Description
本発明者らは、TNBC細胞株または乳癌患者由来の異種移植片におけるCDK19の発現または活性をマウスにおいて低下させると、トリプルネガティブ乳癌(TNBC)腫瘍の増殖と転移が阻害されることを見出した。以下の§4(実施例)を参照。本発明者らはまた、CDK19の生物学的機能はそのパラログCDK8の生物学的機能とは異なること、およびTNBC腫瘍に対するCDK19を介した効果はCDK8活性とは無関係であることも示した。これらのデータは、TNBCが、CDK19を阻害するがCDK8を阻害しない薬剤、またはCDK8と比較してCDK19を優先的に阻害する薬剤によって治療できることを示している。CDK19の阻害がTNBCの増殖と転移の抑制に必要かつ十分であるという発見は、一つには、治療標的としてのCDK19の潜在的な利点のために重要である。CDK8、CDK9、およびBRD4などの他のユビキタスな転写補因子と比較して、CDK19の組織分布はより限定的であり、CDK19阻害剤の毒性の低下とより広い治療ウィンドウを示唆している。
2.1:トリプルネガティブ乳癌(TNBC)
トリプルネガティブ乳癌(TNBC)は、エストロゲン受容体(ER)、プロゲステロン受容体(PR)、およびヒト上皮成長因子受容体2(Her2)の発現の欠如を特徴とする乳癌サブタイプである。受容体の発現は、免疫組織化学染色または他の方法で測定できる。TNBCは通常、除外診断される。これらの受容体を標的とする広く使用されている乳癌治療は、TNBCに対して効果的ではなく、TNBC治療を特に困難にしている。
サイクリン依存性キナーゼ19(CDK19)は、Broudeら、Curr. 本明細書で使用する場合、「薬剤」という用語は、CDK19の発現または活性を低下または阻害することができる生体分子(例えば、核酸、タンパク質、ペプチド、抗体)または(例えば、分子量が1000未満、通常500未満の)小さな有機分子をいう。
本明細書で使用する場合、CDK19の文脈で使用される「阻害剤」という用語は、CDK19の発現または活性を低下させる化合物、組成物またはシステムをいう。薬剤はまた、CDK19の発現または活性をCDK8のものよりも選択的に阻害し得る。
本明細書で使用する場合、「ノックダウン」という用語は、CDK19遺伝子の発現レベルの低下を指す。CDK19遺伝子発現レベルをノックダウンするには、該遺伝子に対応するmRNA転写産物の量を減らし、CDK19タンパク質の発現レベルを低下させる。CDK19遺伝子発現レベルをノックダウンすることは、CDK19タンパク質の量を減らすことによっても達成できる。ノックダウン剤は阻害剤の例である。

本明細書で使用する場合、「投与する」または「投与」という用語は、TNBCと診断された対象にCDK19の発現または活性を阻害する薬剤を導入または送達する任意の経路を含む。投与は、薬剤の送達に適した任意の経路によって実施することができる。したがって、送達経路は、例えば、静脈内、筋肉内、腹腔内、または皮下送達を含むことができる。いくつかの実施形態では、薬剤は、例えば腫瘍中への注射によって、腫瘍に直接投与される。
本明細書で使用する場合、「治療有効量」という用語は、対象または患者に所望の生物学的効果を誘発するか、または本明細書に記載のTNBCを有する患者を治療するのに有効な量、例えば医薬用量を指す。「治療有効量」という用語は、疾患、障害、または状態、例えばTNBCをある程度治療し、治療される疾患の1つまたはそれ以上の症状を軽減する、投与される活性剤の量、および/または治療される対象が有するまたは発症するリスクがある疾患の1つまたはそれ以上の症状をある程度防止する量をいう。例えば、所定のパラメーター(例えば、腫瘍容積、腫瘍直径、転移など)について、治療有効量は、少なくとも5%、10%、15%、20%、25%、30%、40%、50%、60%、70%、80%、90%、または100%、または少なくとも1倍、2倍、または3倍の治療効果の増加または減少を示すであろう。治療有効量は、通常、数日、数週間、または数ヶ月の期間にわたって延在する可能性がある治療過程にわたって送達される。薬剤の治療有効量は、単独で、または他の治療剤と組み合わせて摂取され得る。場合によっては、CDK19阻害剤の治療有効量は、TNBC患者の部分的な応答を達成するのに十分な量(例えば、病変の測定可能な直径が20%以上減少、時には30%以上減少)である。

「アンチセンス鎖」は、標的配列(例えば、5'UTR、オープンリーディングフレーム(ORF)のエクソン、または3'UTRを含むヒトCDK8またはCDK19 mRNA)に相補的または実質的に相補的である領域を含む二本鎖RNAi剤(siRNAまたはshRNA)の鎖を指す。「相補的」または「実質的に相補的」な領域は標的配列に完全に相補的である必要はなく、少なくとも70%、75%、80%、85%、90%、95%、または100%の配列%同一性または%類似性であってよい。
本明細書で使用される「センス鎖」は、アンチセンス鎖の領域に相補的または実質的に相補的である領域を含むRNAi剤(siRNAまたはshRNA)の鎖を指す。
1つのアプローチにおいて、本発明は、サイクリン依存性キナーゼ19(CDK19)の発現または活性を阻害する薬剤の治療有効量を投与することを含む、トリプルネガティブ乳癌(TNBC)と診断された患者を治療する方法を提供する。いくつかの実施形態では、治療により、治療開始から6ヶ月以内に腫瘍容積が少なくとも10%減少する。
3.1.1:ポリヌクレオチド
実施例で示されるように、CDK19遺伝子はTNBCの増殖に不可欠である。本明細書に記載されるような対象におけるTNBCを治療する方法は、CDK19遺伝子の発現を減少または阻害するために対象にポリヌクレオチド(例えば、オリゴヌクレオチド)を投与することにより達成され得る。いくつかの実施形態では、ポリヌクレオチドは、例えば、DNAオリゴヌクレオチドまたはRNAオリゴヌクレオチドであり得る。他の実施形態では、オリゴヌクレオチドは、CRISPR/Casシステムで使用されてよい。CDK19遺伝子の発現を阻害または減少させるオリゴヌクレオチドは、対象においてCDK19遺伝子(例えば、TNBC細胞におけるCDK19遺伝子)をノックアウトまたはノックダウンすることができる。
