Description
本出願と共に提出されたアプリケーションデータシートにおいて外国または国内の優先権主張が確認されたあらゆる全ての出願は、米国特許法施行規則第1.57条の下、参照により本出願に組み込まれるものとする。
本明細書に開示される様々な実施形態は、関節窩ベースプレートを患者の肩甲骨内に移植するための方法及び器具に関する。ガイドピン装着ガイドを使用して関節窩58にガイドピンを取り付け、関節窩58の一部を部分的にリーミングし、関節窩ベースプレート62を関節窩内に移植するために、方法及び器具が使用され得る。患者に適合したアンカーペグ形成ガイドを使用して、関節窩ベースプレートのアンカーペグ用のアンカーチャネルが形成され得る。スクリュー経路ガイドを使用して、関節窩ベースプレートにスクリュー穴が形成され得る。
上記で説明されたように、一部の患者には、関節窩58を部分的にのみリーミングすることが望ましい場合がある。図2は、関節窩ベースプレート262Aを示し、アンカーペグ264が関節窩ベースプレート262Aから延在する。関節窩ベースプレート262Aは、患者に適合した内側を有する患者適合部分267と、リーミングまたは他の方法で修正された関節窩骨表面と嵌合するように構成された平面部分265とを有し得る。患者適合部分267は、図1Bに関連して上記で論じられたように、患者の関節窩58の3Dモデルを使用して形成され得る。平面部分265は、部分リーミング対象面と嵌合するために、実質的に平坦であり得る。例えば、図3に示されるように、関節窩58は、部分リーミング対象部分75及び非リーミング対象部分77を含み得る。部分77はリーミングされていないが、軟骨除去による露出など、本明細書に開示されるガイドを使用して、修正され得る。示されるように、リーミング対象部分75は、関節窩58全体のうちの限られた部分のみである。従って、移植時、平面部分265は、リーミング対象部分75に対して配置され得、患者特有部分267は、関節窩58のリーミングされていない自然な部分77に対して配置され得る。
関節窩58の準備及び関節窩ベースプレート62の移植に使用される器具は、ベースプレート62の適切な移植を保証するために、確実に肩甲骨の好適な部分と正確に位置を合わせることが重要であり得る。図4は、関節窩58を準備する方法の間の関節窩の概略図であり、ガイドピンガイド400が関節窩58の縁68及び/または関節窩58の周りの肩甲骨の部分に対して配置されている。ガイドピンガイド400は、中央部分405と、複数の接触部材とを含み得、複数の接触部材は、中央部分405から延在する突出部または脚部402として形成され得る。脚部402は、脚部402が関節窩58の縁68に対して配置されるように、患者に適合し得る。関節窩58の縁68に対して脚部402を配置することは、ガイド400を解剖学的構造と正確に位置を合わせるのに役立ち得る。
上記で説明されたように、一部の患者には、関節窩58を部分的にのみリーミングすることが望ましい場合がある。図5では、部分リーミングガイド300が関節窩58の表面に対して配置され得、これは、患者適合面302(図3A~図3Cを参照)が関節窩58に対して配置される。部分リーミングガイド300の第1の穴304は、ガイドピン403に沿って誘導され、関節窩58に対して配置され得る。いくつかの実施形態では、第2の穴305は、第2のガイドピン406に沿って誘導され得る。あるいは、ガイド300が関節窩58と接触して配置された後に、第2のガイドピン406が第2の穴305を通して配置され得る。ガイドピン403は、リーミング軸Rに沿って、または平行に配備され得る(図5Aを参照)。第2のガイドピン406は、所望の方位に部分リーミングガイド300の回転位置合わせが行われるように、配置され得る。いくつかの用途では、第2のガイドピン406の所望の位置は、ガイド400の第2の開口部404(またはそのうちの1つ)の位置と同じである。
図6~図6Bは、臨床医が患者の肩甲骨にアンカーペグチャネル605を形成することを支援するように構成されたアンカーペグチャネル形成ガイド600を示す。本明細書で説明されるいくつかの方法では、図4及び図5のうちの1つ以上で示されるステップに続き得る方法において、アンカーペグチャネル形成ガイド600が使用される。言い換えると、部分的にリーミングされた関節窩またはリーミングされていない関節窩に、アンカーペグチャネルを形成するために、ガイド600が使用され得る。
