Description
The present invention relates to a balloon catheter in which a drug is held on the outer surface of the balloon.
It is known that various diseases occur due to stenosis of blood vessels, which is a flow path for blood circulation in the body, and stenosis of blood circulation. In particular, stenosis of the coronary arteries that supply blood to the heart may lead to serious diseases such as angina pectoris and myocardial infarction. As one of the methods for treating such a stenotic part of a blood vessel, there is angioplasty (PTA, PTCA, etc.) for dilating the stenotic part using a balloon catheter. Angioplasty is widely used because it is a minimally invasive therapy that does not require thoracotomy such as bypass surgery.
By the way, in the case of angioplasty, restenosis may occur in the dilated stenosis, and as a treatment method for reducing the frequency of such restenosis (restenosis rate), drug elution having a drug layer on the surface Treatment with stents is being performed. Further, a drug-eluting balloon catheter in which a drug is held in a balloon portion of the balloon catheter has also been proposed (for example, Patent Documents 1 to 4). If a drug-eluting balloon catheter is used, the drug can be transferred to the lumen wall by expanding the balloon at the lesion site, and the occurrence of restenosis can be expected to be suppressed. The advantages of the treatment method using a drug-eluting balloon catheter include that no foreign matter is left in the body and that small blood vessels into which a stent cannot be inserted can be treated.
However, it has been difficult to perform a conventional drug-eluting balloon catheter for a lesion in a place where a blood vessel is bent or a lesion in a place where a blood vessel is tapered (tapered). For example, when the treatment is performed at a place where the blood vessel is bent, the originally bent blood vessel is forcibly stretched by the expansion of the balloon, which gives an unnecessarily large load to the blood vessel or the lesion. In addition, when the balloon expands at the tapered portion, only the distal portion of the balloon contacts the luminal wall, and the proximal portion of the balloon does not contact the luminal wall, allowing the drug to efficiently contact the luminal wall. Does not migrate well.
An object of the present invention is to provide a balloon catheter that is expandable along the shape of a blood vessel, thereby efficient drug transfer, and has a low load and a minimal invasiveness to a blood vessel or a lesion. Is.

As a method of using a balloon catheter having a shaft and a plurality of balloons arranged at different positions in the axial direction of the shaft and holding a drug on the outer surface of at least one balloon, the balloon catheter is used in a blood vessel. It is preferable to have a step of inserting into the balloon, a second step of initiating the expansion of one balloon, and a third step of initiating the expansion of the balloon downstream of the one balloon after the second step.
Other uses include processing the tip of the guide wire into a shape that facilitates delivery of the balloon catheter to the lesion, advancing the guide wire to the target lesion, and inserting the balloon catheter into the blood vessel. And the step of arranging the balloon according to the position of the bent or tapered part of the lumen such as the target blood vessel, or the position of the indwelling stent, and the inside of the blood vessel between the target lesion and the balloon. The step of confirming the positional relationship in, the step of aligning the balloon with respect to the target lesion, the step of starting the expansion of one balloon, and the step of confirming that the blood flow in the target lesion is suppressed. A step of initiating expansion of a balloon located downstream of the one balloon in the blood flow direction, a step of measuring the expansion time of the balloon, and a step of transferring more drug to the target lesion. Slowly contract the dilated balloon, confirm that the balloon is contracting, remove the balloon catheter from the target lesion site, and check the condition of the target lesion after dilation and drug transfer. May have steps to do.
The step of processing the tip of the guide wire into a shape that facilitates delivery of the balloon catheter to the lesion is to improve the passage of the guide wire to the lesion or the distal side of the lesion, or to remove the balloon portion of the balloon catheter. The operator bends or bends the shape of the tip in order to improve workability when accurately aligning the lumen such as a blood vessel where the lumen is bent or tapered. By doing so, it means the step of habituating and processing. The step of slowly contracting the expanded balloon is that the drug transferred to the blood vessel wall in the previous step is lost by elution and outflow from the blood vessel wall into the blood due to the sudden resumption of blood flow. By contracting the balloon over time in order to suppress the above, the time required for contraction at this time is preferably 10 seconds or more.
In the balloon catheter of the present invention, since a plurality of balloons are arranged at different positions in the axial direction of the shaft, the shaft between the plurality of balloons can be flexed, and an organ such as a blood vessel is bent or tapered. Each expanded balloon can easily follow the luminal wall even in the shape of the balloon, and the drug can be efficiently transferred to the luminal wall. Moreover, the load and invasiveness on the organs and lesions are low. It is a thing.
Hereinafter, the present invention will be described in more detail based on the following embodiments, but the present invention is not limited by the following embodiments as well as the present invention, and appropriate changes are made to the extent that it can be adapted to the purpose of the above and the following. In addition, it is of course possible to carry out, and all of them are included in the technical scope of the present invention. In each drawing, hatching, member reference numerals, and the like may be omitted for convenience, but in such cases, the specification and other drawings shall be referred to. In addition, the dimensions of various members in the drawings may differ from the actual dimensions because priority is given to contributing to the understanding of the features of the present invention.
(Embodiment 1)
The balloon catheter according to the first embodiment of the present invention has a shaft and a plurality of balloons arranged at different positions in the axial direction of the shaft, and among the plurality of balloons, the most distal and the most recent. The drug is held on the outer surface of at least one balloon excluding the position balloon. In the balloon catheter of the present invention, since a plurality of balloons are arranged at different positions in the axial direction of the shaft, the shaft between the plurality of balloons can be bent, and the lumen such as a blood vessel is bent. Even in the tapered part, each expanded balloon can easily follow the luminal wall, and the drug can be efficiently transferred to the luminal wall. Moreover, the load and invasiveness on the blood vessels and lesions are low. It is a thing. Of the multiple balloons, the most distal and most recent balloons have the effect of fixing the position of the shaft and balloon during the treatment in the blood vessel, and the diameter at the time of expansion is set according to the diameter of the blood vessel at the treatment site. You can also do it. In the present invention, the term "plurality of balloons" does not require that the resin film constituting the balloon is interrupted between one balloon and another balloon, but is continuous in the axial direction. It is also possible to form a plurality of balloons by providing expansion sections at a plurality of locations using one resin film.
The overall configuration of the balloon catheter according to the embodiment of the present invention will be described with reference to FIGS. 1 and 2. 1 is a plan view of the balloon catheter, and FIGS. 2A to 2D are a cross-sectional view taken along the line AA, a cross-sectional view taken along the line BB, and a cross-sectional view taken along the line CC of the balloon catheter shown in FIG. 1, respectively. , And DD cross-sectional views are shown. The balloon catheter shown in FIGS. 1 and 2 is an over-the-wire type in which a wire is inserted from the distal side to the proximal side of the shaft.
As shown in FIGS. 1 and 2, the balloon catheter 1 includes a
The balloon catheter 1 is configured such that a pressure fluid is supplied from the hub 5 to the inside of the

