御された放出組成物中に存在する活
性成分の量に影響を及ぼす因子には十分な治療投
薬量に必要な量、活性成分の嵩密度、活性成分の
親水性または疎水性の特性、安定性などが含まれ
る。これらの特性は公知であり、そして当該分野
に精通せる者には容易に確認されるものである。
L−ドーパ対脱炭酸酵素抑制剤の比は脱炭酸酵
素抑制剤の各1重量部に対するL−ドーパの量と
して約10〜約3部の範囲で変えることができる。
また、L−ドーパ対N1−dl−セリル−N2−
(2,3,4−トリヒドロキシベンジル)−ヒドラ
ジン塩酸塩の比はかなり変えることができる。そ
の比は脱炭酸酵素抑制剤の各1重量部に対してL
−ドーパとして約5〜約3重量部である。好適な
比は約4:1である。
流体力学的に平衡化されて制御された放出組成
物は当該分野において十分に確立された方法で調
製される。多くの場合、必要なことはすべての成
分を均一な混合物となるように十分に混合し、そ
して比較的細かい粒径、即ちすべての粒子が100
メツシユのふるいを通過するように練るか、また
は粉砕することだけである。この混合物、殊にヒ
ドロコロイドを極めて細かい粒経に粉砕すること
は制御された放出機構を損じることはなく、そし
て実際はプラスの効果を与える。ある種の状況下
において、スラツジ化、湿式造粒または押出しの
通常の製薬学的方法が適当なカプセル剤中での充
てん重量を達成するか、または錠剤を調製するた
めに必要とすることができる。しかしながら、胃
液と接触する際にひき続き水和することによる困
難を避けるためにヒドロコロイドをいずれかの顆
粒中に取り込ませるべきではない。カプセル剤に
対しては、混合物を細かい粒径まで粉砕し、そし
てカプセル剤に完全に満たすことが好ましい。
錠剤に対しては、成分を造粒し、次に組成物を
錠剤状に圧縮することが好ましい。錠剤は通常の
錠剤成形(tabletting)装置上で製造される。し
かしながら、この錠剤を胃液と接触する際にこれ
らのものが1より小さい嵩密度を有しなくなる程
度に硬く圧縮すべきではない。従つて、錠剤の硬
さは臨界的であり、そして処方物の最初の密度及
び錠剤の大きさに依存する。なお、本発明の組成
物の有効投与量としては、例えば一般に、後記実
施例1の処方物Aのカプセル剤4〜8個を3〜4
回に分けて投与するのが好適である。
次の実施例は本発明をさらに説明するものであ
る。

びカルボキシメチルセルロースをアルコール中の
ポリビニルピロリドンの一部と共に造粒した。こ

実施例 2
試験管内試験
実施例1のカプセル剤及び錠剤の形態物中、並
びに実施例1の通常のカプセル剤中の流体力学的
に平衡化され制御された放出組成物を試験管内で
L−ドーパ及びN1−dl−セリル−N2−(2,3,
4−トリヒドロキシベンジル)−ヒドラジン塩酸
塩の放出に対して以下の如く評価した。
カプセル剤及び錠剤を瓶の中の胃液60ml中に
個々に置き、そして20rpmで回転させた。用いた
液は1.2のPH値(胃液の通常PH値)であつた。一
定時間後に試料を除去し、そしてL−ドーパ及び
N1−dl−セリル−N2−(2,3,4−トリヒドロ
キシベンジル)−ヒドラジン塩酸塩を分光光度法

N1 -dl-seryl- N2- (2,3,4-trihydroxybenzyl)-hydrazine hydrochloride is a preferred decarboxylase inhibitor of the present invention. An effective range is about 2 to about 20% by weight, preferably about 4 to about 10% by weight. A preferred decarboxylase inhibitor in the practice of this invention is N1 -dl-seryl- N2- (2,3,4-trihydroxybenzyl)-hydrazine hydrochloride.

The amount of -(2,3,4-trihydroxybenzyl)-hydrazine hydrochloride can range from about 3 to about 15% by weight, preferably from about 4 to about 10% by weight. Factors that influence the amount of active ingredient present in these controlled release compositions include the amount required for sufficient therapeutic dosage, the bulk density of the active ingredient, the hydrophilic or hydrophobic properties of the active ingredient, This includes things like stability. This powder mixture was then granulated with polyvinylpyrrolidone in methylene chloride. The granules were dried and the granules were ground. Microcrystalline cellulose, talc and magnesium stearate were added. This composition was mixed thoroughly and poured into capsules. Claims (1)