ショートヘアピンRNAまたはスモールヘアピンRNA(shRNA)は、ヘアピンターンを備えた人工的なRNA分子であり、細胞内で生成された低分子干渉RNA(siRNA)を介して標的遺伝子の発現を抑制することができる。例えば、Fireら、Nature 391:806-811, 1998; Elbashirら、Nature 411:494-498, 2001; Chakrabortyら、Mol Ther Nucleic Acids 8:132-143, 2017;Bouardら、Br. J. Pharmacol. 157:153-165, 2009を参照。細胞におけるshRNAの発現は、典型的にはプラスミドの送達によって、またはウイルスベクターまたは細菌ベクターを介して達成される。適切な細菌ベクターには、アデノ随伴ウイルス(AAV)、アデノウイルス、およびレンチウイルスが含まれるが、これらに限定されない。ベクターが宿主ゲノムに組み込まれると、プロモーターの選択に応じて、shRNAがポリメラーゼIIまたはポリメラーゼIIIによって核で転写される。その結果生じるプレshRNAは、核から転出され、ついでダイサーによってプロセシングされ、RNA誘導サイレンシング複合体(RISC)にロードされる。センス鎖はRISCによって分解され、アンチセンス鎖はRISCを相補的な配列を持つmRNAに誘導する。次に、RISC中のAgo2と呼ばれるタンパク質がmRNAを切断するか、場合によっては、mRNAの翻訳を抑制し、その結果、その破壊およびmRNAによってコードされるタンパク質の最終的な減少に導く。したがって、shRNAは標的遺伝子のサイレンシングにつながる。shRNAは、分解および代謝回転の速度が比較的遅いという点で、siRNAの有利なメディエーターである。
shRNAの有効性に影響を与える可能性のある多数の構造要素がある。所望の標的遺伝子の特定のRNAiノックダウンのために、その複数の構造要素を考慮してshRNAを設計することができる。一般に、shRNAは約80ヌクレオチドの長さであり、shRNAのヘアピン構造を作るために(5'から3'へ)次の構造要素で構成されるように設計されている:(1)センス鎖(たとえば、上部ステム);(2)続いてヘアピンループ;(3)標的mRNAに完全にまたはほぼ完全に相補的であり、標的mRNAに対してアンチセンスであるアンチセンス鎖(例えば、下部ステムまたはガイド鎖);(4-5)ガイド鎖の最初の位置での「U」または「UH」、およびガイド鎖のテール領域での「UUC」または「CUUC」などの2つの切断モチーフ;および(6)ガイド鎖の最初のヌクレオチド「U」モチーフとヘアピンループの間、およびセンス鎖の最後のヌクレオチドとヘアピンループの間の、長さが約2ヌクレオチドの任意のスペーサーヌクレオチド。ステムを構成するセンス鎖およびアンチセンス鎖は、長さが約19~29ヌクレオチドの範囲からなるように設計することができ、これがステムを形成する。ループ構造は、長さが約2~15ヌクレオチドの範囲からなるように、そして好ましくはいかなる内部二次構造も含まないように設計され得る。ヘアピンループを作製するために使用することができる配列のいくつかの例には、配列(TTCAAGAGA)を含む9ヌクレオチドループ、および配列(TCAAGAG)を含む7ヌクレオチドループが含まれるが、これらに限定されない。その他の設計戦略は、Ros XB-D、Gu S、miRNAベースのshRNAの最適設計に関するガイドライン.Methods (San Diego, Calif) 2016;103:157-166(これは、すべての目的のために参照によりその全体が本明細書に組み込まれる)の関連する開示に記載されている。Sigma-AldrichやThermo-Fisher Scientificから入手できるBroad InstituteのRNAi Consortiumソフトウェアなどのいくつかの設計プログラムも利用できる。
本開示はまた、トリプルネガティブ乳癌の患者においてCDK8およびCDK19の発現を阻害するためのsiRNAベースの治療法を提供する。二本鎖RNAi治療薬は、アンチセンス鎖に相補的なセンス鎖を含む。siRNAのセンス鎖またはアンチセンス鎖は、約14、15、16、17、18、19、20、21、22、23、24、25、26、27、28、29、または30ヌクレオチド長であり得る。siRNAベースの治療薬のアンチセンス鎖は、CDK8またはCDK19をコードするmRNAの一部に相補的な領域を含む。治療用siRNAを作製するためのさらなる方法は、米国特許第9399775号に見出すことができ、これは、すべての目的のためにその全体が参照により組み込まれる。
一過性かまたは安定した発現のいずれが望ましいかに応じて、適切な送達ベクターを選択することができる。本開示で使用され得る送達ベクターの例は、ウイルスベクター、プラスミド、エキソソーム、リポソーム、細菌ベクター、またはナノ粒子である。本開示はまた、当技術分野で公知の任意の手段による送達を提供する。
RNase H依存アンチセンスオリゴヌクレオチド(ASO)は、標的mRNAの相補配列に結合し、RNase Hを介した標的RNAの切断とリボソームの立体障害との両者による翻訳の阻害との両方によって遺伝子発現を低下させる一本鎖の化学修飾オリゴヌクレオチドである。
マイクロRNA(miRNA)は、RNAサイレンシングおよび遺伝子発現の転写後調節で機能する小さな非コードRNA分子である。miRNAは、mRNA転写産物内の相補的な配列と塩基対を形成する。その結果、mRNA転写産物は、mRNA鎖の切断、ポリ(A)テールの短縮によるmRNAの不安定化、およびリボソームによるmRNA転写産物のタンパク質への翻訳効率の低下などの1つまたはそれ以上の機構によってサイレンシングされる。いくつかの実施形態では、miRNAは、それ自体が折り返されて短いヘアピンを形成するRNA転写産物の領域に由来することを除いて(これは、pri-miRNAとも呼ばれる)、shRNA経路のsiRNAに似ている。pri-miRNAとして転写されると、ヘアピンはDroshaと呼ばれる酵素によって核内の一次転写産物から開裂される。ついで、ヘアピンまたはpre-miRNAは、核からサイトゾルに輸送される。サイトゾルでは、ヘアピンのループがDicerと呼ばれる酵素によって切断される。結果として得られる産物は、3'末端にオーバーハングを持つ二本鎖RNAとなり、RISCに組み込まれる。RISCに入ると、2番目の鎖は破棄され、現在RISCにあるmiRNAは成熟したmiRNAであり、相補的な配列を持つmRNAに結合する。