Next, the effect of the balloon catheter of the present invention will be described with reference to FIGS. 3 and 4. 3 (a) to 3 (c) are process cross-sectional views showing how a balloon expands in a blood vessel in a conventional balloon catheter, and some
As shown in FIGS. 3 (a) and 4 (a), the blood vessel has a linear portion and a curved portion. Conventionally, when the balloon catheter 1 is inserted into the bent blood vessel as shown in FIG. 3 (b), the balloon catheter 1 is originally bent as shown in FIG. 3 (c) as the
(Embodiment 2)
The balloon catheter according to the second embodiment of the present invention can be described with reference to FIGS. 1 and 2 as in the first embodiment. As shown in FIGS. 1 and 2 (a) to 2 (d), the balloon catheter according to the second embodiment is a balloon having a shaft and a plurality of balloons arranged at different positions in the axial direction of the shaft. That the drug is held on the outer surface of at least one balloon in the catheter, and the circumferential length of the balloon catheter in the portion where the balloon is placed is the same as the circumferential length of the balloon. It is a feature. In particular, as shown in FIGS. 2 (b) to 2 (d), no member or component is arranged outside the
(Embodiment 3)
The balloon catheter according to the third embodiment of the present invention has a plurality of
When the rigidity of the
(Embodiment 4)
FIG. 6 is a plan view of the balloon catheter according to the fourth embodiment of the present invention. In FIG. 6, among the plurality of balloons, at least one of the balloons (intermediate balloon 11) excluding the most
In the conventional balloon catheter, the size of the balloon is appropriately set according to the size of the treatment site and the like. For example, when the treatment site is a blood vessel, the axial length is 5 mm to 300 mm and the outer diameter is 1 mm. It is preferably ~ 12 mm, and when the treatment site is a gastrointestinal tract such as the papilla of Vater, the axial length is 10 mm to 100 mm and the outer diameter is 3 mm to 30 mm. In the present invention, since the number of balloons is a plurality, the axial length per balloon can be designed to be small. The aspect ratio (x / y) of the

FIG. 9 is a plan view of the balloon catheter according to the seventh embodiment of the present invention (blood vessels are not shown). The balloon catheter shown in FIG. 9 has the most

(Embodiment 8)
10 (a) to 10 (c) are process cross-sectional views showing how the balloon expands in the blood vessel in the balloon catheter 1 according to the eighth embodiment of the present invention. As shown in FIG. 10, the
Some blood vessels have a constant thickness, and some blood vessels have a tapered shape as shown in FIG. 10 (a). Conventionally, when the balloon catheter 1 is inserted into such a tapered portion, only the distal portion of the balloon contacts the
In addition to the above features, the balloon catheter 1 according to the eighth embodiment of the present invention has the maximum diameter of the most