いくつかの実施形態では、CDK19遺伝子のノックアウトまたはノックダウンは、CRISPR/Casシステムなどの遺伝子編集システムを使用して実行される。SandersおよびJoung, Nature Biotechnol 32:347-355, 2014, Huangら、J Cell Physiol 10:1-17, 2017およびMitsunobuら、Trends Biotechnol 17:30132-30134, 2017を参照。CRISPR/Casシステムは、Casタンパク質と、Casタンパク質をCDK19配列内の標的モチーフに誘導し、ハイブリダイズできる少なくとも1つまたは2つのリボ核酸を含む。次にCasタンパク質は標的モチーフを切断し、二本鎖切断または一本鎖切断をもたらす。細胞内の標的ポリヌクレオチド配列を変更できるいかなるCRISPR/Casシステムも本明細書で説明する方法で使用できる。いくつかの実施形態では、CRISPR/Casシステムは、CRISPR I型システムである。いくつかの実施形態では、CRISPR/Casシステムは、CRISPR II型システムである。いくつかの実施形態では、CRISPR/Casシステムは、CRISPR V型システムである。
抗体またはそのフラグメントの意味には、例えば米国特許第4,704,692号(その内容は参照によりその全体が本明細書に組み入れられる)に記載されている抗体フラグメントと抗原結合タンパク質(一本鎖抗体)の複合体も含まれる。
1つのアプローチにおいて、TNBCを治療する方法は、CDK19活性の小分子阻害剤を使用して、CDK19タンパク質を標的化することを含む。CDK19の小分子阻害剤の例は、米国特許第9,321,737号、米国特許公開第US20170071942号、Mallingerら、J. Med. Chem. 59:1078, 2016、およびCzodrowskiら、J. Med. Chem. 59:9337, 2016に記載されている。いくつかの実施形態では、小分子阻害剤は、CDK19のATP結合部位に結合してその活性を阻害する。
CDK19の発現または活性を阻害するがCDK8の発現または活性を阻害しない薬剤、またはCDK8の発現または活性が阻害されるよりもCDK19の発現または活性を一層阻害する薬剤は、CDK19およびCDK8タンパク質およびそれらに対応する遺伝子およびmRNAの配列および構造の違いに基づいて設計することができる。例えば、CDK19およびCDK8 mRNA配列のアラインメントは、shRNAまたはCDK19 mRNAと関連するがCDK8配列とは関連しない他の核酸物質を設計するために使用できる非同一または低同一性ヌクレオチド配列を同定できる(図16および17を参照)。同様に、CDK19およびCDK8アミノ酸配列のアラインメントは、相違する領域を特定でき、CDK19タンパク質に特異的に結合する抗体または他の結合剤を生成できる。同様に、CDK19活性を特異的に阻害するかまたはCDK19活性をCDK8活性よりも一層阻害する小分子薬剤を(合理的な薬物設計またはスクリーニングによって)同定することができる。
1つのアプローチでは、患者は、CDK19の発現または活性を阻害する薬剤および(a)放射線療法および/または化学療法を含む併用療法で治療される。1つのアプローチでは、放射線または化学療法は腫瘍塊の大部分を排除し、CDK19阻害剤は生存可能な癌幹細胞(例えば、EpCAMmed/high/CD10-/low)細胞の数を減らす。1つのアプローチでは、化学療法は、アントラサイクリン(例えば、ドキソルビシンまたはエピルビシン)、タキサン(例えば、パクリタキセルまたはドセタキセル)、代謝拮抗物質(例えば、カペシタビンまたはゲムシタビン)、プラチナ剤(例えば、カルボプラチンまたはシスプラチン)、ビノレルビン、またはエリブリンの投与を含む。
CDK19阻害剤による一連の治療は、例えば腫瘍容積の減少、転移の減少、および表現型EpCAMmed/highおよびCD10-/lowを有する腫瘍細胞の減少など、患者にとって有益な結果になる。
本明細書に記載の方法で使用される医薬組成物は、当業者に知られている方法により製剤化することができる、有効成分および1つまたはそれ以上の医薬的に許容される担体または賦形剤を含むことができる。いくつかの実施形態では、本発明の医薬組成物は、治療上有効な量で、CDK19遺伝子の発現レベルを低下させるDNAまたはRNAオリゴヌクレオチドを含む。他の実施形態では、本発明の医薬組成物は、治療有効量のDNAまたはRNAオリゴヌクレオチド、CDK19の活性を阻害する小分子を含む、本発明の医薬組成物を包含する。医薬組成物中の有効成分の治療有効量は、疾患の症状を予防、緩和、または改善するのに、または治療されている対象の生存を延長するのに十分である。治療有効量の決定は、当業者の能力の範囲内である。
4.1:実施例1-材料および実験方法
化学試薬:
以下は、この研究で使用した化合物の化学名である。CCT152921は、4-[(2-フェニルエチル)アミノ]-6-キナゾリンカルボニトリル(NIH NCAT)である。この化合物をビヒクル(PBS+0.5%Methocel(w/v)+0.25%Tween 20(v/v))に3mg/mLの濃度に再懸濁し、マウスに30mg/kgを投与した。CCT251921またはビヒクルは、毎日強制経口投与した。
センス標的配列、15-merループ配列(5'-GTTAATATTCATAGC-3':配列番号19)、およびセンス配列の逆相補体を含む相補的ssDNAオリゴヌクレオチドのペアを合成した(Elim Biopharmaceuticals)。オリゴヌクレオチドを、50μMアニーリング緩衝液(10mM Tris-HCl pH8.0、50mM NaCl、1mM EDTA)でアニーリングした。その後、二本鎖DNAオリゴテンプレートを、構成的に活性なshRNAベクター構築物のためのBbsIで消化したpRSI12-U6-(sh)-HTS4-UbiC-TagRFP-2A-Puro shRNA発現ベクター(Cellecta)および誘導可能なshRNAベクター構築物のためのBbsIで消化したpRSITUR-U6Tet-(sh)-UbiC-TetRep-2A-TagRFPにクローニングした。この研究で使用したshRNAベクターのセンス鎖は、5'-GCG AGA ATT GAA GTA CCT TAA-3'(shCDK19-1(配列番号1))、5'-ACC AGC AAA TAT CCT AGT AAT-3'(shCDK19-2(配列番号2))、および5'-GCA GGG TAA TAA CCA CAT TAA-3'(shCDK8-2(配列番号3))であった。未修飾の上記のpRSI12-U6-(sh)-HTS4-UbiC-TagRFP-2A-Puro shRNA発現ベクターは「空の」対照shRNAとして使用した。pHIV-ZsGreen発現ベクター(Addgene)を使用してGFP陽性腫瘍細胞を作製した。RNAiスクリーニングに使用されるDECIPHER 27K Pooled shRNAレンチウイルスライブラリー-Human Module 1 (Cellecta)には、同じpRSI12 shRNA発現ベクター内に5,043のヒト遺伝子を標的とする27,500の固有のshRNA構築物(遺伝子当たり約5または6の冗長shRNA)が含まれている。
MDA-MB231、MDA-MB468、HS578T、および293T細胞をATCCから入手した。HMEC細胞はThermoFisher Scientificから入手した。これらの細胞は、マイコプラズマを含まないことが供給業者から認定されている。使用した細胞株はいずれも、ICLACが管理している一般的に誤認されている細胞株のデータベースにリストされていない。使用したすべての細胞株は、供給業者からの元の細胞が解凍されてから10回未満継代した。MDA-MB231、MDA-MB468、293T、およびHS578T細胞はすべて、PSA(Life Technologies)、10%FBS(Hyclone)、Glutamax(ThermoFisher Scientific)、およびピルビン酸ナトリウム(Life Technologies)を補充したDMEM(Invitrogen)で増殖させた。HMEC細胞はHuMEC Ready Medium(ThermoFisher Scientific)で増殖させた。
Nod scidガンマ(NSG)マウス(NOD.Cg-PrkdcscidIL2Rgtm1Wjl/SzJ)をJackson Laboratoryから購入した。PDX実験に使用したマウスは、8~10週齢の成体雌マウスであった。この研究で使用されたすべてのマウスは、スタンフォード大学APLAC委員会によって承認されたプロトコルに従って、スタンフォード動物施設で維持された。マウスは無菌の滅菌条件下、社内で維持した。マウスにオートクレーブした食物と水を投与した。ドキシサイクリン誘導性構築物を利用するPDX実験のため、マウスに通常の齧歯類の餌の代わりに625mgのドキシサイクリンヒクレート/kg餌(Envigo)を含む齧歯類の餌を与えた。
ヒトの試料については、スタンフォード大学の機関審査委員会およびシティー・オブ・ホープによるプロトコルの承認後に、インフォームドコンセントが得られた。この研究で使用されたすべてのPDX腫瘍の完全な説明については、図15を参照。
異種移植片をかみそりの刃で機械的に約1mmに細断し、37℃で3~4時間、120μg/mLのペニシリン、100μg/mLのストレプトマイシン、0.25μg/mLのアンフォテリシンB(PSA)(Life Technologies)を含むAdvanced DMEM/F12(Invitrogen)中、コラゲナーゼおよびヒアルロニダーゼ(Stem Cell Technologies)とともにインキュベートした。次に、細胞をACK溶解緩衝液(Gibco)で処理して赤血球を溶解し、その後、予熱したディスパーゼ(Stem Cell Technologies)とDNAseI(Sigma)で5分間処理し、40μmナイロンメッシュフィルターでろ過した。最後に、細胞をフローサイトメトリー緩衝液(HBBS、2%FCS、PSA)で洗浄した。
PDX腫瘍が単一の細胞に解離した後、生細胞の数をトリパンブルー染色で決定し、血球計算盤で手動にてカウントした。細胞をフローサイトメトリー緩衝液で106生細胞/mLの濃度に再懸濁し、ビオチン抗ヒトCD326(EpCAM)抗体(Biolegend)と1:50(v/v)にて4℃で20分間インキュベートした。細胞をフローサイトメトリー緩衝液で洗浄し、80μLに再懸濁し、20μLの抗ビオチンマイクロビーズ(Miltenyi Biotec)と4℃で20分間インキュベートした。次に、細胞をフローサイトメトリー緩衝液で洗浄し、500μLの緩衝液に再懸濁した。製造業者のプロトコルに従い、細胞を磁化したLSカラム(Miltenyi Biotec)に適用し、洗浄し、磁石から溶出させた。
レンチウイルスは、パッケージングプラスミドミックス(Cellecta)を用いて製造し、293T細胞でLipofectamine 2000(Thermofisher Scientific)を使用し、製造業者の指示に従ってpRSI12shRNA発現プラスミドにサブクローニングした。上清を48時間後と72時間後に収集し、0.45μmフィルターでろ過し、製造業者の指示に従ってレンチウイルス沈殿溶液(Alstem LLC)で沈殿させた。ウイルスを元の量の1/100に再懸濁した。ウイルス力価は、濃縮ウイルスの連続希釈液に感染した293T細胞のフローサイトメトリー分析によって決定した。
インビトロ細胞株実験では、播種時に濃縮レンチウイルス上清(MOIが3になるように)を細胞と混合した。細胞は、蛍光顕微鏡下でRFPを視覚化することによりモニターした。すべてのフローサイトメトリー分析は、感染から少なくとも72時間後に行った。
照射されたL1-Wnt3aフィーダー細胞(Dr. Roel Nusseからの気前の良い贈与品)を成長因子減少マトリゲル(BD Biosciences)と混合し、37℃で固化した。PDX腫瘍細胞の単一細胞懸濁液を、固化したマトリゲル/フィーダー細胞混合基質に移し、オルガノイド培地で増殖させた。細胞を、5%CO2を含む37℃のインキュベーターで約2週間培養した。培地の50%を3~4日ごとに新しい培地と交換した。コロニーを蛍光顕微鏡下でカウントした。RFP陽性コロニー(形質導入細胞を表す)のみをカウントした。CDK19 shRNAの発現を誘導する実験では、ドキシサイクリンヒクレートを100ng/mLの最終濃度で培地に添加した。