(Embodiment 9)
11 (a) to 11 (d) are cross-sectional views of the process showing how the balloon expands in the blood vessel in the balloon catheter of the present invention. In the ninth embodiment, a step of inserting the balloon catheter into the blood vessel, a second step of starting the expansion of the one balloon, and after the second step, expansion of the balloon on the downstream side of the one balloon is performed. A method comprising a third step to start will be described.
First, as a first step, as shown in FIG. 11A, the balloon catheter 1 of the present invention is inserted into a blood vessel. The arrow shown in FIG. 11A indicates the direction of blood flow, and the
Next, as a second step, expansion of the
Next, as a third step, after the second step described above, a pressure fluid is injected into the balloons (
Finally, as shown in FIG. 11D, the
In the ninth embodiment, an example in which the
When the lumen for expanding one balloon and the lumen for expanding another balloon are common, the distance between the balloon and the adjacent balloon is increased in order to suppress blood flow upstream of the blood vessel. It is preferably 5 mm or more and 20 mm or less. By making it 5 mm or more, it can be expected that there will be a time lag between the expansion of the upstream balloon and the expansion of the downstream balloon, which will lead to reliable suppression of blood flow from the upstream side first, and blood flow. It is possible to prevent the drug from being eluted / outflowed and lost. Further, by setting the thickness to 20 mm or less, even if the blood vessel to be treated is a bifurcated blood vessel, the blood flow can be surely suppressed, and the drug can be prevented from being eluted / outflowed and lost due to the blood flow. .. With the above configuration, in the ninth embodiment, a method of using a balloon catheter capable of reducing the amount of the drug that elutes and flows out from the balloon holding the drug into the blood and is lost without migrating to the lesion. Is provided.
(Common subject matter)
Next, detailed matters that can be commonly adopted in any of the balloon catheters in each of the above-described embodiments will be described.
The balloon can be manufactured by molding a resin. For example, a balloon can be manufactured by arranging a resin tube extruded by extrusion molding in a mold and performing biaxial stretching blow molding. The balloon can be formed into any shape depending on the shape of the mold. In addition, the balloon can be manufactured by a known molding method such as dip molding, injection molding, or compression molding.
Examples of the resin constituting the balloon include polyamide-based resin, polyester-based resin, polyurethane-based resin, polyolefin-based resin, vinyl chloride-based resin, silicone-based resin, and natural rubber. Only one of these may be used, or two or more thereof may be used in combination. Of these, polyamide-based resins, polyester-based resins, and polyurethane-based resins are preferably used. As these resins, it is preferable to use an elastomer resin from the viewpoint of thinning the balloon and flexibility. For example, among polyamide resins,
Further, in order to improve the dilation performance for the stenotic lesion hardened by calcification and the dimensional stability against the dilation pressure, the balloon may be integrally reinforced. For example, a reinforcing balloon can be obtained by arranging a mold from an extruded resin tube, performing blow molding, and then joining a reinforcing material with an adhesive. As the reinforcing material, for example, a fiber material can be used, and specifically, polyarylate fiber, aramid fiber, ultra-high molecular weight polyethylene fiber, PBO fiber, carbon fiber and the like are preferably used. These fibrous materials may be monofilaments or multifilaments.
The drug held in the balloon is not particularly limited as long as it is a pharmacologically active substance, and examples thereof include drugs that are acceptable as drugs such as gene therapy drugs, non-gene therapy drugs, small molecules, and cells. In particular, when a balloon catheter is used for the purpose of suppressing restenosis of blood vessels after treatment in angioplasty, anti-restenosis agents such as antiproliferative agents and immunosuppressive agents can be preferably used as agents, and specifically. Can be used with drugs such as paclitaxel, sirolimus (rapamycin), everolimus, and zotarolimus. Only one of these agents may be used, or two or more of these agents may be used.
The drug may be held in the balloon as a drug layer, or may be held in the balloon in the form of microcapsules or the like. In addition, the drug may contain a pharmacologically active substance as well as an auxiliary agent for improving the dispersibility, solubility, transferability to the lumen wall, and storage stability of the drug. As the auxiliary agent, a stabilizer, a base, a binder, an excipient, a disintegrant, a moisture-proofing agent, a preservative, a dissolving agent, a solubilizing agent and the like are used. Specifically, saccharides such as lactose, sucrose, dextrin, xylitol, erythritol, mannitol, carboxymethyl cellulose, oxidized cellulose, hydroxycellulose, and hydroxymethylcellulose, benzoates, ethylenediamine, potassium iodide, urea, polysorbate, dibutylhydroxytoluene, and pyro. Examples thereof include sodium sulfite, ascorbic acid, tocopherol, benzoic acid, paraoxybenzoic acid esters, benzalconium chloride, gum arabic, alginate, glycerin and the like.
The drug may be held in the balloon while being protected by a coating layer to prevent the drug from elution or shedding into the blood during delivery. The coating layer can be formed from a water-soluble polymer, a hydrophobic polymer, a saccharide, a lipid, a surfactant, or the like. Auxiliary agents or coating layers can be used for the purpose of placing the agent on the balloon in a larger amount and more efficiently. Placing more drug in the intended area on the balloon can prevent elution or shedding of the placed drug during delivery and carry more drug to the target lesion. Since some drugs exert a therapeutic effect in a dose-dependent manner, it is possible to contribute to the improvement of the therapeutic effect by arranging a large amount of the drug on the balloon.
The present invention can also be applied to a rapid exchange type balloon catheter in which a wire is inserted halfway from the distal side to the proximal side of the shaft. In that case, the wire insertion passage may be provided in a part of the shaft including the distal side of the shaft, and the hub may not be provided with a treatment portion.