化学物質で処理したまたはレンチウイルスに感染した細胞株について、製造業者の指示に従い、WST-1細胞増殖試薬(Roche)を各ウエルに1:10(v/v)の最終希釈で加えた。その後、細胞を37℃、5%CO2でインキュベートした。試薬添加後1~4時間の間に、プレートをSpectraMax M3バイオアナライザー(Molecular Devices)で分析した。各ウエルの吸光度を450nm(シグナル波長)および650nm(参照波長)で測定した。したがって、各実験試料のシグナルは、吸光度experimental(A450nm-A650nm)であった。培地の影響を補正するために、ブランクウエルの吸光度を測定することにより、吸光度background(A450nm-A650nm)を得た。したがって、各試料のバックグラウンド補正シグナルAcorrectedは吸光度experimental-吸光度backgroundであった。ノックダウンのすべての補正値を対照試料の補正値に正規化し、「相対生存率」を得た。
細胞をTrizol(Life Technologies)で溶解し、製造業者の指示に従ってRNAを抽出した。次にRNAをDNAseIで処理して、混入しているゲノムDNAを除去した。製造業者の指示に従い、SuperScript III First Strand Synthesis Kit(Life Technologies)を使用してRNAをcDNAに逆転写した。製造業者の指示に従い、TaqMan Gene Expression Master Mix(Applied Biosystems)および次のTaqMan Gene Expression Assays(Applied Biosystems)を使用した:ACTB,Hs00357333_g1;CDK19,Hs01039931_m1;CDK8,Hs00993274_m1。データは7900HT Fast Real-Time PCR System(Applied Biosystems)で収集され、データはSDS2.4ソフトウェア(Applied Biosystems)で分析された。各試料の遺伝子発現データは、ベータアクチンの発現に対して正規化した。
PDX細胞の単一細胞懸濁液を、通常のマトリゲル(BD Biosciences)とフローサイトメトリー緩衝液の50%(v/v)混合物に再懸濁し、総量を50~100μLにした。インスリン注射器を用い、細胞を4番目の腹部脂肪体でメスのNSGマウスの乳頭に皮下注射した。限界希釈アッセイのため、マウスに注入された特定の数の細胞をフローサイトメトリーにより、および第二に血球計算盤で手動にてカウントして決定した。
PDX腫瘍は触診により検出された。腫瘍容積は、長さ(l)と幅(w)を測定し、楕円体の計算式1/6x1xw2xπを使用して容積を計算することによって決定した。腫瘍の容積およびマウスの体重は週に2回測定した。
マウスを安楽死させた後、異種移植腫瘍およびマウス肺を外科的に切除した。各腫瘍から3~4mmの切片を切り取り、画像化のため氷冷PBSに保存した。マウスの肺および腫瘍をM205FA蛍光実体顕微鏡(Leica)で画像化し、画像はDFC310FXカメラ(Leica)で記録した。
フローサイトメトリーは、100μmノズルで、フローサイトメトリーAria II(BD Biosciences)とDivaソフトウェア(BD Biosciences)を使用して行った。データ分析は、Flowjoソフトウェア(Flowjo)を用いて行った。すべての実験で、側方散乱プロファイルおよび前方散乱プロファイル(面積および幅)を用いて、残屑および細胞ダブレット(debris and cell doublets)を排除した。死んだ細胞は、4',6-ジアミジノ-2-フェニルインドール(DAPI)陽性細胞(分子プローブ)を除外することにより排除した。PDX腫瘍細胞については、GFP陽性、次にRFP陽性にゲートした(gated)。RFPパーセンテージは、RFP陽性でもあるGFP陽性細胞のパーセンテージである。各試料について、腫瘍のRFP%をベースラインRFP%で除したものであるRFP画分を取得する(「レンチウイルス感染」のセクションを参照)。次に、各試料のRFP画分を、100%に設定されたshRNA対照試料のRFP画分に正規化して、「正規化された%RFP」を取得する。
分析およびセルソーティングのためのフローサイトメトリーは、以前の記載に従って行った。使用したヒト抗体には以下が含まれていた:EpCAM-Alexa Fluor 488(クローン9C4、Biolegend);1μgmL-1、CD49f-APC(クローンGoH3、Biolegend);CD10 PeCy7/Apc-Cy7(クローンH110a、Biolegend);1μgmL-1およびH-2Kdビオチン/パシフィックブルー(クローンSF1-1.1、Biolegend);1μgmL-1。
NSGマウスで増殖したGFP陽性PDX-T1腫瘍を切開し、単一細胞に加工し、前述のようにEpCAMで濃縮した。この時点での細胞の分析は、それらが約98%-100%のGFP陽性であることを示した。
概要について図5Cの模式図を参照。本発明者らは、アルゴリズムを適用して、検証のためにヒットをより扱いやすい数に絞り込んだ。1)個々のshRNAについて、「ドロップアウト比」を決定した。これは、増殖実験試料のshRNAバーコード数をベースライン参照試料のshRNAバーコード数で除したものであった。各スクリーニングにおいて、これらを最低から最高にランク付けした。2)本発明者らは、各実験で最も低いドロップアウト率の上位5%を調べ、>2shRNAによりターゲットした遺伝子を同定した。3)本発明者らは、2つの実験間で重複した遺伝子を同定するために、インビボスクリーニングのshRNA遺伝子標的(208遺伝子)をインビトロスクリーニングのshRNA遺伝子標的(150遺伝子)と相互参照した。これらの46個の重複する「ヒット」遺伝子を図5Aに示す。
PDX腫瘍の切片をホルマリンで一晩固定し、70%エタノールに移した。次に試料をパラフィンに包埋し、組織学用に切片を作成した。ホルマリン固定パラフィン包埋切片をキシレンで脱パラフィンし、エタノール勾配で再水和した。電子レンジで20分間加熱することにより、抗原検索をTris-EDTA緩衝液で行った。一次抗体であるポリクローナルウサギ抗CDK19抗体(Sigma)とポリクローナルニワトリ抗CDK8抗体(Novus Biologicals)を、試料に一晩適用する前に、TBS+1%BSAでそれぞれ1:50と1:100に希釈した。一晩のインキュベーション後、二次抗体であるCy3ロバ抗ウサギ抗体(Jackson ImmunoResearch)およびAlexa488ヤギ抗ニワトリ抗体(Life Technologies)をTBS+1%BSAで1:500に希釈し、試料と室温でインキュベートした。DAPI染色後、ProLongRGold退色防止剤(antifade)(Cell Signaling)で切片をマウントした。Zeiss LSM710共焦点顕微鏡(Carl Zeiss)を使用して、免疫蛍光画像を撮影した。出版用の画像はFijiソフトウェアで処理した。
EpCAM濃縮PDX-T1細胞をshCDK19-2、shCDK8-2または対照shRNAに感染させ、オルガノイド培養条件で72時間培養した。その後、細胞をディスパーゼでマトリゲルから回収し、フローサイトメトリー緩衝液に再懸濁し、フローサイトメトリーでソーティングしてGFPとRFPの両方が陽性の細胞を得た。製造業者の指示に従ってRNeasy plus micro kit(Qiagen)によってこれらの細胞からRNAを抽出し、Agilent 2100バイオアナライザーで定量化した。各試料からの50ngの全RNAを使用した。インビトロ転写、断片化、標識、マイクロアレイへのハイブリダイゼーション、およびスキャニングは、スタンフォードタンパク質および核酸施設(PAN施設)によって行った。試料はPrimeView Human Gene Expression Arrays(Affymetrix)でハイブリダイズした。遺伝子レベルの差異的発現分析は、Transcriptome Analysis Console(Affymetrix)で行った。ダウンレギュレーションされた遺伝子はlog2(試料/対照)<-1.5の遺伝子として、アップレギュレーションされた遺伝子はlog2(試料/対照)>1.5の遺伝子として定義した。
ChIPアッセイは、例えば、Zarnegarら、Nucleic Acids Research, gkx648, July, 2017の記載に従って行った。ChIP当たり約250,000~500,000個のMDA-MB231細胞を使用した。ChIP当たり1μgの抗H3K27ac(Active Motif#39133)を使用した。
ChIP濃縮DNAは、Qubit3.0およびdsDNA HSアッセイを使用して定量化した。転位ベースのNEXTERA XTを使用し(製造業者のプロトコルに従い、インデックス作成のために~14PCRサイクルを使用)、最大1ngのDNAをライブラリー構築に使用した。インデックス付きの試料をプールし、NextSeq500でのシーケンシングのために送り、読み取り深度が~6千万リードの75bpシングルエンドリードを取得した。
生の配列読み取りをGalaxy(usegalaxy.org)にアップロードし、Bowtie2(-very-fast-local)を使用してヒトゲノム(hg19)にアラインメントした。一意にマッピングされた読み取りのみをさらなる分析のために保持した。データを視覚化するため、アライメントファイルを使用してDeepToolsでシグナルトラックを生成し(100bpの瓶に200bpの読み取り拡張子およびRPKM正規化)、BigWigファイルをBroadの統合ゲノムブラウザーにロードした。MACS2を使用して、各複製のピーク(-nomodel、p=0.01、-broad、cuttoff0.1、duplicates=auto、extension200)を呼び出した。Bedtoolsを使用して、すべての複製で生じるピークのみを含むコンセンサスピークリストを生成した。本発明者らは、DiffBindを使用してコンセンサスピーク全体の差異的ピーク分析を行った。UCSCからのヒトRefSeq表の最も近い重複しない特徴をフェッチすることにより、DiffBind出力ピークリストに注釈が付けられた。さまざまなピークセットにわたるChIPシグナルの集約プロットのデータは、DeepToolsのcomputeMatrix(スケール領域:1000;50bp瓶)およびplotProfileを使用して生成された。次に、GraphPad Prismソフトウェアを使用してデータをプロットした。
遺伝子セットエンリッチメント解析(GSEA)は、javaGSEAデスクトップアプリケーション(GSEA 3.0)を使用して実行され、ランキングメトリックとしてのCDK19ノックダウン対対照のlog2倍数変化値、およびエンリッチメントのために試験された遺伝子セットとしてのHallmarks、CDK19KD-EnhancerUpおよびCDK19KD-EnhancerDOWNを使用した。
CDK19KD-EnhancerUPの「コア」遺伝子とCDK19KD-EnhancerDOWNの「コア」遺伝子を使用した特徴遺伝子セットのMetascapeカスタムエンリッチメント分析(次のパラメーターを使用:入力種としてのH. Sapiens、0.01のp値カットオフ、および最小エンリッチメント1.5)をオンライン(www.metascape.org)で行った。
結果を平均値±標準偏差として示す。統計計算は、GraphPad Prismソフトウェア(GraphPad Software Inc)を使用して行った。分散は、F検定を使用して分析した。P値を決定するため、t検定を同分散集団で行い、ウェルチ補正付きのt検定を異なる分散のサンプルに適用した。動物実験では、サンプルサイズは事前に指定された効果サイズを検出するのに十分な検出力を確保するために事前に決定されず、分析から除外された動物はなく、実験は無作為化されておらず、研究者は実験中のグループ割り当てを知らされていなかった。
TNBCの増殖に不可欠な遺伝子を同定するため、TNBC PDXであるPDX-T1の5000遺伝子をターゲットとする27,500shRNAライブラリーを使用して、2つのプールされたRNAiドロップアウト生存スクリーニングを行った(図15)。スクリーニングは、インビトロでのオルガノイド培養として、およびインビボでのnod scidガンマ(NSG)マウスにおけるPDXとしての2つの異なるフォーマットで行われた(図1A)。各実験試料およびベースライン参照試料における個々のshRNAの存在量は、shRNAバーコードのハイスループットシーケンスによって決定された。目標は、shRNAによるノックダウンがさまざまな実験条件でPDX腫瘍細胞の増殖を阻害する遺伝子を特定することであった。他の腫瘍におけるスクリーニングと一致して、インビボスクリーニングは、インビトロスクリーニング(図5B)と比較して、より有意なshRNAドロップアウト率を有した(図5A)。図5Aおよび5Bは、ベースライン試料におけるshRNA数に対するインビボ増殖実験試料(図5A)およびインビトロ増殖実験試料(図5B)におけるshRNA数を示すグラフである。ルシフェラーゼを標的とする対照shRNA(薄い灰色のドット)とCDK19を標的とするshRNA(暗い灰色のドット)を強調表示してある。他のshRNAはすべて黒いドットで示してある(各実験は1回行った)。最終的な候補リストは、2つ以上のshRNAによってターゲットにされ、インビトロとインビボの両方で同定された、各スクリーニングで最低5%のshRNA比を持つ遺伝子に限定した(図5C)。これにより、46個の候補遺伝子が同定された(図5D)。
CDK19ノックダウンの増殖阻害効果を検証するために、3つの一般的に使用されるTNBC細胞株:MDA-MB231、MDA-MB468、およびHS578Tを使用した。独立してCDK19を標的とする2つの異なるshRNA(shCDK19-1(配列番号1)およびshCDK19-2(配列番号2))を使用して、CDK19のノックダウンを確認した(図7Aおよび7B)。図7Aおよび7Bの両方の図について、CDK19ノックダウン細胞におけるCDK19の相対発現は、対照shRNAを形質導入した細胞におけるCDK19の平均発現に対して正規化される。各条件での遺伝子発現は、ハウスキーピング遺伝子としてのベータアクチンに正規化される(**P<0.01;****P<0.0001、平均±標準偏差、(図7Aおよび7B)n=3(図7C)n=2、実験は2回行った)。CDK19のノックダウンはまた、3つのすべてのTNBC細胞株において増殖の低下を引き起こすことも示した(図1B~1D)。図1B~1Dは、CDK19ノックダウンがTNBC細胞の生存を大幅に減少させたことを示している(MDA-MB231細胞の生存率、****P<0.0001(図1B)、MDA-MB468細胞、***P<0.001;****P<0.0001(図1C)、またはHS578T細胞、*P<0.05;****P<0.0001(図1D);対照shRNAまたはCDK19標的shRNA(shCDK19-1、shCDK19-2)での形質導入の4日後に評価)。図1B~1Dのすべての値は、対照shRNA試料に正規化された(平均±標準偏差、n=3、実験は2回行われ、P値は対応のないt検定によって決定された)。
CDK19ノックダウンが、発癌性を評価するために一般的に使用される2つの独立したアッセイ(インビボでのPDX増殖とインビトロでのオルガノイドコロニー形成)で増殖を阻害し、腫瘍の開始に重要な遺伝子がしばしば癌のサブセットで増幅または過剰発現されるとすると、腫瘍開始細胞(TIC)はCDK19阻害に感受性があると仮定される。それゆえ、本発明者らは、TNBC PDX内のTICを同定しようとした。以前は、正常な***組織内および乳癌内の細胞亜集団を分離するのにEpCAMとCD49fが使用された。しかしながら、多くのTNBC PDXでは、EpCAMおよびCD49fはしばしば細胞を明確に別個の亜集団に分離することができない(図2A、左)。したがって、本発明者らは、基底細胞マーカーであるCD10とEpCAMを併用して、乳癌PDXをFACSでソートした。本発明者らは、CD10およびEpCAMがPDX細胞を3つの異なる亜集団、EpCAMmed/high/CD10-/low、EPCAMlow/med/CD10low/+、およびEpCAM-/CD10-に分離できることを発見した(図2A、右)。図2Aにおいて、EpCAMとCD49fを使用して見られる大きな分離できない細胞集団(左)は、EpCAMとCD10を使用して3つの異なる亜集団になる(右):EpCAMmed/high/CD10-/low(ゲート(1))、EPCAMlow/med/CD10low/+(ゲート(2))およびEpCAM-/CD10-(ゲート(3))。EpCAMおよびCD49fを使用したこれらの3つの亜集団の重複も示す(図8A)。
CDK19とそのよく説明されているパラログであるCDK8との間には84%のアミノ酸配列相同性がある(図9)。CDK8は、結腸癌、急性骨髄性白血病、および黒色腫などのさまざまな悪性腫瘍に役割を果たすことが示されている。CDK8の高発現は、結腸癌の予後不良と関連している(Firesteinら、Nature 455:547-551, 2008)。胚性幹細胞におけるCDK8ノックアウトは、多能性幹細胞の表現型におけるその本質的な役割により、胚発生を防止することが示された(Porterら、Proc Natl Acad Sci USA, 109:13799-13804, 2012)。CDK8の既知の癌関連の活性には、Wnt/β-カテニン経路の正の調節、成長因子誘導転写、およびTGFPシグナル伝達が含まれる。状況に応じて、CDK8は転写を負または正に制御することも示されている。しかしながら、最近の証拠は、CDK19がCDK8とは異なって機能することが示唆されている。CDK19とCDK8とが他のCKM成員への結合に相互に排他的に関与することをインビトロ研究が示したのに対し、子宮頸癌および結腸癌の細胞株での遺伝子ノックダウン研究は、CDK19とCDK8とが異なる遺伝子を制御することを示した。本発明者らの目標は、TNBCにおいて、CDK19またはCDK8の標的ノックダウンから生じる包括的な遺伝子発現の変化を調べることにより、CDK19およびCDK8が異なる生物学的機能を持っているかどうかを調べることであった。
最近の研究では、ヒストン3リジン27アセチル化(H3K27Ac)でマークされる、エンハンサー(「スーパーエンハンサー」とも呼ばれる)の大きなクラスターで作用することにより重要な発癌性遺伝子の転写を調節する上で、CDK19とCDK8、およびその他の転写CDK(CDK7、CDK12/CDK13)の役割が強調されている。この遺伝子調節の正確なメカニズムは不明であるが、一部は、RNAPII-メディエーター相互作用を調節するメディエーターとのCKMの相互作用を介して、および一部は、RNAPIIのC末端ドメインのセリン残基をリン酸化することによって生じると考えられている。エンハンサーで機能する転写CDKの傾向を考慮して、本発明者らは、CDK19およびCDK8が、遺伝子発現を制御するメカニズムとしてエンハンサー部位でのエピジェネティックな修飾を調節できるかどうかを調べたいと考えた。他のシグナル伝達経路を介したエンハンサー修飾は同定されているが、この遺伝子制御のメカニズムはCDKについては未だ報告されていない。
本発明者らは、CDK19ノックダウンが事前に確立されたオルガノイドのインビトロでの増殖と事前に確立されたPDX腫瘍にインビボで及ぼす影響を調べた。これは、患者の既存の腫瘍の治療をモデル化することを目的としていた。インビトロでは、(CDK19 shRNAを誘導するために)ドキシサイクリンを処理群に加えると、対照(ドキシサイクリンなし)と比較して、事前に確立されたオルガノイドの数が有意に減少した(図4Aおよび4B)。図4Aおよび4Bにおいて、ドキシサイクリン処理を開始した後の0日目(図4A)および16日目(図4B)でのオルガノイドコロニーの数を示す、****P<0.0001;nsはP>0.05(平均±標準偏差、n=6、実験は2回行った、P値は対応のないt検定によって決定した)。インビボでは、事前に確立されたinducCDK19KD-PDX-T1またはinducCDK19KD-PDX-T3(ドキシサイクリン誘導性CDK19ノックダウン構築物で形質導入されたPDX-T3細胞)腫瘍を有するマウスにドキシサイクリンを給餌すると、これらの腫瘍の増殖に有意な影響を与えた(図4Cおよび4D)。図4Cおよび4Dにおいて、ドキシサイクリン給餌NSGマウスおよび対照NSGマウスにおける事前に確立された腫瘍の増殖を示す:inducCDK19KD-PDX-T1、****P<0.0001;***P<0.001(平均±標準偏差、n=5、実験は2回行った、P値は対応のないt検定によって決定した)(図4C)およびinducCDK19KD-PDX-T3、****P<0.0001;***P<0.001(平均±標準偏差、n=5、実験は1回行った、P値は対応のないt検定によって決定した)(図4D)。CDK19 shRNA誘導腫瘍は、対照腫瘍と比較した場合、inducCDK19KD-PDX-T1腫瘍では最終的に82%小さく、inducCDK19KD-PDX-T3腫瘍では38%小さかった(図4Cおよび4D)。inducCDK19KD-PDX-T1実験およびinducCDK19KD-PDX-T3実験の両方において、マウスの総体重は、処理群と対照群との間で有意差はなかった(図14Aおよび14B)。最後に、生存研究は、PDX-T1腫瘍が対照shRNAで形質導入されたマウスと比較してPDX-T1腫瘍がCDK19 shRNAで形質導入されたマウスにおいて全生存が有意に長かったことを示した(図4E)。図4Eに示すのは、対照shRNA(黒線)、shCDK19-1(実線灰色)またはshCDK19-2(破線灰色)で形質導入されたPDX-T1異種移植片を移植されたマウスのカプランマイヤー生存曲線である。マウスを腫瘍の直径を毎週測定して追跡した。腫瘍の最長径が17mmを超えたときにマウスを屠殺した。shCDK19-2群の2匹のマウスはPDX腫瘍を発症せず、実験の終わりに屠殺した。これらのマウスは、shCDK19-2群の生存曲線を作成するときに打ち切られた、***P<0.001(n=9、実験は3回行った、ログランク(Mantel-Cox)検定を用いてP値を決定した)。要約すると、これらの実験は、事前に確立された腫瘍であっても、CDK19の特異的なノックダウンは腫瘍の増殖を有意に減少させること、およびCDK19ノックダウンはマウスの生存を延長できることを示した。
CDK19標的療法の使用を臨床的にモデル化するため、本発明者らは、CDK19および密接に関連するパラログCDK8の両方の経口で生物学的に利用可能な阻害剤であるCCT251921(図4F)で、事前に確立されたPDX腫瘍を有するマウスを処置した。PCT-T1腫瘍は、CCT251921またはビヒクルの毎日の経口投与(30mg/kg)を開始する前に、マウスで事前に確立された。CCT251921による処置は、14日目までに腫瘍増殖の有意な減少をもたらした(図4G)。CCT251921で処置されたマウスにおける腫瘍の最終的な容積は、ビヒクルで処置されたマウスの腫瘍よりも30%以上小さかった(図4G)。事前に確立されたPDX-T1異種移植腫瘍を有するNSGマウスを、CCT251921またはビヒクルの毎日の強制経口投与で処置した。マウスを週に2回、腫瘍容積を測定して追跡した、****P<0.0001;***P<0.001(平均±標準偏差、n=5、実験は1回行った、P値は対応のないt検定で決定した)。CCT251921コホートおよびビヒクルコホートの両方のマウスは全体的な体重減少を被ったが、これは2群間で有意差はなく、おそらくそれらの摂食習慣に対する毎日の強制経口投与の影響によるものであった(図14C)。遺伝子のノックダウンと化学的阻害とで異なる生物学的結果が生じる可能性があることはよく知られている。本発明者らは、本実施例において、CDK19キナーゼ活性の化学的阻害が、ノックダウン研究で示されたCDK19の総損失の影響を再現できることを、事前に確立された腫瘍で示す。

- トリプルネガティブ乳癌(TNBC)の治療における使用のための、サイクリン依存性キナーゼ19(CDK19)の発現または活性の阻害剤を含む医薬組成物であって、阻害剤は、CDK19標的shRNA、CDK19標的siRNA、または、CDK19遺伝子にハイブリダイズし、かつCRISPR/Casシステムで使用されるガイドRNAである、医薬組成物。
- 該癌が、EpCAMmed/high/CD10-/low上皮細胞を含む腫瘍であることを特徴とする、TNBCの治療における使用のための請求項1に記載の医薬組成物。
- 阻害剤が、CDK19の発現または活性を阻害するが、CDK8の発現または活性を有意に阻害しない、TNBCの治療における使用のための請求項1または2に記載の医薬組成物。
- 阻害剤が、CDK19の5'UTRに相補的または実質的に相補的であるがCDK8の5'UTRには相補的または実質的に相補的でない配列を含む、CDK19標的shRNAまたはsiRNAである、TNBCの治療における使用のための請求項1~3のいずれか一項に記載の医薬組成物。
